1Esophageal cancer: A Review of epidemiology, pathogenesis, staging workup and treatment modalities显示文摘Esophageal cancer is a serious malignancy with regards to mortality and prognosis. It is a growing health concern that is expected to increase in incidence over the next 10 years. Squamous cell carcinoma is the most common histological type of esophageal cancer worldwide, with a higher incidence in developing nations. With the increased prevalence of gastroesophageal reflux disease and obesity in developed nations, the incidence of esophageal adenocarcinoma has dramatically increased in the past 40 years. Esophageal cancer is staged according to the widely accepted TNM system. Staging plays an integral part in guiding stage specific treatment protocols and has a great impact on overall survival. Common imaging modalities used in staging include computed tomography, endoscopic ultrasound and positron emission tomography scans. Current treatment options include multimodality therapy mainstaysof current treatment include surgery, radiation and chemotherapy. 2Value of screening endoscopy in evaluation of esophageal,gastric and colon cancers显示文摘Esophageal,gastric,and colorectal cancers are deadly diseases that continue to plague our world today.The value of screening endoscopy in evaluating these types of cancers is a critical area of discussion due to a potential reduction in morbidity and mortality.This article describes how to identify a good screening test and explains what are important criteria in the field of screening endoscopy.Furthermore,the current status and progress of screening endoscopy for esophageal,gastric,and colorectal cancer will be evaluated and discussed.Mass screening programs have not been implemented for esophageal and gastric carcinomas in those with average or low risk populations.However,studies of high-risk populations have found value and a cost-benefit in conducting screening endoscopy.Colorectal cancer,on the other hand,has had mass screening programs in place for many years due to the clear evidence of improved outcomes.As the role of endoscopy as a screening tool has continued to develop,newer technology and techniques have emerged to improve its utility.Many new image enhancement techniques and computer processing programs have shown promise and may have a significant role in the future of endoscopic screening.These developments are paving the way for improving the diagnostic and therapeutic capability of endoscopy in the field of gastroenterology.Tae H Ro Michelle A Mathew Subhasis Misra 2015World Journal of Gastroenterology2015,21,33:9

4Thrombocytopenia with multiple splenic lesions-histiocytic sarcoma of the spleen without splenomegaly: A case report显示文摘BACKGROUND Histiocytic sarcoma(HS)of the spleen is reported to be a rare and lethal disease.The clinicopathological features of splenic HS have not been well described.The objective of this paper is to describe the diagnosis and treatment of a case of this rare disease and provide a review of the literature.CASE SUMMARY In this article,we discuss the case of a 40-year-old Hispanic female who presented with progressive thrombocytopenia and multiple hypoechoic lesions in the spleen without splenomegaly.Positron emission tomography-computed tomography showed increased activity in cervical lymph nodes,as well as multiple bone and splenic lesions with positive uptake.Two bone marrow biopsies and fine-needle aspiration of the cervical lymph node were inconclusive.Laparoscopic splenectomy was performed,and gross examination showed a 110.1 g spleen with multiple rubbery,nodular lesions within the subcapsular sinus and splenic parenchyma.The microscopic findings showed multinodular histiocyte proliferation with atypia and multilobulated nuclei,which were positive for CD163,CD4,and CD68 by immunohistochemical analysis.The final pathologic diagnosis was difficult and was found to be low-grade HS of the spleen,after consultations with two renowned hematopathology institutions.At the patient’s five-month follow-up visit,her bone marrow metastasis had progressed.She is waiting to be enrolled in a clinical trial.CONCLUSION Pathologic diagnosis of splenic HS can be challenging.Low-grade differentiation may be associated with a slow progressive disease.Kai Huang Alvaro Frometa Columbie Robert W Allan Subhasis Misra 2020World Journal of Clinical Oncology2020,11,3:1

9Acute epiploic appendagitis at the tip of the appendix mimicking acute appendicitis: A rare case report with literature review显示文摘BACKGROUND Acute epiploic appendagitis of the appendix (AEAA) is a rare self-limiting inflammatory disorder of the epiploic appendages (EA) close to the vermiform appendix, which often times mimicking the presentation of acute appendicitis (AA). To date, very few cases of AEAA have been reported. We report a case of a 52-year old man with the clinical suspicion of AA, but post-operative specimen examination confirmed AEAA as the final diagnosis. CASE SUMMARY A 52-year-old morbidly obese man presented to the emergency department with a 1-d history of the right lower quadrant (RLQ) abdominal pain. Physical examination revealed localized RLQ tenderness mimicking AA. The computed tomography abdomen was inconclusive, and a decision was made to perform laparoscopic appendectomy (LA). During the LA, an infarcted epiploic appendage at the tip of appendix and adherent to the abdominal wall was found, which was entirely excised. Final pathology showed congested and hemorrhagic epiploic appendage without any accompanied acute inflammatory changes in the wall of the appendix. Postoperative course was uneventful and he was doing well at seven months follow-up. CONCLUSION The possibility of AEAA should be considered in patients clinically suspected of having AA.
