3Emergency bedside ultrasound for the diagnosis of pediatric intussusception: a retrospective review显示文摘BACKGROUND: Ultrasound diagnosis of pediatric intussusception is feasible with limited operator training. The authors report the test characteristics of bedside ultrasound(BUS) for the diagnosis of pediatric intussusception at a single institution.METHODS: Inclusion criteria were: 1) patients of 0–18 years old were seen in the pediatric emergency department(ED) with a clinical presentation suspicious for intussusception; 2) BUS was performed to identify intussusception and bedside impression documented in the medical record; 3) a 'formal' diagnostic study(such as computed tomography, ultrasound, or barium enema) was performed by the radiology department after BUS was completed. Electronic medical record and ED BUS archive were screened for potentially eligible ED visits between January 1st 2009 to October 3rd 2012. These records were then reviewed to identify patients for inclusion in the study. All emergency physicians who performed the BUS had undergone a minimum of 1-hour didactic training on the use of BUS to diagnose pediatric intussusception.RESULTS: A total of 1 631 charts were reviewed, with 49 meeting inclusion criteria. Five of those were later excluded for incomplete documentation or lack of saved BUS images. The prevalence of intussusception was 23%. The mean age of the subjects was 31 months. BUS was 100% sensitive(95%CI 66%–100%) and 94% specifi c(95%CI 79%–99%) for detection of pediatric intussusception compared to radiology study results. Positive and negative likelihood ratios were 16.5(95%CI 4.30%–63.21%) and 0(95%CI 0–0) respectively.CONCLUSIONS: BUS is an accurate means of diagnosing acute intussusception in pediatric patients. 4Drug sensitivity and drug resistance profiles of human intrahepatic cholangiocarcinoma cell lines显示文摘AIM: To study the effect of a number of chemotherapeutic drugs on five human intrahepatic cholangiocarcinoma (CCA) cell lines. The expressions of genes that have been proposed to influence the resistance of chemotherapeutic drugs including thymidylate synthase (TS), dihydropyrimidine dehydrogenase (DPD), glutathione-S-transferase P1 (GSTP1), multidrug resistance protein (MDR1) and multidrug resistance-associated proteins (MRPs) were also determined.METHODS: Five human CCA cell lines (KKU-100, KKU M055, KKU-M156, KKU-M214 and KKU-OCA17) weretreated with various chemotherapeutic drugs and growth inhibition was determined by 3-(4,5-dimethylthiazol-2-yl)5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium (MTS) assay. Semi-quantitative levels of gene expression were determined by a reverse transcriptase polymerase chain reaction (RT-PCR). Results of IC50 values and the ratios of gene expression were analyzed by linear regression to predict their relationship. RESULTS: Among five CCA cell lines, KKU-M055 was the most sensitive cell line towards all chemotherapeutic drugs investigated, particularly taxane derivatives with IC50 values of 0.02-3 nmol/L, whereas KKU-100 was apparently the least sensitive cell line. When compared to other chemotherapeutic agents, doxorubicin and pirarubicin showed the lowest IC50 values (<5 μmol/L) in all five CCA cell lines. Results from RT-PCR showed that TS, MRP1, MRP3 and GSTP1 were highly expressed in these five CCA cell lines while DPD and MRP2 were only moderately expressed. It should be noted that MDR1 expression was detected only in KKU-OCA17 cell lines. A strong correlation was only found between the level of MRP3 expression and the IC50 values of etoposide, doxorubicin and pirarubicin (r = 0.86-0.98, ,P<0.05). CONCLUSION: Sensitivity to chemotherapeutic agents is not associated with the histological type of CCA. Choosing of the appropriate chemotherapeutic regimen for the treatment of CCA requires knowledge of drug sensitivity. MRP3 was correlated with resistance of CCA cell lines to etoposide, doxorubicin and pirarubicin, whereas other chemotherapeutic drugs showed no association. 6中国西南地区儿童土源性线虫感染及影响因素调查显示文摘目的了解中国西南贫困地区儿童土源性线虫感染情况和主要影响因素,为制定土源性线虫病防治措施提供依据。方法采用改良加藤厚涂片法对受检儿童粪便进行病原学检查,其中部分儿童进行透明胶纸肛拭法检查蛲虫卵。采用问卷调查和Probit估计多元统计方法,分析儿童服药驱虫、个体特征、饮食卫生习惯和家庭特征等16个相关因素对土源性线虫感染的影响。结果共粪检1 707名儿童,土源性线虫感染率为22.2%,其中蛔虫、钩虫、鞭虫感染率分别为16.9%、3.8%和6.6%,且以蛔虫感染率和感染度为最高;蛲虫检查495名,感染率为5.1%。Probit估计多元统计显示,兄弟姐妹数量、母亲受教育程度、饮用生水、饲养畜禽等4个因素差异均有统计学意义(P均<0.05)。其中,母亲受教育程度能降低土源性线虫感染率,边际效应为-0.074;兄弟姐妹数量、饮用生水和饲养畜禽均能提高土源性线虫感染率,边际效应分别为0.028、-0.112和0.080。结论我国西南贫困地区儿童土源性线虫感染率仍然较高,且以蛔虫感染为主;改变儿童饮用生水的不良卫生习惯、家庭饲养畜禽习惯和针对其母亲开展寄生虫病防治知识的健康教育,对儿童土源性线虫病的预防和控制具有重要意义。王晓兵 王国飞 张林秀 罗仁福 田洪春 唐丽娜 王聚君 Alexis Medina Paul Wise Scott Rozelle 2012中国血吸虫病防治杂志2012,24,3:10

