| 1 | 尿激酶治疗急性心肌梗塞多中心临床试验1406例总结显示文摘为观察尿激酶天普洛欣(UKTP)经静脉溶栓治疗急性心肌梗塞(AMI)的临床有效性及安全性。收集协作组148家医院1994年11月至1996年4月经静脉UKTP溶栓治疗AMI患者1406例,观察临床疗效、副作用及病死率等。其中124例行90分钟冠状动脉造影评价梗塞血管开通情况。结果:梗塞血管临床再灌注率为73.5%,90分钟冠状动脉造影血管开通率为72.6%,5周总病死率为7.8%(109/1406),轻度出血10.2%(143/1406),中重度出血0.43%(6/1406),脑出血0.50%(7/1406)。老年(>65岁)甚至高龄(>75岁)患者溶栓及距发病超过6小时者,其用药仍然安全有效,UKTP合适的用药剂量可能为150万U左右。结果提示UKTP治疗AMI安全有效。 | The collaborative study group for national multicenter clinical trial of urokinase thrombolytic therapy (Correspondence: Hu Dayi, Xu Zhimin. | 2 | 不稳定性心绞痛、急性非Q波心肌梗死不同抗栓疗法的对比研究显示文摘目的 观察不同抗栓方案对急性冠状动脉综合征心脏事件、出血风险和预后的影响。方法 本研究为前瞻性、多中心、随机、开放试验 ,入选患者随机分为静脉滴注普通肝素组和皮下注射低分子量肝素组。入选对象为不稳定性心绞痛或非Q波心肌梗死 ,入选前 4 8小时以内至少有一次心绞痛发作 ,ST段无抬高。肝素 10 0IU/kg静注 ,续 10 0 0IU/h ,维持活化的部分凝血活酶时间 (APTT)或活化的全血凝固时间 (ACT)于正常的 1 5~ 2 0倍 ,连续 7日。低分子量肝素 0 4~ 0 6ml,每日两次皮下注射 ,连续 7日。主要观察终点 :随访治疗 3 0日内发生急性心肌梗死、心脏性或非心脏性死亡和药物治疗无法控制心绞痛 ,需行急性血运重建术。住院至少 7日 ,随访至治疗后 3 0日。结果 本研究共入选符合条件的患者 4 0 2例 ,两组在性别、年龄、心血管危险因素和心绞痛发作方面差异无显著性。治疗后 7日 ,两组用药期间平均胸痛发作次数差异无显著性 ,但肝素组有更多的患者需口服硝酸甘油缓解胸痛 ;病死率在低分子量肝素组较普通肝素组低 ,两组比较P值为 0 0 62 ,复合终点事件 (死亡、心肌梗死和紧急血管重建 )在低分子量肝素组明显下降。低分子量肝素组出血事件明显少于肝素组。治疗开始后 3 0日 ,低分子量肝素组死亡和复合终点事? | Clinical Collaborative Study Group of Low Molecular Weight Heparin (Correspondent: HU Dayi, XU Juntang. In higher eucaryotic cells, such as mammalian cells, signals that arrest the cycle usually act at a G1 checkpoint. Cells that pass this restriction point are committed to complete the cycle. | 8 | Vein transplantation using human umbilical cord blood stem cells in the treatment of stroke sequela显示文摘BACKGROUND: Transplanted mononuclear cell(MNC)of umbilical blood can survive in central nervous system (CNS)of host through blood brain barrier,differentiate into nerver cells,migrate to damaged site and integrate morphological strucgh and function with nerve cells of host so as to improve deficiencies of sensatory function,motor function and cognitive function and influence on stroke sequela.OBJECTIVE: To observe the vein transplantation of human umbilical cord blood stem cells(HUCBSC) for improving neurological function,limb funtion and activity of daily living of patients with stroke and evaluate the reliability.DESIGN: Self-controlled study.SETTING: Department of Neurosurgery,the Second People's Hospital of Zhengzhou City;Red-crossed Blood Center of Henan Province;Department of Neurosurgery,the Fist Affiliated Hospital of Zhenzhou University.PARTICIPANTS:A total of 10 patients with stoke sequela were selected from Department of Cerebral Surgery,the Second People's Hospital of Zhengzhou City from April to December 2005.There were 9males and 1 female aged from 35to 75years with the mean age of 56 years.All of them were diagnosed with CT and MRI examination and coincidence with diagnostic criteria of stroke established by the Fourth National Academic Meeting for Cerebrovascular Disease.All patients provided informed consent. METHODS:80-140 mL umbilical blood of term birth of newborn was selected hermetically and maintained in sterile plastic bag.And then,the blood was centrifugated at the speed of 1500 r/min for 30 minutes at 22℃ in order to separate MNC,i.e.,HUCBSC.In addition,after final diagnosis during hospitalization,stroke patients were perfused with HUCBSC through superficial vein of back of the hand.Each patient was averagely penfused with 6 portions of HUCBSC(cellular numbers≥1×108/portion)and the interval between each portion was 1-7 days with the mean interval of 4 days.MAIN OUTCOME MEASURES: ①Neurological function of stroke patients was evaluated with neurological function deficiency(NFD)before treatment and at 3 months after