1Insulin-like growth factor(IGF)signaling in tumorigenesis and the development of cancer drug resistance显示文摘One of the greatest obstacles to current cancer treatment efforts is the development of drug resistance by tumors.Despite recent advances in diagnostic practices and surgical interventions,many neoplasms demonstrate poor response to adjuvant or neoadjuvant radiation and chemotherapy.As a result,the prognosis for many patients afflicted with these aggressive cancers remains bleak.The insulin-like growth factor(IGF)signaling axis has been shown to play critical role in the development and progression of various tumors.Many basic science and translational studies have shown that IGF pathway modulators can have promising effects when used to treat various malignancies.There also exists a substantial body of recent evidence implicating IGF signaling dysregulation in the dwindling response of tumors to current standard-of-care therapy.By better understanding both the IGF-dependent and-independent mechanisms by which pathway members can influence drug sensitivity,we can eventually aim to use modulators of IGF signaling to augment the effects of current therapy.This review summarizes and synthesizes numerous recent investigations looking at the role of the IGF pathway in drug resistance.We offer a brief overview of IGF signaling and its general role in neoplasia,and then delve into detail about the many types of human cancer that have been shown to have IGF pathway involvement in resistance and/or sensitization to therapy.Ultimately,our hope is that such a compilation of evidence will compel investigators to carry out much needed studies looking at combination treatment with IGF signaling modulators to overcome current therapy resistance.Sahitya K.Denduluri Olumuyiwa Idowu Zhongliang Wang Zhan Liao Zhengjian Yan Maryam K.Mohammed Jixing Ye Qiang Wei Jing Wang Lianggong Zhao Hue H.Luu 2015Genes & Diseases2015,2,1:21

3Pancreatic duct stones in patients with chronic pancreatitis:surgical outcomes显示文摘BACKGROUND: Pancreatic duct stone (PDS) is a common complication of chronic pancreatitis. Surgery is a common therapeutic option for PDS. In this study we assessed the surgical procedures for PDS in patients with chronic pancreatitis at our hospital. METHOD: Between January 2004 and September 2009, medical records from 35 patients diagnosed with PDS associated with chronic pancreatitis were retrospectively reviewed and the patients were followed up for up to 67 months. RESULTS: The 35 patients underwent ultrasonography, computed tomography, or both, with an overall accuracy rate of 85.7%. Of these patients, 31 underwent the modified Puestow procedure, 2 underwent the Whipple procedure, 1 underwent simple stone removal by duct incision, and 1 underwent pancreatic abscess drainage. Of the 35 patients, 28 were followed up for 4-67 months. There was no postoperative death before discharge or during follow-up. After the modified Puestow procedure, abdominal pain was reduced in patients with complete or incomplete stone clearance (P>0.05). Steatorrhea and diabetes mellitus developed in several patients during a long-term follow-up. CONCLUSIONS: Surgery, especially the modified Puestow procedure, is effective and safe for patients with PDS associated with chronic pancreatitis. 4Joint Inversion of the 3D P Wave Velocity Structure of the Crust and Upper Mantle under the Southeastern Margin of the Tibetan Plateau Using Regional Earthquake and Teleseismic Data显示文摘The special seismic tectonic environment and frequent seismicity in the southeastern margin of the Qinghai–Tibet Plateau show that this area is an ideal location to study the present tectonic movement and background of strong earthquakes in China's Mainland and to predict future strong earthquake risk zones. Studies of the structural environment and physical characteristics of the deep structure in this area are helpful to explore deep dynamic effects and deformation field characteristics, to strengthen our understanding of the roles of anisotropy and tectonic deformation and to study the deep tectonic background of the seismic origin of the block's interior. In this paper, the three-dimensional(3D) P-wave velocity structure of the crust and upper mantle under the southeastern margin of the Qinghai–Tibet Plateau is obtained via observational data from 224 permanent seismic stations in the regional digital seismic network of Yunnan and Sichuan Provinces and from 356 mobile China seismic arrays in the southern section of the north–south seismic belt using a joint inversion method of the regional earthquake and teleseismic data. The results indicate that the spatial distribution of the P-wave velocity anomalies in the shallow upper crust is closely related to the surface geological structure, terrain and lithology. Baoxing and Kangding, with their basic volcanic rocks and volcanic clastic rocks, present obvious high-velocity anomalies. The Chengdu Basin shows low-velocity anomalies associated with the Quaternary sediments. The Xichang Mesozoic Basin and the Butuo Basin are characterised by lowvelocity anomalies related