1Alcoholic hepatitis 2010:A clinician's guide to diagnosis and therapy显示文摘Alcoholic hepatitis(AH)remains a common and life threatening cause of liver failure,especially when it is severe.Although the adjective'acute'is frequently used to describe this form of liver injury,it is usually subacute and has been developing for weeks to months before it becomes clinically apparent.Patients with this form of alcoholic liver disease usually have a history of drinking heavily for many years.While certain aspects of therapy,mainly nutritional support and abstinence are well established,significant debate has surrounded the pharmacologic treatment of AH,and many institutions practice widely varying treatment protocols. In recent years a significant amount of literature has helped focus on the details of treatment,and more data have accumulated regarding risks and benefits of pharmacologic treatment.In particular,the efficacy of pentoxifylline has become increasingly apparent,and when compared with the risks associated with prednisolone,has brought this drug to the forefront of therapy for severe AH.This review will focus on the clinical and laboratory diagnosis and pharmacologic therapies that should be applied during hospitalization and continued into outpatient management.We conclude that the routine use of glucocorticoids for severe AH poses significant risk with equivocal benefit,and that pentoxifylline is a better,safer and cheaper alternative.While the full details of nutritional support lie beyond the scope of this article,nutrition is a cornerstone of therapy and must be addressed in every patient diagnosed with AH. 3On Density-Based Data Streams Clustering Algorithms: A Survey显示文摘Clustering data streams has drawn lots of attention in the last few years due to their ever-growing presence. Data streams put additional challenges on clustering such as limited time and memory and one pass clustering. Furthermore, discovering clusters with arbitrary shapes is very important in data stream applications. Data streams are infinite and evolving over time, and we do not have any knowledge about the number of clusters. In a data stream environment due to various factors, some noise appears occasionally. Density-based method is a remarkable class in clustering data streams, which has the ability to discover arbitrary shape clusters and to detect noise. Furthermore, it does not need the number of clusters in advance. Due to data stream characteristics, the traditional density-based clustering is not applicable. Recently, a lot of density-based clustering algorithms are extended for data streams. The main idea in these algorithms is using densitybased methods in the clustering process and at the same time overcoming the constraints, which are put out by data stream's nature. The purpose of this paper is to shed light on some algorithms in the literature on density-based clustering over data streams. We not only summarize the main density-based clustering algorithms on data streams, discuss their uniqueness and limitations, but also explain how they address the challenges in clustering data streams. Moreover, we investigate the evaluation metrics used in validating cluster quality and measuring algorithms' performance. 4EXPERIMENTAL AND FIELD STUDY ON MINING-PIT MIGRATION显示文摘Rivers are one of the most essential sources of sand and gravel supply for civil works. However, undesirable effects of irregular in-stream mining have been reported on natural sources, environment and infrastructures close to rivers. Therefore, it is necessary to find the effects of mining on rivers in more details. This research concentrates on mining-pit migration phenomenon and its effects on the channel bed. This paper reports an experimental study on the migration of rectangular mining pits and variation of longitudinal profile in the channel bed composed of rather uniform sediments. Different values of widths and lengths were used for pit while pit depths and flow variables were kept constant. The results show that the migration speed changes with the length/width ratio of the pit. The migration speed in convection period is higher than that in diffusion period. In addition, by increasing the length or width, filling rate of pit increases, where the effect of width is more important than the effect of the length. Also is reported in this paper a field study on the changes of three pits excavated at different locations of a river. 