| 1 | A+C群脑膜炎球菌多糖疫苗安全性评估显示文摘目的 评估A+C群脑膜炎球菌多糖疫苗的安全性。方法 以A+C群脑膜炎球菌多糖疫苗为观察组,伤寒Vi多糖疫苗为对照组,按整群随机分层配对的原则,将研究现场分为108个组群,观察组和对照组各分配54个组。对两组同时建立接种反应监测系统,按统一表格和方法对两组的接种反应进行监测、记录,并进行流行病学调查。结果 两组共接种34543人,其中A+C群脑膜炎球菌多糖疫苗接种18167人,伤寒Vi疫苗接种16376人。A+C群脑膜炎球菌多糖疫苗速发接种反应率为0.44‰,一般接种反应率为0.38%o;伤寒Vi疫苗速发接种反应率为0.79%,一般接种反应率为0.73%o;跟踪随访接种反应对象共1239人,其中接种A+C群脑膜炎球菌多糖疫苗的771人,接种伤寒Vi疫苗的468人。接种后第1天,A+C群脑膜炎球菌多糖疫苗组的局部反应发生率要明显高于伤寒Vi疫苗(X^2。=13.98,P=0.0002);接种后第2—3天局部反应和全身反应的发生率,两组差异无统计学意义;两组各自接种人群和未接种人群发热发生率的差异无统计学意义,两组已接种人群的发热发生率差异也无统计学意义;两组均未发现严重反应。结论 A+C群脑膜炎球菌多糖疫苗与伤寒Vi疫苗均可在不同年龄组人群开展大规模接种,局部及全身接种反应轻微,接种反应率低,有良好的安全性。 | 董柏青 叶强 崔萱林 杨进 龚健 杨明 廖和壮 韦士良 张杰 吴兴华 司国爱 杨宏徽 Ataru Tsuzuki JinKyung Park Mohammad Ali R. | 2 | Lactogenic hormones alter cellular and extracellular microRNA expression in bovine mammary epithelial cell culture显示文摘Background: Bovine milk contains not only a variety of nutritional ingredients but also micro RNAs(mi RNAs) that are thought to be secreted by the bovine mammary epithelial cells(BMECs). The objective of this study was to elucidate the production of milk-related mi RNAs in BMECs under the influence of lactogenic hormones.Results: According to a microarray result of milk exosomal mi RNAs prior to cellular analyses, a total of 257 mi RNAs were detected in a Holstein cow milk. Of these, 18 major mi RNAs of interest in the milk were selected for an expression analysis in BMEC culture that was treated with or without dexamethasone, insulin, and prolactin(DIP) to induce a lactogenic differentiation. Quantitative polymerase chain reaction(q PCR) results showed that the expressions of mi R-21–5p(P = 0.005), mi R-26a(P = 0.016), and mi R-320a(P = 0.011) were lower in the DIP-treated cells than in the untreated cells. In contrast, the expression of mi R-339a(P = 0.017) in the cell culture medium were lower in the DIP-treated culture than in the untreated culture. Intriguingly, the mi R-148 a expression in cell culture medium was elevated by DIP treatment of BMEC culture(P = 0.018). The medium-to-cell expression ratios of mi R-103(P = 0.025), mi R-148a(P < 0.001), and mi R-223(P = 0.013) were elevated in the DIP-treated BMECs, suggesting that the lactogenic differentiation-induced secretion of these three mi RNAs in BMECs. A bioinformatic analysis showed that the mi RNAs down-regulated in the BMECs were associated with the suppression of genes related to transcriptional regulation, protein phosphorylation, and tube development.Conclusion: The results suggest that the mi RNAs changed by lactogenic hormones are associated with milk protein synthesis, and mammary gland development and maturation. | 17 | iStent inject? and cataract surgery for mild-to-moderate primary open angle glaucoma in Japan: a cost-utility analysis显示文摘AIM: To evaluate the cost-utility of iStent inject;with cataract surgery vs cataract surgery alone in patients with mild-to-moderate primary open angle glaucoma(POAG) in the Japanese setting from a public payer’s perspective.METHODS: A Markov model was adapted to estimate the cost-utility of iStent inject;plus cataract surgery vs cataract surgery alone in one eye in patients with mild-tomoderate POAG over lifetime horizon from the perspective of Japanese public payer. Japanese sources were used for patients’ characteristics, clinical data, utility, and costs whenever available. Non-Japanese data were validated by Japanese clinical experts. RESULTS: In the probabilistic base case analysis, iStent inject;with cataract surgery was found to be cost-effective compared with cataract surgery alone over a lifetime horizon when using the ￥5 000 000/quality-adjusted life year(QALY)willingness-to-pay threshold. The incremental cost-utility ratio(ICUR) was estimated to be ￥1 430 647/QALY gained and the incremental cost-utility ratio(ICER) was estimated to be ￥12 845 154/blind eye avoided. iStent inject;with cataract surgery vs cataract surgery alone was found to increase costs(￥1 025 785 vs ￥933 759, respectively) but was more effective in increasing QALYs(12.80 vs 12.74) and avoiding blinded eyes(0.133 vs 0.141). The differences in costs were mainly driven by costs of primary surgery(￥279 903 vs ￥121 349).
