1Persistent alanine aminotransferase elevation among the general Iranian population: Prevalence and causes显示文摘AIM: To determine the prevalence and causes of persistently elevated alanine aminotransferase (ALT) levels among the general population in northern Iran. METHODS: A total of 2292 (1376 female, aged 18-75 year), were selected by systematic clustered random sampling from the cities and villages of Gonbad and Kalaleh in Golestan Province and invited to participate in the study. A comprehensive history regarding alcohol drinking and medication was taken. Body mass index (BMI), viral markers and ALT levels were measured. If ALT level was ≥ 40 U/L, it was rechecked twice within 6 mo. Those with ≥ 2 times elevation of ALT were considered as having persistently elevated ALT level. Non-alcoholic fatty liver disease (NAFLD) was diagnosed based on evidence of fatty liver upon sonography and excluding other etiology. RESULTS: A total of 2049 (1351 female) patients participated in the study, 162 (7.9%) had elevated ALT level at the first measurement. Persistently elevated ALT level was detected in 64 (3.1%) participants, with51 (79.6%) with no obvious etiology, six (9.3%) with Hepatitis B, four (6.2%) with Hepatitis C virus (HCV) infection and three (4.6%) with alcoholic hepatitis. The prevalence of NAFLD and alcoholic hepatitis was 2.04% (42 patients) and 0.1% (three), respectively. There was correlation between NAFLD and male gender, overweight, diabetes and living in an urban area [odds ratio = 3.03 (95% CI: 1.6-5.72), 4.21 (95% CI: 1.83-9.68), 2.86 (95% CI: 1.05-7.79) and 2.04 (95% CI: 1.00-4.16) respectively]. 2Achalasia and thyroid disease显示文摘AIM: To investigate some possible etiologies of achalasia by screening patients with achalasia for some autoimmune diseases such as thyroid disease. METHODS: We examined 30 known cases of achalasia (20 females, 10 males). Their age ranged 15-70 years. All of them were referred to our institute for treatment. Their sera were evaluated to detect some possible associations with rheumatoid disease, thyroid disease, inflammatory process, anemia, etc. RESULTS: Seven out of 30 patients (23%) had thyroid disease including four patients with hypothyroidism (13.3%), two patients with hyperthyroidism (6.6%), and one had only thyroid nodule but was in euthyroid state (3.3%). Two of these hypothyroid patients had no related clinical symptoms (subclinical) and two had clinical manifestations of hypothyroidism. There were no correlations between the intensity of thyroid diseases and the severity of achalasia symptoms. CONCLUSION: The etiology of achalasia is unknown although autoimmunity has been implicated and is supported by several studies. 3Elevated alanine aminotransferase activity is not associated with dyslipidemias,but related to insulin resistance and higher disease grades in non-diabetic non-alcoholic fatty liver disease显示文摘Objective:To explore demographic and metabolic factors associated with increased alanine aminotransferase(ALT)activity in non-diabetic non-alcoholic fatty liver disease(NAFLD)patients.Methods:Overall 372 patients who consecutively attended to Gastroenterology Clinic of Baqiyatallah University of Medical Sciences,Tehran,Iran awere diagnosed as NAFLD entered into analysis.Exclusion criteria were having diabetes mellitus and fasting blood glucose over126 mg/dL,active hepatitis B virus infection,having hepatitis C virus positive serology,and to be under corticosteroid therapy.ALT levels were considered pathologically high when it was over30 IU/L for men and over 19 IU/L for women.Results:Bivariate analyses using t test and chisquare test showed that patients with pathologically augmented ALT levels had significantly higher NAFLD grades in their ultrasonographic evaluations(P=0.003).Moreover,these patients represented significantly higher homeostatic model assessment levels(P=0.003),levels of serum insulin(P=0.002),fasting blood glucose(P<0.001),and uric acid(P=0.02).The prevalence of insulin resistance was also higher in patients with increased serum ALT concentrations.Multifactorial logistic regression models showed that ultrasonographic grading of NAFLD(P=0.027)and insulin resistance(P=0.013)were the only variables significantly associated with abnormal ALT levels.Conclusions:This study shows that the associations of increased ALT serum levels in NAFLD patients are different from what are supposed before.By excluding diabetic patients from our population,we find that increased ALT levels are not associated with dyslipidemias but are independently associated with insulin resistance and NAFLD grading on ultrasonographic evaluations.Further studies are needed to confirm our results.Mohammad Ebrahim Ghamar-Chehreh Mohsen Amini Hossein Khedmat Seyed Moayed Alavian Fatemeh Daraei Reza Mohtashami Reza Hadi Bent-Al-Hoda Beyram Saeed Taheri 2012Asian Pacific Journal of Tropical Biomedicine2012,2,9:5

