| 1 | Vaccine against Helicobacter pylori : Inevitable approach显示文摘Over three decades have passed since the discovery of Helicobacter pylori(H. pylori), and yet many questions about its treatment remain unanswered. For example, there is no certainty regarding continued use of current antibiotic therapy against H. pylori. The bad news is that even combined regimens are also unable to eradicate bacterial colonization. The worst problem with H. pylori chemotherapy is that even if we identify the most successful regimen, it cannot eliminate the risk of re-infection. This problem is further complicated by the fact that clinicians have no information as to whether probiotics are useful or not. Moreover, to date, we have no large scale produced vaccine effective against H. pylori. Due to the relatively rapid and abundant dissemination of guidelines globally reported concerning management of gastric cancer prevention and therapeutic regimens, clinicians may choose a vaccine as better effective weapon against H. pylori. Therefore, a radical shift in adopted strategies is needed to guide ultimate decisions regarding H. pylori management. In light of failures in vaccine projects, we should identify better vaccine design targeting conserved/essential genes. The unique character and persistence of H. pylori pose obstacles to making an effective vaccine. Preferably, in developing countries, the best reasonable and logical approach is to recommend prophylactic H. pylori vaccine among children as an obligatory national program to limit primary colonization. Trying to produce a therapeutic vaccine would be postponed until later. In reality, we should not forget to prescribe narrow spectrum antibiotics. | 2 | Strategies used by helicobacter pylori to establish persistent infection显示文摘Helicobacter pylori(H. pylori) is a Gram-negative and motile bacterium that colonizes the hostile microniche of the human stomach, then persists for the host's entire life, if not effectively treated. Clinically, H. pylori plays a causative role in the development of a wide spectrum of diseases including chronic active gastritis, peptic ulceration, gastric adenocarcinoma, and gastric mucosaassociated lymphoid tissue lymphoma. Due to the global distribution of H. pylori, it is no exaggeration to conclude that smart strategies are contributing to adaptation of the bacterium to its permanent host. Thirty-four years after the discovery of this bacterium, there are still many unanswered questions. For example, which strategies help the bacterium to survive in this inhospitable microniche? This question is slightly easier to answer if we presume the same clinical concept for both persistent infection and disease. Understanding the mechanisms governing H. pylori persistence will improve identification of the increased risk of diseases such as gastric cancer in patients infected with this bacterium. A well-defined and longterm equilibrium between the human host and H. pylori allows bacterial persistence in the gastric microniche; although this coexistence leads to a high risk of severe diseases such as gastric cancer. To escape the bactericidal activity of stomach acid, H. pylori secretes large amounts of surface-associated and cytosolic urease. The potential to avoid acidic conditions and immune evasion are discussed in order to explain the persistence of H. pylori colonization in the gastric mucosa, and data on bacterial genetic diversity are included. Information on the mechanisms related to H. pylori persistence can also provide the direction for future research concerning effective therapy and management of gastroduodenal disorders. | 3 | Transplantation of stem cells from human exfoliated deciduous teeth for bone regeneration in the dog mandibular defect显示文摘AIM: To investigate the effect of stem cells from human exfoliated deciduous teeth(SHED) transplanted for bone regeneration in the dog mandibular defect.METHODS: In this prospective comparative study, SHEDs had been isolated 5 years ago from human exfoliated deciduous teeth. The undifferentiated stem cells were seeded into mandibular bone through-andthrough defects of 4 dogs. Similar defects in control group were filled with cell-free collagen scaffold. After 12 wk, biopsies were taken and morphometric analysis was performed. The percentage of new bone formation and foreign body reaction were measured in each case. The data were subject to statistical analysis using the Mann-Whitney U and Kruskalwalis statistical tests. Differences at P < 0.05 was considered as significant level.RESULTS: There were no significant differences between control and SHED-seeded groups in connective tissue(P = 0.248), woven bone(P = 0.248) and compact bone(P = 0.082). | 4 | Resistance to clarithromycin and gastroenterologist's persistence roles in nomination for Helicobacter pylori as high priority pathogen by World Health Organization显示文摘Due to the increasing prevalence of clarithromycin resistance, future of management of Helicobacter pylori(H. pylori) infections need to be recognized. To now, clarithromycin was the best effective, well-tolerated and safe