| 1 | Highly efficient GaAs solar cells by limiting light emission angle显示文摘In a conventional flat plate solar cell under direct sunlight,light is received from the solar disk,but is re-emitted isotropically.This isotropic emission corresponds to a significant entropy increase in the solar cell,with a corresponding drop in efficiency.Here,using a detailed balance model,we show that limiting the emission angle of a high-quality GaAs solar cell is a feasible route to achieving power conversion efficiencies above 38%with a single junction.The highest efficiencies are predicted for a thin,light trapping cell with an ideal back reflector,though the scheme is robust to a non-ideal back reflector.Comparison with a conventional planar cell geometry illustrates that limiting emission angle in a light trapping geometry not only allows for much thinner cells,but also for significantly higher overall efficiencies with an excellent rear reflector.Finally,we present ray-tracing and detailed balance analysis of two angular coupler designs,show that significant efficiency improvements are possible with these couplers,and demonstrate initial fabrication of one coupler design. | Emily D Kosten Jackson H Atwater James Parsons Albert Polman Harry A Atwater | 2013 | Light(Science & Applications)2013,2,1: | 9 |
| 2 | Laparoscopic sleeve gastrectomy:More than a restrictive bariatric surgery procedure?显示文摘Sleeve gastrectomy(SG) is a restrictive bariatric surgery technique that was first used as part of restrictive horizontal gastrectomy in the original Scopinaro type biliopancreatic diversion. Its good results as a single technique have led to a rise in its use, and it is currently the second most performed technique worldwide. SG achieves clearly better results than other restrictive techniques and is comparable in some aspects to the Roux-en-Y gastric bypass, the current gold standard in bariatric surgery. These benefits have been associated with different pathophysiologic mechanisms unrelated to weight loss such as increased gastric emptying and intestinal transit, and activation of hormonal mechanisms such as increased GLP-1 hormone and decreased ghrelin. | 3 | Gastric cancer in Alaska Native people: A cancer health disparity显示文摘AIM To evaluate recent trends in gastric cancer incidence, response to treatment, and overall survival among Alaska Native(AN) people. METHODS A retrospective analysis of the Alaska Native Medical Center patient database was performed. Patient history, clinical, pathological, response to treatment and patient outcomes were collected from one-hundred and thirty-two AN gastric cancer patients. The Surveillance, Epidemiology and End Result database 18 was used to collect comparison United States non-Hispanic White(NHW) and AN gastric cancer patient data between 2006-2014.RESULTS AN gastric cancer patients have a higher incidence rate, a poorer overall survival, and are diagnosed at a significantly younger age compared to NHW patients. AN patients differ from NHW patients in greater prevalence of non-cardia, diffuse subtype, and signet ring cell carcinomas. AN females were more likely to be diagnosed with later stage cancer, stage IV, compared to AN males. Diminished overall survival was observed among AN patients with increasing stage, O+ blood type, < 15 lymph nodes examined at resection, and no treatment. This study is the first report detailing the clinicopathologic features of gastric cancer in AN people with outcome data.CONCLUSION Our findings confirm the importance of early detection, treatment, and surgical resection for optimizing AN patient outcomes. | 5 | Acute pancreatitis in aging animals:Loss of pancreatitis-associated protein protection?显示文摘AIM:To investigate the effect of age on severity of acute pancreatitis(AP) using biochemical markers,histology and expression of the protective pancreatitisassociated proteins(PAPs).METHODS:AP was induced via intraductal injection of 4% sodium taurocholate in young and old rats.Sera and pancreata were assayed at 24 h for the parameters listed above;we also employed a novel molecular technique to assess bacterial infiltration using polymerase chain reaction to measure bacterial genomic ribosomal RNA.RESULTS:At 24 h after induction of AP,the pancreata of older animals had less edema(mean ± SE histologic score of young vs old:3.11 ± 0.16 vs 2.50 ±-0.11,P < 0.05),decreased local inflammatory response(histologic score of stromal infiltrate:3.11 ± 0.27 vs 2.00 ± 0.17,P < 0.05) and increased bacterial infiltration(174% ± 52% increase from sham vs 377% ± 4%,P < 0.05).A decreased expression of PAP1 and PAP2 was demonstrated by Western blotting analysis and immunohistochemical staining.There were no differences in serum amylase and lipase activity,or tissue myeloperoxidase or monocyte chemotactic protein-1 levels.However,in the most-aged group,serum C-reactive protein levels were higher(young vs old:0.249 ± 0.04 mg/dL vs 2.45 ± 0.68 mg/dL,P < 0.05).CONCLUSION:In older animals,there is depressed PAP expression related to a blunted inflammatory response in AP which is associated with worsened bacterial infiltration and higher C-reactive protein level;this may explain the more aggressive clinical course. | Sophia Fu Albert Stanek Cathy M Mueller Nefertti A Brown Chongmin Huan Martin H Bluth Michael E Zenilman | 2012 | World Journal of Gastroenterology2012,18,26: | 5 |

