1Anterior rectopexy for full-thickness rectal prolapse: Technical and functional results显示文摘AIM:To assess effectiveness, complications, recurrence rate, and recent improvements of the anterior rectopexy procedure for treatment of total rectal prolapse.METHODS:MEDLINE, Pub Med, EMBASE, and other relevant database were searched to identify studies.Randomized controlled trials, non-randomized studies and original articles in English language, with more than 10 patients who underwent laparoscopic ventral rectopexy for full-thickness rectal prolapse, with a follow-up over 3 mo were considered for the review.RESULTS:Twelve non-randomized case series studies with 574 patients were included in the review.No surgical mortality was described.Conversion was needed in 17 cases(2.9%), most often due to difficult adhesiolysis.Twenty eight patients(4.8%) presented with major complications.Seven(1.2%) mesh-related complications were reported.Most frequent complications were urinary tract infection and urinary retention.Mean recurrence rate was 4.7% with a median follow-up of 23 mo.Improvement of constipation ranged from 3%-72% of the patients and worsening or new onset occurred in 0%-20%.Incontinence improved in 31%-84% patients who presented fecal incontinence at various stages.Evaluation of functional score was disparate between studies.CONCLUSION:Based on the low long-term recurrence rate and favorable outcome data in terms of low de novo constipation rate, improvement of anal incontinence, and low complications rate, laparoscopic anterior rectopexy seems to emerge as an efficient procedure for the treatment of patients with total rectal prolapse.Jean-Luc Faucheron Bertrand Trilling Edouard Girard Pierre-Yves Sage Sandrine Barbois Fabian Reche 2015World Journal of Gastroenterology2015,21,16:7
2The impact of fluorination on both donor polymer and non-fullerene acceptor:The more fluorine,the merrier显示文摘Fluorination of the donor polymer or non-fullerene acceptor (NFA) in an organic photovoltaic device is an effective method to improve device efficiency.Although there have been many studies on donor polymer fluorination,blends containing both a fluorinated donor and fluorinated NFA have rarely been reported.In this study,we use two donor polymers (4'-FT-HTAZ and 4'-FT-FTAZ) and two NFAs (ITIC-Th and ITIC-Th1) with different amounts of fluorine (from 2F to 6F) to investigate how the degree of fluorination in a blend impacts device performance.We find that fluorinating the NFA leads to a higher short-circuit current density (Jsc) and fill factor (FF),however,the open-circuit voltage (Voc) is decreased due to a depressed lowest unoccupied molecular orbital (LUMO) level.Adding additional fluorine to the donor polymer does not have a large effect on the Jsc or FF,but it does lead to an improved Voc.By fluorinating the NFA and having more fluorine on the donor polymer,we obtain both a high Jsc and Voc simultaneously,leading to a power conversion efficiency over 10% in the case of 4'-FT-FTAZ:ITIC-Th1,which has the most amount of fluorine (6F).Nicole Bauer Qianqian Zhang Jeromy James Rech Shuixing Dai Zhengxing Peng Harald Ade Jiayu Wang Xiaowei Zhan Wei You 2019Nano Research2019,12,9:3
3Overview of extended release tacrolimus in solid organ transplantation显示文摘Tacrolimus(Prograf?, Astellas Pharma Europe Ltd, Staines, United Kingdom; referred to as tacrolimusBID) is an immunosuppressive agent to prevent and treat allograft rejection in kidney transplant recipients in combination with mycophenolate mofetil, corticosteroids,with or without basiliximab induction. The drug has also been studied in liver, heart and lung transplant; however, these are currently off-label indications. An extended release tacrolimus formulation(Advagraf?, Astagraf XL?) allows for once-daily dosing, with the potential to improve adherence. Extended release tacrolimus has similar absorption, distribution, metabolism and excretion to tacrolimus-BID. Phase Ⅰ pharmacokinetic trials comparing extended release tacrolimus and tacrolimus-BID have demonstrated a decreased maximum concentration(C max) and delayed time to maximum concentration(t max) with the extended release formulation; however, AUC0-24 was comparable between formulations. Overall extended release tacrolimus has a very similar safety and efficacy profile to tacrolimus-BID. It is not recommended in the use of liver transplant patient's due to the increased risk of mortality in female recipients. There has been minimal data regarding the use of extended release tacrolimus in heart and lung transplant recipients.