antibiotic used in treatment of the bacterium, but, increasing trend of resistance reduced efficacy of recommended regimens. Indeed, gastroenterologists are mostly unable to start appropriate therapyaccording to the sensitivity profile-due to the certain difficulties in routine H. pylori culture procedure and being time consuming method. This announcement by World Health Organization(WHO) was an onset to reconsider current challenging dilemma about H. pylori clarithromycin resistant isolates. Therefore, investigating of various factors affecting this nomination by WHO is highly welcomed. In fact, WHO enumerated more than 16 pathogens which seriously threats human life and public health, thus better management or effective guidelines are necessary. Here for the first time, we nominated this phenomenon as ‘‘gastroenterologist's persistence'' which should be equally investigated as antibiotic resistance. The ability of gastroenterologists to win the game against H. pylori infections is highly influenced by their collaboration with diagnostic laboratories to apply susceptibility patterns before any prescription. | 5 | Effects of aromatherapy with Rosa damascena on nulliparous women's pain and anxiety of labor during first stage of labor显示文摘Background: Reducing labor pain and anxiety is one of the most important goals of maternity care.Objective: This study aimed to assess the effects of aromatherapy with Rosa damascena on pain and anxiety in the first stage of labor among nulliparous women.Design, setting, participants and interventions: This was a randomized clinical trial of 110 nulliparous women. The eligible participants were randomly assigned to two groups of aromatherapy and control in an Iranian maternity hospital. The participants received 0.08 m L of Rosa damascena essence in the aromatherapy group and 0.08 m L of normal saline in the control group, every 30 min. Pain was measured 3 times, once each at three stages of cervical dilation(4–5, 6–7, and 8–10 cm). Anxiety was measured twice,once each at two stages of cervical dilation(4–7 and 8–10 cm). The tools for data collection were the Spielberger anxiety questionnaire, numerical pain rating scale, demographic and obstetric questionnaire,and an observational checklist. Data analyses included the t-test, Mann–Whitney U test and Chi-square test.Main outcome measures: Severity of labor pain and severity of anxiety were used as primary outcome measures. Labor and delivery characteristics(including number of contractions, duration of contractions in second stage, Bishop score, augmentation by oxytocin, Apgar score, and mode of delivery), demographic characteristics, and fertility information were used as secondary outcome measures.Results: Pain severity in the group receiving aromatherapy with R. damascena was significantly lower than in the control group after treatment at each pain assessment(cervical dilation of 4–5, 6–7, and8–10 cm; P < 0.05). Anxiety levels were also significantly lower in the treatment group than in the control group after treatment at each time of measurement(cervical dilation of 4–7 and 8–10 cm; P < 0.05).Conclusion: Aromatherapy with R. damascena reduced the severity of pain and anxiety in the first stage of labor. | 6 | Coinfection of the secondary symbionts,Hamiltonella defensa and Arsenophonus sp.contribute to the performance of the major aphid pest,Aphis gossypii(Hemiptera:Aphididae)显示文摘Bacterial endosymbionts play important roles in ecological traits of aphids.In this study,we characterize the bacterial endosymbionts of A.gossypii collected in Karaj,Iran and their role in the performance of the aphid.Our results indicated that beside Buchnera aphidicola,A.gossypii,also harbors both Hamiltonella defensa and Arsenophonus sp.Quantitative PCR(qPCR)results revealed that the populations of the endosymbionts increased throughout nymphal development up to adult emergence;thereafter,populations of Buchnera and Arsenophonus were diminished while the density of H.defensa constantly increased.Buchnera reduction caused prolonged development and no progeny production.Furthermore,secondary symbiont reduction led to reduction of the total life span and intrinsic rate of natural increase as well as appearance of the deformed dead offspring in comparison with the control insects.Reduction of the secondary symbionts did not affect parasitism rate of the aphid by the parasitic wasp Aphidius matricariae.Together these findings showed that H.defensa and Arsenophonus contributed to the fitness of A.gossypii by enhancing its performance,but not through parasitoid resistance. | Aida Ayoubi Ali Asghar Talebi Yaghoub Fathipour Mohammad Mehrabadi | 2020 | Insect Science2020,27,1: | 3 |
