| 7 | Is it time to replace propranolol with carvedilol for portal hypertension?显示文摘Beta-adrenergic receptor antagonists(β-blockers) have been well established for use in portal hypertension for more than three decades. Different Non-selective β-blockers like propranolol, nadolol, timolol, atenolol, metoprolol and carvedilol have been in clinical practice in patients with cirrhosis. Carvedilol has proven 2-4 times more potent than propranolol as a beta-receptor blocker in trials conducted testing its efficacy for heart failure. Whether the same effect extends to its potency in the reduction of portal venous pressures is a topic of on-going debate. The aim of this review is to compare the hemodynamic and clinical effects of carvedilol with propranolol, and attempt assess whether carvedilol can be used instead of propranolol in patients with cirrhosis. Carvedilol is a promising agent among the beta blockers of recent time that has shown significant effects in portal hypertension hemodynamics. It has also demonstrated an effective profile in its clinical application specifically for the prevention of variceal bleeding. Carvedilol has more potent desired physiological effects when compared to Propranolol. However, it is uncertain at the present juncture whether the improvement in hemodynamics also translates into a decreased rate of disease progression and complications when compared to propranolol. | 9 | Health risks associated with heavy metals in the drinking water of Swat,northern Pakistan显示文摘The concentrations of heavy metals such as Cd, Cr, Cu, Mn, Ni, Pb and Zn were investigated in drinking water sources(surface and groundwater) collected from Swat valley, Khyber Pakhtunkhwa, Pakistan. The potential health risks of heavy metals to the local population and their possible source apportionment were also studied. Heavy metal concentrations were analysed using atomic absorption spectrometer and compared with permissible limits set by Pakistan Environmental Protection Agency and World Health Organization. The concentrations of Cd, Cr, Ni and Pb were higher than their respective permissible limits, while Cu, Mn and Zn concentrations were observed within their respective limits. Health risk indicators such as chronic daily intake(CDI) and health risk index(HRI) were calculated for adults and children separately. CDIs and HRIs of heavy metals were found in the order of Cr > Mn >Ni > Zn > Cd > Cu > Pb and Cd > Ni > Mn > Cr > Cu > Pb > Zn, respectively. HRIs of selected heavy metals in the drinking water were less than 1, indicating no health risk to the local people. | 10 | Scour and flow field around a spur dike in a 90° bend显示文摘Spur dike is an important element in river training that creates rapid variations in flow field, sediment transport and bed topography. The mechanism of flow and sediment transport in a channel bend is very complex, especially when a spur dike is constructed in a bend. Most of previous investigations on flow behavior and scour around spur dike were carried out in straight channels. In this paper results of experiments on flow field and scour around a spur dike in a 90 degree channel bend are presented. Sand with uniform grain size was used as the bed material. Experiments were conducted for different locations and different lengths of spur dikes at the bend with different values of discharge. The three dimensional flow fields around a spur dike were investigated. The present review describes the evidencebased consensus on the diagnosis, prevention and management of HCV disease. Various techniques, for the detection of anti-HCV immunoglobulin G immunoassays, detection of HCV RNA by identifying virus-specific molecules nucleic acid testings, recognition of core antigen for diagnosis of HCV, quantitative antigenassay, have been used to detect HCV RNA and core antigen. Advanced technologies such as nanoparticlebased diagnostic assays, loop-mediated isothermal amplification and aptamers and Ortho trak-C assay have also come to the front that provides best