| 1 | Lean techniques for the improvement of patients' flow in emergency department显示文摘BACKGROUND: Emergency departments(EDs) face problems with overcrowding, access block, cost containment, and increasing demand from patients. In order to resolve these problems, there is rising interest to an approach called 'lean' management. This study aims to(1) evaluate the current patient flow in ED,(2) to identify and eliminate the non-valued added process, and(3) to modify the existing process.METHODS: It was a quantitative, pre- and post-lean design study with a series of lean management work implemented to improve the admission and blood result waiting time. These included structured re-design process, priority admission triage(PAT) program, enhanced communication with medical department, and use of new high sensitivity troponin-T(hsTnT) blood test. Triage waiting time, consultation waiting time, blood result time, admission waiting time, total processing time and ED length of stay were compared.RESULTS: Among all the processes carried out in ED, the most time consuming processes were to wait for an admission bed(38.24 minutes; SD 66.35) and blood testing result(mean 52.73 minutes, SD 24.03). The triage waiting time and end waiting time for consultation were significantly decreased. The admission waiting time of emergency medical ward(EMW) was significantly decreased from 54.76 minutes to 24.45 minutes after implementation of PAT program(P<0.05).CONCLUSION: The application of lean management can improve the patient flow in ED. | 3 | 胎儿期恰逢1959~1961年饥荒的中国成人精神分裂症的发病率显示文摘背景:精神分裂症是一种常见的中度精神障碍。子宫内营养缺乏可增大患精神分裂症的危险。其主要依据来自1944~1945年荷兰饥饿冬天(Dutch Hunger Winter)研究,当时食品摄入在短期内极剧下降。这项有关饥荒期间受孕人群的大型队列研究显示,发生精神分裂症的危险增大两倍。
目的:确定经受1959~1961年大饥荒的中国人是否有相似的结果。
设计、地点及参试者:发生精神分裂症危险的调查于安徽省芜湖地区进行,这是当初受灾最重的地区之一。对饥荒之前、饥荒期间和饥荒以后出生的人群发病率进行比较。芜湖及其周围6个县是由一所精神病院负责的。对1971~2001年所有的精神病病历记录均进行检查。精神分裂症患者的临床和社会人口学资料由对自然灾害暴露情况不知情的研究者摘录。有关饥荒年出生人数和死亡人数的数据是有效可用的,累积死亡率根据以后的人口学调查进行估计。
主要观测指标:饥荒的证据已经核实,计算未校正的以及对死亡率进行校正后的发生精神分裂症的相对危险度。
结果:安徽省的出生率(每1000人)在饥荒期问降低了约80%,从1958年的28.28降至1959年的20.97、1960年的8.61和1961年的11.06。在饥荒期间出生的人,晚年发生精神分裂症的校正后危险明显增高,从1959年的0.84%增至1960年的2.15%和1961年的1.81%。死亡率校正相对危险在1960年出生者为2.30(95%可信区间,1.99~2.05),1961年出生者为1.93(95%可信区间,1.68~2.23)。

| 4 | Effects of Delayed Second Cochlear Implant显示文摘Objective Since Helms' successful bilateral cochlear implantation with good results in 1996, there have been increasing number of reports on bilateral cochlear implantation. Most second device have been implantated within one year after the first. Considering effects of long time auditory deprivation, it is not clear whether a delayed second cochlear implant serves to add additional benefits and how it may interact with central nervous system plasticity. Methods Three cases who received delayed second cochlear implants at People's Hospital of Peking University from 2002 to 2005 were reviewed. The interval between the first and second implants was longer than 2 years in all three patients. Sound perception, and unilateral/bilateral speech discrimination in quiet and noise were evaluated. In addition, GAP detection test was conducted in one patient. Results In one case, having both implants on provided improved performance compared to using only one implant both in quiet and noise. Presumably due to visual interference from lip-reading or short interval between second implant and testing, one patient showed no improvement from using the second implant either in quiet or noise, while the last case demonstrated additional benefits from the second implant only in quiet. In all three patients, performance in recognizing the four tones in Mandarin was superior over word recognition. Conclusions Considerable plasticity in the cerebral auditory center is preserved, despite long acoustic deprivation in some children who have received unilateral cochlear implant. Delayed second implants can result in significant improvements in some of these children. Visual interference from lip-reading may be an obstacle during retraining.