| 7 | Role of dup A in virulence of Helicobacter pylori显示文摘Helicobacter pylori(H. pylori) is a gastric human pathogen associated with acute and chronic gastritis, 70% of all gastric ulcers, 85% of all duodenal ulcers, and both forms of stomach cancer, mucosal-associated lymphoid tissue(MALT) lymphoma and adenocarcinoma. Recently, attention has focused on possible relationship between presence of certain virulence factor and H. pylori-associated diseases. Some contradictory data between this bacterium and related disorders has been observed since not all the colonized individuals develop to severe disease. The reported diseases plausibility related to H. pylori specific virulence factors became an interesting story about this organism. Although a number of putative virulence factors have been identified including cytotoxin-associated gene a(cag A) and vac A, there are conflicting data about their actual participation as specific risk factor for H. pylori-related diseases. Duodenal ulcer promoting gene a(dup A) is a virulence factor of H. pylori that is highly associated with duodenal ulcer development and reduced risk of gastric cancer. The prevalence of dup A in H. pylori strains isolated from western countries is relatively higher than in H. pylori strains from Asian countries. Current confusing epidemiological reports will continue unless future sophisticated and molecular studies provide data on functional and complete dup A cluster in H. pylori infected individuals. | 10 | Analysis of maximum pressure in VVER1000/V446 reactor containment for LOCA and MSLB显示文摘The highest thermal-hydraulic pressure in the containment occurs when reactor coolant in the first loop and steam in the secondary loop discharge simultaneously,and when the maximum amount of energy from reactor unit enters to containment volume. In this paper, we investigate temperature and pressure variations in the VVER 1000 containment compartments owing to concurrent break in the pipelines of the primary and secondary loops. A two-phase, multicellular model is applied in the presence of non-condensable gases. Convection and conduction through the main heat structures inside the containment are also considered. The predicted results agree well with available data. Maximum values of pressure and temperature in the containment are then calculated and compared to the design values. | 11 | Chinese Helicobacter pylori vaccine: Solution for an old challenge?显示文摘Helicobacter pylori(H. pylori) is an important cause for gastric cancer in high risk individuals. H. pylori colonizes more than 50% of the world's population and associated peptic ulcer disease and gastric malignancy have important public health implications. It has been classified as a class Ⅰ carcinogen in 1994 by the World Health Organization. Clinicians are often prompted to eliminate the infection the moment it is detected. This also, unfortunately, led to reckless use of antibiotics and reports of increasing resistance are now worldwide. Each year, many of people die from gastric cancer; thus application of effective vaccine can reduce this relatively high mortality worldwide. H. pylori can be eliminated by antibiotics but efficacy is sharply decreasing. Moreover, current therapy is also expensive and with side effects. Vaccine may be the best solution to the above problem but there are many challenges in producing such an effective therapeutic vaccine. Recently, the Chinese group published in Lancet, a single-center, randomized, phase Ⅲ study of an oral recombinant vaccine(Urease B subunit fused with heat-labile enterotoxin B derived from Escherichia coli) prescribed in the Chinese children(6-15 years) without a history of H. pylori infection. | 12 | Quality control of the parasitoid wasp Trichogramma brassicae (Hymenoptera: Trichogrammatidae) over 45 generations of rearing on Sitotroga cerealella显示文摘Trichogramma brassicae(Bezdenko)is an important biological control agent that has been used widely against many lepidopteran pests.Commonly,colonies of Trichogramma are mass reared on factitious hosts such as Sitotroga cerealella(Olivier)over many generations.In this study,we evaluated the quality and performance of a colony of T.brassicae that had been reared for over 45 generations(G)using two-sex life table parameters and parasitism capacity.We found that female adult longevity was significantly different among sequential generations,ranging from 5.58±2.5 d(at G5)to 3.75±1.42 d(at G45).However,no significant difference was found in male adult longevity among different generations.Although female wasps survived longer until the 15th gener・ation,they allocated more days for egg laying at G5 and GIO.The highest values of gross reproductive rate(GRR),net reproductive rate(7.),intrinsic rate of increase(r),finite rate of increase(λ)and mean generation time(T)were found in G5 and GIO,which also showed significantly higher cq.No significant difference in the finite parasitism rate(co)was found among generations up to G15.These results suggest that T.brassicae wasps held under continuous laboratory rearing can be used successfully in biological control programs until the 15th generation without any loss of quality or performance.However,laboratory mass rearing colonies declined in quality after 15 generations and we suggest that they should be rejuvenated regularly by adding field-collected parasitoids periodically. | Ehsan Ghaemmaghami Yaghoub Fathipour Abdoolnabi Bagheri Ali Asghar Talebi Gadi V.P.Reddy | 2021 | Insect Science2021,28,1: | 2 |
