| 1 | Duodenal variceal bleeding with large spontaneous portosystemic shunt treated with transjugular intrahepatic portosystemic shunt and embolization: A case report显示文摘BACKGROUND Duodenal variceal bleeding is a rare cause of gastrointestinal bleeding. The most common site is the duodenal bulb. It is usually detected endoscopically but it can be very challenging to diagnose if it is located distal to the second part od duodenum. The pre- transjugular intrahepatic portosystemic shunt (TIPS) presence of spontaneous portosystemic shunt (SPSS) was found to be associated with an increased risk of early morbidity and mortality after TIPS placement. CASE SUMMARY A 43-year-old cirrhotic male presented with melena for three days. Upper endoscopy was performed and showed active blood oozing from the distal duodenum concerning for ectopic duodenal varix. A computed tomography (CT) angiogram was performed and showed an enlarged cluster of venous collaterals around the distal duodenum. He underwent TIPS placement. He had another episode of melena three days later. Push enteroscopy with injection sclerotherapy into the duodenal varices was performed with no success. A repeat CT angiogram showed occluded TIPS shunt. Therefore, a TIPS revision was performed and there was an extensive portal venous thrombosis with a large shunt between the inferior mesenteric vein and left renal vein via the left gonadal vein. Thrombectomy and TIPS shunt balloon angioplasty was performed, followed by embolization of the portosystemic. The melena was resolved, and patient was discharged with arranged hepatology follow up. | 2 | Parallel transjugular intrahepatic portosystemic shunt with Viatorr^(®)? stents for primary TIPS insufficiency:Case series and review of literature显示文摘BACKGROUND Transjugular intrahepatic portosystemic shunts(TIPS) can alleviate complications of portal hypertension such as ascites and variceal bleeding by decreasing the portosystemic gradient.In limited clinical situations,parallel TIPS may be only solution to alleviate either variceal bleeding or ascites secondary to portal hypertension when the primary TIPS fails to do so.Data specifically addressing the use of this partially polytetrafluoroethylene covered nitinol stent(Viatorr~?) is largely lacking despite Viatorr~? being the current gold standard for modern TIPS placement.CASE SUMMARY All three patients had portal hypertension and already had a primary Viatorr~?TIPS placed previously.All patients have undergone failed endoscopy to manage acute variceal bleeding before referral for a parallel stent(PS).PS were placed in patients presenting with recurrent variceal bleeding despite existence of a widely patent primary TIPS.Primary stent patency was verified with either Doppler ultrasound or intra-procedural TIPS stent venography.Doppler ultrasound follow-up imaging demonstrated complete patency of both primary and parallel TIPS.All three patients did well on clinical follow-up of up to six months and no major complications were recorded.A review of existing literature on the role of PS in the management of portal hypertension complications is discussed.There are three case reports of use of primary and PS Viatorr~? stents placement,only one of which is in a patient with gastrointestinal variceal bleeding despite a patent primary Viatorr~? TIPS.CONCLUSION Viatorr~? | 5 | Mechanistic, Energetic and Structural Aspects of the Adsorption of Carmustine on the Functionalized Carbon Nanotubes显示文摘Using density functional theory, noncovalent interactions and two mechanisms of covalent functionalization of drug carmustine with functionalized carbon nanotube(CNT) have been investigated. Quantum molecular descriptors of noncovalent configurations were studied. It was specified that binding of drug carmustine with functionalized CNT is thermodynamically suitable. NTCOOH and NTCOCl can bond to the NH group of carmustine through OH(COOH mechanism) and Cl(COCl mechanism) groups, respectively. The activation energies, activation enthalpies and activation Gibbs free energies of two pathways were calculated and compared with each other. The activation parameters related to COOH mechanism are higher than those related to COCl mechanism, and therefore COCl mechanism is suitable for covalent functionalization. COOH functionalized CNT(NTCOOH) has more binding energy than COCl functionalized CNT(NTCOCl) and can act as a favorable system for carmustine drug delivery within biological and chemical systems(noncovalent).