| 7 | Performance of the Montreal classification for inflammatory bowel diseases显示文摘AIM:To validate the Montreal classification system for Crohn's disease(CD) and ulcerative colitis(UC) within the Netherlands.METHODS:A selection of 20 de-identified medical records with an appropriate representation of the inflammatory bowel disease(IBD) sub phenotypes were scored by 30 observers with different professions(gastroenterologist specialist in IBD,gastroenterologist in training and IBD-nurses) and experience level with IBD patient care.Patients were classified according to the Montreal classification.In addition,participants were asked to score extra-intestinal manifestations(EIM) and disease severity in CD based on their clinical judgment.The inter-observer agreement was calculated by percentages of correct answers(answers identical to the 'expert evaluation') and Fleiss-kappa(k).Kappa cutoffs:< 0.4-poor; 0.41-0.6-moderate; 0.61-0.8-good; > 0.8 excellent.RESULTS:The inter-observer agreement was excellent for diagnosis(k = 0.96),perianal disease(k = 0.92) and disease location in CD(k = 0.82) and good for age of onset(k = 0.67),upper gastrointestinal disease(k = 0.62),disease behaviour in CD(k = 0.79) and disease extent in UC(k = 0.65).Disease severity in UC was scored poor(k = 0.23).The additional items resulted in a good inter-observer agreement for EIM(k = 0.68) and a moderate agreement for disease severity in CD(k = 0.44).Percentages of correct answers over all Montreal items give a good reflection of the inter-observer agreement(> 80%),except for disease severity(48%-74%).IBD-nurses were significantly worse in scoring upper gastrointestinal disease in CD compared to gastroenterologists(P = 0.008) and gastroenterologists in training(P = 0.040).Observers with less than 10 years of experience were significantly better at scoring UC severity than observers with 10-20 years(P = 0.003) and more than 20 years(P = 0.003) of experience with IBD patient care.Observers with 10-20 years of experience with IBD patient care were significantly better at scoring upper gastrointestinal disease in CD than observers with less than 10 years(P = 0.007) and more than 20 years(P = 0.007) of experience with IBD patient care.CONCLUSION:We found a good to excellent interobserver agreement for all Montreal items except for disease severity in UC(poor). | Lieke M Spekhorst Marijn C Visschedijk Rudi Alberts Eleonora A Festen Egbert-Jan van der Wouden Gerard Dijkstra Rinse K Weersma | 2014 | World Journal of Gastroenterology2014,20,41: | 4 |

| 9 | Pyruvate-fortified resuscitation stabilizes cardiac electrical activity and energy metabolism during hypovolemia显示文摘AIM: To test the hypothesis that fluid resuscitation with Ringer's solution enriched with pyruvate(PR), a physiological antioxidant and energy substrate, affords protection of myocardial metabolism and electrophysiological performance superior to lactated Ringer's(LR) during hypovolemia and hindlimb ischemia-reperfusion.METHODS: Male domestic goats(25-30 kg) were exsanguinated to a mean arterial pressure of 48 ± 1 mm Hg. Right hindlimb ischemia was imposed for 90 min by applying a tourniquet and femoral crossclamp. LR or PR, infused iv, delivered 0.05 mmol/kg per minute L-lactate or pyruvate, respectively, from 30 min hindlimb ischemia until 30 min post-ischemia. Time controls(TC) underwent neither hemorrhage, hindlimb ischemia nor resuscitation. Goats were sacrificed and left ventricular myocardium biopsied at 90 min fluid resuscitation(n = 6 per group) or 3.5 h later(n = 9 LR, 10 PR, 8 TC).RESULTS: Myocardial 8-isoprostane content, phosphocreatine phosphorylation potential, creatine kinase activity, and heart rate-adjusted QT interval(QTc) vari- ability were evaluated at 90 min resuscitation and 3.5 h post-resuscitation. PR sharply lowered pro-arrhythmic QTc variability vs LR(P < 0.05); this effect persisted 3.5 h post-resuscitation. PR lowered myocardial 8-isoprostane content, a product of oxidative stress, by 39 and 37% during and 3.5 h after resuscitation, respectively, vs LR. Creatine kinase activity fell 42% post-LR vs TC(P < 0.05), but was stable post-PR(P < 0.02 vs post-LR). | 10 | Chronic antiepileptic drug use and functional network efficiency: A functional magnetic resonance imaging study显示文摘AIM To increase our insight in the neuronal mechanisms underlying cognitive side-effects of antiepileptic drug(AED) treatment.METHODS The relation between functional magnetic resonance-acquired brain network measures, AED use, and cognitive function was investigated. Three groups of patients with epilepsy with a different risk profile for developing cognitive side effects were included: A 'low risk' category(lamotrigine or levetiracetam, n=16), an 'intermediate risk' category(carbamazepine, oxcarbazepine, phenytoin, or valproate, n=34) and a 'high risk' category(topiramate, n=5). Brain connectivity was assessed using resting state functional magnetic resonance imaging and graph theoretical network analysis. The Computerized Visual Searching Task was used to measure central information processing speed, a common cognitive side effect of AED treatment. RESULTS Central information processing speed was lower in patients taking AEDs from the intermediate and high risk categories, compared with patients from the low risk category. The effect of risk category on global efficiency was significant(P < 0.05, ANCOVA), with