5溶胶-凝胶新方法合成介孔铁酸铜纳米粉体催化剂上CO低温氧化(英文)显示文摘Mesoporous CuFe2O4 solid solution nanopowders with high specific surface areas were synthesized by a novel, very simple and inexpensive sol-gel route using propylene oxide as gelation agent, and used as the catalyst in low temperature CO oxidation. The samples were characterized by X-ray diffraction, N2 adsorption-desorption, thermogravimetric/differential thermal analysis, Fourier transform infrared spectroscopy, scanning electron microscopy, transmission electron microscopy, and temperature-programmed reduction. The results revealed that the samples have a nanocrystalline structure with crystals in the range of 10 to 25 nm, and that all the catalysts have mesoporous pores. The addition of Cu into iron oxide affected its structural and catalytic properties. 6Analytical study of care quality and moral distress in clinical situations and patient care显示文摘Objective: Nursing profession conventionally meets a high standard of ethical behavior and action. One of the ethical challenges in nursing profession is moral distress. Nurses frequently expose to this phenomenon which leads to different consequences such as being bored by delivering patient care that decline care quality and make it challenging to achieve health purposes. This study was conducted to investigate the association between the aspects of moral distress and care quality.Methods: In this descriptive–analytical study, 545 nurses of intensive and cardiac care units and dialysis and psychiatric wards were recruited by census sampling. Three questionnaires, Sociodemographics, Moral Distress Scale, and Quality Patient Care Scale, were distributed among the participants and collected within 9 months. Data analysis was conducted by descriptive statistics, analysis of variance, and the least significant difference in SPSS 13.Results: Investigating moral distress domains(ignoring patient, decision-making power, and professional competence) and care quality domains(psychosocial, physical, and communicational) demonstrated that in being exposed to moral distress, ignoring patient had no effect on psychosocial domain(P=0.056), but decision-making and professional competence of moral distress had positive effect on psychosocial, physical(bodily), and communication domains of care quality.Conclusions: Because moral distress domains are effective on patient care quality, it is recommended to enhance the knowledge of nurses, especially beginners, about moral distress, increase their strength alongside standardizing nursing services in decisionmaking domains, improve the professional competence, and pay attention to patients.Masoumeh Hasanlo Arezo Azarm Parvaneh Asadi Kourosh Amini Hossein Ebrahimi Mohammad Asghari Jafarabadi 2019Frontiers of Nursing2019,6,4:3

8Soil and Rhizosphere Associated Fungi in Gray Mangroves(Avicennia marina) from the Red Sea—A Metagenomic Approach显示文摘Covering a quarter of the world's tropical coastlines and being one of the most threatened ecosystems, mangroves are among the major sources of terrestrial organic matter to oceans and harbor a wide microbial diversity. In order to protect, restore, and better understand these ecosystems, researchers have extensively studied their microbiology, yet few surveys have focused on their fungal communities. Our lack of knowledge is even more pronounced for specific fungal populations, such as the ones associated with the rhizosphere. Likewise, the Red Sea gray mangroves(Avicennia marina) remain poorly characterized, and understanding of their fungal communities still relies on cultivation-dependent methods. In this study, we analyzed metagenomic datasets from gray mangrove rhizosphere and bulk soil samples collected in the Red Sea coast, to obtain a snapshot of their fungal communities. Our data indicated that Ascomycota was the dominant phylum(76%–85%), while Basidiomycota was less abundant(14%–24%), yet present in higher numbers than usually reported for such environments. Fungal communities were more stable within the rhizosphere than within the bulk soil, both at class and genus level. This finding is consistent with the intrinsic patchiness in soil sediments and with the selection of specific microbial communities by plant roots. Our study indicates the presence of several species on this mycobiome that were not previously reported as mangrove-associated. In particular, we detected representatives of several commercially-used fungi, e.g., producers of secreted cellulases and anaerobic producers of cellulosomes. 9Hydrodynamic mechanisms of aggressive collapse events in leading edge cavitation显示文摘Transient cavities generated from unsteady leading-edge cavitation may undergo aggressive collapses which are responsible for cavitation erosion.In this paper,we studied the hydrodynamic mechanisms of these events in the leading edge cavitation fonned over a modified NACA0009 hydrofoil using experimental and numerical methods.In the experimental investigation,high-speed visualization(HSV)and paint test are employed to study the behavior of the cavitating flow at σ=1.25,α=5°,U∞=20 m/s.In the numerical part,the same cavitating flow is simulated using an inviscid density-based compressible solver with a barotropic cavitation model.The numerical results are first compared with the experimental HSV to show that the simulation is able to reproduce the main features of the cavitating flow.Then,as the compressible solver is capable of capturing the shock wave upon the collapse of cavities,the location of collapse events with high erosion potential are determined.The location of these collapse events are compared with the paint test results with a qualitatively good agreement.It is clearly observed,in both the experiments and the numerical simulation,that there exists four distinct regions along the hydrofoil with higher risks of erosion:(1)A very narrow strip at the leading edge,(2)an area of accumulated collapses at around 60 percent of the sheet cavity maximum length,(3)an area around the closure line of the sheet cavity with the highest erosion damage,and(4)a wide area close to the trailing edge with dispersed collapse events.A combined analysis of the experimental and numerical results reveals that the small-scale structures generated by secondary shedding are more aggressive than the large-scale cloud cavities(primary shedding).It is also observed that the high risk of cavitation erosion in regions 2 and 3 is mainly due to the collapses of the small cavity structures that are formed around the sheet cavity closure line or the rolling cloud cavity.Mohammad Hossein Arabnejad Ali Amini Mohamed Farhat Rickard EBensow 2020Journal of Hydrodynamics2020,32,1:2