5Role of frailty and sarcopenia in predicting outcomes among patients undergoing gastrointestinal surgery显示文摘According to the United States census bureau 20% of Americans will be older than 65 years in 2030 and half of them will need an operation- equating to about 36 million older surgical patients. Older adults are prone to complications during gastrointestinal cancer treatment and therefore may need to undergo special pretreatment assessments that incorporate frailty and sarcopenia assessments. A focused, structured literature review on Pub Med and Google Scholar was performed to identify primary research articles, review articles, as well as practice guidelines on frailty and sarcopenia among patients undergoing gastrointestinal surgery. The initial search identified 450 articles; after eliminating duplicates, reports that did not include surgical patients, case series, as well as case reports, 42 publications on the impact of frailty and/or sarcopenia on outcome of patients undergoing gastrointestinal surgery were included. Frailty is defined as a clinically recognizable state of increased vulnerability to physiologic stressors resulting from aging. Frailty is associated with a decline in physiologic reserve and function across multiple physiologic systems. Sarcopenia is a syndrome characterized by progressive and generalized loss of skeletal muscle mass and strength. Unlike cachexia, which is typically associated with weight loss due to chemotherapy or a general malignancy-related cachexia syndrome, sarcopenia relates to muscle mass rather than simply weight. As such, while weight reflects nutritional status, sarcopenia- the loss of muscle mass- is a more accurate and quantitative global marker of frailty. While chronologic age is an important element in assessing a patient's peri-operative risk, physiologic age is a more important determinant of outcomes. Geriatric assessment tools are important components of the preoperative work-up and can help identify patients who suffer from frailty. 6Heavy metals and their source identification in particulate matter(PM_(2.5)) in Isfahan City, Iran显示文摘The presence of heavy metals(HMs) in particulate matters(PMs) particularly fine particles such as PM_(2.5) poses potential risk to the health of human being. The purpose of this study was to analyze the contents of HMs in PM_(2.5) in the atmospheric monitoring stations in Isfahan city,Iran, in different seasons between March 2014 and March 2015 and their source identification using principle component analysis(PCA). The samples of PM_(2.5) were taken using a high volume sampler in 7 monitoring stations located throughout the city and industrial zones since March 2014 to March 2015. The HMs content of the samples was measured using ICP-MS.The results showed that the concentrations of As, Cd and Ni were in a range of 23–36, 1–12,and 5–76 ng/m^3 at all the stations which exceeded the US-EPA standards. Furthermore,the concentrations of Cr and Cu reached to 153 and 167 ng/m^3 in some stations which were also higher than the standard levels. Depending on the potential sources of HMs, their concentration in PM_(2.5) through the various seasons was different. PCA illustrated that the different potential sources of HMs in the atmosphere, showing that the most important sources of HMs originated from fossil fuel combustion, abrasion of vehicle tires, industrial activities(e.g., iron and steel industries) and dust storms. 7Persistent alanine aminotransferase elevation among the general Iranian population: Prevalence and causes显示文摘AIM: To determine the prevalence and causes of persistently elevated alanine aminotransferase (ALT) levels among the general population in northern Iran. METHODS: A total of 2292 (1376 female, aged 18-75 year), were selected by systematic clustered random sampling from the cities and villages of Gonbad and Kalaleh in Golestan Province and invited to participate in the study. A comprehensive history regarding alcohol drinking and medication was taken. Body mass index (BMI), viral markers and ALT levels were measured. If ALT level was ≥ 40 U/L, it was rechecked twice within 6 mo. Those with ≥ 2 times elevation of ALT were considered as having persistently elevated ALT level. Non-alcoholic fatty liver disease (NAFLD) was diagnosed based on evidence of fatty liver upon sonography and excluding other etiology. RESULTS: A total of 2049 (1351 female) patients participated in the study, 162 (7.9%) had elevated ALT level at the first measurement. Persistently elevated ALT level was detected in 64 (3.1%) participants, with51 (79.6%) with no obvious etiology, six (9.3%) with Hepatitis B, four (6.2%) with Hepatitis C virus (HCV) infection and three (4.6%) with alcoholic hepatitis. The prevalence of NAFLD and alcoholic hepatitis was 2.04% (42 patients) and 0.1% (three), respectively. There was correlation between NAFLD and male gender, overweight, diabetes and living in an urban area [odds ratio = 3.03 (95% CI: 1.6-5.72), 4.21 (95% CI: 1.83-9.68), 2.86 (95% CI: 1.05-7.79) and 2.04 (95% CI: 1.00-4.16) respectively]. 