| 1 | Colonoscopic evaluation of minimal rectal bleeding in average-risk patients for colorectal cancer显示文摘AIM: To assess the prevalence of clinically signifi cant lesions in patients with minimal bright red bleeding per rectum (BRBPR). METHODS: Consecutive outpatients prospectively underwent colonoscopy at Loghman Hakim Hospital, Tehran. Minimal BRBPR was defi ned as small amounts of red blood after wiping or in the toilet bowl. Patients with the following alarm signs were excluded: Positive personal history of colorectal neoplasms or inflamma-tory bowel disease (IBD), positive fi rst degree family history of colorectal neoplasms, history of altered bow- el habits, recent signifi cant weight loss, and presence of iron defi ciency anemia. Neoplastic polyps, colorectal carcinoma, and IBD were def ined as signif icant lesions. RESULTS: A total of 402 patients (183 female and 219 male, aged 43.6 ± 15.7 years) were studied. Hemorrhoids (54.2%), anal fi ssures (14.2%) and ul-cerative colitis (14.2%) were the most common lesions and colonoscopy was normal in 8.0%. Signifi cant le-sions were found in 121 (30.1%) patients, including 26 patients (6.5%) with adenocarcinoma and 30 (7.5%) with adenomatous polyps. Almost all patients with signifi cant lesions had at least one lesion in the distal colon; an adenocarcinoma and an adenomatous polyp in the proximal colon were found in 2 patients with hemorrhoids. CONCLUSION: Flexible sigmoidoscopy appears to be sufficient for the evaluation of average risk patients with minimal BRBPR. Rigid sigmoidoscopy may be used as an alternative in patients less than 40 years of age in settings where the former is not available. | 2 | Healthy ranges of serum alanine aminotransferase levels in Iranian blood donors显示文摘AIM: The healthy ranges for serum alanine aminotransferase(ALT) levels are less well studied. The aim of this study was to define the upper limit of normal (ULN) for serum ALT levels, and to assess factors associated with serum ALT activity in apparently healthy blood donors. METHODS: A total of 1 939 blood donors were included.ALT measurements were performed for all cases using the same laboratory method. Healthy ranges for ALT levels werecomputed from the population at the lowest risk for liver disease. Univariate and multivariate analyses were performed to evaluate associations between clinical factors and ALT levels.RESULTS: Serum ALT activity was independently associated with body mass index (BMI) and male gender, but not associated with age. Association of ALT with BMI was more prominent in males than in females. Upper limit of normal for non-overweight women (BMI of less than 25) was 34 U/L,and for non-overweight men was 40 U/L.CONCLUSION: Serum ALT is strongly associated with sex and BMI. | 4 | Iran supports a great share of biodiversity and floristic endemism for Fritillaria spp.(Liliaceae):A review显示文摘Iran supports a great share of exotic and/or endemic plant genera and species. The genus Fritillaria(Liliaceae) is a precious part of this botanical richness with 19 species, of which 10 are endemic to the country. However, signs are mounting that the country is truly at a crossroads when it comes to preservation of this national wealth. In this regard, an effective conservation strategy should thoroughly consider the classification of Fritillaria, as conservation practices are compromised by knowledge gaps in systematics and taxonomy. | 9 | Analysis, Implementation, and Application of Acoustic and Seismic Arrays显示文摘In this paper, we consider the analysis, implementation, and application of wideband sources using both seismic and acoustic sensors. We use the approximate maximum likelihood (AML) algorithm to perform acoustic direction of arrival (DOA). For non-uniform noise spectra, whitening filtering was applied to the received acoustic signals before the AML operation. For short-range seismic DOA applications, one method was based on eigen-decomposition of the covariance matrix and a second method was based on surface wave analysis. Two well-known optimization schemes were used to estimate the source locations from the estimated DOAs at sensors of known locations. | 18 | Corneal biomechanics after rigid gas permeable contact lens wear in keratoconus eyes显示文摘Background:Evaluation of corneal biomechanical properties 3 months after using rigid gas permeable(RGP)contact lenses in keratoconus.Methods:In this prospective trial study,cases were 32 keratoconic eyes with no history of RGP lens wear.All eyes were examined with the Ocular Response Analyzer(ORA)and the Corneal Visualization Scheimpflug Technology(CORVIS-ST)to measure corneal hysteresis(CH),corneal resistance factor(CRF),deformation amplitude(DA),applanation velocity(AV)1 and 2,applanation length(AL)1 and 2,and peak distance before and 3 months after fitting aspheric RGP lenses.The effect of the correlation between contralateral eyes and maximum keratometry were controlled for in the analysis.Results were compared using repeated measures analysis of covariance.Results:At 3 months,neither the increases in mean CH(0.14±2.77 mm Hg,P=0.789),CRF(0.41±4.35 mm Hg,P=0.612),AV1(0.03±0.17 m/s,P=0.301),AV2(0.11±0.59 m/s,P=0.299),AL1(0.44±1.56 m/s,P=0.118),AL2(1.16±5.06 m/s,P=0.211),and peak distance(0.19±1.29 m/s,P=0.409),nor the decrease in mean DA(0.03±0.17 mm,P=0.402)was statistically significant.Conclusions:Results in our series of patients indicated that 3 months of RGP lens wear had no significant impact on corneal biomechanics,and perhaps non progression of keratoconus.Therefore,RGP lenses can be regarded safe and appropriate in keratoconic patients. | Fereshteh Shokrollahzadeh Hassan Hashemi Ebrahim Jafarzadehpur Ali Mirzajani Mehdi Khabazkhoob Soheila Asgari | 2016 | Eye Science2016,31,2: | 0 |