10Diagenetic and Depositional Impacts on the Reservoir Quality of the Upper Jurassic Arab Formation in the Balal Oilfield, Offshore Iran显示文摘The Kimmeridgian-Tithonian aged Arab Formation, as the main reservoir of the Jurassic succession in the Balal oilfield, located in the offshore region of the Iranian sector of the Persian Gulf, is investigated in this study. The formation is composed of dolomites and limestones with anhydrite interbeds. Based on detailed petrographic studies, six microfacies are recognized, which are classified in four sub-environments including supratidal, intertidal, lagoonal and the high energy shoal of a homoclinal carbonate ramp. The main diagenetic features of the studied succession include dolomitization, anhydritization, cementation, micritization, fracturing and compaction. Based on stable isotope data, dolomitization of the upper Arab carbonates is related to sabkha settings(i.e. evaporative type). In terms of sequence stratigraphy, three shallowing-upward sequences are recognized, based on core and wireline log data from four wells of the studied field. Considering depositional and diagenetic effects on the reservoir quality, the studied facies are classified into eight reservoir rock types(RRT) with distinct reservoir qualities. Dolomitization has played a major role in reservoir quality enhancement, whereas anhydritization, carbonate cementation, and compaction have damaged the pore throat network. 14Assessment of land suitability and agricultural production sustainability using a combined approach (Fuzzy-AHP-GIS):A case study of Mazandaran province,Iran显示文摘This research was conducted to evaluate the sustainability of rice production using fuzzy logic in Mazandaran province,Iran.The untapped use of modern agricultural technology,such as agricultural machinery,has become a global dilemma,which has forced governments and environmentalists to take a serious stand against the dangers of massive non-standard agricultural practices in the environment.Three types of mechanization,socioeconomic,and economic modeling were used to determine the sustainability and their values were collected through interview method using a 320-item questionnaire from 2016 to 2018.Land evaluation was carried out using soil and climate characteristics,so that about 27.4%of the area was in the perfectly suitable range(S1),35.2%in the appropriate range(S2),19.5%in the critical range(S3),and 17.9%in the inappropriate range(N)for rice cultivation.Farmers were selected from two regions(R1 and R2)by the GIS on two lower levels and more than one hectare(A1,A2)according to agricultural methods(mechanical and traditional).The results showed that the highest and lowest means of the product were 4314.44 and 3501.95 kg ha1,respectively.The sustainability of production was within the range of“sustainability”and“almost sustainable”in terms of mechanistic and economic factors in the fuzzy range of“moderate sustainability”and also with respect to social factors that are in a better position than mechanistic and economic indicators.Results of final sustainability model showed that the average of the lasting sustainability index was in the range of“almost unsustainable”and“moderate sustainability”.Sherwin Amini Abbas Rohani Mohammad Hossein Aghkhani Mohammad Hossein Abbaspour-Fard Mohammad Reza Asgharipour 2020Information Processing in Agriculture2020,7,3:1

17Selecting the most appropriate tractor using Analytic Hierarchy Process–An Iranian case study显示文摘With regard to the importance of selecting an appropriate tractor in rural areas which is one of the main tasks for most farmers,the aim of this study is to select the best tractor in Ghaemshahr and Ahvaz cities of Iran.The methodology of the paper is descriptiveanalytic and data were collected through library and field works(interviews and questionnaires).The statistic population of this study was 25 tractors in Ghaemshahr and Ahvaz,in that 15 tractors were chosen randomly,and the data were analyzed using AHP(analytic hierarchy process).The results showed that the maximum effect regarding the selection of tractor belonged to the maintenance with 49.4%and the minimum went to ergonomic effect with 7%in Ahvaz and the maximum effect was related to price with 29.6%and the minimum went to ergonomic effect with 6.8%.ITM285 and Romania 650 with 83%were the best tractors in Ahvaz and John Deere 6150 with 83%was the best tractor in Ghaemshahr city.Incompatibility ratio for all the comparisons was zero,so the criteria are completely compatible with the aims and options.Sherwin Amini Mohammad Amin Asoodar 2016Information Processing in Agriculture2016,3,4:1
