1Hepatocellular carcinoma: Review of disease and tumor biomarkers显示文摘Hepatocellular carcinoma(HCC) is a common malignancy and now the second commonest global cause of cancer death. HCC tumorigenesis is relatively silent and patients experience late symptomatic presentation. As the option for curative treatments is limited to early stage cancers, diagnosis in non-symptomatic individuals is crucial. International guidelines advise regular surveillance of high-risk populations but the current tools lack sufficient sensitivity for early stage tumors on the background of a cirrhotic nodular liver. A number of novel biomarkers have now been suggested in the literature, which may reinforce the current surveillance methods. In addition, recent metabonomic and proteomic discoveries have established specific metabolite expressions in HCC, according to Warburg's phenomenon of altered energy metabolism. 2Protective effects of ischemic preconditioning and application of lipoic acid prior to 90 min of hepatic ischemia in a rat model显示文摘AIM: To compare different preconditioning strategies to protect the liver from ischemia/reperfusion injury focusing on the expression of pro- and anti-apoptotic proteins. Interventions comprised different modes of ischemic preconditioning (IP) as well as pharmacologic pretreatment by α-lipoic acid (LA). METHODS: Several groups of rats were compared: sham operated animals, non-pretreated animals (nt), animals receiving IP (10 min of ischemia by clamping of the portal triad and 10 min of reperfusion) prior to sustained ischemia, animals receiving selective ischemic preconditioning (IPsel, 10 min of ischemia by selective clamping of the ischemic lobe and 10 min of reperfusion) prior to sustained ichemia, and animals receiving 500 μmol α-LA injected i.v. 15 min prior to the induction of 90 min of selective ischemia. RESULTS: Cellular damage was decreased only in the LA group. TUNEL-positive hepatocytes as well as necrotic hepatocyte injury were also decreased only by LA (19 ± 2 vs 10 ± 1, P < 0.05 and 29 ± 5 vs 12 ± 1, P < 0.05). Whereas caspase 3- activities in liver tissue were unchanged, caspase 9- activity in liver tissue was decreased only by LA pretreatment (3.1 ± 0.3 vs 1.8 ± 0.2, P < 0.05). Survival rate as the endpoint of liver function was increased after IP and LA pretreatment but not after IPsel. Levels of lipid peroxidation (LPO) in liver tissue were decreased in the IP as well as in the LA group compared to the nt group. Determination of pro- and anti-apoptotic proteins showed a shift towardsanti-apoptotic proteins by LA. In contrast, both our IP strategies failed to influence apototic cell death. CONCLUSION: IP, consisting of 10 min of ischemia and 10 min of reperfusion, protects only partly against ischemia/reperfusion injury of the liver prior to 90 min of selective ischemia. IPsel did not influence ischemic tolerance of the liver. 4Successful surgical management of ruptured umbilical hernias in cirrhotic patients显示文摘Acute umbilical hernia rupture in patients with hepatic cirrhosis and ascites is an unusual,but potentially lifethreatening complication,with postoperative morbidity about 70% and mortality between 60%-80% after supportive care and 6%-20% after urgent surgical repair.Management options include primary surgical repair with or without concomitant portal venous system decompression for the control of the ascites.We present a retrospective analysis of our centre's experience over the last 6 years.Our cohort consisted of 11 consecutive patients(median age:53 years,range:36-63 years) with advanced hepatic cirrhosis and refractory ascites.Appropriate patient resuscitation and optimisation with intravenous fluids,prophylactic antibiotics and local measures was instituted.One failed attempt for conservative management was followed by a successful primary repair.In all cases,with one exception,a primary repair with non-absorbable Nylon,interrupted sutures,without mesh,was performed.The perioperative complication rate was 25% and the recurrence rate 8.3%.No mortality was recorded.Median length of hospital stay was 14 d(range:4-31 d).Based on our experience,the management of ruptured umbilical hernias in patients with advanced hepatic cirrhosis and refractory ascites is feasible without the use of transjugular intrahepatic portosystemic shunt routinely in the preoperative period,provided that meticulous patient optimisation is performed.Nikolaos A Chatzizacharias J Andrew Bradley Simon Harper Andrew Butler Asif Jah Emmanuel Huguet Raaj K Praseedom Michael Allison Paul Gibbs 2015World Journal of Gastroenterology2015,21,10:4
