It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. | 3 | Puerarin protects rat brain against ischemia/reperfusion injury by suppressing autophagy via the AMPK-mT OR-ULK1 signaling pathway显示文摘Puerarin suppresses autophagy to alleviate cerebral ischemia/reperfusion injury, and accumulating evidence indicates that the AMPKm TOR signaling pathway regulates the activation of the autophagy pathway through the coordinated phosphorylation of ULK1. In this study, we investigated the mechanisms underlying the neuroprotective effect of puerarin and its role in modulating autophagy via the AMPK-m TOR-ULK1 signaling pathway in the rat middle cerebral artery occlusion model of cerebral ischemia/reperfusion injury. Rats were intraperitoneally injected with puerarin, 50 or 100 mg/kg, daily for 7 days. Then, 30 minutes after the final administration, rats were subjected to transient middle cerebral artery occlusion for 90 minutes. Then, after 24 hours of reperfusion, the Longa score and infarct volume were evaluated in each group. Autophagosome formation was observed by transmission electron microscopy. LC3, Beclin-1 p62, AMPK, m TOR and ULK1 protein expression levels were examined by immunofluorescence and western blot assay. Puerarin substantially reduced the Longa score and infarct volume, and it lessened autophagosome formation in the hippocampal CA1 area following cerebral ischemia/reperfusion injury in a dose-dependent manner. Pretreatment with puerarin(50 or 100 mg/kg) reduced Beclin-1 expression and the LC3-II/LC3-I ratio, as well as p-AMPK and p S317-ULK1 levels. In comparison, it increased p62 expression. Furthermore, puerarin at 100 mg/kg dramatically increased the levels of p-m TOR and p S757-ULK1 in the hippocampus on the ischemic side. Our findings suggest that puerarin alleviates autophagy by activating the APMK-m TOR-ULK1 signaling pathway. | 7 | Variations of pore water sulfate gradients in sediments as indicator for underlying gas hydrate in Shenhu Area, the South China Sea显示文摘Shenhu Area is one of the most promising areas for gas hydrate exploration in the northern South China Sea (SCS). Pore water sulfate gradient, sulfate-methane interface (SMI) depth, and sulfate flux were analyzed at 53 sites in this area. SO42 gradient ranges between 0.33 and 4.43 mmol L-1 m-1 . SMI depths are from 7.7 to 87.9mbsf. Sulfate flux varies between 2.0 and 26.9 mmol m-2 yr-1 , with a mean of 11.7 mmol m-2 yr-1 . Correlation coefficient between SMI depth and methane flux for the 53 sites is 0.80, implying that methane flux regulates the rate of anaerobic methane oxidation (AMO), SMI depth, and sulfate flux. Twelve anomalous fields with high methane flux and steep sulfate gradients were recognized. Bottom simulating reflector (BSR) is distributed mainly in areas where SMI depth is less than 50 mbsf or places with sulfate flux larger than 3.5 mmol m-2 yr-1 . | 16 | Liver transplant recipients with portal vein thrombosis:a single center retrospective study显示文摘BACKGROUND:Portal vein thrombosis(PVT)used to be a contraindication for liver transplantation(LT).This obstacle has been delt with following the improvement of LT-related techniques and therapeutic approaches to thrombosis. But the effect of PVT on LT outcomes is still controversial. We reviewed retrospectively the outcome of LT patients with PVT as well as risk factors and surgical management according to PVT grades. METHODS:A total of 465 adult LTs were performed from December 2002 through December 2006.Operative findings and the result of preoperative ultrasonography and imaging were reviewed for PVT grading(Yerdel grading).Comparison of risk factors,variables associated with perioperative period and prognosis between recipients with and without PVT is based on the grades. RESULTS:In the 465 LTs,42 were operatively confirmed to have PVT(9.0%)(19 recipients with grade 1,14 with grade 2,7 with grade 3,and 2 with grade 4).Increased age and treatment of portal hypertension were associated with PVT.Grade 1 or 2 PVT was treated by direct anastomosis or single thrombectomy.In grade 3 PVT patients,the donor PV was directly anastomosed to the dilated branch of the recipient portal venous system or to the distal open superior mesenteric vein through an interposition vein graft if needed.Grade 4 PVT was managed by our modified cavoportal hemitransposition technique.The comparison between PVT patients and controls showed no significant difference in operative duration and blood transfusion(P>0.05).The flow rate of the PV was lower in the PVT patients(48.881±12.788 cm/s)than in the controls(57.172±21.715 cm/s,P<0.05).The PVT patients had such postoperative complications as renal failure and PV rethrombosis(P<0.05).The 1-year survival rates in PVT and non-PVT patients were 78.6%and 76.4%respectively (P>0.05);the 3-year survival rates were 58.8%and 