1Blumgart's technique of pancreaticojejunostomy: Analysis of safety and outcomes显示文摘Background: Blumgart's pancreaticojejunostomy(PJ) has been described with low pancreatic leak rates. This study aimed to evaluate our experience with this technique regarding the pancreatic leak and other perioperative outcomes. Methods: We performed a single-center retrospective analysis of a cohort of 81 patients who underwent pancreaticoduodenectomy in our department from January 2011 to February 2018. The primary endpoint was the occurrence of a clinically relevant postoperative pancreatic fistula(CR-POPF) and analysis of its risk factors. Results: The CR-POPF rate was 12.3%. Fistula risk score(FRS) was the only significant risk factor for the occurrence of overall POPF in multivariate analysis. However, none of the other factors including FRS was found to be significantly associated with CR-POPF risk. A strong positive correlation was found between the CR-POPF and the incidence of delayed gastric emptying, post-pancreatectomy hemorrhage and increased length of hospital stay. Conclusion: Blumgart's technique is a safe technique of pancreatico-enteric anastomosis with low rates of CR-POPF. CR-POPF with this technique is independent of most of the preoperative and intraoperative factors. 2Toxicity Evaluation of Freshwater Cyanobacterium Microcystis aeruginosa PCC 7806: Ⅱ Nephrotoxicity in Rats显示文摘Nephrotoxic potential of laboratory cultures of freshwater cyanobacterium (blue-green al ga) Microcystis aeruginosa PCC 7806 (Pasteur Institute) was assessed in male rats. The ani mals were injected intraperitoneally with 0. 5, 1. 0 and 2. 0 LD50 doses of lyophilized cell ex tract. Elevated plasma urea and creatinine levels were accompanied by decrease in protein and albumin levels, followed by hematuria, proteinuria and bilirubinuria. Also decrease in kidney lactate dehydrogenase and glutamic oxaloacetic transaminase indicated possible nephrotoxic po tential of the cyanobacteria. 3Solar radiation-use characteristics of indica/japonica hybrid rice (Oryza sativa L.) in the late season in southeast China显示文摘New indica and japonica hybrid rice cultivars,such as the Yongyou series,provide farmers with very high yield potential.However,information on their canopy light capture and solar radiation use efficiency in the late season is limited.Field experiments were performed to compare the radiation-use parameters of four rice types:indica rice(IR),inbred japonica rice(IJR),hybrid japonica rice(HJR),and hybrid indica/japonica rice(HIJR),from 2016 to 2018 during the late season in Hangzhou,China.The grain yield,aboveground biomass,intercepted solar radiation(SI),and radiation-use efficiency(RUE)of the HIJR were on average respectively 13.4%–53.4%,14.3%–30.6%,7.6%–21.4%,and 8.2%–14.9%higher than those of the HJR,IJR,and IR.The leaf area index(LAI)of the HIJR was 18.2%–57.0%greater than that of the IJR and HJR at four growth stages,resulting in respectively 17.8%–38.5%and 10.7%–42.8%greater canopy light interception rates(LIR)and amount of intercepted solar radiation during the vegetative stage.The prolonged grain-filling stage also led to respectively 33.9%–52.6%and 30.5%–51.4%increases in amounts of incident and intercepted radiation for the HIJR relative to the IR during grain filling.These results indicate that the SI superiority of the HIJR was caused by canopy closure as rapid as that of the IR during the vegetative stage(greater LAI and canopy LIR during the growing season)and a grain-filling stage as long as that of the HJR.For grain-filling stage,differences in leaf Pn between HIJR,IR,and IJR were not significant,suggesting that the greater RUE of the HIJR(12.7%–52.8%higher)than that of the other rice types resulted from improved canopy architecture after flowering(FL).Principal components analysis(PCA)revealed that the superiority of the HIJR in terms of solar radiation use resulted from the greater canopy light capture capability of IR and the prolonged growth period(especially during grain filling)of japonica rice in the late growing season.Min Yin Shaowen Liu Xi Zheng Guang Chu Chunmei Xu Xiufu Zhang Dangying Wang Song Chen 2021The Crop Journal2021,9,2:3
4Emerging approaches and technologies in transplantation:the potential game changers显示文摘Newly emerging technologies are rapidly changing conventional approaches to organ transplantation.In the modern era,the key challenges to transplantation include(1)how to best individualize and possibly eliminate the need for life-long immunosuppression and(2)how to expand the donor pool suitable for human transplantation.This article aims to provide readers with an updated review of three new technologies that address these challenges.First,single-cell RNA sequencing technology is rapidly evolving and has recently been employed in settings related to transplantation.The new sequencing data indicate an unprecedented cellular heterogeneity within organ transplants,as well as exciting new molecular signatures involved in alloimmune responses.Second,sophisticated nanotechnology platforms provide a means of therapeutically delivering immune modulating reagents to promote transplant tolerance.Tolerogenic nanoparticles with regulatory molecules and donor antigens are capable of targeting host immune responses with tremendous precision,which,in some cases,results in donor-specific tolerance.Third,CRISPR/Cas9 gene editing technology has the potential to precisely remove immunogenic molecules while inserting desirable regulatory molecules.This technology is particularly useful in generating genetically modified pigs for xenotransplantation to solve the issue of the shortage of human organs.Collectively,these new technologies are positioning the transplant community for major breakthroughs that will significantly advance transplant medicine.Anil Dangi Shuangjin Yu Xunrong Luo 2019Cellular & Molecular Immunology2019,16,4:3

6Gallbladder perforation:A single-center experience in north India and a step-up approach for management显示文摘Background: Spontaneous gallbladder perforation(GBP) is an uncommon diagnosis. This study presented the experience of managing spontaneous GBP over nine years at a large, tertiary care university hospital in north India and investigated the outcomes and treatment strategies. Methods: A retrospective review of prospectively maintained digital database of consecutive patients was performed. All patients received medical and/or surgical treatment for spontaneous GBP in our department between January 2010 and June 2018. Results: We identifed 151 patients(81 females and 70 males) with mean age of 53 years. Most common presenting features were pain(96.7%), fever(54.3%) and jaundice(31.1%). Most common cause was gallbladder stones(84.8%) followed by common bile duct stones(30.5%), xanthogranulomatous cholecystitis(17.9%) and malignancy(11.9%). As per Niemeier classifcation, 8.6% had type 1 GBP(free perforation in peritoneal cavity), 76.2% had type 2 GBP(localized perforation) and 13.2% had type 3 GBP(cholecystoenteric fstula). About 60% of the perforations were diagnosed preoperatively. Type 1 was more common in patients with diabetes and also had the worst prognosis. Surgery was performed in 109 patients(72.2%). Seven patients(4.6%) had a postoperative morbidity of Clavien-Dindo Ⅲ or higher. There were three mortalities in patients who underwent surgery. Conclusions: High index of suspicion is required for preoperative diagnosis of GBP, especially in types 2 and 3. Laparoscopic cholecystectomy can be difficult in these patients and patients may require open or partial cholecystectomy.