a significantly higher global efficiency for patient from the low category compared with the high risk category(P < 0.05, post-hoc test). Risk category had no significant effect on the clustering coefficient(ANCOVA, P > 0.2). Also no significant associations between information processing speed and global efficiency or the clustering coefficient(linear regression analysis, P > 0.15) were observed. | 13 | Correlation between the proportion of breast volume involved by locally advanced tumors and invasion of the skin and posterior structures显示文摘AIM: To evaluate any differences between the percentages of involved breast volume, pathologic attributes,and tumor marker expression of T3 and T4a-c tumors in locally advanced breast cancers(BC).METHODS: All patients with T3 N > 0 and T4a-c BC without evidence of distant metastasis(M0), presenting to the Breast Clinic from 1980 to 2010, were examinedto determine whether their BC's involved ≥ 50% of their breast volumes, defined by gross replacement of at least one hemisphere. Core needle biopsy or postmastectomy specimens from tumors involving a known percent of breast volume were evaluated for:(1)pathological grades and lympho-vascular invasion(LVI);(2) hormone receptor(ER/PR) expression > 0; and(3)epidermoid growth factor 2(her2) over-expression(3+)by immune-histochemical staining or fluorescent in situ hybridization.RESULTS: The data base included 98 patients with T3N> 0 M0 and 120 with T4a-c, any N disease, M0 disease. T3 tumor masses involved 50% or more of the breast in 23/98(24%), and T4a-c tumors 65/120(54%)(P < 0.001). Only 1% of T3 tumors and 23% of T4a-c tumors presented with total breast replacement. | 16 | Modeling cardiac arrest and resuscitation in the domestic pig显示文摘Cardiac arrest remains a leading cause of death and permanent disability worldwide. Although many victims are initially resuscitated, they often succumb to the extensive ischemia-reperfusion injury inflicted on the internal organs, especially the brain. Cardiac arrest initiates a complex cellular injury cascade encompassing reactive oxygen and nitrogen species, Ca2+ overload, ATP depletion, pro- and anti-apoptotic proteins, mitochondrial dysfunction, and neuronal glutamate excitotoxity, which injures and kills cells, compromises function of internal organs and ignites a destructive systemic inflammatory response. The sheer complexity and scope of this cascade challenges the development of experimental models of and effective treatments for cardiac arrest. Many experimental animal preparations have been developed to decipher the mechanisms of damage to vital internal organs following cardiac arrest and cardiopulmonary resuscitation(CPR), and to develop treatments to interrupt the lethal injury cascades. Porcine models of cardiac arrest and resuscitation offer several important advantages over other species, and outcomes in this large animal are readily translated to the clinical setting. This review summarizes porcine cardiac arrest-CPR models reported in the literature, describes clinically relevant phenomena observed during cardiac arrest and resuscitation in pigs, and discusses numerous methodological considerations in modeling cardiac arrest/CPR. | 18 | Evaluation of mannitol effect in patients with acute hepatic failure and acute-on-chronic liver failure using conventional MRI,diffusion tensor imaging and in-vivo proton MR spectroscopy显示文摘AIM:To evaluate the effect of an intravenous bolus of mannitol in altering brain metabolites, brain water content, brain parenchyma volume, cerebrospinal fluid (CSF) volume and clinical signs in controls and in patients with acute liver failure (ALF) and acute-on-chronic liver failure (ACLF), by comparing changes in conventional magnetic resonance imaging (MRI), in vivo proton magnetic resonance spectroscopy (PMRS) and diffusion tensor imaging (DTI) before and after its infusion.METHODS: Five patients each with ALF and ACLF in grade 3 or 4 hepatic encephalopathy and with clinical signs of raised intracranial pressure were studied along with five healthy volunteers. After baseline MRI, an intravenous bolus of 20% mannitol solution was given over 10 min in controls as well as in patients with ALF and ACLF. Repeat MRI for the same position was acquired 30 min after completing the mannitol injection. RESULTS: No statistically significant difference was observed between controls and patients with ALF and ACLF in metabolite ratios, DTI metrics and brain volume or CSF volume following 45 min of mannitol infusion. There was no change in clinical status at the end of post-mannitol imaging. We searched PUB MED and MEDLINE with several key words-namely,'leishmaniasis';'cutaneous','diffuse cutaneous','mucosal', and'visceral leishmaniasis'; 'kala azar'and 'post kala azar dermal leishmaniasis'-for recent clinical and basic science articles related to leishmaniasis in countries in the Eastern Africa region. Poverty, wars, conflicts and migration have significantly aggravated leishmaniases in Eastern Africa. Of particular concern is the increasing incidence of Leishmania-HIV co-infection in Ethiopia where 20~40% of the persons affected by visceral leishmaniasis are HIV-co-infected. Sudan has the highest prevalence rate of post kala-azar dermal leishmaniasis(PKDL) in the world, a skin complication of visceral leishmaniasis(VL) that mainly afflicts children below age ten.