detection results with greater ease and specificity for detection of HCV. It is of immense importance to prevent this infection especially among the sexual partners, injecting drug users, mother-to-infant transmission of HCV, household contact, healthcare workers and people who get tattoos and piercing on their skin. Management of this infection is intended to eradicate it out of the body of patients. Management includes examining the treatment(efficacy and protection), assessment of hepatic condition before commencing therapy, controlling the parameters upon which dual and triple therapies work, monitoring the body after treatment and adjusting the co-factors. However, undesirable effects of irregular in-stream mining have been reported on natural sources, environment and infrastructures close to rivers. Therefore, it is necessary to find the effects of mining on rivers in more details. This research concentrates on mining-pit migration phenomenon and its effects on the channel bed. This paper reports an experimental study on the migration of rectangular mining pits and variation of longitudinal profile in the channel bed composed of rather uniform sediments. Different values of widths and lengths were used for pit while pit depths and flow variables were kept constant. The results show that the migration speed changes with the length/width ratio of the pit. The migration speed in convection period is higher than that in diffusion period. In addition, by increasing the length or width, filling rate of pit increases, where the effect of width is more important than the effect of the length. Also is reported in this paper a field study on the changes of three pits excavated at different locations of a river. | 15 | 社区吸烟者心理特质及其对尼古丁依赖程度的影响研究显示文摘背景心理社会因素是影响尼古丁依赖的重要因素,关于吸烟原因和心理社会因素联合影响尼古丁依赖程度的研究较少见。目的调查北京市社区吸烟人群心理健康状况,并探讨其对尼古丁依赖程度的影响。方法2018-2019年,采用横断面研究方法,通过现场发放问卷调查北京市12家社区的吸烟者407人,纳入戒烟干预研究队列,采用自编社会人口学问卷收集吸烟者的人口学信息,采用尼古丁依赖量表评估吸烟者尼古丁依赖程度,采用拒烟自我效能问卷、Russell吸烟原因问卷、领悟社会支持量表及特质应对方式问卷进行吸烟者心理特质的调查。探讨人口学信息和心理特质得分与尼古丁依赖量表得分的相关关系;以尼古丁依赖程度为因变量,采用多重线性回归分析探讨影响尼古丁依赖程度的心理特质指标。结果本调查共发放问卷407份,回收有效问卷309份,有效问卷回收率为75.9%。不同文化程度、平均月收入吸烟者尼古丁依赖程度比较,差异有统计学意义(P<0.05)。不同尼古丁依赖程度吸烟者拒烟自我效能问卷总分及积极/社会情景、消极/情绪情景、习惯/成瘾情景维度得分,Russell吸烟原因问卷中心理意向、手口活动、享乐、镇静、刺激、成瘾及自动因子得分比较,差异有统计学意义(P<0.05)。Spearman秩相关分析结果显示,文化程度(rs=-0.224)、拒烟自我效能问卷总分(rs=-0.398)及积极/社会情景维度(rs=-0.292)、消极/情绪情景维度(rs=-0.226)、习惯/成瘾情景维度(rs=-0.465)得分与尼古丁依赖程度呈负相关(P<0.05),Russell吸烟原因问卷中的心理意向(rs=0.120)、手口活动(rs=0.240)、享乐(rs=0.354,P<0.05)、镇静(rs=0.239)、刺激(rs=0.227)、成瘾(rs=0.426)和自动(rs=0.298)因子得分与尼古丁依赖程度呈正相关(P<0.05)。多重线性回归分析结果显示,文化程度(t=-3.151)、习惯/成瘾情景维度得分(t=-5.401)、享乐因子得分(t=2.736)和成瘾因子得分(t=3.111)是吸烟者尼古丁依赖程度的影响因素(P<0.05)。结论影响吸烟者尼古丁依赖程度的因素包括文化程度、习惯成瘾情境下的拒烟自我效能和吸烟原因中的享乐因素以及成瘾因素,应从社会心理层面与成瘾方面入手进行吸烟者的戒烟干预。 | 马含俏 刘涵 金倩莹 乔昆 Ali Akbar 张曼华 李星明 | 2020 | 中国全科医学2020,23,15: | 9 |

| 17 | Prevalence and antibiogram study of Salmonella and Staphylococcus aureus in poultry meat显示文摘Objective:To evaluate the presence and antibiogram pattern of Salmonella and Staphylococcus aureus(S.aureus)in retail poultry meat products.Methods:Foodborne pathogens(Salmonella and S.aureus)were isolated from poultry meat and confirmed with the help of biochemical and immunological test.Antibiogram of the isolates were examined by following CLS1 methods.Results:A total number of 209 poultry meat samples were collected and studied in this study.Out of which,5.26%were found contaminated with Salmonella while 18.18%were found contaminated with S.aureus.All the Salmonella and S.aureus isolates were found resistant to at least one antibiotic.About 72.72%of the Salmonella isolates showed resistance to tetracycline,while S.aureus isolates were also found highly resistant to tetracycline equal to 44.73%.One of the Salmonella isolates showed multi-drug resistance to almost six antibiotics out of nine antibiotics used in the study.Multidrug resistant S.aureus isolates were also found in the study.Conclusions:The study confirmed the presence of Salmonella and S.aureus in retail poultry meat.It is a potential threat to consumer health.To reduce the risk of contamination,good hygiene practices are necessary from processing to storage. | Ali Akbar Anil Kumar Anal | 2013 | Asian Pacific Journal of Tropical Biomedicine2013,3,2: | 8 |