to very thick sedimentary layers. The upper and middle crust beneath the Chuan–Dian and Songpan–Ganzi Blocks has apparent lateral heterogeneities, including low-velocity zones of different sizes. There is a large range of low-velocity layers in the Songpan–Ganzi Block and the sub–block northwest of Sichuan Province, showing that the middle and lower crust is relatively weak. The Sichuan Basin, which is located in the western margin of the Yangtze platform, shows high-velocity characteristics. The results also reveal that there are continuous low-velocity layer distributions in the middle and lower crust of the Daliangshan Block and that the distribution direction of the low-velocity anomaly is nearly SN, which is consistent with the trend of the Daliangshan fault. The existence of the low-velocity layer in the crust also provides a deep source for the deep dynamic deformation and seismic activity of the Daliangshan Block and its boundary faults. The results of the 3D P-wave velocity structure show that an anomalous distribution of high-density, strong-magnetic and high-wave velocity exists inside the crust in the Panxi region. This is likely related to late Paleozoic mantle plume activity that led to a large number of mafic and ultra-mafic intrusions into the crust. In the crustal doming process, the massive intrusion of mantle-derived material enhanced the mechanical strength of the crustal medium. The P-wave velocity structure also revealed that the upper mantle contains a low-velocity layer at a depth of 80–120 km in the Panxi region. 6Left atrial enlargement and non-paroxysmal atrial fibrillation as risk factors for left atrial thrombus/spontaneous Echo contrast in patients with atrial fibrillation and low CHA2DS2-VASc score显示文摘Objective To determine the risk factors for thromboembolism in lower risk patients with non-valvular atrial fibrillation(AF)and low CHA2DS2-VASc scores,which remain undefined.Methods We retrospectively analyzed the baseline clinical characteristics,routine laboratory parameters,and echocardiographic measurements of 705 patients(71.1%male;mean age:52.10±9.64 years)with low CHA2DS2-VASc score(0 or 1;1 point for female sex)out of 1346 consecutive patients with non-valvular AF who underwent transesophageal echocardiography(TEE)at Guangdong Cardiovascular Institute between January 2013 and December 2015.Results Patients with left atrial thrombus(LAT)or spontaneous echo contrast(SEC)on TEE(24/705,4%)showed a higher incidence rate of vascular disease(54.2%vs.32.9%,P=0.045)and non-paroxysmal AF(79.2%vs.29.4%,P<0.001),larger left atrial diameter(43.08±4.59 vs.36.02±5.53 mm,P<0.001),and lower left ventricular ejection fraction(58.23±8.82%vs.64.15±7.14%,P<0.001)than those without.Multivariate logistic regression analysis identified left atrial diameter[odds ratio(OR)=1.171,95%confidence interval(CI):1.084–1.265,P<0.001]and non-paroxysmal AF(OR=3.766,95%CI:1.282–11.061,P=0.016)as independent risk factors for LAT/SEC.In ROC curve analysis,a left atrial dimeter cutoff of 37.5 mm yielded 95.0%sensitivity and 62.7%specificity(AUC:0.847,P<0.0001,95%CI:0.793–0.914).Conclusion In patients with non-valvular AF with low CHA2DS2-VASc score,the presence of LAT or SEC was associated with left atrial enlargement,which had moderate predictive value,and non-paroxysmal AF.Wei-Dong LIN Yu-Mei XUE Fang-Zhou LIU Xian-Hong FANG Xian-Zhang ZHAN Hong-Tao LIAO Gary Tse Shu-Lin WU 2020Journal of Geriatric Cardiology2020,17,3:8
7Aerosolized STAT1 Antisense Oligodeoxynucleotides Decrease the Concentrations of Inflammatory Mediators in Bronchoalveolar Lavage Fluid in Bleomycin-Induced Rat Pulmonary Fibrosis显示文摘It has been demonstrated that alveolar macrophages (AMs) play a key role in the pathogenesis of pulmonary fibrosis by releasing a variety of cytokines and inflammatory mediators. In addition, abnormal signal transducer and activator of transcription-1 (STAT1) activation in AMs may play a pivotal role in the process of alveolitis and pulmonary fibrosis. In this study, we transfected STAT1 antisense oligodeoxynucleotide (ASON) into rats by aerosolization, and then investigated the effect of STAT1 ASON on inflammatory mediators such as TGF-β, PDGF and TNF-α in bronchoalveolar lavage fluid (BALF) from rats with bleomycin (BLM)-induced rat pulmonary fibrosis. Our results showed that STAT1 ASON by aerosolization could enter into lung tissues and AMs. STAT1 ASON could inhibit mRNA and protein expressions of STAT1 and ICAM-1 in AMs of rat with pulmonary fibrosis, and had no toxic side effect on liver and kidney. Aerosolized STAT1 ASON could ameliorate the alveolitis through inhibiting the secretion of inflammatory mediators in BLM-induced rat pulmonary fibrosis. 8Grain refinement of Mg-Al alloys inoculated by MgAl_(2)O_(4)powder显示文摘As a potent nucleating substrate forα-Mg grain,MgAl_(2)O_(4) powder was used to inoculate the Mg-Al melt in this study.The effects of MgAl_(2)O_(4)amount,holding time and Al content on the grain size and grain refining ratio of the inoculated Mg-Al alloys are systematically investigated.The results show that the minimum grain size of Mg-3Al alloy is achieved by adding 2wt.%MgAl_(2)O_(4)powder and this alloy exhibits higher grain refining ratio than Mg-5Al and Mg-8Al alloys.The crystallographic misfit calculation indicates the wellmatching and possible orientation relationships(ORs)betweenα-Mg and MgAl_(2)O_(4).Among these predicted ORs,[10–10]α−Mg//[110]MgAl2O4 in(0002)α−Mg//(1–13)MgAl2O4 possesses the smallest misfit,i.e.,2.34%(fr).Both results of the experiment and crystallographic calculation demonstrate that the grain refinement of Mg-Al alloys is attributed to the MgAl_(2)O_(4)particles acting as the heterogeneous nucleation substrates forα-Mg grains.Hengbin Liao Meiyan Zhan Chengbo Li Zhiqiang Ma Jun Du 2021Journal of Magnesium and Alloys2021,9,4:7
