| 1 | Role of liver transplantation in the management of hepatoblastoma in the pediatric population显示文摘Hepatoblastoma(HB) is the most common primary liver tumor in children and accounts for two-thirds of all malignant liver neoplasms in the pediatric population. For patients with advanced HB(unresectable or unresponsive to chemotherapy), combined treatment with chemotherapy and liver transplantation is an excellent option. The etiology of HB is mostly obscure because of its extreme rarity although some inherited syndromes and very low birth weight have been associated with it. The prognosis for children with HB has significantly improved in the past three decades thanks to advancements in chemotherapy, surgical resection and postoperative care. In 2002 a surgical staging system called pretreatment extent of disease(PRETEXT) was designed to allow a universal, multidisciplinary approach to patients with HB. Between one-third to two-thirds of patients initially present with unresectable tumors or distant metastases, but up to 85% of these tumors become operable after neoadjuvant chemotherapy. Patients with PRETEXT categories 1, 2, and some 3 are referred for neoadjuvant chemotherapy followed by surgical resection with the goal of complete tumor removal. Classic treatments regimens include a combination of cisplatin, fluorouracil, and vincristine or cisplatin and doxorubicin. Liver transplantation is the only treatment option for unresectable HB. In 2010 the pediatric end-stage liver disease, a pediatric-specific scoring system that determines a patient's ranking on the liver transplant list, began to award additional 'exception' points for patients with HB. We analyzed the Standard Transplant Analysis and Research dataset to assess the impact of changes in exception point criteria for HB on outcomes after liver transplantation at Texas Children's Hospital in Houston, Texas. | 2 | Beyond the Pediatric end-stage liver disease system: Solutions for infants with biliary atresia requiring liver transplant显示文摘Biliary atresia(BA), a chronic progressive cholestatic disease of infants, is the leading cause for liver transplant in children, especially in patients under two years of age. BA can be successfully treated with the Kasai portoenterostomy; however most patients still require a liver transplant, with up to one half of BA children needing a transplant by age two. In the current pediatric end-stage liver disease system, children with BA face the risk of not receiving a liver in a safe and timely manner. In this review, we discuss a number of possible solutions to help these children. | 4 | 髁突矢状骨折动物模型的实验研究显示文摘目的:建立髁突囊内矢状骨折动物模型,观察髁突骨折后不同时间的病理形态改变。方法:对10只绵羊进行实验,右侧关节为手术侧,从耳前区切开皮肤,到达颞下颌关节,切断关节盘前后附着,从髁突外侧嵴到内侧髁颈锯开髁突,建立斜形矢状骨折。髁突碎片和关节盘推向前下内侧,左侧关节为对照侧。分别于术后1周、4周和12周处死2只、4只和4只动物。记录术前和术后每组动物的体质量、最大开口度、侧方活动度,进行X线、CT和组织病理学观察。采用SPSS12.0软件包对数据进行统计分析。结果:各手术组体质量、左侧向活动度未发现显著差异,但是在4周和12周组中,最大开口度和右侧向活动度明显减小。在这2组中,X线检查发现髁突残端外侧有新骨生成,并向外侧生长,同时关节间隙变狭窄和模糊。三维CT检查发现,髁突残端、髁突碎片和关节结节均发生改建。组织病理学观察发现,在4周和12周组动物,关节腔充满纤维组织、软骨和骨组织。结论:该研究表明,此类骨折可以导致髁突骨关节病理改变,颞下颌关节有发展为关节强直的可能。 | 龙星 Goss AN | 2010 | 中国口腔颌面外科杂志2010，8，5： | 4 |
| 5 | Failure of P-selectin blockade alone to protect the liver from ischemia-reperfusion injury in the isolated blood-perfused rat liver显示文摘AIM:To determine if blockade of P-selectin in the isolated blood-perfused cold ex vivo rat liver model protects the liver from ischemia-reperfusion injury. METHODS:The effect of P-selectin blockade was assessed by employing an isolated blood-perfused cold ex vivo rat liver with or without P-selectin antibody treatment before and after 6 h of cold storage in University of Wisconsin solution. RESULTS:In our isolated blood-perfused rat liver model,pre-treatment with P-selectin antibody failed to protect the liver from ischemia-reperfusion injury,as judged by the elevated aspartate aminotransferase activity. In addition,P-selectin antibody treatment did not significantly reduced hepatic polymorphonuclear leukocyte accumulation after 120 min of perfusion. Histological evaluation of liver sections obtained at 120 min of perfusion showed significant oncotic necrosis in liver sections of both ischemic control and P-selectin antibody-treated groups. However,total bile production after 120 min of perfusion was signifi cantly greater in P-selectin antibody-treated livers,compared to control livers. No signifi cant difference in P-selectin and ICAM-1 mRNAs and proteins,GSH,GSSG,and nuclear NF-κB was found between control and P-selectin antibody-treated livers.