treatment.The scale includes consciousness,level fix function.facial paralysis,language,muscle force of upper limbs,muscle force of lower limb and step function.The total scores ranged from 0 to 45; meanwhile,the lower the scores were,the better the neurological function of limbs was. ②Motor function of injured limbs was evaluated with Fugl-Meyer Assessment (FMA),including motor function of upper limbs.motor function of lower limbs,balance ability,sensory function and motion of joint.The total scores ranged from 0 to 226;meanwhile,the higher the scores were,the better the motor function of limbs was.③Activities of daily living(ADL)was evaluated with Barthel Index(BI),including having meals,taking a bath,dressing oneself,putting on clothes,walking in balance and stair activity.The total scores ranged from 0 to 100;meanwhile,the higher the scores were,the stronger the ADL was.RESULTS:A total of 10 patients were involved in the final analysis.After treatment,NFD of stroke patients was (10.9±5.09)points,which was lower than that before treatment[(25.4±6.09)points,t=8.213,P<0.01].In addition,after treatment,FMA and BI of stroke patients were(80.9±25.00)points and(81.1±15.93)points,re spectively,which were higher than those before treatment[(31.9+21.85)points,(36.2+19.41)points,t=13.024,13.670,P<0.01]. | 9 | 失独者抑郁的心理弹性多因素作用模型显示文摘目的:中国失独家庭已多达百万个,65.6%的失独者存在严重而持久的抑郁情绪,需要及时干预。本研究旨在探索心理弹性与其影响因素的交互作用,比较心理弹性及其影响因素对于失独者抑郁的效应。方法:采用Connor-Davidson心理弹性量表、Zung抑郁自评量表、简易应对方式问卷、艾森克简式量表中国版、社会支持评定量表、一般自我效能感量表对长沙市9个行政区域212名失独者进行入户调查。基于Kumpfer心理弹性框架和心理应激多因素作用过程的理论模式构建假设模型并进行验证。结果:心理弹性的影响因素有:积极应对(总效应值为0.480)、支持利用(总效应值为0.359)、精神质(总效应值为-0.326)、消极应对(总效应值为0.279)、内外倾(总效应值为0.219)、客观支持(总效应值为0.077)。心理弹性对抑郁的影响以直接效应为主(总效应值为-0.344,占67.1%),也通过自我效能感发挥间接效应(总效应值为-0.169),心理弹性的总效应值占所有变量总效应的20.1%。结论:心理弹性受到人格、积极应对、社会支持等应激中介变量的直接和间接作用;心理弹性对抑郁主要产生直接负性效应,是定位于自我效能感之前的重要应激中介变量。个体-环境互动作用过程对心理弹性的影响较大,人格和支持利用通过影响消极应对、积极应对进而间接影响心理弹性。提高心理弹性应当关注个体-环境作用过程,即利用支持、积极应对、适当的消极应对,进而影响个体对自我的积极评价,这将是开发心理弹性干预方案以改善失独者抑郁的重要内容。 | 王安妮 张雯 郭玉芳 Wendy Cross Virginia Plummer Louisa Lam 张静平 | 2021 | 中南大学学报(医学版)2021,46,1: | 7 |

The response of B cells to their specific Ag can be activation and proliferation, leading to the immune response, or anergy and activation-induced cell death (AICD), leading to tolerance. AICD in B lymphocytes is a highly regulated event initiated by crosslinking of the B cell receptor (BCR). BCR engagement initiates several signaling events such as activation of PLCr, Ras, and PI3K, which generally speaking, lead to survival. However, in the absence of survival signals (CD40 or IL-4R engagement), BCR crosslinking can also promote apoptotic signal transduction pathways such as activation of effector caspases, expression of pro-apoptotic genes, and inhibition of pro-survival genes. | 13 | Characteristics of Some Heavy Metals in Acid Sulfate Topsoils, Eastern Australia显示文摘Forty-five acid sulfate topsoil samples (depth < 0.5 m) from 15 soil cores at 11 locations along the New South Wales coast, Australia, were selected to investigate the chemical behavior of Zn, Mn, Cr, Co and Pb in these soils. The amount of HCI-extractable Mn was much smaller than the mean value of the total Mn documented for other soils. This may be attributed to enhanced mobilization of Mn from the soils under the extremely acidic and seasonally flooded conditions encountered in the investigated soils. The pH-dependency of soluble Zn and Mn was strongly affected by the availability of acid reactive Zn and Mn compounds. There were fairly good relationships between soluble Zn and acid reactive Zn compounds, and between soluble Mn and