5Recurrent ectopic pancreatitis of the jejunum and mesentery over a 30-year period显示文摘BACKGROUND:Ectopic pancreas is defined as pancreatic tissue found outside its usual anatomical position,with no ductal or vascular communication with the native pancreas. We describe a case of ectopic pancreas of the small bowel and mesentery causing recurrent episodes of pancreatitis, initially suspected on computed tomography(CT)and magnetic resonance cholangiopancreatography(MRCP),and confirmed on histological review of the resection. METHODS:A 67-year-old woman presented with clinical symptoms and biochemical evidence of pancreatitis.She had similar episodes over the past 30 years with unrevealing investigations,and was concluded to have idiopathic pancreatitis. She underwent CT and MRCP,with findings suggestive of ectopic pancreas,a diagnosis confirmed on histology of the resection. RESULTS:MRCP identified a mass in the proximal small bowel mesentery isointense to the native pancreas,with a small duct draining into a proximal jejunal loop.The resected specimen consisted of normal parenchyma with lobulated acinar tissue with scattered islets of Langerhans,an occasional ductular structure,and admixed areas of adipose tissue.The patient remained asymptomatic with normal biochemistry six months post-operatively. 6Meso-Cenozoic tectonic evolution of the Talas-Fergana region of the Kyrgyz Tien Shan revealed by low-temperature basement and detrital thermochronology显示文摘This study provides new low-temperature thermochronometric data, mainly apatite fission track data on the basement rocks in and adjacent to the Talas-Fergana Fault, in the Kyrgyz Tien Shan in the first place.In the second place, we also present new detrital apatite fission track data on the Meso-Cenozoic sediments from fault related basins and surrounding intramontane basins. Our results confirm multistaged Meso-Cenozoic tectonic activity, possibly induced by the accretion of the so-called Cimmerian blocks to the Eurasian margin. New evidence for this multi-staged thermo-tectonic activity is found in the data of both basement and Meso-Cenozoic sediment samples in or close to the Talas-Fergana Fault.Zircon(U-Th)/He and apatite fission track data constrain rapid Late TriassiceE arly Jurassic and Late JurassiceE arly Cretaceous basement cooling in the Kyrgyz Tien Shan around 200 Ma and 130 -100 Ma respectively. Detrital apatite fission track results indicate a different burial history on both sides of the Talas-Fergana Fault. The apatite fission track system of the Jurassic sediments in the Middle Tien Shan unit east of the Talas-Fergana Fault is not reset, while the Jurassic sediments in the Fergana Basin and Yarkand-Fergana Basin, west of the fault zone, are partially and in some cases even totally reset. 7Machine learning in orthopaedic surgery显示文摘Artificial intelligence and machine learning in orthopaedic surgery has gained mass interest over the last decade or so.In prior studies,researchers have demonstrated that machine learning in orthopaedics can be used for different applications such as fracture detection,bone tumor diagnosis,detecting hip implant mechanical loosening,and grading osteoarthritis.As time goes on,the utility of artificial intelligence and machine learning algorithms,such as deep learning,continues to grow and expand in orthopaedic surgery.The purpose of this review is to provide an understanding of the concepts of machine learning and a background of current and future orthopaedic applications of machine learning in risk assessment,outcomes assessment,imaging,and basic science fields.In most cases,machine learning has proven to be just as effective,if not more effective,than prior methods such as logistic regression in assessment and prediction.With the help of deep learning algorithms,such as artificial neural networks and convolutional neural networks,artificial intelligence in orthopaedics has been able to improve diagnostic accuracy and speed,flag the most critical and urgent patients for immediate attention,reduce the amount of human error,reduce the strain on medical professionals,and improve care.Because machine learning has shown diagnostic and prognostic uses in orthopaedic surgery,physicians should continue to research these techniques and be trained to use these methods effectively in order to improve orthopaedic treatment.Simon P Lalehzarian Anirudh K Gowd Joseph N Liu 2021World Journal of Orthopedics2021,12,9:4

10Effect of process parameters on additive-free electrospinning of regenerated silk fibroin nonwovens显示文摘Silk fibroin is a biomaterial with multiple beneficial properties for use in regenerative medicine and tissue engineering.When dissolving and processing the reconstituted silk fibroin solution by electrospinning,the arrangement and size of fibers can be manifold varied and according fiber diameters reduced to the nanometer range.Such nonwovens show high porosity as well as potential biocompatibility.Usually,electrospinning of most biomaterials demands for the application of additives,which enable stable electrospinning by adjusting viscosity,and are intended to evaporate during processing or to be washed out afterwards.However,the use of such additives