| 2 | Current management of spontaneous intracerebral haemorrhage显示文摘Intracerebral haemorrhage(ICH)is the most devastating and disabling type of stroke.Uncontrolled hypertension(HTN)is the most common cause of spontaneous ICH.Recent advances in neuroimaging,organised stroke care,dedicated Neuro-ICUs,medical and surgical management have improved the management of ICH.Early airway protection,control of malignant HTN,urgent reversal of coagulopathy and surgical intervention may increase the chance of survival for patients with severe ICH.Intensive lowering of systolic blood pressure to<140 mm Hg is proven safe by two recent randomised trials.Transfusion of platelets in patients on antiplatelet therapy is not indicated unless the patient is scheduled for surgical evacuation of haematoma.In patients with small haematoma without significant mass effect,there is no indication for routine use of mannitol or hypertonic saline(HTS).However,for patients with large ICH(volume>30 cbic centmetre)or symptomatic perihaematoma oedema,it may be beneficial to keep serum sodium level at 140–150 mEq/L for 7–10 days to minimise oedema expansion and mass effect.Mannitol and HTS can be used emergently for worsening cerebral oedema,elevated intracranial pressure(ICP)or pending herniation.HTS should be administered via central line as continuous infusion(3%)or bolus(23.4%).Ventriculostomy is indicated for patients with severe intraventricular haemorrhage,hydrocephalus or elevated ICP.Patients with large cerebellar or temporal ICH may benefit from emergent haematoma evacuation.It is important to start intermittent pneumatic compression devices at the time of admission and subcutaneous unfractionated heparin in stable patients within 48 hours of admission for prophylaxis of venous thromboembolism.There is no benefit for seizure prophylaxis or aggressive management of fever or hyperglycaemia.Early aggressive comprehensive care may improve survival and functional recovery. | Cyrus K Dastur Wengui Yu | 2017 | Stroke & Vascular Neurology2017,2,1: | 18 |
| 3 | Stenting for intracranial stenosis:potential future for the prevention of disabling or fatal stroke显示文摘Intracranial stenosis is a common cause of ischaemic strokes,in particular,in the Asian,African and Hispanic populations.The randomised multicentre study Stenting and Aggressive Medical Management for the Prevention of Recurrent stroke in Intracranial Stenosis(SAMMPRIS)showed 14.7% risk of stroke or death in the stenting group versus 5.8% in the medical group at 30 days,and 23% in the stenting group versus 15% in the medical group at a median follow-up of 32.4 months.The results demonstrated superiority of medical management over stenting and have almost put the intracranial stenting to rest in recent years.Of note,16 patients(7.1%)in the stenting group had disabling or fatal stroke within 30 days mostly due to periprocedural complications as compared with 4 patients(1.8%)in the medical group.In contrast,5 patients(2.2%)in the stenting group and 14 patients(6.2%)in the medical group had a disabling or fatal stroke beyond 30 days,indicating significant benefit of stenting if periprocedural complications can be reduced.Recently,the results of the Chinese Angioplasty and Stenting for Symptomatic Intracranial Severe Stenosis trial and the Wingspan Stent System Post Market Surveillance Study(WEAVE trial)showed 2%-2.7% periprocedural complications.It is time to evaluate the role of intracranial stenting for the prevention of disabling or fatal stroke. | Wengui Yu Wei-Jian Jiang | 2018 | Stroke & Vascular Neurology2018,3,3: | 11 |
| 4 | Effects of Jisuikang on hemorheology and inflammatory factors in rats following spinal cord injury显示文摘BACKGROUND:Trauma can damage the spinal cord or cauda equina to different degrees. Previous studies have verified that traditional Chinese medicine has effects on spinal cord injury via a variety of pathways. OBJECTIVE:To observe changes in hemorheology and inflammatory factors in spinal cord injury rats following treatment with the Chinese medicine Jisuikang,to verify the dose-dependent effect of Jisuikang,and to compare its effects with the effects of prednisone. DESIGN,TIME AND SETTING: A randomized study was performed at the Research Institute of Orthopedics,and Experimental Center of First Clinical Medical College,Nanjing University of Traditional Chinese Medicine,China from September 2007 to March 2008. MATERIALS: Jisuikang powdered extract,composed of milkvetch root (30g),Chinese angelica (12g),red peony root (12g),earthworm (10g),szechwan lovage rhizome (10g),peach