| 2 | Imaging response predictors following drug eluting beads chemoembolization in the neoadjuvant liver transplant treatment of hepatocellular carcinoma显示文摘BACKGROUND Drug-eluting bead transarterial chemoembolization(DEB-TACE)is an endovascular treatment to release chemotherapeutic agents within a target lesion,minimizing systemic exposure and adverse effects to chemotherapeutics.Therefore,identifying which patient characteristics may predict imaging response to DEB-TACE can improve treatment results while selecting the best candidates.Predictors of the response after DEB-TACE still have not been fully elucidated.This is the first prospective study performed with standardized DEBTACE technique that aim to identify predictors of radiological response,assessing patients clinical and laboratory characteristics,diagnostic imaging and intraprocedure data of the hepatocellular carcinoma treated in the neoadjuvant context for liver transplantation.AIM To identify pre-and intraoperative clinical and imaging predictors of the radiological response of drug-eluting bead transarterial chemoembolization(DEB-TACE)for the neoadjuvant treatment of hepatocellular carcinoma(HCC).METHODS This is prospective,cohort study,performed in a single transplant center,from 2011 to 2014.Consecutive patients with HCC considered for liver transplant who underwent DEB-TACE in the first session for downstaging or bridging purposes were recruited.Pre and post-chemoembolization imaging studies were performed by computed tomography or magnetic resonance.The radiological response of each individual HCC was evaluated by objective response using mRECIST and the percentage of necrosis.RESULTS Two hundred patients with 380 HCCs were examined.Analysis of the objective response(nodule-based analysis)demonstrated that HCC with pseudocapsules had a 2.01 times greater chance of being responders than those without pseudocapsules(P=0.01),and the addition of every 1mg of chemoembolic agent increased the chance of therapeutic response in 4%(P<0.001).Analysis of the percentage of necrosis through multiple linear regression revealed that the addition of each 1mg of the chemoembolic agent caused an average increase of 0.65%(P<0.001)in necrosis in the treated lesion,whereas the hepatocellular carcinoma with pseudocapsules presented 18.27%(P<0.001)increased necrosis compared to those without pseudocapsules.CONCLUSION The presence of a pseudocapsule and the addition of the amount of chemoembolic agent increases the chance of an objective response in hepatocellular carcinoma and increases the percentage of tumor necrosis following drug-eluting bead chemoembolization in the neoadjuvant treatment,prior to liver transplantation. | Francisco Leonardo Galastri Felipe Nasser Breno Boueri Affonso Leonardo Guedes Moreira Valle Bruno Calazans Odísio Joaquim Mauricio Motta-Leal Filho Paolo Rogério Salvalaggio Rodrigo Gobbo Garcia Márcio Dias de Almeida Ronaldo Hueb Baroni Nelson Wolosker | 2020 | World Journal of Hepatology2020,12,1: | 2 |

| 6 | Metabolomics in chronic hepatitis C:Decoding fibrosis grading and underlying pathways显示文摘BACKGROUND Chronic Hepatitis C(CHC)affects 71 million people globally and leads to liver issues such as fibrosis,cirrhosis,cancer,and death.A better understanding and prognosis of liver involvement are vital to reduce morbidity and mortality.The accurate identification of the fibrosis stage is crucial for making treatment decisions and predicting outcomes.Tests used to grade fibrosis include histological analysis and imaging but have limitations.Blood markers such as molecular biomarkers can offer valuable insights into fibrosis.AIM To identify potential biomarkers that might stratify these lesions and add information about the molecular mechanisms involved in the disease.METHODS Plasma samples were collected from 46 patients with hepatitis C and classified into fibrosis grades F1(n=13),F2(n=12),F3(n=6),and F4(n=15).To ensure that the identified biomarkers were exclusive to liver lesions(CHC fibrosis),healthy volunteer participants(n=50)were also included.An untargeted metabolomic technique was used to analyze the plasma metabolites using mass spectrometry and database verification.Statistical analyses were performed to identify differential biomarkers among groups.RESULTS Six differential metabolites were identified in each grade of fibrosis.This six-metabolite profile was able to establish a clustering tendency in patients with the same grade of fibrosis;thus,they showed greater efficiency in discriminating grades.CONCLUSION This study suggests that some of the observed biomarkers,once validated,have the potential to be applied as prognostic biomarkers.Furthermore,it suggests that liquid biopsy analyses of plasma metabolites are a good source of molecular biomarkers capable of stratifying patients with CHC according to fibrosis grade. | Adriana Camargo Ferrasi Samara Vitória Granja Lima Aline Faria Galvani Jeany Delafiori Flavia Luísa Dias-Audibert Rodrigo Ramos Catharino Giovanni Faria Silva Roberta Rodrigues Praxedes Driele Bretones Santos Dayane Trevisan de Macedo Almeida Estela Oliveira Lima | 2023 | World Journal of Hepatology2023,15,11: | 1 |