9Radiation-induced inflammation and autoimmune diseases显示文摘Currently,ionizing radiation(IR)plays a key role in the agricultural and medical industry,while accidental exposure resulting from leakage of radioactive sources or radiological terrorism is a serious concern.Exposure to IR has various detrimental effects on normal tissues.Although an increased risk of carcinogenesis is the best-known long-term consequence of IR,evidence has shown that other diseases,particularly diseases related to inflammation,are common disorders among irradiated people.Autoimmune disorders are among the various types of immune diseases that have been investigated among exposed people.Thyroid diseases and diabetes are two autoimmune diseases potentially induced by IR.However,the precise mechanisms of IR-induced thyroid diseases and diabetes remain to be elucidated,and several studies have shown that chronic increased levels of inflammatory cytokines after exposure play a pivotal role.Thus,cytokines,including interleukin-1(IL-1),tumor necrosis factor(TNF-α)and interferon gamma(IFN-α),play a key role in chronic oxidative damage following exposure to IR.Additionally,these cytokines change the secretion of insulin and thyroid-stimulating hormone(TSH).It is likely that the management of inflammation and oxidative damage is one of the best strategies for the amelioration of these diseases after a radiological or nuclear disaster.In the present study,we reviewed the evidence of radiation-induced diabetes and thyroid diseases,as well as the potential roles of inflammatory responses.In addition,we proposed that the mitigation of inflammatory and oxidative damage markers after exposure to IR may reduce the incidence of these diseases among individuals exposed to radiation.Rasoul Yahyapour Peyman Amini Saeed Rezapour Mohsen Cheki Abolhasan Rezaeyan Bagher Farhood Dheyauldeen Shabeeb Ahmed Eleojo Musa Hengameh Fallah Masoud Najafi 2018Military Medical Research2018,5,1:7
10多壁碳管/二氧化钛改性棉织物及其自清洁和抗紫外性能(英文)显示文摘以丁二酸为交联剂,采用纳米TiO2和多壁碳管对棉织物进行改性。通过暴露于紫外和阳光照射,分析Direct Green6浓度的减少来评价改性织物的光催化自洁性能。探讨样品的紫外吸收、耐磨性与扩张强度。结果表明,在紫外和阳光照射下,经纳米TiO2和多壁碳管处理后的棉织物能有效提高Direct Green 6的光降解效率。改性织物的耐麿性和抗紫外性能提高,抗张强度略降低。Direct Green 6光降解效率的提高归因于电子空穴对重组的减少及由TiO2和多壁碳管产生协同作用而使吸收光的增加。Loghman Karimi Salar Zohood Atefeh Amini 2014新型炭材料2014,29,5:7
11Weathering and soils formation on different parent materials in Golestan Province,Northern Iran显示文摘Geochemical,mineralogical,and micromorphological characteristics of soils and their relevant parent rocks including loess,ignimbrite,sandstone and limestone were investigated to identify the soil-parent material uniformity and the weathering degree of soils in Golestan Province,northern Iran.Highly developed Calcixerolls and moderately developed Haploxerepts were formed on loess and limestone,respectively.In contrast,the soils formed on ignimbrite and sandstone were nondeveloped Entisols.Illite was the dominant clay mineral found in ignimbrite and sandstone in both the A horizon and parent material.In loess derived soils however,smectite was dominant especially in the Bt horizon compared to its parent material indicating partly to its pedogenic formation.In limestone,illite and vermiculite were dominant both in the A and C horizons.Ti/Zr ratio proved that the studied soils were closely related to their underlying parent materials geochemically.Chemical index of alteration(CIA),micromorphological index of soil development(MISECA),smectite/illite+chlorite ratio and magnetic susceptibility were applied to investigate the degree of soil development.Results showed that the most and the least developed soils were those formed on loess deposits and limestone,respectively.Application of the different geochemical and pedogenetic approaches was proved to be useful in identifying the relevance of soils to their underlying parent materials and also their degree of development.Maryam MAHMOODI Farhad KI-IORMALI Arash AMINI Shamsollah AYOUBI 2016Journal of Mountain Science2016,13,5:6