seed (10g) and safflower (10g),was provided by the Experimental Center,First Clinical Medical College,Nanjing University of Traditional Chinese medicine. Each gram of powdered extract was equivalent to 6.47 g crude drug. METHODS: A total of 72 Sprague Dawley rats were randomly assigned into 6 groups (n = 12). Rat models of spinal cord injury were established using the occlusion method. Rats in the model group were treated with distilled water. Rats in the 25 g/kg,12.5 g/kg,and 6.25 g/kg Jisuikang groups were given 25 g/kg,12.5 g/kg,or 6.25 g/kg Jisuikang by gavage,for 14 days. Rats in the prednisone group received 0.06 g/kg prednisone by gavage,for 7 days. Rats in the normal group were given the same volume of distilled water. The volume of administration was 15 mL/kg. MAIN OUTCOME MEASURES: Rat serum interleukin-10,tumor necrosis factor-α (TNF-α),nitric oxide,nitric oxide synthase levels,malondialdehyde content,superoxide dismutase activity and whole blood viscosity were measured in each group. Spinal cord around the site of the model was collected. Half the spinal cord was used for histopathologic examination. The other half was used for measurement of nitric oxide and NOS levels,malondialdehyde contents,and superoxide dismutase activity. RESULTS: Superoxide dismutase activity was higher in the 25 g/kg Jisuikang group than in the model group. Malondialdehyde contents,nitric oxide and NOS levels were lower in the 25 g/kg and 12.5 g/kg Jisuikang groups compared with the model group. Whole blood viscosity was lower in the 25 g/kg and 12.5 g/kg Jisuikang groups compared with the model group (P < 0.05-0.01). Serum TNF-αcontent was lower in each Jisuikang group compared with the model group (P < 0.05-0.01). Serum interleukin-10 levels were greater in the prednisone group and each Jisuikang group compared with the model group (P < 0.01). Mild hemorrhage and necrosis in the rat spinal cord,and unclear neural cell swelling were seen in the 25 g/kg Jisuikang group. Severe hemorrhage and necrosis in the rat spinal cord,and distinct neural cell swelling were seen in the 12.5 g/kg Jisuikang group. Edema in the white matter was found in the 6.25 g/kg Jisuikang group. Pathological changes in the prednisone group were identical to the 25 g/kg and 12.5 g/kg Jisuikang groups. CONCLUSION: Jisuikang inhibits nitric oxide synthase expression,reduces nitric oxide and TNF-α levels,decreases malondialdehyde content,increases interleukin-10 levels and superoxide dismutase activity,improves indices of hemorheology,and prevents secondary changes in spinal cord injury,resulting in relieving pathological changes in spinal cord tissue. | 5 | The Holocene Activity Evidence of the Yema River-Daxue Mountain Fault in Western Qilian Mountain显示文摘Altyn Tagh fault controls the deformation characteristics of the northern margin of the Qinghai-Tibet Plateau.The sinistral slip rate of the eastern segment of the fault reduces gradually where the reduction transforms into the deformation within Qilian Mountain,forming a series of thrust faults and strike-slip faults.Among them,the Yema River-Daxue Mountain fault is one of the important structural transform faults in the study area.Based on the differences of the geometrical characteristics and activities,the fault is divided into four segments,the Yema River segment,the Shibandun segment,the Liushapo segment and the Baishitougou segment,among which the former three are Holocene active faults,and the Baishitougou segment belongs to late Pleistocene fault.The excavated trenches imply a total of 6paleoearthquake events,and at least 4 events have occurred during Holocene,whose occurrence times are 8300±700 yr BP,6605±140 yr BP,4540±350 yr BP,2098±47 yr BP,respectively.The recurrence interval is 2600±600 yr BP that is close to the lapsed time of the last one,2098±47 yr BP,which suggests that the Yema River-Daxue Mountain fault is in a high risk of major earthquakes in the future.The vertical coseismic displacements of the four Holocene paleoearthquake events are 100 cm,42 cm,40 cm and 50 cm,respectively,the horizontal coseismic displacements are 5 m,4.5–5.5 m,5–8 m and 4–5.5 m,separately,and then the reference magnitude of the paleoearthquake events is conjectured to be M7.6±0.1. | LUO Hao HE Wengui YUAN Daoyang SHAOYanxiu | 2013 | Acta Geologica Sinica(English Edition)2013,87,6: | 7 |
