| 1 | Endoscopic submucosal dissection for premalignant lesions and noninvasive early gastrointestinal cancers显示文摘AIM: To investigate the indication, feasibility, safety, and clinical utility of endoscopic submucosal dissection (ESD) in the management of various gastrointestinal pathologies. METHODS: The medical records of 60 consecutive patients (34 female, 26 male) who underwent ESD at the gastroenterology department of Kocaeli University from 2006-2010 were examined. Patients selected for ESDhad premalignant lesions or non-invasive early cancers of the gastrointestinal tract and had endoscopic and histological diagnoses. Early cancers were considered to be confined to the submucosa, with no lymph node involvement by means of computed tomography and endosonography. RESULTS: Sixty ESD procedures were performed. The indications were epithelial lesions (n = 39) (33/39 adenoma with high grade dysplasia, 6/39 adenoma with low grade dysplasia), neuroendocrine tumor (n = 7), cancer (n = 7) (5/7 early colorectal cancer, 2/7 early gastric cancer), granular cell tumor (n = 3), gastrointestinal stromal tumor (n = 2), and leiomyoma (n = 2). En bloc and piecemeal resection rates were 91.6% (55/60) and 8.3% (5/60), respectively. Complete and incomplete resection rates were 96.6% (58/60) and 3.3% (2/60), respectively. Complications were major bleeding [n = 3 (5%)] and perforations [n = 5 (8.3%)] (4 colon, 1 stomach). Two patients with colonic perforations and two patients with submucosal lymphatic and microvasculature invasion (1 gastric carcinoid tumor, 1 colonic adenocarcinoma) were referred to surgery. During a mean follow-up of 12 mo, 1 patient with adenoma with high grade dysplasia underwent a second ESD procedure to resect a local recurrence. CONCLUSION: ESD is a feasible and safe method for treatment of premalignant lesions and early malignant gastrointestinal epithelial and subepithelial lesions. | 2 | Long-term vision-threatening complications of phakic intraocular lens implantation for high myopia显示文摘AIM:To report the long-term vision-threatening complications in patients who underwent phakic intraocular lens(pIOLs)implantation for high myopia.METHODS:Thisstudywasdesignedfromaconsecutive series of phakic intraocular lens complication and corrective surgeries.Sixteen eyes of 13 patients had implantation of phakic intraocular lens for correction high myopia and developed serious complications have been included in this study.The mean age of patients was 38.6±6.35y(range 32-50y)and the mean time of history of pIOL implantation for high myopia was 6±2y(range 2-10y).Before corrective surgery,best spectaclecorrective visual acuity(BSCVA)ranged from perception to 20/200 in the eyes in which severe complications occurred.RESULTS:Corneal decompensation occurred in 12eyes of 9 high myopic patients after anterior chamber pIOL implantation.Rhegmatogenous retinal detachment(RRD)occurred in 4 eyes of 4 high myopic patients following anterior chamber and posterior chamber pIOL implantation.Patients with corneal decompensation,had combined procedures consisting of pIOL removal and penetrating keratoplasty(PKP).Removals of pIOL,phacoemulsification and pars plana vitrectomy(PPV)with silicone oil tamponade were performed in patients with RRD.After corrective surgeries,all patients but one(P+,patient 2,right eye)achieved moderate BSCVA ranged from 20/200 to 20/50 at the last visit.CONCLUSION:Phakic IOLs may be effective for thecorrection of high myopia.Although these IOLs may have severe complications and it affects safety and efficacy of this surgery.As seen here,corneal decompensation and rhegmatogenous retinal detachment are possible postoperative vision-threatening complications of phakic IOLs.Patients must be carefully examined before and after surgery for possible endothelial cell loss and vitreoretinal problems. | Isil Bahar Sayman Muslubas Baran Kandemir Ayse Yesim Aydin Oral Suleyman Kugu Metin Dastan | 2014 | International Journal of Ophthalmology(English edition)2014，7，2： | 14 |

| 4 | A predictive score for retinopathy of prematurity by using clinical risk factors and serum insulin-like growth factor-1 levels显示文摘AIM: To detect the impact of insulin-like growth factor-1(IGF-1) and other risk factors for the early prediction of retinopathy of prematurity(ROP) and to establish a scoring system for ROP prediction by using clinical criteria and serum IGF-1 levels.METHODS: The study was conducted with 127 preterm infants. IGF-1 levels in the 1 st day of life, 1 st, 2 nd, 3 rd and 4 th week of life was analyzed. The score was established after logistic regression analysis, considering the impact of each variable on the occurrences of any stage ROP. A validation cohort containing 107 preterm infants was included in the study and the predictive ability of ROP score was calculated.RESULTS: Birth weights(BW), gestational weeks(GW) and the prevalence of breast milk consumption were lower, respiratory distress syndrome(RDS), bronchopulmonarydysplasia(BPD) and necrotizing enterocolitis(NEC) were more frequent, the duration of mechanical ventilation and oxygen supplementation was longer in patients with ROP(P<0.05). Initial serum IGF-1 levels tended to be lower in newborns who developed ROP. Logistic regression analysis revealed that low BW(<1250 g), presence of intraventricular hemorrhage(IVH) and formula feeding increased the risk of ROP. Afterwards, the scoring system was validated on 107 infants. The negative predictive values of a score less than 4 were 84.3%, 74.7% and 79.8% while positive predictive values were 76.3%, 65.5% and 71.6% respectively.CONCLUSION: In addition to BW <1250 g and IVH, formula consumption was detected as a risk factor for the development of ROP. | 5 | Fatty liver in childhood显示文摘Fatty liver is a growing health problem worldwide. It might evolve to nonalcoholic steatohepatitis, cirrhosis and cause hepatocellular carcinoma. This disease, which has increased because of eating habits, changes in food content and lifestyle, affects people from childhood. The most important risk factors are obesity and insulin resistance. Besides these factors, gender, ethnicity, genetic predisposition and some medical problems are also important. Cirrhosis in children is rare but is reported. Nonalcoholic fatty liver disease(NAFLD) has no specific symptoms or signs but should be considered in obese children. NAFLD does not have a proven treatment. Weight loss with family based treatments is the most acceptable management. Exercise and an applicable diet with low glycemic index and appropriate calorie intake are preferred.
