1Proso Millet Protein Elevates Plasma Level of High-Density Lipoprotein:A New Food Function of Proso Millet显示文摘We examined the effects of dietary proso-millet protein on plasma levels of high-density lipoprotein (HDL) cholesterol in different rats from animals reported in our previous studies. The results showed also, in this animal, that the ingestion of the millet protein elevates plasma levels of HDL-cholesterol like our earlier works. Taking into account the anti-atherogenic function of HDL, therefore, the millet protein would be useful as a new food ingredient which has the function that regulates cholesterolN. NISHIZAWA S. 3Endoscopic ultrasonography can diagnose distal biliary strictures without a mass on computed tomography显示文摘AIM:To assess the diagnostic ability of endoscopic ultrasonography(EUS)for evaluating causes of dista biliary strictures shown on endoscopic retrograde chol angiopancreatography(ERCP)or magnetic resonance cholangiopancreatography(MRCP),even without iden tifiable mass on computed tomography(CT). METHODS:The diagnostic ability of EUS was retro spectively analyzed and compared with that of routine cytology(RC)and tumor markers in 34 patients with distal biliary strictures detected by ERCP or MRCP a Dokkyo Medical School Hospital from December 2005 to December 2008,without any adjacent mass or ec centric thickening of the bile duct on CT that could cause biliary strictures.Findings considered as benign strictures on EUS included preservation of the normasonographic layers of the bile duct wall,irrespective of the presence of a mass lesion.Other strictures were considered malignant.Final diagnosis of underlying diseases was made by pathological examination in 18 cases after surgical removal of the samples,and by clinical follow-up for>10 mo in 16 cases. RESULTS:Seventeen patients(50%)were finally di- agnosed with benign conditions,including 6'normal' subjects,while 17 patients(50%)were diagnosed with malignant disease.In terms of diagnostic ability,EUS showed 94.1%sensitivity,82.3%specificity,84.2% positive predictive value,93.3%negative predictive value(NPV)and 88.2%accuracy for identifying ma- lignant and benign strictures.EUS was more sensi- tive than RC(94.1%vs 62.5%,P=0.039).NPV was also better for EUS than for RC(93.3%vs 57.5%,P= 0.035).In addition,EUS provided significantly higher sensitivity than tumor markers using 100 U/mL as the cutoff level of carbohydrate antigen 19-9(94.1%vs 53%,P=0.017).On EUS,biliary stricture that was fi- nally diagnosed as malignant showed as a hypoechoic, irregular mass,with obstruction of the biliary duct and invasion to surrounding tissues. 5Carbon dioxide insufflation in esophageal endoscopic submucosal dissection reduces mediastinal emphysema: A randomized, double-blind, controlled trial显示文摘AIM To assess the efficacy of CO2 insufflation for reduction of mediastinal emphysema(ME) immediately after endoscopic submucosal dissection(ESD).METHODS A total of 46 patients who were to undergo esophageal ESD were randomly assigned to receive either CO2 insufflation(CO2 group, n = 24) or air insufflation(Air group, n = 22). Computed tomography(CT) was carried out immediately after ESD and the next morning. Pain and abdominal distention were chronologically recorded using a 100-mm visual analogue scale(VAS). The volume of residual gas in the digestive tract was measured using CT imaging. RESULTS The incidence of ME immediately after ESD in the CO2 group was significantly lower than that in the Air group(17% vs 55%, P = 0.012). The incidence of ME the next morning was 8.3% vs 32% respectively(P= 0.066). There were no differences in pain scores or distention scores at any post-procedure time points. The volume of residual gas in the digestive tract immediately after ESD was significantly smaller in the CO2 group than that in the Air group(808 m L vs 1173 m L, P = 0.013).CONCLUSION CO2 insufflation during esophageal ESD significantly reduced postprocedural ME. The present paper addresses the elucidation of the generating mechanism and parameter dependency such as the kind and degree of mistuning. 8Measurement and Numerical Simulation of a Small Centrifugal Compressor Characteristics at Small or Negative Flow Rate显示文摘For centrifugal compressors used in automotive turbochargers, the extension of the surge margin is demanded because of lower engine speed. In order to estimate the surge line exactly, it is required to acquire the compressor characteristics at small or negative flow rate. In this paper, measurement and numerical simulation of the characteristics at small or negative flow rate are carried out. In the measurement, an experimental facility with a valve immediately downstream of the compressor is used to suppress the surge. In the numerical work, a new boundary condition that specifies mass flow rate at the outlet boundary is used to simulate the characteristics around the zero flow rate region. Furthermore, flow field analyses at small or negative flow rate are performed with the numerical results. 10CFD Simulation of Pulsation Noise in a Small Centrifugal Compressor with Volute and Resonance Tube显示文摘The rotational frequency tone noise emitted from the automobile turbocharger is called the pulsation noise. The cause of the pulsation noise is not fully understood, but is considered to be due to some manufacturing errors, which is called the mistuning. The effects of the mistuning of the impeller blade on the noise field inside the flow passage of the compressor are numerically investigated. Here, the flow passage includes the volute and duct located downstream of the compressor impeller. Our numerical approach is found to successfully capture the wavelength of the pulsation noise at given rotational speeds by the comparison with the experiments. 14Angiosarcoma of the scalp diagnosed by the presence of neck inflammation:a case report显示文摘Angiosarcoma is an uncommon malignancy,which spread out from the endothelial cells of vessels.Scalp angiosarcoma with cervical lymph node metastasis is particularly rare.This article describes a rare case of angiosarcoma of the scalp,presenting as neck inflammation.Imaging procedures such as computed tomography(CT),magnetic resonance image(MRI) and ultrasonography(US) were not sufficient to diagnose this case.A needle biopsy provided an effective and accurate diagnosis of cervical lymph node metastasis.Additional observation and physical examination was required to diagnose the origin of the primary cancerous lesion.Once the angiosarcoma diagnosis was confirmed histologically,sequential weekly and monthly docetaxel(DTX) treatment was effective in preventing reoccurrence.Nonetheless,the optimization of angiosarcoma treatment remains a future goal.Although patients generally describe pain and swelling at the primary lesion site,this patient complained only of painful neck inflammation,without any indication of pain or swelling of the scalp.A revised diagnostic protocol should note that cervical lymph node metastasis of unknown primary origin may result from angiosarcoma of the scalp.Rei Karube Hiroyoshi Sasaki Keiji Shinozuka Yasuhiro Fujisawa Toru Yanagawa Kenji Yamagata Kojiro Onizawa Fujio Otsuka Hiroki Bukawa 2012International Journal of Oral Science2012,4,3:2