increases costs and has to be taken into account in terms of biological risks when used for biomedical applications.In this study,we explored the possibilities of additive-free electrospinning of pure fibroin nonwovens and tried to optimize process parameters to enable stable processing.We used natural silk derived from the mulberry silkworm Bombyx mori.After degumming,the silk fibroin was dissolved and the viscosity of the spinning solution was controlled by partial evaporation of the initial solving agent.This way,we were able to completely avoid the use of additives and manufacture nonwovens,which potentially offer higher biocompatibility and reduced immunogenicity.Temperature and relative humidity during electrospinning were systematically varied(25–35°C,25–30%RH).In a second step,the nonwovens optionally underwent methanol treatment to initiate beta-sheet formation in order to increase structural integrity and strength.Comprehensive surface analysis on the different nonwovens was performed using scanning electron microscopy and supplemented by additional mechanical testing.Cytotoxicity was evaluated using BrdU-assay,XTT-assay,LDH-assay and live-dead staining.Our findings were,that an increase of temperature and relative humidity led to unequal fiber diameters and defective nonwovens.Resistance to penetration decreased accordingly.The most uniform fiber diameters of 998±63 nm were obtained at 30°C and 25%relative humidity,also showing the highest value for resistance to penetration(0.20 N).The according pure fibroin nonwoven also showed no signs of cytotoxicity.However,while the biological response showed statistical evidence,the material characteristics showed no statistically significant correlation to changes of the ambient conditions within the investigated ranges.We suggest that further experiments should explore additional ranges for temperature and humidity and further focus on the repeatability of material properties in dependency of suitable process windows.Alexander Kopp Ralf Smeets Martin Gosau Nadja Kröger Sandra Fuest Marius Köpf Magnus Kruse Judith Krieger Rico Rutkowski Anders Henningsen Simon Burg 2020Bioactive Materials2020,5,2:3
11A family study of endophenotypes for psychosis within an early intervention programme in Hong Kong: Rationale and preliminary findings显示文摘The study of endophenotypes may be a viable strategy to tackle the genetic complexity and phenotypic heterogeneity of psychosis, but this research direction is relatively under-developed in China as compared to Western countries. We have recently initiated one of the first family studies of endophenotypes for psychosis in China. Patients entering an established early psychosis intervention service are recruited into this research project for phenotyping, endophenotyping and genotyping. At the endophenotypic level, four domains (neurological soft signs, neurocognition of prospective memory, social cognition of facial emotion recognition, and affective cognition of anticipatory and consummatory pleasure) are studied in the sample of patients with psychosis and their unaffected siblings. 12A prospective trial of volumetric intensity-modulated arc therapy vs conventional intensity modulated radiation therapy in advanced head and neck cancer显示文摘AIM: To prospectively compare volumetric intensitymodulated arc therapy(VMAT) and conventional intensity-modulated radiation therapy(IMRT) in coverage of planning target volumes and avoidance of multiple organs at risk(OARs) in patients undergoing definitive chemoradiotherapy for advanced(stage Ⅲ or Ⅳ)squamous cell cancer of the head and neck. METHODS: Computed tomography scans of 20 patients with advanced tumors of the larynx, naso-, oroand hypopharynx were prospectively planned using IMRT(7 field) and VMAT using two arcs. Calculated doses to planning target volume(PTV) and OAR were compared between IMRT and VMAT plans. Dose-volume histograms(DVH) were utilized to obtain calculated doses to PTV and OAR, including parotids, cochlea,spinal cord, brainstem, anterior tongue, pituitary and brachial plexus. DVH's for all structures were compared between IMRT and VMAT plans. In addition the planswere compared for dose conformity and homogeneity. The final treatment plan was chosen by the treating radiation oncologist. RESULTS: VMAT was chosen as the ultimate plan in 18 of 20 patients(90%) because the plans were thought to be otherwise clinically equivalent. The IMRT plan was chosen in 2 of 20 patients because the VMAT plan produced concentric irradiation of the cord which was not overcome even with an avoidance structure. For all patients, VMAT plans had a lower number of average monitor units on average(MU = 542.85) than IMRT plans(MU = 1612.58)(P < 0.001). Using the conformity index(CI), defined as the 95% isodose volume divided by the PTV, the IMRT plan was more conformal with a lower conformity index(CI = 1.61) than the VMAT plan(CI = 2.00)(P = 0.003). Dose homogeneity, as measured by average standard deviation of dose distribution over the PTV, was not different with VMAT(1.45 Gy) or IMRT(1.73 Gy)(P = 0.069). There were no differences in sparing organs at risk.CONCLUSION: In this prospective study, VMAT plans were chosen over IMRT 90% of the time. Compared to IMRT, VMAT plans used only one third of the MUs, had shorter treatment times, and similar sparing of OAR. Overall, VMAT provided similar dose homogeneity but less conformity in PTV irradiation compared to IMRT. 15Incidental non-benign gallbladder histopathology after cholecystectomy in an United Kingdom population: Need for routine histological analysis?