| 1 | 健康素养及运动干预控制糖尿病患者血糖水平的效果评估显示文摘目的评估健康素养及运动干预降低2型糖尿病(T2DM)患者血糖水平的效果。方法以上海市闵行区和长宁区799名最近一次糖化血红蛋白(HbA1c)≥7.5%(或FPG≥10mmol/L)的T2DM患者为研究对象,采用整群随机分组方法分为健康素养干预组、运动干预组、综合干预组(健康素养+运动干预)和对照组,每组各约200人。基线调查、身体测量及生化检测完成后,各干预组分别接受为期1年的干预。第3、6和12个月时,对所有患者进行问卷调查随访、身体测量和HbA1c检测。结果3、6和12个月时患者的总应答率分别为99.4%、98.4%和95.2%。干预满3个月时,综合干预组HbA1c达标率(HbA1c〈7.0%)最高,为25.3%,6和12个月时运动干预组达标率最高,分别为25.3%和34.6%。与基线时相比,各干预组HbA1c值在不同随访时段均有下降,以6个月时最明显,综合干预组平均下降0.48%(95%CI:-0.71%—-0.25%),健康素养干预组下降0.33%(95%a:-0.55%-0.11%),运动干预组下降0.70%(95%CI:-0.92%.0.48%),而对照组略升高0.03%(95%CI:-0.19%~0.25%),差异有统计学意义(P〈0.001)。以对照组为参比,调整可能的混杂因素后,3个月时综合干预组HbA1c效果最佳(β=-0.47,95%CI:-0.73—0.20);随后运动干预的效果逐渐凸现,6和12个月时HbA1c降幅分别达-0.73(95%CI:-0.98—0.47)和-0.75(95%CI:-1.05—-0.45)。分层分析显示,在健康素养较低的患者中3种干预手段均有效,而在算术能力高者中运动干预更为有效。结论健康素养及运动干预均可有效降低上海市社区T2DM患者的HbA1c水平,这将有助于降低患者并发症的发生和早死风险。 | 刘晓娜 夏庆华 方红 李锐 陈英耀 严玉洁 周鹏 姚保栋 姜玉 Russell L Rothman 徐望红 | 2018 | 中华流行病学杂志2018,39,3: | 19 |
| 2 | 健康素养和运动干预对社区2型糖尿病患者医疗不信任的影响研究显示文摘背景医疗不信任可降低患者的医嘱遵从度和服药依从性,影响临床结局,可通过有效的干预得以改善,但相关研究较少。目的评估健康素养干预和运动干预对2型糖尿病(T2DM)患者医疗不信任的影响。方法本研究基于2015年2月至2016年3月开展的一项整群随机对照试验(RCT)。采用多阶段抽样法在上海市闵行区和长宁区各4家社区卫生服务中心的糖尿病管理系统中招募T2DM患者800例,以社区为群随机分入对照组和3个干预组。所有患者接受常规护理,3个干预组在此基础上分别实施基于提高糖尿病教育伙伴关系(PRIDE)工具包的健康素养干预、运动干预、健康素养+运动干预(综合干预)。基线时和干预实施3、6、12个月后及干预结束12个月后对患者开展调查,调查工具包括汉化版健康素养管理表(c-HeLMS)、汉化版糖尿病计算能力测试量表(c-DNT-5)、汉化版医疗不信任指数量表(c-MMI)。采用广义估计方程评估干预效果。结果最终纳入780例患者。c-MMI量表在本人群中具有较好的信度和效度,Cronbach'sα系数为0.826。c-MMI的中位得分为31(7)分,医疗不信任(≥30分)率为65.9%(514/780)。随访期间,四组患者的c-MMI得分及医疗不信任率整体呈下降趋势。与对照组相比,健康素养干预组在3、6个月时医疗不信任风险下降,OR(95%CI)分别为0.42(0.23,0.78)和0.46(0.24,0.88);运动干预组在6、12、24个月时医疗不信任风险下降,OR(95%CI)分别为0.50(0.25,0.99)、0.43(0.22,0.86)、0.37(0.19,0.72);综合干预组在6个月时医疗不信任风险降低,OR(95%CI)为0.30(0.16,0.56)。结论基于PRIDE工具包的健康素养干预和步行运动干预均可有效降低T2DM患者的c-MMI得分,有助于改善患者的医疗不信任。 | 王梦妍 王蕾 陈英耀 方红 夏庆华 Russell L Rothman 徐望红 | 2022 | 中国全科医学2022,25,1: | 15 |
| 3 | Abdominal applications of diffusion-weighted magnetic resonance imaging: Where do we stand?显示文摘Diffusion-weighted imaging (DWI) is one of the magnetic resonance imaging (MRI) sequences providing qualitative as well as quantitative information at a cellular level. It has been widely used for various applications in the central nervous system. Over the past decade, various extracranial applications of DWI have been increasingly explored, as it may detect changes even before signal alterations or morphological abnormalities become apparent on other pulse sequences. Initial results from abdominal MRI applications are promising, particularly in oncological settings and for the detection of abscesses. | 5 | Hematopoietic stem cell-derived adipocytes and fibroblasts in the tumor microenvironment显示文摘The tumor microenvironment(TME) is complex and constantly evolving. This is due, in part, to the crosstalk between tumor cells and the multiple cell types that comprise the TME, which results in a heterogeneous population of tumor cells and TME cells. This review will focus on two stromal cell types, the cancerassociated adipocyte(CAA) and the cancer-associated fibroblast(CAF). In the clinic, the presence of CAAs and CAFs in the TME translates to poor prognosis in multiple