5Long-term rotational stability and visual outcomes of a single-piece hydrophilic acrylic toric IOL:a 1.5-year follow-up显示文摘AIM:To evaluate the rotational stability and visual outcomes of a single-piece hydrophilic acrylic toric intraocular lenses(IOL) over a 1.5-year follow-up period.Data from the 6-month follow-up study have been previously reported.METHODS:Forty eyes of 26 cataract patients(mean age:72.8±7.9y) with pre-existing corneal astigmatism of 1.0 to 2.6 D were enrolled in the study.Mean axial length was 23.17±0.88 mm.Main outcome measures after implantation of the Torica-a A IOL(Human Optics) were IOL rotational stability,subjective refraction,astigmatism correction,uncorrected and corrected distance visual acuity(UDVA,CDVA).RESULTS:There was no significant change in the parameters evaluated between the 6-month and 1.5-year follow-ups.At last visit,mean absolute IOL rotation(objective method) was 1.81°±1.87°(range 0.00° to 7.20°) with 78.6% of eyes having IOL rotation <3°,92.9% of eyes <5° and 100% of eyes <8°.No patient required secondary IOL repositioning during the course of the study.Mean residual refractive cylinder was-0.60±0.40 D.There was a significant reduction in the magnitude of the J0 vector postoperatively(P<0.0001) with a mean absolute change of 0.76±0.40 D.The mean J45 vector was close to zero preoperatively and postoperatively and didn't change significantly(P=0.28).Mean monocular UDVA and CDVA was 0.09±0.12 log MAR and-0.01±0.12 log MAR,respectively.No treatment with Nd:YAG laser was required.CONCLUSION:The Torica-a A IOL shows good and stable visual performance and rotational stability over a 1.5-year follow-up period.Balázs Gyongyossy Paul Jirak Ulrich Schonherr 2017International Journal of Ophthalmology(English edition)2017,10,4:7
6Relevance of α-defensins(HNP1-3) and defensin β-1 in diabetes显示文摘AIM: To investigate the genetic background of human defensin expression in type 1 and 2 diabetes.METHODS: Associations between DEFA1/DEFA3 gene copy number polymorphism and diabetes as well as between the promoter polymorphisms of DEFB1 and diabetes were studied. The copy number variation of the DEFA1/DEFA3 genes was determined in 257 diabetic patients(117 patients with type 1 and 140 with type 2 diabetes). The control group consisted of 221 age- and gender-matched healthy blood donors. The cumulative copy numbers of the DEFA1/DEFA3 genes were detected by using quantitative PCR analysis. To evaluate the HNP 1-3(human neutrophil peptide 1-3 or α-defensin) levels in the circulation, plasma HNP 1-3 concentrations were measured by ELISA. The expression of DEFA1/A3 in peripheral leukocytes of the diabetic patients was measured by quantitative RT PCR analysis. Three SNPs of the human DEFB1(human defensin β-1) gene: DEFB1 G-20A(rs11362), DEFB1 C-44G(rs1800972) and DEFB1 G-52A(rs1799946) were genotyped by Custom TaqMan? Real Time PCR assay.RESULTS: Significant differences were observed in HNP1-3 levels between the healthy subjects and both groups of diabetic patients. The mean ± SE was 28.78 ± 4.2 ng/mL in type 1 diabetes, and 29.82 ± 5.36 ng/mL in type 2 diabetes, vs 11.94 ± 2.96 ng/mL in controls; P < 0.01 respectively. There was no significant difference between patients with type 1 and type 2 diabetes in the high plasma concentrations of HNP1-3. The highest concentrations of α-defensin were found in diabetic patients with nephropathy(49.4 ± 4.8 ng/mL), neuropathy(38.7 ± 4.8 ng/mL) or cardiovascular complications(45.6 ± 1.45 ng/L). There was no significant difference in the cumulative copy numbers of DEFA1/DEFA3 genes between controls and patients, or between patients with the two types of diabetes. Comparisons of HNP 1-3 plasma level and DEFA1/A3 copy number of the same patient did not reveal significant relationship between defensin-α levels and the gene copy numbers(r2 = 0.01). Similarly, no positive correlation was observed between the copy numbers and the mRNA expression levels of DEFA1/A3. 7一项中国幽门螺杆菌耐药基因型及菌株谱系的全基因组测序研究显示文摘幽门螺杆菌(Helicobacter pylori,H.pylori)是胃癌Ⅰ类致癌物,根除H.pylori能显著降低胃癌的发生率和死亡率。抗生素耐药是导致H.pylori根除治疗失败的最重要因素,因此药敏指导下的个性化诊疗日益重要。为了加强对H.pylori耐药机制的了解,并寻找新的耐药基因型位点,该研究从中国幽门螺杆菌分子医学中心(CCHpMM)菌株库复苏了60株H.pylori,进行了耐药表型检测和全基因组测序(whole genome sequencing,WGS),并评价了耐药表型和基因型的一致性。结果显示克拉霉素和左氧氟沙星耐药基因型与表型几乎完全一致,Kappa系数分别为0.867和0.833。所有携带R16H/C突变和rdxA截断的H.pylori都具有甲硝唑耐药性,特异性为100%。此外,研究还发现HP0399基因的A1378G突变和FabH基因的A149G突变两个新位点,可能分别导致了四环素耐药和多药耐药性。该研究结果表明,用克拉霉素和左氧氟沙星耐药基因型检测耐药是可靠的,WGS对发现新的H.pylori耐药位点有很大帮助,并为发现H.pylori新耐药机制提供了基础。Zhou Y Zhong ZS Hu SJ 钟子劭(编译) 2022中华医学杂志2022,102,22:4