| 10 | Diagnosis and management of hepatic artery in-stent restenosis after liver transplantation by optical coherence tomography:A case report显示文摘BACKGROUNDPercutaneous transluminal angioplasty and stenting represent an effectivetreatment for hepatic artery stenosis after liver transplantation. In the first yearafter stenting, approximately 22% of patients experience in-stent restenosis,increasing the risk of artery thrombosis and related complications, and 50%experience liver failure. Although angiography is an important tool for diagnosisand the planning of therapeutic interventions, it may raise doubts, especially insmall-diameter arteries, and it provides low resolution rates compared withnewer intravascular imaging methods, such as optical coherence tomography(OCT).CASE SUMMARYA 64-year-old male developed hepatic artery stenosis one year after orthotropicliver transplantation and was successfully treated with percutaneous transluminalangioplasty with stenting. Five months later, the Doppler ultrasound resultsindicated restenosis. Visceral arteriography confirmed hepatic artery tortuositybut was doubtful for significant in-stent restenosis (ISR) and intrahepatic flowreduction. To confirm ISR, identify the etiology and guide treatment, OCT wasperformed. OCT showed severe stenosis due to four mechanisms: Focal andpartial stent fracture, late stent malapposition, in-stent neointimal hyperplasia,and neoatherosclerosis.CONCLUSIONIntravascular diagnostic methods can be useful in evaluating cases in which initialangiography results are not sufficient to provide a proper diagnosis of significantstenosis, especially with regard to ISR. A wide range of diagnoses are provided byOCT, resulting in different treatment options. | 11 | Queens remate despite traumatic mating in stingless bees显示文摘Males can control female reproduction using genital plugs to impede access by rivals.In social bees,ants,and wasps,plugging may involve traumatic mating,with females being harmed.In stingless bees,chances are that plugs may promote ovarian activan,and are thought to ensure single mating—a general tendency among the social Hymenoptera.However,understanding on relationships between mating plugs,traumatic mating,and mating systems in stingless bees remains limited.To address this,we(1)compared mated queens of 7 Neotropical species to understand the patterns of copulatory marks in females and(2)compared pre-and post-mating genitalia of males and females in Melipona fasciculata to depict plug functional morphology.Data revealed an unprecedented consequence of mating in stingless bees:the characteristic marks left by mating plugs on female abdomens and the inferences that can be made from them.To our surprise,in 1 species M.fasciculata we found that queens retain the plug long after mating,and may carry it for the rest of their lives.All the other 6 species retained the plug for only a short period.Remated queens were only found in M.seminigra,whose multiple copulatory marks match previous findings of polyandry in this species.Our study shows that queens can remate,and suggests that male genital morphology may determine in part the time persistence of plugs.We conclude that traumatic mating plugs do not fully prevent remating in stingless bees and that mating systems are not uniform in this group.Nonetheless,exceptional cases of facultative polyandry in social insects—for example,when mating plugs fail—may confirm a general tendency for single mating in close link with efficient mating plugs. | Jamille Costa VEIGA Gustavo Rodrigo Sanches RUIZ Gislene Almeida CARVALHO-ZILSE Cristiano MENEZES Felipe Andres Leon CONTRERA | 2022 | Current Zoology2022,68,1: | 0 |
| 12 | Treatment of acute severe ulcerative colitis using accelerated infliximab regimen based on infliximab trough level:A case report显示文摘BACKGROUND Acute severe ulcerative colitis (ASUC) is a complication of ulcerative colitisassociated with high levels of circulating tumor necrosis factor alpha, due to theintense inflammation and faster stool clearance of anti-tumor necrosis factordrugs. Dose-intensified infliximab treatment can be beneficial and is associatedwith lower rates of colectomy. The aim of the study was to present a case of apatient with ASUC and megacolon, treated with hydrocortisone and acceleratedscheme of infliximab that was monitored by drug trough level.CASE SUMMARYA 22-year-old female patient diagnosed with ulcerative colitis, presented