9Mechanisms of biliary carcinogenesis and growth显示文摘Cholangiocarcinoma is a rare cancer originating from the neoplastic transformation of the epithelial cells (i.e. cholangiocytes) that line the biliary tract. The prognosis for patients with cholangiocarcinoma is grim due to lack of viable treatment options. The increase in world-wide incidence and mortality from cholangiocarcinoma highlights the importance of understanding the intracellular mechanisms that trigger the neoplastic transformation of cholangiocytes and the growth of biliary cancers. The purpose of the following review is to address what has been learned over the past decade concerning the molecular basis of cholangiocarcinogenesis. The material presented is divided into two sections:(1) mechanisms regulating neoplastic transformation of cholangiocytes; and (2) factors regulating cholangiocarcinoma growth. 10Apoptotic activity of caged xanthones from Garcinia hanburyi in cholangiocarcinoma cell lines显示文摘AIM:To investigate the growth inhibitory mechanism of four caged xanthones from Garcinia hanburyi in cholangiocarcinoma(CCA) KKU-100 and KKU-M156 cells.METHODS:Four caged xanthones,selected on the basis of their anticancer potency and chemical structure diversities(i.e.isomorellin,isomorellinol,forbesione and gambogic acid) were used in this study.Growth inhibition of these caged xanthones was determined using the sulforhodamine B assay.Induction of apoptosis was assessed by observing cell morphology,ethidium bromide and acridine orange staining and DNA fragmentation assay.Levels of apoptotic-related gene and protein expressions were determined by a real-time reverse transcriptase polymerase chain reaction and Western blotting analysis,respectively.RESULTS:The compounds were found to inhibit growth of both cell lines in a dose-dependent manner and also showed selective cytotoxicity against the cancer cells when compared with normal peripheral blood mononuclear cells.Growth suppression by these compounds was due to apoptosis,as evidenced by the cell morphological changes,chromatin condensation,nuclear fragmentation,and DNA ladder formation.At the molecular level,these compounds induced down-regulation of Bcl-2 and survivin proteins with up-regulation of Bax and apoptosisinducing factor proteins,leading to the activation of caspase-9 and-3 and DNA fragmentation.The functional group variations did not appear to affect the anticancer activity with regard to the two CCA cell lines;however,at a mechanistic level,isomorellinol exhibited the highest potency in increasing the Bax/Bcl-2 protein expression ratio(120 and 41.4 for KKU-100 and KKU-M156,respectively) and in decreasing survivin protein expression(0.01 fold as compared to control cells in both cell lines).Other activities at the molecular level indicate that functional groups on the prenyl side chain may be important.CONCLUSION:Our findings for the first time demonstrate that four caged xanthones induce apoptosis in CCA cells which is mediated through a mitochondriadependent signaling pathway.Chariya Hahnvajanawong Wongwarut Boonyanugomol Tapanawan Nasomyon Watcharin Loilome Nisana Namwat Natthinee Anantachoke Wichittra Tassaneeyakul Banchob Sripa Wises Namwat Vichai Reutrakul 2010World Journal of Gastroenterology2010,16,18:5