显示文摘AIM To analyse the range of histopathology detected in the largest published United Kingdom series of cholecystectomy specimens and to evaluate the rational for selective histopathological analysis.METHODS Incidental gallbladder malignancy is rare in the United Kingdom with recent literature supporting selective histological assessment of gallbladders after routine cholecystectomy.All cholecystectomy gallbladder specimens examined by the histopathology department at our hospital during a five year period between March 2008 and March 2013 were retrospectively analysed.Further data was collected on all specimens demonstrating carcinoma,dysplasia and polypoid growths.RESULTS The study included 4027 patients.The majority(97%) of specimens exhibited gallstone or cholecystitis related disease.Polyps were demonstrated in 44(1.09%),the majority of which were cholesterol based(41/44).Dysplasia,ranging from low to multifocal high-grade was demonstrated in 55(1.37%).Incidental primary gallbladder adenocarcinoma was detected in 6 specimens(0.15%,5 female and 1 male),and a single gallbladder revealed carcinoma in situ(0.02%).This large single centre study demonstrated a full range of gallbladder disease from cholecystectomy specimens,including more than 1% neoplastic histology and two cases of macroscopically occult gallbladder malignancies.CONCLUSION Routine histological evaluation of all elective and emergency cholecystectomies is justified in a United Kingdom population as selective analysis has potential to miss potentially curable life threatening pathology.Krashna Patel Khaled Dajani Satheesh Iype Nikolaos A Chatzizacharias Saranya Vickramarajah Susan Davies Rebecca Brais Siong S Liau Simon Harper Asif Jah Raaj K Praseedom Emmanuel L Huguet 2016World Journal of Gastrointestinal Surgery2016,8,10:2
16Effect of weekend admission on geriatric hip fractures显示文摘BACKGROUND The care discrepancy for patients presenting to a hospital on the weekend relative to the work week is well documented.With respect to hip fractures,however,there is no consensus about the presence of a so-called“weekend effect”.This study sought to determine the effects,if any,of weekend admission on care of geriatric hip fractures admitted to a large tertiary care hospital.It was hypothesized that geriatric hip fracture patients admitted on a weekend would have longer times to medical optimization and surgery and increased complication and mortality rates relative to those admitted on a weekday.AIM To determine if weekend admission of geriatric hip fractures is associated with poor outcome measures and surgical delay.METHODS A retrospective chart review of operative geriatric hip fractures treated from 2015-2017 at a large tertiary care hospital was conducted.Two cohorts were compared:patients who arrived at the emergency department on a weekend,and those that arrived at the emergency department on a weekday.Primary outcome measures included mortality rate,complication rate,transfusion rate,and length of stay.Secondary outcome measures included time from emergency department arrival to surgery,time from emergency department arrival to medical optimization,and time from medical optimization to surgery.RESULTS There were no statistically significant differences in length of stay(P=0.2734),transfusion rate(P=0.9325),or mortality rate(P=0.3460)between the weekend and weekday cohorts.Complication rate was higher in patients who presented on a weekend compared to patients who presented on a weekday(13.3%vs 8.3%;P=0.044).Time from emergency department arrival to medical optimization(22.7 h vs 20.0 h;P=0.0015),time from medical optimization to surgery(13.9 h vs 10.8 h;P=0.0172),and time from emergency department arrival to surgery(42.7 h vs 32.5 h;P<0.0001)were all significantly longer in patients who presented to the hospital on a weekend compared to patients who presented to the hospital on a weekday.CONCLUSION This study provided insight into the“weekend effect”for geriatric hip fractures and found that day of presentation has a clinically significant impact on delivered care.Jordan B Pasternack Matthew L Ciminero Michael Silver Joseph Chang Ronald J Simon Kevin K Kang 2020World Journal of Orthopedics2020,11,9:2