acid reactive Mn compounds. Soluble Zn and soluble Mn concentrations were important controls on exchangeable Zn and Mn concentrations, respectively. In contrast to the suggestion by other authors that adsorption of Co was closely associated with Mn oxides present in soils, the exchangeable Co in the investigated acid sulfate soils was not clearly related to the abundance of Mn minerals. In addition to the fact that there are few Mn minerals present in the soils, this might also be because the availability of canon exchange sites on the crystal surfaces of Mn oxides was reduced under extremely acidic conditions. | C. | 14 | Costimulation of resting B lymphocytes alters the IL-4-activated IRS2 signaling pathway in a STAT6 independent manner: implications for cell survival and proliferation显示文摘IL-4 is an important B cell survival and growth factor. IL-4 induced the tyrosine phosphorylation of IRS2 in resting B lymphocytes and in LPS- or CD40L-activated blasts. Phosphorylated IRS2 coprecipitated with the p85 subunit of PI 3’ kinase in both resting and activated cells. By contrast, association of phosphorylated IRS2 with GRB2 was not detected in resting B cells after IL-4 treatment although both proteins were expressed. However, IL-4 induced association of IRS2 with GRB2 in B cell blasts. The pattern of IL-4- induced recruitment of p85 and GRB2 to IRS2 observed in B cells derived from STAT6 null mice was identical to that observed for normal mice. While IL-4 alone does not induce activation of MEK, a MEKI inhibitor suppressed the IL-4-induced proliferative response of LPS-activated B cell blasts. These results demonstrate that costimulation of splenic B cells alters IL-4-induced signal transduction independent of STAT6 leading to proliferation. | 15 | Characteristics of Phosphorus in Some Eastern Australian Acid Sulfate Soils显示文摘Forty-five acid sulfate topsoil samples (depth < 0.5 m) from 15 soil cores were collected from 11 locations along the New South Wales coast, Australia. There was an overall trend for the concentration of the HC1extractable P to increase along with increasing amounts of organic C and the HCl-extractable trivalent metals in the topsoils of some less-disturbed acid sulfate soils (pH <4.5). This suggests that inorganic P in these soils probably accumulated via biological cycling and was retained by complexation with trivalent metals or their oxides and hydroxides. While there was no clear correlation between pH and the water-extractable P, the concentration of the water-extractable P tended to increase with increasing amounts of the HCl-extractable P. This disagrees with some established models which suggest that the concentration of solution P in acid soils is independent of total P and decreases with increasing acidity. The high concentration of sulfate present in acid sulfate soils appeared to affect the chemical behavior of P in these soil systems. Comparison was made between a less disturbed wetland acid sulfate soil and a more intensively disturbed sugarcane acid sulfate soil.The results show that reclamation of wetland acid sulfate soils for sugarcane production caused a significant decrease in the HCl-extractable P in the topsoil layer as a result of the reduced bio-cycling of phosphorus following sugarcane farming. Simulation experiment shows that addition of hydrated lime had no effects on the immobilization of retained P in an acid sulfate soil sample within a pH range 3.5~4.6. When the pH was raised to above 4.6, soluble P in the soil extracts had a tendency to increase with increasing pH until the 15th extraction (pH 