| 2 | Non-invasive diagnosis of liver fibrosis and cirrhosis显示文摘The evaluation and follow up of liver fibrosis and cirrhosis have been traditionally performed by liver biopsy. However, during the last 20 years, it has become evident that this 'gold-standard' is imperfect; even according to its proponents, it is only 'the best' among available methods. Attempts at uncovering non-invasive diagnostic tools have yielded multiple scores, formulae, and imaging modalities. All are better tolerated, safer, more acceptable to the patient, and can be repeated essentially as often as required. Most are much less expensive than liver biopsy. Consequently, their use is growing, and in some countries the number of biopsies performed, at least for routine evaluation of hepatitis B and C, has declined sharply. However, the accuracy and diagnostic value of most, if not all, of these methods remains controversial. In this review for the practicing physician, we analyze established and novel biomarkers and physical techniques. We may be witnessing in recent years the beginning of the end of the first phase for the development of non-invasive markers. Early evidence suggests that they might be at least as good as liver biopsy. | 3 | Transcriptional and epigenetic basis of Treg cell development and function: its genetic anomalies or variations in autoimmune diseases显示文摘Naturally arising regulatory CD4^+T(Treg)cells,which specifically express the transcription factor FoxP3 in the nucleus and CD25 and CTLA-4 on the cell surface,are a T-cell subpopulation specialized for immune suppression,playing a key role in maintaining immunological self-tolerance and homeostasis.FoxP3 is required for Treg function,especially for its suppressive activity.However,FoxP3 expression per se is not necessary for Treg cell lineage commitment in the thymus and insufficient for full Treg-type gene expression in mature Treg cells.It is Treg-specific epigenetic changes such as CpG demethylation and histone modification that can confer a stable and heritable pattern of Treg type gene expression on developing Treg cells in a FoxP3-independent manner.Anomalies in the formation of Treg-specific epigenome,in particular,Treg-specific super-enhancers,which largely include Tregspecific DNA demethylated regions,are indeed able to cause autoimmune diseases in rodents.Furthermore,in humans,single nucleotide polymorphisms in Treg-specific DNA demethylated regions associated with Treg signature genes,such as IL2RA(CD25)and CTLA4,can affect the development and function of naive Treg cells rather than effector T cells.Such genetic variations are therefore causative of polygenic common autoimmune diseases including type 1 diabetes and rheumatoid arthritis via affecting endogenous natural Treg cells.These findings on the transcription factor network with FoxP3 at a key position as well as Tregspecific epigenetic landscape facilitate our understanding of Treg cell development and function,and can be exploited to prepare functionally stable FoxP3-expressing Treg cells from antigen-specific conventional T cells to treat autoimmune diseases. | Naganari Ohkura Shimon Sakaguchi | 2020 | Cell Research2020,30,6: | 29 |
| 4 | Normal vitamin D levels are associated with spontaneous hepatitis B surface antigen seroclearance显示文摘AIM: To investigate a possible association between serum vitamin D levels and spontaneous hepatitis B surface antigen (HBsAg) seroclearance. METHODS: Fifty-three patients diagnosed with chronic inactive hepatitis B and spontaneous HBsAg seroclearance were followed up in two Israeli liver units between 2007 and 2012. This retrospective study reviewed medical charts of all the patients, extracting demographic, serological and vitamin D rates in the serum, as well as medical conditions and current medical therapy. Spontaneous HBsAg seroclearance was defined as the loss of serum HBsAg indefinitely. Vitamin D levels were compared to all patients who underwent spontaneousHBsAg seroclearance.HBsAg seroclearance. RESULTS: Out of the 53 patients who underwent hepatitis B antigen seroclearance, 44 patients (83%) had normal levels of 25-hydroxyvitamin vitamin D compared to 9 patients (17%) who had below normal levels. Multivariate analysis showed that age (>35 years) OR = 1.7 (95%CI: 1.25-2.8, P=0.05), serum vitamin D levels (>20 