| 6 | First Discovery of North-South Striking Normal Faults near the Potential Eastern End of Altyn Tagh Fault显示文摘The Altyn Tagh fault is the northern boundary of Tibetan Plateau. As one of the most well-known strike-slip fault in the world, great achievements on tectonic deformation and Late Quaternary slip rate have been made. However, there is a long-lasting debate on whether the Altyn Tagh fault extends into the Jinta Basin or even eastward. In this paper, we use satellite image interpretation, field investigation, trench excavation, and optical stimulated luminescence dating to study newly found NS striking scarps in the eastern end of Jinta Nan Shan. The results are as follows: firstly, a group of normal faults develop on terrace T2 of Heihe River, the total length amounts to ~40 km, total scarp height is 30±5 m; secondly, four paleoseismic events have been interpreted from three trenches, approximate ages of events are 79.97±19.14 ka BP, 62.55±13.10~55.41±10.77 ka BP, before 16.89±2.08 ka BP, 8.52±1.49 ka BP, respectively; thirdly, just like NS normal faults in the western end of Altyn Tagh fault, the newly found NS extensional faults are likely the terminating tectonics of the eastern end of Altyn Tagh fault, the large Altyn Tagh fault may end in the eastern end of Jinta Nan Shan. | Bo Zhang Daoyang Yuan Wengui He Wei Pang Pengtao Wang Ming Wu | 2018 | Journal of Earth Science2018,29,1: | 6 |

| 8 | 20 Years Advances in Preparation of Potassium Salts from Potassic Rocks:A Review显示文摘Long-term research on key techniques of clean utilization of potassic rocks from over twenty localities has been performed to settle the shortage of soluble potassium resources in China. The results show that K-feldspar as the principal mineral enriched in potassium could be decomposed in the media of Ca(OH)_2, NaOH, KOH-H_2O solution via hydrothermal treatment, into tobermorite, hydroxylcancrinite, and kalsilite respectively. By further processing, these compounds are feasible for being as slow-release carrier of potassium nitrate, extracting alumina, and preparing farm-oriented fertilizers of potassium sulphate and nitrate. Correspondingly, the filtrate is KOH,(Na, K)_2SiO_3, and K_2SiO_3 solution, from which potassium carbonate, sulphate, nitrate, and phosphate could be easily fabricated. As NaO H and KOH are recycled in the processing chains by causticizing sodium and/or potassium metasilicate solutions, the hydrothermal alkaline techniques as developed in this research have several advantages as lower consumption of disposable mineral resources and energy, maximized utilization of potassic mineral resources, as well as clean productions etc. Based on the approaches presented in this paper, the technical system of efficiently utilizing insoluble potassium resources has been established. | 9 | Slip Rate of Yema River–Daxue Mountain Fault since the Late Pleistocene and Its Implications on the Deformation of the Northeastern Margin of the Tibetan Plateau显示文摘The slip rate of Yema River–Daxue Mountain fault in the western segment of Qilian Mountains was determined by the dated offset of river risers or gullies. Results indicate that the left-lateral fault slip rate is 2.82 ± 0.20 mm/a at Dazangdele site,2.00 ± 0.24 mm/a at Shibandun site,and 0.50 ± 0.36 and 2.80 ± 0.33 mm/a at two sites in Zhazihu. The ideal average slip rate of the whole fault is 2.81 ± 0.32 mm/a. The lower slip rate confirms part of the displacement of Altyn Tagh fault was transformed into an uplifting of the strap mountains in the western segment of Qilian Mountains,whereas another part transformed into sinistral displacement of Haiyuan fault. | 10 | Preclinical and clinical applications of specific molecular imaging for HER2-positive breast cancer显示文摘Precision medicine and personalized therapy are receiving increased attention, and molecular-subtype classification has become crucial in planning therapeutic schedules in clinical practice for patients with breast cancer. Human epidermal growth factor receptor 2(HER2) is associated with high-grade breast tumors, high rates of lymph-node involvement, high risk of recurrence, and high resistance to general chemotherapy. Analysis of HER2 expression is highly important for doctors to identify patients who can benefit from trastuzumab therapy and monitor the response and efficacy of treatment. In recent years, significant efforts have been devoted to achieving specific and noninvasive HER2-positive breast cancer imaging in vivo. A trust management mechanism based on the jury system for distributed cognitive radio networks is proposed in this paper. The 'jury user' is designed to collaboratively examine the reputation of the cognitive user in the networks and to perform data fusion and spectrum allocation for distributed cognitive radio networks.
