4Sentinel lymph node navigation surgery for early stage gastric cancer显示文摘We attempted to evaluate the history of sentinel node navigation surgery(SNNS),technical aspects,tracers,and clinical applications of SNNS using Infrared Ray Electronic Endoscopes(IREE)combined with Indocyanine Green(ICG).The sentinel lymph node(SLN)is defined as a first lymph node(LN)which receives cancer cells from a primary tumor.Reports on clinical application of SNNS for gastric cancers started to appear since early 2000s.Two prospective multicenter trials of SNNS for gastric cancer have also been accomplished in Japan.Kitagawa et al reported that the endoscopic dual(dye and radioisotope)tracer method for SN biopsy was confirmed acceptable and effective when applied to the early-stage gastric cancer(EGC).We have previously reported the usefulness of SNNS in gastrointestinal cancer using ICG as a tracer,combined with IREE(Olympus Optical,Tokyo,Japan)to detect SLN.LN metastasis rate of EGC is low.Hence,clinical application of SNNS for EGC might lead us to avoid unnecessary LN dissection,which could preserve the patient’s quality of life after operation.The most ideal method of SNNS should allow secure and accurate detection of SLN,and real time observation of lymphatic flow during operation.Norio Mitsumori Hiroshi Nimura Naoto Takahashi Masahiko Kawamura Hiroaki Aoki Atsuo Shida Nobuo Omura Katsuhiko Yanaga 2014World Journal of Gastroenterology2014,20,19:15
5Prediction of lymph node metastasis and sentinel node navigation surgery for patients with early-stage gastric cancer显示文摘Accurate prediction of lymph node(LN) status is crucially important for appropriate treatment planning in patients with early gastric cancer(EGC). However,consensus on patient and tumor characteristics associated with LN metastasis are yet to be reached. Through systematic search,we identified several independent variables associated with LN metastasis in EGC,which should be included in future research to assess which of these variables remain as significant predictors of LN metastasis. On the other hand,even if we use these promising parameters,we should realize the limitation and the difficulty of predicting LN metastasis accurately. The sentinel LN(SLN) is defined as first possible site to receive cancer cells along the route of lymphatic drainage from the primary tumor. The absence of metastasis in SLN is believed to correlate with the absence of metastasis in downstream LNs. In this review,we have attempted to focus on several independent parameters which have close relationship between tumor and LN metastasis in EGC. 6Multiple UAVs cooperative formation forming control based on back-stepping-like approach显示文摘To ensure multiple unmanned aerial vehicles(UAVs)reach stable formation quickly, a cooperative guidance law based on the back-stepping-like approach is designed in this paper.Adopting the guidance mechanism of virtue leader vehicle, the dynamic equation of tracking errors for each UAV is built. The communication interactive relationships are described based on graph theory, and the guidance law for formation reaching is obtained by the back-stepping-like approach. The formation stability is analyzed by constructing an appropriate Lyapunov function. 8Sphingosine kinase 1 is upregulated with lysophosphatidic acid receptor 2 in human colorectal cancer显示文摘AIM: To examine the expression of Sph K1, an oncogenic kinase that produces sphingosine 1-phosphate(S1P), and its correlation with the expression of LPAR2, a major lysophosphatidic acid(LPA) receptor overexpressed in various cancers, in human colorectal cancer.METHODS: Real-time reverse-transcription polymerase chain reaction was used to measure the m RNA expression of Sph K1, LPAR2, and the three major S1 P receptors in 27 colorectal cancer samples and corresponding normal tissue samples. We also examined the correlation between the expression of Sph K1 and LPAR2.RESULTS: C o l o r e c t a l c a n c e r t i s s u e i n 2 2 o f 2 7 patients had higher levels of Sph K1 m RNA than in normal tissue. In two-thirds of the samples, Sph K1 m RNA expression was more than two-fold higher than in normal tissue. Consistent with previous reports, LPAR2 m RNA expression in 20 of 27 colorectal cancer tissue samples was higher compared to normal tissue samples. Expression profiles of all three major S1 P receptors, S1PR1, S1PR2, and S1PR3, varied without any trend, with no significant difference in expression between cancer and normal tissues. A highly significant positive correlation was found between Sph K1 and LPAR2 expression [Pearson's correlation coefficient(r) = 0.784 and P < 0.01]. 