13Achalasia and thyroid disease显示文摘AIM: To investigate some possible etiologies of achalasia by screening patients with achalasia for some autoimmune diseases such as thyroid disease. METHODS: We examined 30 known cases of achalasia (20 females, 10 males). Their age ranged 15-70 years. All of them were referred to our institute for treatment. Their sera were evaluated to detect some possible associations with rheumatoid disease, thyroid disease, inflammatory process, anemia, etc. RESULTS: Seven out of 30 patients (23%) had thyroid disease including four patients with hypothyroidism (13.3%), two patients with hyperthyroidism (6.6%), and one had only thyroid nodule but was in euthyroid state (3.3%). Two of these hypothyroid patients had no related clinical symptoms (subclinical) and two had clinical manifestations of hypothyroidism. There were no correlations between the intensity of thyroid diseases and the severity of achalasia symptoms. CONCLUSION: The etiology of achalasia is unknown although autoimmunity has been implicated and is supported by several studies. 14Triaxial shear behavior of a cement-treated sand——gravel mixture显示文摘A number of parameters,e.g.cement content,cement type,relative density,and grain size distribution,can influence the mechanical behaviors of cemented soils.In the present study,a series of conventional triaxial compression tests were conducted on a cemented poorly graded sandegravel mixture containing 30% gravel and 70% sand in both consolidated drained and undrained conditions.Portland cement used as the cementing agent was added to the soil at 0%,1%,2%,and 3%(dry weight) of sandegravel mixture.Samples were prepared at 70% relative density and tested at confining pressures of 50 kPa,100 kPa,and150 kPa.Comparison of the results with other studies on well graded gravely sands indicated more dilation or negative pore pressure in poorly graded samples.Undrained failure envelopes determined using zero Skempton's pore pressure coefficient (= 0) criterion were consistent with the drained ones.Energy absorption potential was higher in drained condition than undrained condition,suggesting that more energy was required to induce deformation in cemented soil under drained state.Energy absorption increased with increase in cement content under both drained and undrained conditions.Younes Amini Amir Hamidi 2014Journal of Rock Mechanics and Geotechnical Engineering2014,6,5:5
15Influence of welding speed on corrosion behaviour of friction stir welded AA5086 aluminium alloy显示文摘The plates of AA5086 aluminium alloy were joined together by friction stir welding at a fixed rotation speed of 1000 r/min various welding speeds ranging from 63 to 100 mm/min.Corrosion behavior of the parent alloy(PA),the heat affected zone(HAZ),and the weld nugget zone(WNZ)of the joints were studied in 3.5%(mass fraction)aerated aqueous Na Cl solution by potentiodynamic polarization and electrochemical impedance spectroscopy(EIS).The corrosion susceptibility of the weldments increases when the welding speed increases to 63 and 100 mm/min.However,the value of corrosion rate in the weldments is lower than that in the PA.Additionally,the corrosion current density increases with increasing the welding speed in the HAZ and the WNZ.On the contrary,the corrosion potential in the WNZ appears more positive than in the HAZ with decreasing the welding speed.The WNZ exhibits higher resistance compared to the HAZ and the PA as the welding speed decreases.The results obtained from the EIS measurements suggest that the weld regions have higher corrosion resistance than the parent alloy.With increasing the welding speed,the distribution and extent of the corroded areas in the WNZ region are lower than those of the HAZ region.In the HAZ region,in addition to the pits in the corroded area,some cracks can be seen around the corroded areas,which confirms that intergranular corrosion is formed in this area.The alkaline localized corrosion and the pitting corrosion are the main corrosion mechanisms in the corroded areas within the weld regions.Crystallographic pits are observed within the weld regions.Kamran Amini Farhad Gharavi 2016Journal of Central South University2016,23,6:5