tumor types. CAAs and CAFs have an activated phenotype and produce growth factors, inflammatory factors, cytokines, chemokines, extracellular matrix components, and proteases in an accelerated and aberrant fashion. Through this activated state, CAAs and CAFs remodel the TME, thereby driving all aspects of tumor progression, including tumor growth and survival, chemoresistance, tumor vascularization, tumor invasion, and tumor cell metastasis. Similarities in the tumorpromoting functions of CAAs and CAFs suggest that a multipronged therapeutic approach may be necessary to achieve maximal impact on disease. While CAAs and CAFs are thought to arise from tissues adjacent to the tumor, multiple alternative origins for CAAs and CAFs have recently been identified. Recent studies from our lab and others suggest that the hematopoietic stem cell, through the myeloid lineage, may serve as a progenitor for CAAs and CAFs. We hypothesize that the multiple origins of CAAs and CAFs may contribute to the heterogeneity seen in the TME. | 7 | Preoperative immunonutrition in patients undergoing liver resection:A prospective randomized trial显示文摘BACKGROUND Preoperative supplementation with immunonutrients, including arginine and n-3 fatty acids, has been shown in a number of systematic reviews to reduce infectious complications in patients who have undergone gastrointestinal surgery. Limited information, however, is available on the benefits of nutritional supplementation enriched with arginine and n-3 fatty acids in patients undergoing liver resection.AIM To evaluate the effects of preoperative nutritional supplementation enriched with arginine and n-3 fatty acids on inflammatory and immunologic markers and clinical outcome in patients undergoing liver resection.METHODS Thirty-four patients undergoing liver resection were randomized to either five days of preoperative Impact? [1020 kcal/d, immunonutrition(IMN) group], or standard care [no supplementation, standard care(STD) group]. Nutritional status was measured at study entry by subjective global assessment(SGA).Functional assessments(grip strength, fatigue and performance status) were carried out at study entry, on the day prior to surgery, and on postoperative day(POD) 7 and 30. Inflammatory and immune markers were measured at study entry, on the day prior to surgery, and POD 1, 3, 5, 7, 10 and 30. Postoperative complications were recorded prospectively until POD30.RESULTS A total of 32 patients(17 IMN and 15 STD) were analysed. All except four patients were SGA class A. The plasma ratio of(eicosapentaenoic acid plus docosahexaenoic acid) to arachidonic acid was higher in IMN patients on the day prior to surgery and POD 1, 3, 5 and 7(P < 0.05). Plasma interleukin(IL)-6 concentrations were elevated in the IMN group(P = 0.017 for POD7). No treatment effect was detected for functional measures, immune response(white cell count and total lymphocytes) or markers of inflammation(C-reactive protein,tumour necrosis factor-α, IL-8, IL-10). There were 10 patients with infectious complications in the IMN group and 4 in the STD group(P = 0.087).