11High efficiency, high power 808nm laser array and stacked arrays optimized for elevated temperature operation显示文摘Operation of 808-nm laser diode pumping at elevated temperature is crucial to many applications. Reliable operation at high power is limited by high thermal load and low catastrophic optical mirror damage (COMD) threshold at elevated temperature range. We demonstrated high efficiency and high power operation at elevated temperature with high COMD power. These results were achieved through device design optimization such as growth conditions, doping profile, and materials composition of the quantum-well and other layers. Electrical-to-optical efficiency as high as 62% was obtained through lowered threshold current, lowered series resistance and increased slope efficiency. The performance of single broad-area laser diodes scales to that of high power single bars on water-cooled copper micro-channel heatsinks or conductively-cooled CS heatsinks. 12Identification of pathologic features associated with “ulcerative colitis-like” Crohn's disease显示文摘AIM: To identify pathologic features associated with this 'ulcerative colitis(UC)-like' subgroup of Crohn's disease(CD).METHODS: Seventeen subjects diagnosed as having UC who underwent proctocolectomy(RPC) from 2003-2007 and subsequently developed CD of the ileal pouch were identified. UC was diagnosed based on preoperative clinical, endoscopic, and pathologic studies. Eighteen patients who underwent RPC for UC within the same time period without subsequently developing CD were randomly selected and used as controls. Pathology reports and histological slides were reviewed for a wide range of gross and microscopic pathological features, as well as extent of disease. The demographics, gross description and histopathology of the resection specimens were reviewed and compared between the two groups. RESULTS: Patients with 'UC-like' CD were on average 13 years younger than those with 'true' UC(P < 0.01). More severe disease in the proximal involved region and active ileitis with/without architectural distortion were observed in 6 of 17(35%) and 7 of 17(41%) 'UC-like' CD cases, respectively, but in none of the 'true' UC cases(P < 0.05). Active appendicitis occurred in 8 of 16(50%) 'UC-like' CD cases but in only two(11%) 'true' UC cases(P < 0.05). Conspicuous lamina propria neutrophils were more specific for 'UClike' CD(76% vs 22%, P < 0.05). In addition, prominent lymphoid aggregates tended to be more common in 'UC-like' CD(P = 0.07). The 'true' UC group contained a greater number of cases with severe activity(78% vs 47%). Therefore, the features more commonly seen in 'UC-like' CD were not due to a more severe disease process. 16脐血单核细胞移植治疗慢性完全性脊髓损伤Ⅱ期临床试验的安全性和疗效评估显示文摘目的评估人类白细胞抗原(HLA≥4:6)配型的脐血单核细胞(UCBMC)移植治疗慢性完全性脊髓损伤(SCI)的安全性、可行性、有效性以及最佳细胞剂量和联合使用甲基强的松龙(MP)、碳酸锂对细胞移植的作用。方法选取联勤保障部队第九二〇医院神经外科自2011年2月至2014年1月收治的20例SCI患者,按照随机数字表法分为5组(A、B、C、D、E组),每组4例受试者。在患者损伤上下缘脊神经背根入处的4个点(上下各2点)注射UCBMC,A、B、C组分别为每点各注射4、8和16μL(100000 UCBMC/μL,640万细胞)到SCI部位的上方和下方脊神经背根入处,D组为16μL细胞注射加30 mg/kg MP静脉注射,E组为16μL细胞注射加MP和口服碳酸锂(750 mg/d),一个疗程6周。受试者进行3~6个月的强化步行训练,以美国脊髓损伤协会(ASIA)为主要评分指标,脊髓损伤步行指数(WISCI)和脊髓独立量表(SCIM)为次要评分指标。结果20例受试者的SCI平面分别位于C3~T11,平均受伤7年。治疗前,16例(80%)患者行走<10 m,18例(90%)患者自理生活需要协助。治疗后41~87周,受试者ASIA运动评分无明显变化,而WISCI和SCIM评分明显提高。15例(75%)患者可在辅助下步行10 m,12例(60%)患者恢复膀胱和肠道控制,5例(25%)患者由完全性SCI变为不完全性SCI;所有患者无神经功能丢失。结论UCBMC移植和步行训练能提高慢性完全性SCI患者的WISCI和SCIM评分,但对运动评分没有改善,因此推测UCMBC移植能促进轴突的生长,从而激活中枢模式发生器,改善运动、排尿和排便等功能,而且联合使用MP和碳酸锂对细胞移植治疗无明显作用。封亚平 Wise Young 朱辉 封雨 邓洵鼎 谢佳芯 张社敏 方芳 石静 陈永和 郑爽 2020中华神经创伤外科电子杂志2020,6,4:2

20Concealed fault analysis based on the CT projection matrix显示文摘This paper proposes the concept of projection curves based on the theory of CT image reconstruction to probe the internal structure of the working panel prior to formal mining of the working panel.As well as reducing costs,this method provides safe and efficient excavation of the working panel.According to the results of the numerical model and the actual working panel,the new method has been proven to be accurate in detecting the location of the fault that extends into the face.Concealed faults of the internal working panel,as well as the start and end points of the fault,can be detected by this method.Engineering practice has proven that the method is highly reliable,has a highly decisive impact on faults for coal mining,and can be used to guide the safe mining of the working panel.Yang Zhen Yao Wenli Ma Liuzhu Wise Lucas 2016International Journal of Mining Science and Technology2016,26,2:2
