| 2 | Impact of gout on in-hospital outcomes of acute coronary syndrome-related hospitalizations and revascularizations: Insights from the national inpatient sample显示文摘BACKGROUND Previous studies have established a role of gout in predicting risk and prognosis of cardiovascular diseases. However, large-scale data on the impact of gout on inpatient outcomes of acute coronary syndrome (ACS)-related hospitalizations and post-revascularization is inadequate. AIM To evaluate the impact of gout on in-hospital outcomes of ACS hospitalizations, subsequent healthcare burden and predictors of post-revascularization inpatient mortality. METHODS We used the national inpatient sample (2010-2014) to identify the ACS and goutrelated hospitalizations, relevant comorbidities, revascularization and postrevascularization outcomes using the ICD-9 CM codes. A multivariable analysis was performed to evaluate the predictors of post-revascularization in-hospital mortality. RESULTS We identified 3144744 ACS-related hospitalizations, of which 105198 (3.35%) also had gout. The ACS-gout cohort were more often older white males with a higher prevalence of comorbidities. Coronary artery bypass grafting was required more often in the ACS-gout cohort. Post-revascularization complications including cardiac (3.2% vs 2.9%), respiratory (3.5% vs 2.9%), and hemorrhage (3.1% vs 2.7%) were higher whereas all-cause mortality was lower (2.2% vs 3.0%) in the ACSgout cohort (P < 0.001). An older age (OR 15.63, CI: 5.51-44.39), non-elective admissions (OR 2.00, CI: 1.44-2.79), lower household income (OR 1.44, CI: 1.17- 1.78), and comorbid conditions predicted higher mortality in ACS-gout cohort undergoing revascularization (P < 0.001). Odds of post-revascularization inhospital mortality were lower in Hispanics (OR 0.45, CI: 0.31-0.67) and Asians (OR 0.65, CI: 0.45-0.94) as compared to white (P < 0.001). However, postoperative complications significantly raised mortality odds. Mean length of stay, transfer to other facilities, and hospital charges were higher in the ACS-gout cohort. | 3 | Serum immunoglobulin A concentration is a reliable biomarker for liver fibrosis in non-alcoholic fatty liver disease显示文摘AIM:To evaluate the diagnostic accuracy of serum Immunoglobulin A(IgA)for differentiating early stage nonalcoholic fatty liver disease(NAFLD)from nonalcoholic steatohepatitis(NASH).METHODS:All cases had fatty liver change confirmed by ultrasound and aminotransferases of at least twice the normal level.Clinical and biochemical data,including serum IgA,were obtained from 50 histologically proven NAFLD cases and 54 healthy controls.Fasting whole blood samples were obtained from the study population.Immunoturbidimetric methods were used to measure the IgA levels.All NAFLD cases were hospitalized for liver biopsy.Liver specimens were examined for steatosis,steatohepatitis and fibrosis within hepatocytes.Patients were categorized into two groups:NASH and non-NASH.Variables were compared within cases(NASH vs non-NASH)and controls.Cut-off values of serum IgA were evaluated using analysis of receiver operating characteristic(ROC curves).Associations between the variables were tested using calculations of correlation coefficients.Statistical significances were assigned to P values<0.05.RESULTS:The extent of liver fibrosis correlated positively with IgA levels.Subjects with no fibrosis in their liver biopsies had a lower IgA level(301.5±91.2 mg/dL)than subjects with any degree of fibrosis(388.8±140.8 mg/dL),(P=0.01).IgA levels were higher in NASH cases,and its value was significantly higher for higher degrees of fibrosis.Patients with perisinusoidal or pericellular fibrosis had significantly higher levels of IgA(403.5±133.9 mg/dL,418.2±129.5 mg/dL)compared to those without it(301.8±94.9 mg/dL,297.7±91.5 mg/dL),respectively.No significant correlation was found between steatosis grade and serum IgA levels.Based on ROC analysis,the best predictive IgA cutoff value for detecting liver fibrosis was 360mg/dL(61%sensitivity,81%specificity).CONCLUSION:The serum IgA level is useful to evaluate the severity of liver fibrosis and can be used serially for evaluation and follow-up of NAFLD cases. | Iradj Maleki Mahmood Reza Aminafshari Tarang Taghvaei Vahid Hosseini Alireza Rafiei Zhila Torabizadeh Maryam Barzin Elahe Orang | 2014 | World Journal of Gastroenterology2014，20，35： | 4 |

| 5 | Modeling of coastal effluent transport:an approach to linking far-field and near-field models显示文摘One of the challenges in effluent transport modeling in coastal tidal environments is the proper specification of initial dilution in connection with the far-field transport phenomena. An approach of external linkage of far-field and near-field effluent transport models is presented, and applied to simulating the effluent transport in the Port Angeles Harbor, Washington in the Strait of Juan de Fuca. A near-field plume model was used to calculate the effluent initial dilution and a three-dimensional (3-D) hydrodynamic model was developed to simulate the tidal circulation and far-field effluent transport in the Port Angeles Harbor. The hydrodynamic model was driven by tides and surface winds. Observed water surface elevation and velocity data were used to calibrate the model over a period covering the neap-spring tidal cycle. The model was also validated with observed surface drogue trajectory data. The model successfully reproduced the tidal dynamics in the study area and good agreements between model results and observed data were obtained. | 7 | The power of microsatellite markers and AFLPs in revealing the genetic diversity of Hashemi aromatic rice from Iran显示文摘Hashemi, a popular aromatic rice among Iranians, is famous for its fragrance and taste. Such features are major reasons for its higher price compared to non-aromatic varieties available in Iran. Therefore, the knowledge of genetic diversity of this profitable crop is a fundamental ineterst for plant breeders in future breeding programs. In the present research, genetic diversity among 35 genotypes of Hashemi aromatic rice(Oryza sativa L.) from Guilan and Mazandaran provinces of Iran was estimated using simple sequence repeat(SSR) and amplified fragment length polymorphism(AFLP) markers. Out of 21 SSR and 12 Eco RI-Mse I AFLP marker combinations, only 16 SSRs and 10 AFLPs exhibited polymorphic patterns while others were monomorphic. The 10 AFLP primer combinations produced a total of 142 of bands and 20 were polymorphic(14.08%). Moreover, 40 out of 47 bands amplified with 16 SSR markers showed polymorphism(85.1%). The average number of alleles identified by SSR primers was 2.56 alleles per locus with a range of 2 to 4. The average value of polymorphic information content(PIC) was 0.393 and 0.468 for AFLP and SSR markers, respectively. However, the genetic similarity values ranged from 0.26 to 1 for SSRs and 0.21 to 1 for AFLPs. Later, a unweighted pair group method with arithmetic mean(UPGMA) dendrogram was generated and genotypes were clustered into four groups with SSRs at similarity coefficient of 0.55 while AFLPs clustered them into six groups at similarity coefficient of 0.41.