withdiarrhea, rectal bleeding, abdominal pain, vomiting, and distended abdomen.During investigation, a positive toxin for Clostridium difficile and colonic dilatationof 7 cm consistent with megacolon were observed. She was treated with oralvancomycin for pseudomembranous colitis and intravenous hydrocortisone forsevere colitis, which led to the resolution of megacolon. Due to the persistentsevere colitis symptoms, infliximab 5 mg/kg was prescribed, monitored by drugtrough level (8.8 μg/mL) and fecal calprotectin of 921 μg/g (< 30 μg/g). Based onthe low infliximab trough level after one week from the first infliximab dose, thepatient received a second infusion at week 1, consistent with the acceleratedregimen (infusions at weeks 0, 1, 2 and 6). We achieved a positive clinical andendoscopic response after 6 mo of therapy, without the need for a colectomy.CONCLUSIONInfliximab accelerated infusions can be beneficial in ASUC unresponsive to thetreatment with intravenous corticosteroids. | 18 | Heart failure as an adverse effect of infliximab for Crohn's disease: A case report and review of the literature显示文摘BACKGROUND Anti-tumor necrosis factor agents were the first biologic therapy approved for the management of Crohn's disease(CD).Heart failure(HF)is a rare but potential adverse effect of these medications.The objective of this report is to describe a patient with CD who developed HF after the use of infliximab.CASE SUMMARY A 50-year-old woman with a history of hypertension and diabetes presented with abdominal pain,diarrhea,and weight loss.Colonoscopy and enterotomography showed ulcerations,areas of stenosis and dilation in the terminal ileum,and thickening of the intestinal wall.The patient underwent ileocolectomy and the surgical specimen confirmed the diagnosis of stenosing CD.The patient started infliximab and azathioprine treatment to prevent post-surgical recurrence.At 6 mo after initiating infliximab therapy,the patient complained of dyspnea,orthopnea,and paroxysmal nocturnal dyspnea that gradually worsened.Echocardiography revealed biventricular dysfunction,moderate cardiac insufficiency,an ejection fraction of 36%,and moderate pericardial effusion,consistent with HF.The cardiac disease was considered an infliximab adverse effect and the drug was discontinued.The patient received treatment with diuretics for HF and showed improvement of symptoms and cardiac function.Currently,the patient is using anti-interleukin for CD and is asymptomatic.CONCLUSION This reported case supports the need to investigate risk factors for HF in inflammatory bowel disease patients and to consider the risk-benefit of introducing infliximab therapy in such patients presenting with HF risk factors. | Thais Gagno Grillo Luciana Rocha Almeida Rodrigo Fedatto Beraldo Mariana Barros Marcondes Diego Aparecido Rios Queiróz Daniel Luiz da Silva Rodrigo Quera Julio Pinheiro Baima Rogerio Saad-Hossne Ligia Yukie Sassaki | 2021 | World Journal of Clinical Cases2021,9,33: | 0 |
| 19 | SARS-CoV-2 and hearing:An audiometric analysis of COVID-19 hospitalized patients显示文摘Purpose COVID-19 associated hearing loss is still an ongoing matter of debate.No original studies exist on audiological effects of SARS-CoV-2 infection in hospitalized patients.The main objective was to determine whether SARS-CoV-2 may affect auditory function in clinically ill COVID-19 patients.Materials and methods COVID-19 patients with moderate-severe disease and without prior history of hearing abnormalities were enrolled from a tertiary referral center,and matched with controls.Participants performed an audiometric evaluation,and thresholds were compared.Results 120 ears from 60 patients were enrolled.Patients with COVID-19 showed worse mean auditory thresholds starting from 1000 Hz through higher frequencies,when compared to controls(1000 Hz:18.52±5.49 dB HL in controls vs 25.36±6.79 dB HL in COVID-19,p<0.001;2000Hz:17.50±5.57 dB HL in controls vs 21.96±7.05 dB HL in COVID-19,p=0.010;3000Hz:17.97±8.07 dB HL in controls vs 25±9.38 dB HL in COVID-19,p=0.003;4000 Hz:20.16±10.12 dB HL in controls vs 29.55±11.26 dB HL in COVID-19,p=0.001;8000 Hz:31.09±12.75 dB HL in controls vs 40.71±19.40 dB HL in COVID-19,p=0.030;Pure Tone Average:20.42±4.29 dB HL in controls vs 24.85±5.62 dB HL in COVID-19,p=0.001).Statistical significance persisted after adjusting for confounders such as age,gender and various comorbidities(p<0.05).Conclusions SARS-CoV-2 may affect hearing in COVID-19 patients with moderate-severe disease.Results are in line with the previous suggested effects of COVID-19 on auditory system.This study is expected to encourage further research on this topic. | Francisco Alves de Sousa Rodrigo Pinto Costa Sandra Xará Ana Nóbrega Pinto Cecília Almeida e Sousa | 2021 | Journal of Otology2021,16,3: | 0 |