56.4% respectively(P>0.05). Higher antioxidant enzyme activities and lower malonaldehyde (MDA) content in rice anthers were observed in Jin 23B than in Zhenshan 97B under drought stress. Further, a great increase in the content of soluble sugar in rice anthers of Jin 23B was observed across the whole drought exposure, while Zhenshan 97B showed significant decrease in soluble sugar during 9-12 d after drought stress (DADS). Accordingly, a marked decline of pollen fertility and activity, pollen numbers in an anther and pollen numbers on a stigma was observed in Zhenshan 97B, whereas little difference was found in Jin 23B. | 19 | Construction and external validation of a nomogram that predicts lymph node metastasis in early gastric cancer patients using preoperative parameters显示文摘Objective: To create a nomogram to predict the incidence of lymph node metastasis(LNM) in early gastric cancer(EGC) patients and to externally validate the nomogram.Methods: To construct the nomogram,we retrospectively analyzed a primary cohort of 272 EGC patients.Univariate analysis and a binary logistic regression were performed.A nomogram predicting the incidence of LNM in EGC patients was created.The discrimination ability of the nomogram was measured using the concordance index(c-index),and the nomogram was also calibrated.Then,another prospective cohort of 81 patients was analyzed to validate the nomogram.Results: In the primary cohort,LNM was pathologically confirmed in 37(13.6%) patients.In multivariate analysis,the presence of an ulcer,the maximum lesion diameter observed via gastroscopy,the thickness of the lesion observed via endoscopic ultrasonography,and the presence of enlarged lymph nodes on computed tomography(CT) were independent risk factors for LNM.A nomogram was then created based on the regression model with the c-index of 0.905,and the calibration curve of the nomogram fell approximately on the ideal 45-degree line.The cut-off score of the nomogram was 110,and the sensitivity,specificity,positive predictive and negative predictive values of the nomogram in the primary cohort were 81.1%,86.0%,47.6% and 96.7%,respectively,and in the prospective validation cohort were 75.0%,91.0%,60.0% and 95.5%,respectively.The calibration curve of the external validation cohort was almost on the 45-degree line.Conclusions: We developed an effective nomogram predicting the incidence of LNM for EGC patients. | Yinan Zhang Yiqiang Liu Ji Zhang Xiaojiang Wu Xin Ji Tao Fu Ziyu Li Qi Wu Zhaode Bu Jiafu Ji | 2018 | Chinese Journal of Cancer Research2018,30,6: | 12 |
| 20 | Protective effect of bone marrow mesenchymal stem cells in intestinal barrier permeability after heterotopic intestinal transplantation显示文摘AIM:To explore the protective effect of bone marrow mesenchymal stem cells(BM MSCs)in the small intestinal mucosal barrier following heterotopic intestinal transplantation(HIT)in a rat model.METHODS:BM MSCs were isolated from male Lewis rats by density gradient centrifugation,cultured,and analyzed by flow cytometry.The HIT models were divided into a non-rejection group,saline-treated rejection group(via penile vein),and BM MSC–treated group(via penile vein).Intestinal mucosal barrier injury was estimated by diamine oxidase(DAO)and D-lactic acid(D-LA)expression levels.Tumor necrosis factor-α(TNF-α),interferon-γ(INF-γ),interleukin-10(IL-10),and transforming growth factor-β(TGF-β)were detected by enzyme-linked immunosorbent assay.Ultrastructural change of tight junctions(TJs)was observed under transmission electron microscope.Expression levels of the TJ proteins occludin and zona occludens(ZO)-1,affected by the inflammatory factors,were measured using real-time polymerase chain reaction and Western blotting.RESULTS:The pathological score at each time point after surgery indicated significantly less serious injury in the BM MSCs-treated group than in the rejection group(P<0.05).In the former,graft levels of DAO and D-LA were reduced,and TNF-αand INF-γproduction was inhibited(at day 7:10.6473±0.0710vs 17.2128±0.4991,P<0.05;545.1506±31.9416vs 810.2637±25.1175,P<0.05).IL-10 and TGF-βproduction was increased greatly(at day 7:125.7773±4.7719 vs 80.3756±2.5866,P<0.05;234.5273±9.3980 vs 545.1506±31.9416,P<0.05).There was increased expression of occludin and ZO-1 protein(at day 7:0.2674±0.0128 vs 0.1352±0.0142,P<0.05;at day 5:0.7189±0.0289 vs 0.4556±0.0242,P<0.05)and mRNA(at day 7:0.3860±0.0254 vs 0.1673±0.0369,P<0.05;at day 5:0.5727±0.0419 vs0.3598±0.0242,P<0.05).CONCLUSION:BM MSCs can improve intestinal barrier permeability,repair TJs,and increase occludin and ZO-1 protein expression.With altered cytokine levels,they can protect the intestinal mucosa after transplantation. | Wen Zhang Zhong-Yang Shen Hong-Li Song Yang Yang Ben-Juan Wu Nan-Nan Fu Tao Liu | 2014 | World Journal of Gastroenterology2014,20,23: | 12 |