| 18 | Discernment scheme for paraquat poisoning: A five-year experience in Shiraz, Iran显示文摘AIM To evaluate various schemes for paraquat poisoning and different variables that influence the outcome of acute paraquat poisoning.METHODS In a cross-sectional study, the information about all cases of acute paraquat poisoning who were admitted to teaching hospitals affiliated to Shiraz University of Medical Sciences, in a five year period(September 2010 to September 2015) were evaluated. The variables included: Demographic data, medical assessment, therapeutic options, laboratory findings, and the outcomes. Data were analyzed using SPSS, version 22. Significant difference between groups was tested using t-test for continues outcomes and χ~2 test for categorical. The significance level was considered to be P < 0.05. RESULTS A total of 104 patients(66.3% male) were evaluated. The mean age of the female patients was 22.81 ± 9.87 years and the male patients' was 27.21 ± 11.06 years. Ninety seven(93.3%) of all the cases were suicide attempts with mortality rate of 43.2%. Despite the necessity for emergency hemodialysis during the first 6 h of intoxication, none of the patients had dialysis during this time. Immunosuppressive and corticosteroid medications were not administrated in adequate dosage in 31.1% and 60% of the patients, respectively. | 19 | Application of artificial neural networks in optimal tuning of tuned mass dampers implemented in high-rise buildings subjected to wind load显示文摘High-rise buildings are usually considered as flexible structures with low inherent damping. Therefore, these kinds of buildings are susceptible to wind-induced vibration. Tuned Mass Damper(TMD) can be used as an effective device to mitigate excessive vibrations. In this study, Artificial Neural Networks is used to find optimal mechanical properties of TMD for high-rise buildings subjected to wind load. The patterns obtained from structural analysis of different multi degree of freedom(MDF) systems are used for training neural networks. In order to obtain these patterns, structural models of some systems with 10 to 80 degrees-of-freedoms are built in MATLAB/SIMULINK program. Finally, the optimal properties of TMD are determined based on the objective of maximum displacement response reduction. The Auto-Regressive model is used to simulate the wind load. In this way, the uncertainties related to wind loading can be taken into account in neural network's outputs. After training the neural network, it becomes possible to set the frequency and TMD mass ratio as inputs and get the optimal TMD frequency and damping ratio as outputs. | 20 | Corrosion and Wear Resistance Characterization of Environmentally Friendly Sol-gel Hybrid Nanocomposite Coating on AA5083显示文摘Environmentally friendly organic-inorganic hybrid nanocomposite films have been developed by sol-gel method for corrosion protection of AA5083 alloy.The hybrid nanocomposite coatings have been synthesized from tetraethylorthosilicate(TEOS) and 3-glycidoxypropyltrimethoxysilane(GPTMS) precursors.The multilayer coatings were prepared by dip-coating technique.Attenuated total reflectance-Fourier transform infrared (ATR-FTIR) spectroscopy was carried out to show the formation of the Si-O-Si structural backbone of the hybrid coatings.Structure and surface morphology of the coatings were studied by optical microscopy(OM), scanning electron microscopy(SEM) and atomic force microscopy(AFM).Characterization of the coatings with respect to pencil scratch hardness,adhesive and abrasion resistance was performed.The corrosion protection performance of these coatings was examined by using cyclic potentiodynamic polarization technique in Persian Gulf water.The results revealed that crack-free films with smooth surface were obtained.