ng/mL) OR = 2.6 (95%CI: 2.4-3.2, P=0.02), hepatitis B e antigen negativity OR = 2.1 (95%CI: 2.2-3.1, P=0.02), low viral load (hepatitis B virus DNA < 100 IU/mL) OR = 3 (95%CI: 2.6-4.2, P = 0.01) and duration of HBsAg seropositivity (> 8 years) OR = 1.6 (95%CI: 1.15-2.6, P=0.04) were also associated with spontaneous HBsAg seroclearance. | 5 | Success of acupuncture treatment in patients with initially low sperm output is associated with a decrease in scrotal skin temperature显示文摘Poor spermatogenesis in patients with inflammation of the genital tract is associated with scrotal hyperthermia.These patients can benefit from acupuncture treatment.We conducted a study to verify whether the influence of acupuncture treatment on sperm output in patients with low sperm density is associated with a decrease in scrotal temperature.The experimental group included 39 men who were referred for acupuncture owing to low sperm output.The control group,which comprised 18 normal fertile men,was used to define a threshold(30.5℃)above which scrotal skin temperature was considered to be high.Accordingly,34 of the 39 participants in the experimental group initially had high scrotal skin temperature;the other five had normal values.Scrotal skin temperature and sperm concentration were measured before and after acupuncture treatment.The five patients with initially normal scrotal temperatures were not affected by the acupuncture treatment.Following treatment,17 of the 34 patients with hyperthermia,all of whom had genital tract inflammation,had normal scrotal skin temperature;in 15 of these 17 patients,sperm count was increased.In the remaining 17 men with scrotal hyperthermia,neither scrotal skin temperature nor sperm concentration was affected by the treatment.About 90%of the latter patients suffered from high gonadotropins or mixed etiological factors.Low sperm count in patients with inflammation of the genital tract seems to be associated with scrotal hyperthermia,and,consequently,acupuncture treatment is recommended for these men. | Shimon Siterman Fina Eltes Liora Schechter Yair Maimon Hanny Lederman Benjamin Bartoov | 2009 | Asian Journal of Andrology2009,11,2: | 8 |
| 6 | Trabeculectomy with Ex-PRESS implant versus Ahmed glaucoma valve implantation-a comparative study显示文摘AIM:To compare the surgical outcomes of trabeculectomy with Ex-PRESS implant and Ahmed glaucoma valve(AGV) implantation. METHODS: Patients who underwent trabeculectomy with Ex-PRESS implants or AGV implantation separately were included in this retrospective chart review.Main outcome measures were surgical failure and complications.Failure was defined as intraocular pressure(IOP) >21 mm Hg or <5 mm Hg on two consecutive visits after 3mo,reoperation for glaucoma,or loss of light perception.Eyes that had not failed were considered as complete success if they did not required supplemental medical therapy.RESULTS: A total of 64 eyes from 57 patients were included: 31 eyes in the Ex-PRESS group and 33 eyes in the AGV group.The mean follow-up time was 2.6 ±1.1y and 3.3±1.6y,respectively.Patients in the AGV group had significantly higher baseline mean IOP(P =0.005),lower baseline mean visual acuity(VA)(P =0.02),and higher proportion of patients with history of previous trabeculectomy(P <0.0001).Crude failure rates were 16.1%,n =5/31 in the Ex-PRESS group and 24.2%,n =8/33 in the AGV group.The cumulative proportion of failure was similar between the groups,P =0.696.The proportion of eyes that experienced postoperative complications was32.3% in the Ex-PRESS group and 60.1% in the AGV group(P =0.0229).CONCLUSION: Trabeculectomy with Ex-PRESS implant and AGV implantation had comparable failure rates.The AGV group had more post-operative complications,but also included more complex cases with higher baselinemean IOP,worse baseline mean VA,and more previous glaucoma surgeries.Therefore,the results are limited to the cohort included in this study. | Michael Waisbourd Naomi Fischer Hadas Shalev Oriel Spierer Elad Ben Artsi Rony Rachmiel Gabi Shemesh Shimon Kurtz | 2016 | International Journal of Ophthalmology(English edition)2016,9,10: | 6 |