17Elevated alanine aminotransferase activity is not associated with dyslipidemias,but related to insulin resistance and higher disease grades in non-diabetic non-alcoholic fatty liver disease显示文摘Objective:To explore demographic and metabolic factors associated with increased alanine aminotransferase(ALT)activity in non-diabetic non-alcoholic fatty liver disease(NAFLD)patients.Methods:Overall 372 patients who consecutively attended to Gastroenterology Clinic of Baqiyatallah University of Medical Sciences,Tehran,Iran awere diagnosed as NAFLD entered into analysis.Exclusion criteria were having diabetes mellitus and fasting blood glucose over126 mg/dL,active hepatitis B virus infection,having hepatitis C virus positive serology,and to be under corticosteroid therapy.ALT levels were considered pathologically high when it was over30 IU/L for men and over 19 IU/L for women.Results:Bivariate analyses using t test and chisquare test showed that patients with pathologically augmented ALT levels had significantly higher NAFLD grades in their ultrasonographic evaluations(P=0.003).Moreover,these patients represented significantly higher homeostatic model assessment levels(P=0.003),levels of serum insulin(P=0.002),fasting blood glucose(P<0.001),and uric acid(P=0.02).The prevalence of insulin resistance was also higher in patients with increased serum ALT concentrations.Multifactorial logistic regression models showed that ultrasonographic grading of NAFLD(P=0.027)and insulin resistance(P=0.013)were the only variables significantly associated with abnormal ALT levels.Conclusions:This study shows that the associations of increased ALT serum levels in NAFLD patients are different from what are supposed before.By excluding diabetic patients from our population,we find that increased ALT levels are not associated with dyslipidemias but are independently associated with insulin resistance and NAFLD grading on ultrasonographic evaluations.Further studies are needed to confirm our results.Mohammad Ebrahim Ghamar-Chehreh Mohsen Amini Hossein Khedmat Seyed Moayed Alavian Fatemeh Daraei Reza Mohtashami Reza Hadi Bent-Al-Hoda Beyram Saeed Taheri 2012Asian Pacific Journal of Tropical Biomedicine2012,2,9:5

19A two-dimensional limit equilibrium computer code for analysis of complex toppling slope failures显示文摘Evaluation of blocky or layered rock slopes against toppling failures has remained of great concern for engineers in various rock mechanics projects.Several step-by-step analytical solutions have been developed for analyzing these types of slope failures.However,manual application of these analytical solutions for real case studies can be time-consuming,complicated,and in certain cases even impossible.This study will first examine existing methods for toppling failure analyses that are reviewed,modified and generalized to consider the effects of a wide range of external and dead loads on slope stability.Next,based on the generalized presented formulae,a Windows form computer code is programmed using Visual C#for analysis of common types of toppling failures.Input parameters,including slope geometry,joint sets parameters,rock and soil properties,ground water level,dynamic loads,support anchor loads as well as magnitudes and forms of external forces,are first loaded into the code.The input data are then saved and used to graphically draw the slope model.This is followed by automatic identification of the toppling failure mode and a deterministic analysis of the slope stability against this failure mode.The results are presented using a graphical approach.The developed code allows probabilistic introduction of the input parameters via probability distribution functions(PDFs)and thus a probabilistic analysis of the toppling failure modes using Monte-Carlo simulation technique.This allows calculation of the probability of slope failure.Finally,several published case studies and typical examples are analyzed with the developed code.The outcomes are compared with those of the main references to assess the performance and robustness of the developed computer code.The comparisons demonstrate good agreement between the results.Akbar Ardestani Mehdi Amini Kamran Esmaeili 2021Journal of Rock Mechanics and Geotechnical Engineering2021,13,1:4
20微波辅助燃烧法制备镁铝尖晶石纳米粉体的研究(英文)显示文摘Stoichiometric MgAl2O4 spinel nanoparticles were synthesized by microwave assisted combustion reaction from aluminium nitrate nanohydrate(Al(NO3)3.9H2O) and Sol-Gel prepared magnesium hydroxide(Mg(OH)2) in the presence of urea((NH2)2CO) as a fuel,in about 20 min of irradiation.X-ray diffraction(XRD) studies reveal that mi-crowave assisted combustion synthesis route yields single-phase spinel nanoparticles with larger crystalline size(around 75 nm) than other conventional heating methods.Scanning electronic microscope(SEM) images show nanoparticles with spherical shape and homogenous morphology.The surface area measurements(SBET) show crystals with 2.11 m2/g and 0.0033 mL/g pore volume.Leila Torkian Mostafa M Amini Zohreh Bahrami 2011无机材料学报2011,26,5:4
