1Helicobacter pylori-induced inflammation and epigenetic changes during gastric carcinogenesis显示文摘The sequence of events associated with the development of gastric cancer has been described as 'the gastric precancerous cascade'. This cascade is a dynamic process that includes lesions, such as atrophic gastritis, intestinal metaplasia and dysplasia. According to this model, Helicobacter pylori(H. pylori) infection targets the normal gastric mucosa causing non-atrophic gastritis, an initiating lesion that can be cured by clearing H. pylori with antibiotics or that may then linger in the case of chronic infection and progress to atrophic gastritis. The presence of virulence factors in the infecting H. pylori drives the carcinogenesis process. Independent epidemiological and animal studies have confirmed the sequential progression of these precancerous lesions. Particularly long-term follow-up studies estimated a risk of 0.1% for atrophic gastritis/intestinal metaplasia and 6% in case of dysplasia for the long-term development of gastric cancer. With this in mind, a better understanding of the genetic and epigenetic changes associated with progression of the cascade is critical in determining the risk of gastric cancer associated with H. pylori infection. 2Alcoholic liver disease:Utility of animal models显示文摘Alcoholic liver disease(ALD) is a major cause of acute and chronic liver injury. Extensive evidence has been accumulated on the pathological process of ALD during the past decades. However, effective treatment options for ALD are very limited due to the lack of suitable in vivo models that recapitulate the full spectrum of ALD. Experimental animal models of ALD, particularly rodents, have been used extensively to mimic human ALD. An ideal animal model should recapitulate all aspects of the ALD process, including significant steatosis, hepatic neutrophil infiltration, and liver injury. A better strategy against ALD depends on clear diagnostic biomarkers, accurate predictor(s) of its progression and new therapeutic approaches to modulate stop or even reverse the disease. Numerous models employing rodent animals have been established in the last decades to investigate the effects of acute and chronic alcohol exposure on the initiation and progression of ALD. Although significant progress has been made in gaining better knowledge on the mechanisms and pathology of ALD, many features of ALD are unknown, and require further investigation, ideally with improved animal models that more effectively mimic human ALD. 3Probiotic effects on intestinal fermentation patterns in patients with irritable bowel syndrome显示文摘AIM: To determine whether Lactobacillus casei strain Shirota (Yakult ) can alter small intestinal bacterial overgrowth (SIBO), as tested by the lactulose breath test, and whether this is associated with changes in symptoms in irritable bowel syndrome (IBS). METHODS: 18 patients with IBS (Rome Ⅱ criteria), who showed an early rise in breath hydrogen with lactulose (ERBHAL), consumed 65 mL of Yakult daily for 6 wk. Lactulose breath test was repeated at the end of the treatment period. Symptoms were recorded daily using a 10 cm visual analogue scale. RESULTS: 14 patients completed the study, 9 (64%) had reversal of ERBHAL, with the median time of f irst rise in breath hydrogen increasing from 45 to 75 min (P = 0.03). There was no signifi cant improvement in the symptom score with probiotic therapy, except for wind (P=0.04). Patients commencing with at least moderate symptoms and who no longer had ERBHAL at the end of treatment, showed improvement in the overall symptoms scores [median fi nal score 5.3 (IQR3.9-5.9), 55% reduction; n=6] to a greater extent than those who had had persisting ERBHAL [final score 6.9 (5.0-7.0), 12% reduction; n = 5; P = 0.18]. CONCLUSION: Yakult is effective in altering fermentation patterns in the small bowel, consistent with reducing SIBO. The loss of ERBHAL was associated with reduced symptoms. 4Ten years of sorafenib in hepatocellular carcinoma: Are there any predictive and/or prognostic markers?显示文摘Sorafenib has been considered the standard of care for patients with advanced unresectable hepatocellular carcinoma(HCC) since 2007 and numerous studieshave investigated the role of markers involved in the angiogenesis process at both the expression and genetic level and clinical aspect. What results have ten years of research produced? Several clinical and biological markers are associated with prognosis. The most interesting clinical parameters are adverse events, Barcelona Clinic Liver Cancer stage, and macroscopic vascular invasion, while several single nucleotide polymorphisms and plasma angiopoietin-2 levels represent the most promising biological biomarkers. A recent pooled analysis of two phase III randomized trials showed that the neutrophil-to-lymphocyte ratio, etiology and extra-hepatic spread are predictive factors of response to sorafenib, but did not identify any predictive biological markers. After 10 years of research into sorafenib there are still no validated prognostic or predictive factors of response to the drug in HCC. 6Assessment of rock slope stability by probabilistic-based Slope Stability Probability Classification method along highway cut slopes in Adilcevaz-Bitlis(Turkey)显示文摘Rock slope stability is of great concern along highway routes as stability problems on cut slopes may cause fatal events as well as loss of property.In rock slope engineering,stability evaluations are commonly performed by means of analytical or numerical analyses,principally considering the factor of safety concept.As a matter of fact,the probabilistic assessment of slope stability is progressively getting popularity due to difficulties in assigning the most appropriate values to design parameters in analytical or numerical methods.Additionally,the effect of heterogeneities in rock masses and discontinuities on the analysis results is minimized through the probabilistic concept.In this study,slope stability of high and steep sedimentary rock cut slopes along a state highway in AdilcevazBitlis(Turkey) was evaluated on the basis of probabilistic approach using the Slope Stability Probability Classification(SSPC) system.The probabilistic assessment indicates major slope stability problems because of discontinuity controlled and discontinuity orientation independent mass movements.Almost all studied cut slopes suffer from orientation-independent stability problems with very low stability probabilities.Additionally,the probability of planar and toppling failures issignificantly high with respect to the SSPC system.The stability problems along the investigated rock slopes were also verified by field reconnaissance.Remedial measures such as slope re-design and reinforcement at the studied locations should be taken to prevent hazardous events along the highway.On the other hand,the probabilistic approach may be a useful tool during rock slope engineering to overcome numerous uncertainties when probabilistic and analytic results are compared.Akgun CANAL Mutluhan AKIN 2016Journal of Mountain Science2016,13,11:4
7Important parameters in plasma jets for the production of RONS in liquids for plasma medicine: A brief review显示文摘Reactive oxygen and nitrogen species (RONS) are among the key factors in plasma medicine. They are generated by atmospheric plasmas in biological fluids, living tissues and in a variety of liquids. This ability of plasmas to create a delicate mix of RONS in liquids has been used to design remote or indirect treatments for oncological therapy by treating biological fluids by plasmas and putting them in contact with the tumour;Documented effects include selective cancer cell toxicity, even though the exact mechanisms involved are still under investigation. However, the 'right' dose for suitable therapeutical activity is crucial and still under debate. The wide variety of plasma sources hampers comparisons. This review focuses on atmospheric pressure plasma jets as the most studied plasma devices in plasma medicine and compiles the conditions employed to generate RONS in relevant liquids and the concentration ranges obtained.