| 7 | Etiology and long-term outcome of extrahepatic portal vein obstruction in children显示文摘AIM:To study the management and outcome of children with extrahepatic portal vein obstruction(EHPVO) in a whole country population.METHODS:A nationwide multicenter retrospective case series of children with EHPVO was conducted.Data on demographics,radiographic studies,laboratory workup,endoscopic and surgical procedures,growth and development,were extracted from the patients' charts.Characteristics of clinical presentation,etiology of EHPVO,management and outcome were analyzed.RESULTS:Thirty patients,13 males and 17 females,19(63.3%) Israeli and 11(36.7%) Palestinians,were included in the analysis.Age at presentation was 4.8± 4.6 years,and mean follow-up was 4.9±4.3 years.Associated anomalies were found in 4 patients.The incidence of EHPVO in Israeli children aged 0-14 years was 0.72/million.Risk factors for EHPVO were detected in 13(43.3%)patients,including 9 patients(30%) with perinatal risk factors,and 4 patients(13.3%) with prothrombotic states:two had low levels of protein S and C,one had lupus anticoagulant,and one was homozygous for methyltetrahydrofolate reductase mutations.In 56.6% of patients,no predisposing factors were found.The most common presenting symptoms were an incidental finding of splenomegaly(43.3%),and upper gastrointestinal bleeding(40%).No differences were found between Israeli and Palestinian children with regard to age at presentation,etiology and clinical symptoms.Bleeding occurred in 18 patients(60%),at a median age of 3 years.Sclerotherapy or esophageal banding was performed in 20 patients.No sclerotherapy complications were reported.Portosystemic shunts were performed in 11 patients(36.6%),at a median age of 11(range 3-17)years:splenorenal in 9,mesocaval in 1,and a meso-Rex shunt in 1 patient.One patient underwent splenectomy due to severe pancytopenia.Patients were followed up for a median of 3(range 0.5-15)years.One patient died aged 3 years due to mucopolysaccharidase deficiency type Ⅲ.None of the patients died due to gastrointestinal bleeding.CONCLUSION:EHPVO is a rare disorder.The etiological factors are still mostly unknown,and the endoscopic and surgical treatment options ensure a good long-term prognosis. | Batia Weiss Eyal Shteyer Asaf Vivante Drora Berkowitz Shimon Reif Zvi Weizman Yoram Bujanover Rivka Shapiro | 2010 | World Journal of Gastroenterology2010,16,39: | 6 |

| 18 | The Tomato Hybrid Proline-rich Protein regulates the abscission zone competence to respond to ethylene signals显示文摘The Tomato Hybrid Proline-rich Protein(THyPRP)gene was specifically expressed in the tomato(Solanum lycopersicum)flower abscission zone(FAZ),and its stable antisense silencing under the control of an abscission zone(AZ)-specific promoter,Tomato Abscission Polygalacturonase4,significantly inhibited tomato pedicel abscission following flower removal.For understanding the THyPRP role in regulating pedicel abscission,a transcriptomic analysis of the FAZ of THyPRP-silenced plants was performed,using a newly developed AZ-specific tomato microarray chip.Decreased expression of THyPRP in the silenced plants was already observed before abscission induction,resulting in FAZ-specific altered gene expression of transcription factors,epigenetic modifiers,post-translational regulators,and transporters.Our data demonstrate that the effect of THyPRP silencing on pedicel abscission was not mediated by its effect on auxin balance,but by decreased ethylene biosynthesis and response.Additionally,THyPRP silencing revealed new players,which were demonstrated for the first time to be involved in regulating pedicel abscission processes.These include:gibberellin perception,Ca2+-Calmodulin signaling,Serpins and Small Ubiquitin-related Modifier proteins involved in post-translational modifications,Synthaxin and SNARE-like proteins,which participate in exocytosis,a process necessary for cell separation.These changes,occurring in the silenced plants early after flower removal,inhibited and/or delayed the acquisition of the competence of the FAZ cells to respond to ethylene signaling.Our results suggest that THyPRP acts as a master regulator of flower abscission in tomato,predominantly by playing a role in the regulation of the FAZ cell competence to respond to ethylene signals. | Srivignesh Sundaresan Sonia Philosoph-Hadas Chao Ma Cai-Zhong Jiang Joseph Riov Raja Mugasimangalam Betina Kochanek Shoshana Salim Michael SReid Shimon Meir | 2018 | Horticulture Research2018,5,1: | 2 |