11Characterization and validation of somatic mutation spectrum to reveal heterogeneity in gastric cancer by single cell sequencing显示文摘Gastric cancer(GC) is a highly heterogeneous disease with multiple cellular types and poor prognosis.However, the cellular evolution and molecular basis of GC at the individual intra-tumor level has not been well demonstrated. We performed single-cell whole exome sequencing to detect somatic singlenucleotide variants(SNVs) and significantly mutated genes(SMGs) among 34 tumor cells and 9 normal cells from a patient with GC. The Complete Prediction for Protein Conformation(CPPC) approach directly predicting the folding conformation of the protein 3D structure with Protein Folding Shape Code, combined with functional experiments were used to confirm the characterization of mutated SMGs in GC cells. We identified 201 somatic SNVs, including 117 non-synonymous mutations in GC cells. Further analysis identified 24 significant mutated genes(SMGs) in single cells, for which a single amino acid change might affect protein conformation. Among them, two genes(CDC27 and FLG) that were mutated only in single cells but not in the corresponding tumor tissue, were recurrently present in another GC tissue cohort, and may play a potential role to promote carcinogenesis, as confirmed by functional characterization. 12Hierarchical structures in climate and atmospheric turbulence显示文摘The climatic time series encompass many time scales, forming multi-scale climatic variations and hierarchies. The periods and magnitudes of cooling or warming, especially for extreme climatic events, vary with the time scales. On the other hand, high-frequency atmospheric turbulent motions, which are certainly related to the climatic changes, are also typical multi-scale phenomena. In this paper, certain properties of the fluctuations are examined that are invariant with the change of the time scales. Specifically, the existence of a statistical similarity law is demonstrated in climatic time series and in atmospheric turbulent motions. This is the hierarchical similarity law. Further analyses show that the hierarchical similarity law is a general property of multi-scale fluctuation systems. 15Early gastric cancer frequently has high expression of KKLC-1, a cancer-testis antigen显示文摘AIM To assess cancer-testis antigens(CTAs) expression in gastric cancer patients and examined their associations with clinicopathological factors.METHODS Eighty-three gastric cancer patients were evaluated in this study. Gastric cancer specimens were evaluated for the gene expression of CTAs, Kitakyushu lung cancer antigen-1(KK-LC-1), melanoma antigen(MAGE)-A1, MAGE-A3 and New York esophageal cancer-1(NYESO-1), by reverse transcription PCR. Clinicopathological background information, such as gender, age, tumor size, macroscopic type, tumor histology, depth of invasion, lymph node metastasis, lymphatic invasion, venous invasion, and pathological stage, was obtained. Statistical comparisons between the expression of each CTA and each clinicopathological background were performed using the χ2 test. RESULTS The expression rates of KK-LC-1, MAGE-A1, MAGE-A3, and NY-ESO-1 were 79.5%, 32.5%, 39.8%, and 15.7%, respectively. In early stage gastric cancer specimens, the expression of KK-LC-1 was 79.4%, which is comparable to the 79.6% observed in advanced stage specimens. The expression of KK-LC-1 was not significantly associated with clinicopathological factors, while there were considerable differences in the expression rates of MAGE-A1 and MAGE-A3 with vs without lymphatic invasion(MAGE-A1, 39.3% vs 13.6%, P = 0.034; MAGE-A3, 47.5% vs 18.2%, P = 0.022) and/or vascular invasion(MAGE-A1, 41.5% vs 16.7%, P = 0.028; MAGE-A3, 49.1% vs 23.3%, P = 0.035) and, particularly, MAGE-A3, in patients with early vs advanced stage(36.5% vs 49.0%, P = 0.044), respectively. Patients expressing MAGE-A3 and NYESO-1 were older than those not expressing MAGE-A3 and NY-ESO-1(MAGE-A3, 73.7 ± 7.1 vs 67.4 ± 12.3, P = 0.009; NY-ESO-1, 75.5 ± 7.2 vs 68.8 ± 11.2, P = 0.042). 18Emphysematous cholecystitis with massive gas in the abdominal cavity显示文摘Emphysematous cholecystitis is a rare variant of acute cholecystitis with a high mortality rate.The combination of emphysematous cholecystitis and pneumoperitoneum is also rare.We herein describe a case of emphysematous cholecystitis with massive gas in the abdominal cavity.A 77-year-old male presented with epigastric pain and lassitude lasting for one week.A computed tomography scan demonstrated massive gas in the abdominal cavity.Gas was also detectable inside the gallbladder.Massive ascites as well as a pleural effusion were also detected.Under the diagnosis of perforation of the digestive tract,we performed emergency surgery.Beyond our expectations,the perforation site was not in the alimentary tract,but rather in the gallbladder.We then diagnosed the patient with emphysematous cholecystitis with perforation,and performed cholecystectomy.A pathological examination of the resected gallbladder revealed necrosis in the mucosa and thinning of the wall.Cultures of the ascites detected Clostridium perfringens,a gas-producing microorganism.Hiroyuki Miyahara Dai Shida Hiroki Matsunaga Yukiko Takahama Sachio Miyamoto 2013World Journal of Gastroenterology2013,19,4:2