9Synchronous electrogastrographic and manometric study of the stomach as an esophageal substitute显示文摘AIM: To investigate the electric and contractile mechanisms involved in the deranged function of the transposed stomach in relation to the course of the symptoms and the changes in contractile and electrical parameters over time.METHODS: Twenty-one patients after subtotal esophagectomy and 18 healthy volunteers were studied.Complaints were compiled by using a questionnaire, and a symptom score was formed. Synchronous electrogastrography and gastric manometry were performed in the fasting state and postprandially.RESULTS: Eight of the operated patients were symptomfree and 13 had symptoms. The durations of the postoperative periods for the symptomatic (9.1±6.5 mo)and the asymptomatic (28.3±8.8 mo) patients were significantly different. The symptom score correlated negatively with the time that had elapsed since the operation. The percentages of the dominant frequency in the normogastric, bradygastric and tachygastric ranges differed significantly between the controls and the patients.A significant difference was detected between the power ratio of the controls and that of the patients. The occurrence of tachygastria in the symptomatic and the symptom-free patients correlated negatively both with the time that had elapsed and with the symptom score. There was a significant increase in motility index after feeding in the controls, but not in the patients. The contractile activity of the stomach increased both in the controls and in the symptom-free patients. In contrast, in the group of symptomatic patients, the contractile activity decreased postprandially as compared with the fasting state.CONCLUSION: The patients' post-operative complaints and symptoms change during the post-operative period and correlate with the parameters of the myoelectric and contractile activities of the stomach. 11Nutritional Status,Sarcopenia and the Importance of Prehabilitation of Gastrointestinal Tumor Patients:a Surgical Point of View显示文摘Background Surgery may be the field of healthcare where malnutrition and sarcopenia have their greatest impact on patient morbidity and mortality.However,there are limited data on the nutritional status of surgical patients and the effects of prehabilitation on the outcomes of surgery.Methods A prospective analysis was conducted on all patients surgically-treated for malignant gastrointestinal tumors at St.John Hospital during a two-year period.The patient’s gender,age,body weight,height,BMI and weight loss were registered,then a risk score was determined by the MUST survey.Measurement of the triceps and thigh skin-fold thickness and the circumference of the upper arm and thigh were done to calculate muscle area and muscle index,respectively.The body composition was assessed using an OMRON-BF511 device.Muscle function was evaluated based on hand clamping force measurement and activity tests.Patients who were diagnosed as being at-risk received preoperative prehabilitation,which included physiotherapy and nutritional therapy.Results A total of 231 patients(133 males/98 females)were analyzed.They had a mean age of 68.9 years(18~98).Seventy-four patients(32%)lost weight,with an average loss of 7 kilograms(3~15 kg).Anthropometric data showed an average upper-arm circumference of 27.4 cm(14.3~38.1)and thigh circumference of 44.7 cm(19.3~60.1),so the median muscle index was 1.29.The mean BMI was 26,which is above normal,and the elevated BMI was consistent documented in each patient subgroup stratified by age and tumor type.A body composition analysis was performed for 75 patients(44 male/31 female),who had a median age of 68(37~88 y).The average BMI of these patients was 25.7 y and their average MUST score was 1.12.The total body fat percentage(of the total body mass)was 29.5%,total muscle was 30.1%and visceral fat was 10%.Thirty patients(40%)had sarcopenia,with a mean BMI of 28.7,fat comprising 34.2%of the body mass,visceral-fat 11%,and muscle 27.1%,and their median MUST score was 1.23.Patients who received preoperative training(physiotherapy)showed improvements in physical function ranging from 12%~33%.Conclusion Gastrointestinal tumor patients have a higher than normal BMI regardless of age or tumor type.Patients with sarcopenia show measurable improvement after two weeks of prehabilitation.Péter Lukovich Ákos Nagy Bianka Barok Borbála Csiba Ram Rokka Balázs Pőcze 2020Journal of Nutritional Oncology2020,5,3:2

15Thymidine kinase gene mutation leads to reduced virulence of pseudorabies virus显示文摘To explore correlation between the tk gene structure of pseudorabies virus (PRV) and its virulence, to study the effect of the gene mutation on PRV biological properties, and to investigate mechinism of reduced virulence, thymidine kinase (TK)-deficient mutant of pseudorabies virus strain Hubei (PRV HB) was isolated by selection for resistance to 5-bromodeoxyuridine. The tk genes of PRV HB and its TK mutant were cloned and sequenced. 1587 base pairs of the tk gene and flanking regions of wild-type (wt) virus were sequenced, which included an open reading frame (ORF) of 1098 bp encoding a protein of 366 amino acids. The ORF contained two 137-bp repeated sequences, which were connected by an adenosine. 1458 bp of the tk and flanking regions of TK- mutant were sequenced. Analysis of the tk gene sequence of TK mutant indicated that one of 137 bp repeated sequence and the connecting adenosine in the tk gene of the wt virus was deleted and a repeated sequence of 8 nucleotides (GCGCGCC) was inserted.
