| 1 | Diagnostic criteria for acute liver failure due to Wilson disease显示文摘AIM: To describe the diagnostic criteria for acute liver failure due to Wilson disease (WD), which is an uncommon cause of acute liver failure (ALF). METHODS: We compared findings of patients presenting with ALF due to WD to those with ALF of other etiologies. RESULTS: Previously described criteria, such as low alkaline phosphatase activity, ratio of low alkaline phosphatase to total bilirubin or ratio of high aspartate aminotransferase (AST) to alanine aminotransferase (ALT), failed to identify patients with ALF due to WD. There were significant differences in low ALT and AST activities (53 ± 43 vs 1982 ± 938, P < 0.0001 and 87 ± 44 vs 2756 ± 2941, P = 0.037, respectively), low choline esterase activity (1.79 ± 1.2 vs 4.30 ± 1.2, P = 0.009), high urine copper concentrations (93.4 ± 144.0 vs 3.5 ± 1.8, P = 0.001) and low hemoglobin (7.0 ± 2.2 vs 12.6 ± 1.8, P < 0.0001) in patients with ALF caused by WD as compared with other etiologies. Interestingly, 4 of 7 patients with ALF due to WD survived without liver transplantation. CONCLUSION: In ALF, these criteria can help establish a diagnosis of WD. Where applicable, slit- lamp examination for presence of Kayser-Fleischer rings and liver biopsy for determination of hepatic copper concentration still remain important for the diagnosis of ALF due to WD. | 2 | Catalytic synthesis of acetals and ketals with H3PW12O40/PAn显示文摘A new environmental friendly catalyst H3PW12O40/PAn was prepared and identified by means of FT-IR,XRD,and TG/DTA.The optimum conditions have been found;that is,the mass ratio of PAn to H3PW12O40 is 1:1.5,the volume of methanol is 20 mL,and the reflux reaction time is 3 h.The structural identity of Keggin units is preserved after the incorporation into polyaniline matrix.Catalytic activities of H3PW12O40/PAn in synthesizing 2-methyl-2-ethoxycarbonylmethyl-1,3-dioxolane,2,4-dimethyl-2-ethoxycarbonylmethyl-1,3-dioxolane,cyclohexanone ethylene ketal,cyclohexanone 1,2-propanediol ketal,butanone ethylene ketal,butanone 1,2-propanediol ketal,2-phenyl-1,3-dioxolane,4-methyl-2-phenyl-1,3-dioxolane,2-propyl-1,3-dioxolane,and 4-methyl-2-propyl-1,3-dioxolane were reported.It has been demon-strated that H3PW12O40/PAn is an excellent catalyst.Various factors concerned in these reactions were investigated.The optimum conditions are as follows:the molar ratio of aldehyde/ketone to glycol(r) is 1:1.5,the mass ratio of the catalyst used to the reactants is 0.6%,and the reaction time is 1.0 h.Under these conditions,the yield is as follows:2-methyl-2-ethoxycarbonylmethyl-1,3-dioxolane,69.0%;2,4-dimethyl-2-ethoxycarbonylmethyl-1,3-dioxolane,79.5%;cyclohexanone ethylene ketal,78.9%;cyclohexanone 1,2-propanediol ketal,85.3%;butanone ethylene ketal,56.9%;butanone 1,2-propanediol ketal,78.1%;2-phenyl-1,3-dioxolane,76.3%;4-methyl-2-phenyl-1,3-dioxolane,94.2%;2-propyl-1,3-dioxolane,70.7%;and 4-methyl-2-propyl-1,3-dioxolane,79.2%. | YANG Shuijin ZHANG Yijun DU Xinxian Philippe G. | 3 | Combination therapy for advanced hepatocellular carcinoma:do we see the light at the end of the tunnel?显示文摘Importance:Combination therapies of anti-PD-1 and anti-angiogenesis regimens are emerging rapidly and exhibit more promising anti-tumor efficacy for advanced hepatocellular carcinoma(HCC),and consistently it is the hotspot in clinical studies.Objective:To elaborate several issues which are warranted further consideration as more regimens are being investigated in combination therapies.Evidence Review:We searched PubMed,MEDLINE,Cochrane Library and Google Scholar by 2021 February for publications on combination therapies for HCC.Findings:Several clinical issues are worth reconsidering,such as the evaluation on appropriate primary endpoints in phase III clinical trials as for different practical problems,the translation of surrogate endpoint objective response rate(ORR)benefits into overall survival(OS)benefits,and whether conversion surgery contributes to initial expectations of long-term survival or not.New concepts in novel immunotherapy and targeted therapy in combination with loco-regional therapies may improve overall survival for HCC.Conclusions and Relevance for Reviews:Comprehensive understanding of the mechanism of immunotherapy and targeted therapy contributes to better prognosis of advanced HCC and more explorative combination therapies are needed. | Ti Zhang Philippe Merle Huaqi Wang Haitao Zhao Masatoshi Kudo | 2021 | Hepatobiliary Surgery and Nutrition2021，10，2： | 7 |

| 9 | Determination of polyfluoroalkyl compounds in water and suspended particulate matter in the river Elbe and North Sea,Germany显示文摘The distribution of polyfluoroalkyl compounds(PFCs)in the dissolved and particulate phase and their discharge from the river Elbe into the North Sea were studied.The PFCs quantified included C_(4)-C_(8) perfluorinated sulfonates(PFSAs),6∶2 fluorotelomer sulfonate(6∶2 FTS),C_(6) and C_(8) perfluorinated sulfinates(PFSiAs),C_(4)-C_(12) perfluorinated carboxylic acids(PFCAs),perfluoro-3,7-dimethyl-octanoic acid(3,7m_(2)-PFOA),perfluorooctane sulfonamide(FOSA),and n-ethyl perfluroctane sulfonamidoethanol(EtFOSE).PFCs were mostly distributed in the dissolved phase,where perfluorooctanoic acid(PFOA)dominated with 2.9–12.5 ng/L.In the suspended particulate matter FOSA and perfluorooctane sulfonate(PFOS)showed the highest concentrations(4.0 ng/L and 2.3 ng/L,respectively).The total flux ofΣPFCs from the river Elbe was estimated to be 802 kg/year for the dissolved phase and 152 kg/year for the particulate phase.This indicates that the river Elbe acts as a source of PFCs into the North Sea.However,the concentrations of perfluorobutane sulfonate(PFBS)and perfluorobutanoic acid(PFBA)in the North Sea were higher than that in the river Elbe,thus an alternative source must exist for these compounds. | Lutz AHRENS Merle PLASSMANN Zhiyong XIE Ralf EBINGHAUS | 2009 | Frontiers of Environmental Science & Engineering2009，3，2： | 3 |

| 11 | An unusual case of incidental rupture of liver hemangioma during labor显示文摘Cavernous hemangiomas are the most common benign tumors of the liver and are found in about 2% of autopsy patients. 1,2 The vast majority of these tumors are small and asymptomatic; however, there have been a few reported cases in which these lesions led to spontaneous fatal hemorrhage. 1,2 Although liver hemangiomas occur in both sexes, most studies point to a female predominance; one study reported a ratio of female predominance of 4. 5: 1.3 It has been suggested that estrogen may be associated with the growth of liver hemangiomas, but the incidence of these lesions in pregnancy and the effect on them of the increased estrogen levels during pregnancy are unknown. 2 Symptomatic liver hemangiomas have been treated by steroids, radiation therapy, surgical resection and recently embolization, but surgeons may sometimes be confronted with intraabdominal hemorrhage originating from rupture of asymptomatic liver hemangiomas.
