1Unique cytologic features of thyroiditis caused by immune checkpoint inhibitor therapy for malignant melanoma显示文摘Blockade of immune checkpoint molecules to reverse cancer-induced immune suppression can improve anti-tumor immune responses in cancer patients.Monoclonal antibodies targeting two such molecules,Programmed cell death protein 1(PD-1)and cytotoxic T-lymphocyte associated protein 4(CTLA-4)have shown clinical benefit in the treatment of advanced malignancies,including metastatic melanoma.Adverse effects of these immune checkpoint inhibitors include immune-related adverse events(irAE),of which one of the most common is autoimmune thyroiditis.Though thyroiditis is increasingly recognized,there are no reports of the pathological findings that occur in immunotherapy-induced thyroiditis.We present a case of immunotherapy-induced thyroiditis demonstrating its unique cytopathologic features.A 51-year-old woman with metastatic melanoma was found to have a suppressed TSH and elevated free thyroxine concentration 14 days after starting treatment with nivolumab(PD-1 antagonist)plus ipilimumab(CTLA-4 antagonist)therapy.A thyroid biopsy was performed based on ultrasound findings and cytopathology revealed unique features including abundant clusters of necrotic cells,lymphocytes and CD163-positive histiocytes.This case reports cytopathologic features found in immune checkpoint inhibitor related thyroiditis.These appear to be unique findings and may help inform future research regarding the pathophysiology and mechanisms of this condition.Trevor E.Angell Le Min Tad J.Wieczorek F.Stephen Hodi 2018Genes & Diseases2018,5,1:13
2The crystal structure of the non-liganded 14-3-3σ protein: insights into determinants of isoform specific ligand binding and dimerization显示文摘Seven different, but highly conserved 14-3-3 proteins are involved in diverse signaling pathways in human cells. It isunclear how the 14-3-3σ isoform, a transcriptional target of p53, exerts its inhibitory effect on the cell cycle in thepresence of other 14-3-3 isoforms, which are constitutively expressed at high levels. In order to identify structuraldifferences between the 14-3-3 isoforms, we solved the crystal structure of the human 14-3-3σ protein at a resolutionof 2.8 ? and compared it to the known structures of 14-3-3ζ and 14-3-3τ. The global architecture of the 14-3-3σ foldis similar to the previously determined structures of 14-3-3ζ and 14-3-3τ: two 14-3-3σ molecules form a cup-shapeddimer. Significant differences between these 14-3-3 isoforms were detected adjacent to the amphipathic groove, whichmediates the binding to phosphorylated consensus motifs in 14-3-3-ligands. Another specificity determining region islocalized between amino-acids 203 to 215. These differences presumably select for the interaction with specific ligands,which may explain the different biological functions of the respective 14-3-3 isoforms. Furthermore, the two 14-3-3σmolecules forming a dimer differ by the spatial position of the ninth helix, which is shifted to the inside of the ligandinteraction surface, thus indicating adaptability of this part of the molecule. In addition, 5 non-conserved residues arelocated at the interface between two 14-3-3σ proteins forming a dimer and represent candidate determinants of homo-and hetero-dimerization specificity. 4Indications and contraindications for shock wave lithotripsy and how to improve outcomes显示文摘For over 35 years shock wave lithotripsy has proven to be an effective,safe and truly minimally invasive option for the treatment of nephrolithiasis.Various technical factors as well as patient selection can impact the success of the procedure.We used published work focusing on outcomes of shock wave lithotripsy,risk of complications,and strategies for improving stone fragmentation to create this review.Multiple patient and technical factors have been found to impact success of treatment.Skin to stone distance,stone density and composition,size and location of the stone within the urinary system all influence stone free rates.A slower rate with a gradual increasing voltage,precise targeting,proper coupling will improve stone fragmentation and decrease risk of complications.The selection of appropriate patients through a shared decision making process and attention to the technical factors that improve stone free rates is key to providing an effective treatment and patient satisfaction.Luke F.Reynolds Tad Kroczak Kenneth T.Pace 2018Asian Journal of Urology2018,5,4:4
