| 1 | Device-associated infection rates, mortality, length of stay and bacterial resistance in intensive care units in Ecuador: International Nosocomial Infection Control Consortium's findings显示文摘AIM To report the results of the International Nosocomial Infection Control Consortium(INICC) study conducted in Quito, Ecuador.METHODS A device-associated healthcare-acquired infection(DAHAI) prospective surveillance study conducted from October 2013 to January 2015 in 2 adult intensive care units(ICUs) from 2 hospitals using the United States Centers for Disease Control/National Healthcare Safety Network(CDC/NHSN) definitions and INICC methods. RESULTS We followed 776 ICU patients for 4818 bed-days. The central line-associated bloodstream infection(CLABSI) rate was 6.5 per 1000 central line(CL)-days, the ventilator-associated pneumonia(VAP) rate was 44.3 per 1000 mechanical ventilator(MV)-days, and the catheterassociated urinary tract infection(CAUTI) rate was 5.7 per 1000 urinary catheter(UC)-days. CLABSI and CAUTI rates in our ICUs were similar to INICC rates [4.9(CLABSI) and 5.3(CAUTI)] and higher than NHSN rates [0.8(CLABSI) and 1.3(CAUTI)]- although device use ratios for CL and UC were higher than INICC and CDC/NSHN's ratios. By contrast, despite the VAP rate was higher than INICC(16.5) and NHSN's rates(1.1), MV DUR was lower in our ICUs. Resistance of A. baumannii to imipenem and meropenem was 75.0%, and of Pseudomonas aeruginosa to ciprofloxacin and piperacillin-tazobactam was higher than 72.7%, all them higher than CDC/NHSN rates. Excess length of stay was 7.4 d for patients with CLABSI, 4.8 for patients with VAP and 9.2 for patients CAUTI. Excess crude mortality in ICUs was 30.9% for CLABSI, 14.5% for VAP and 17.6% for CAUTI. | 2 | Stem cell therapy in the management of shoulder rotator cuff disorders显示文摘Rotator cuff tears are frequent shoulder problems that are usually dealt with surgical repair. Despite improved surgical techniques, the tendon-to-bone healing rate is unsatisfactory due to difficulties in restoring the delicate transitional tissue between bone and tendon. It is essential to understand the molecular mechanisms that determine this failure. The study of the molecular environment during embryogenesis and during normal healing after injury is key in devising strategies to get a successful repair. Mesenchymal stem cells(MSC) can differentiate into different mesodermal tissues and have a strong paracrine, anti-inflammatory, immunoregulatory and angiogenic potential. Stem cell therapy is thus a potentially effective therapy to enhance rotator cuff healing. Promising results have been reported with the use of autologous MSC of different origins in animal studies: they have shown to have better healing properties, increasing the amount of fibrocartilage formation and improving the orientation of fibrocartilage fibers with less immunologic response and reduced lymphocyte infiltration. All these changes lead to an increase in biomechanical strength. However, animal research is still inconclusive and more experimental studies are needed before human application. | 3 | Functional leaf and size traits determine the photosynthetic response of 10 dryland species to warming显示文摘Aims Relatively few studies so far have assessed how ongoing global warming will affect the photosynthetic performance of dryland plant species.We evaluated the effects of warming on the photosynthetic rates of 10 species with contrasting functional attributes,and whether their functional traits modulated photosynthetic responses to warming.Methods A common garden experiment was conducted over 2 years with distinct environmental conditions(drier vs.wetter year).The experiment was designed as a randomized block design with two treatments:warming(control vs.~2.9℃ temperature increase)and species(Agropyron cristatum,Festuca ovina,Lygeum spartum,Medicago sativa,Plantago lanceolata,Psoralea bituminosa,Sanguisorba minor,Hedysarum coronarium,Dorycnium pentaphyllum and Phlomis herba-venti).We linked functional traits measurements with temporal variations in photosynthetic responses to warming.Important Findings In the drier year,warming increased photosynthetic rates at the beginning of the growing season,suggesting a modification in the growing period(earlier spring).In the wetter year,functional traits modulated photosynthetic responses to warming.Larger species with shorter leaves(e.g.M.sativa)had higher photosynthetic rates under warming compared to smaller species with larger leaves(e.g.F.ovina).Our results highlight the importance of(i)studying photosynthetic responses along different years and(ii)considering functional traits when evaluating photosynthetic responses to climate change,particularly in stressful environments such as drylands. | Enrique Valencia JoséL.Quero Fernando T.Maestre | 2016 | Journal of Plant Ecology2016,9,6: | 2 |