5.13). This, in combination with the poor pH-soluble P relationship observed from the less-disturbed acid sulfate soils, suggests that soluble P was not clearly pH-dependent in acid sulfate soils with pH < 4.5. | C. We conducted a survey of Hg contamination in three species of forage fish, brook silverside (Labidesthes sicculus), threadfin shad (Dorosoma petenense) and gizzard shad (Dorosoma cepedianum), from Caddo Lake, Texas, USA and found species-specific differences in Hg concentrations. We examined total length, age, trophic position (determined using δ15N), and growth rate of forage fish as factors that could have influenced within-and between-species differences in Hg concentration. Further, wind turbines in these locations are often subject to harsh operating conditions. These challenges mean there is a requirement for a high degree of maintenance. The data generated by monitoring systems can be used to obtain models of wind turbines operating under different conditions, and hence predict output signals based on known inputs. A model-based condition monitoring system can be implemented by comparing output data obtained from operational turbines with those predicted by the models, so as to detect changes that could be due to the presence of faults. This paper discusses several techniques for model-based condition monitoring systems: linear models, artificial neural networks, and state dependent parameter 'pseudo' transfer functions.The models are identified using supervisory control and data acquisition(SCADA) data acquired from an operational wind firm. It is found that the multiple-input single-output state dependent parameter method outperforms both multivariate linear and artificial neural network-based approaches. Subsequently, state dependent parameter models are used to develop adaptive thresholds for critical output signals. In order to provide an early warning of a developing fault, it is necessary to interpret the amount by which the threshold is exceeded, together with the period of time over which this occurs. | 19 | Iron Monosulfide Distribution in Three Coastal Floodplain Acid Sulfate Soils, Eastern Australia显示文摘The distribution of iron monosulfide (quantified as acid volatile sulfur: SAV) was compared with geo- chemical properties that are known to affect its formation and accumulation in three coastal Holocene acid sulfate soils (ASS) at Tuckean Swamp, McLeods Creek and Bungawalbyn Swamp respectively. These properties included PH, reactive iron (FeR), pore-water sulfate (SO:42-) and organic carbon (OC). Iron monosulfide was concentrated at the oxic/anoxic boundary. The Tuckean Swamp and McLeods Creek sites are Holocene sediments, whereas the Bungawalbyn Swamp is a Holocene peat. The concentration of SAV averaged 0.2 g kg-l in a 0.5 m thick soil layer at the Tuckean Swamp, but was an order of magnitude lower in the oxic/anoxic transition layers at McLeods Creek and Bungawalbyn Swamp. The SAV mineral greigite (Fe3S4) was identified in the Tuckean Swamp by X-ray diffraction and scanning electron microscopy with quantitative energy dispersive X-ray analysis (SEM-EDX). Very small concentrations of greigite were also observed in the McLeods Creek, based on crystal morphology and elemental composition. The concentration of SAV was a small fraction of the total reduced sulfur, representing at most 3% of the Pyrite sulfur. However, the presence of this highly reactive sulfide mineral, distributed within pores where oxygen diffusion is most rapid, has important implications to the potential rate of acid production from these sediments. | R.