9Keyhole Foraminotomy via a Percutaneous Posterior Full-endoscopic Approach for Cervical Radiculopathy:An Advanced Procedure and Clinical Study显示文摘Endoscopic cervical foraminotomy is increasingly used for cervical spondylotic radiculopathy(CSR),but there is great concern about radiation exposure because of the heavy dependence of this surgical method on fluoroscopy.The objective of this study was to introduce in detail an advanced surgical technique of keyhole foraminotomy via a percutaneous posterior full-endoscopic approach as a treatment for CSR and investigate its clinical outcomes.We retrospectively reviewed 33 consecutive patients with CSR who underwent keyhole foraminotomy via a percutaneous posterior full-endoscopic approach from October 2015 to April 2017.The patients’general characteristics,including operative time,blood loss,hospital stay,complications,and recurrence,were obtained.Clinical outcomes were evaluated using the visual analogue scale(VAS)for radicular pain,the neck disability index(NDI)for functional assessment,and the modified MacNab criteria for patient satisfaction.All operations were successfully performed(mean operation time,62 min),with no measurable blood loss or severe related complications.The mean follow-up was 25 months.The VAS and NDI scores were significantly improved as compared with those in the preoperative period(preoperative vs.final follow-up:7.6±1.6 vs.3.83±7.34 for VAS,P<0.01;69.5%±10.5%vs.17.54%±13.40%for NDI,P<0.01).Of the 33 patients,32(97.0%)had good-to-excellent global outcomes and all patients obtained symptomatic improvement.In conclusion,keyhole foraminotomy via a percutaneous posterior full-endoscopic approach is an efficient,safe,and feasible procedure for the treatment of CSR.Its simplified single-step blunt incision for localization appears to decrease radiation exposure risks.Rong-jin LUO Yu SONG Zhi-wei LIAO Hui-peng YIN Sheng-feng ZHAN Sai-deng LU Chao CHEN Cao YANG 2020Current Medical Science2020,40,6:7
10STACKING SEQUENCE OPTIMIZA-TION OF LAMINATED COMPOSITE CYLINDER SHELL FOR MAXIMAL BUCKLING LOAD显示文摘A new optimization method for the optimization of stacking of composite glass fiber laminates is developed. The fiber orientation and angle of the layers of the cylindrical shells are sought considering the buckling load. The proposed optimization algorithm applies both finite element analysis and the mode-pursuing sampling (MPS)method. The algorithms suggest the optimal stacking sequence for achieving the maximal buckling load. The procedure is implemented by integrating ANSYS and MATLAB. The stacking sequence designing for the symmetric angle-ply three-layered and five-layered composite cylinder shells is presented to illustrate the optimization process, respectively. 11Risk factors for postoperative pancreatic fistula in patients with insulinomas:analysis of 292 consecutive cases显示文摘BACKGROUND:Pancreatic fistula (PF) remains the most challenging complication in pancreatic surgery,yet few published studies have focused on the risk factors for postoperative PF in patients undergoing surgery for insulinomas.METHODS:From January 1990 to February 2010,a total of 292 patients with insulinomas underwent surgery at Peking Union Medical College Hospital.Demographic data,intraoperative procedures,and postoperative data were collected.Particular attention was paid to variables associated with PF as defined by the International Study Group of Pancreatic Fistula (ISGPF).Univariate and multivariate analyses were used to identify possible risk factors for PF.RESULTS:PF was found in 132 (45.2%) patients,of whom 90 were classified into ISGPF grade A,33 grade B,and 9 grade C.Multivariate analysis showed that male patients (OR=2.56;P=0.007) and operative time >180minutes (OR=3.756;P<0.0001) were independent risk factors for clinical PF.Pancreatic resection with stapler was a protective factor for both total PF (OR=0.022;P=0.010) and clinical PF (OR=0.097;P=0.007).CONCLUSIONS:Male gender and operative time >180minutes were independent risk factors for clinical PF,while pancreatic resection with a stapler was a protective factor.Whether body mass index (BMI) and other variables during operation are risk factors of PF needs further study.Zhao, Yu-Pei Zhan, Han-Xiang Cong, Lin Zhang, Tai-Ping Liao, Quan Dai, Meng-Hua Cai, Li-Xing Zhu, Yu 2012Hepatobiliary & Pancreatic Diseases International2012,11,1:3
