| 1 | 食物过敏的定义、流行性、诊断及治疗显示文摘食物过敏是异质性群体相关疾病,会通过一种或多种方式对多个器官造成影响。其反应的严重程度可从轻度的局部反应到全身性的过敏反应。从机制上来看,食物过敏是Th2细胞驱动的免疫紊乱疾病,食物特异性IgE是这种速发型不良反应的基础。过敏的部位和症状并不一定相同。食物过敏常会与自身免疫性(如乳糜泻)和非免疫性(如乳糖不耐受)的其他食物不良反应相混淆。为了正确诊断食物过敏,需要详细了解患者病史,检测特异性IgE,并进行经口激发试验来确诊。一些辅助因素(如运动、药物和酒精)有助于诱发食物过敏,会进一步增加诊断的复杂性。以食物提取物为基础的诊断试验并不能反映症状的严重程度,而检测单个食物致敏原IgE的新一代分子诊断技术,则能够为临床医生和患者提供可靠的症状严重程度依据。分子诊断还有助于确定食物过敏是直接源于对食物的暴露还是间接源于花粉过敏(交叉反应)。流行病学调查表明,桃过敏在欧洲主要是源于对桃的食用,而在中国则是源于对艾蒿花粉的初级致敏。但这两种情况都是由来自同一家族的致敏原分子介导的。流行病学调查深入揭示了食物过敏的病因、流行性及其所包含的风险因素,为预防食物过敏提供了循证策略。过去的十年中,食物过敏在发达国家盛行。经济发展和城市化进程都被认为会导致食物过敏流行性的增加,而且饮食习惯不同,过敏的食物也不同。目前,分子变态反应学和生物技术正在开发安全的免疫治疗方法,有望能够减轻日益加重的食物过敏负担。一种使用低致敏性突变重组分子的免疫疗法已经开始进行第一阶段临床试验的评估。食物致敏原名单的不断完善为转基因食品致敏性的评估提供了坚实基础。 | Ronald van Ree Lars K. | 5 | Molecular phylogeny of Solms-laubachia(Brassicaceae) s.l.,based on multiple nuclear and plastid DNA sequences,and its biogeographic implications显示文摘The Hengduan Mountains region of south-west China is a noted biodiversity hotspot,but the geographic origins and historical assembly of its rich endemic flora,including the sky-island species of Solms-laubachia Muschl.(Brassicaceae),have been little studied.Previous molecular studies on the phylogeny of Solms-laubachia showed it to be paraphyletic,leading to considerable expansion not only of its taxonomic limits,but also its geographic range,with the inclusion of taxa from outside the Hengduan region.However,these studies provided little resolution of interspecific relationships,preventing inferences about historical biogeography within the clade.In the present study,new sequence data from two nuclear genes(LEAFY and G3pdh) and two chloroplast intergenic spacers(petN-psbM and psbM-trnD) were combined with existing markers to increase phylogenetic signals.Phaeonychium villosum(Maxim.) Al-Shehbaz was found to be nested within Solms-laubachia s.l.In general,phylogenetic relationships appear to be a good predictor of geography,with the Hengduan Mountain endemics embedded in a paraphyletic grade of species from the western Himalayas and central Asia,but they also imply morphological homoplasy.Incongruence was detected between the nuclear and chloroplast gene trees,perhaps resulting from incomplete lineage sorting of ancestral polymorphisms.The crown age of Solms-laubachia s.l.was estimated to be approximately 1.42-3.68 mya,using Bayesian relaxed molecular clock analysis.Historical biogeographic analysis using a parametric dispersal-extinction-cladogenesis model inferred central Asia and the western Himalayas as most probable ancestral range of Solms-laubachia s.l.,and estimated higher rates of eastward expansion than westward during the diversification of descendant lineages.In summary,our results suggest that Solms-laubachia s.l.originated during the Pliocene in central Asia,and subsequently migrated eastward into the Hengduan Mountains,colonizing sky-island,alpine scree-slope habitats that may have provided novel ecological opportunity and accelerated speciation,ultimately establishing this region as the present center of diversity of the genus. | Ji-Pei YUE Hang SUN David A. | 8 | Refining pathological evaluation of neoadjuvant therapy for adenocarcinoma of the esophagus显示文摘AIM:To assess tumour regression grade(TRG)and lymph node downstaging to help define patients who benefit from neoadjuvant chemotherapy.METHODS:Two hundred and eighteen consecutive patients with adenocarcinoma of the esophagus or gastro-esophageal junction treated with surgery alone or neoadjuvant chemotherapy and surgery between 2005and 2011 at a single institution were reviewed.Triplet neoadjuvant chemotherapy consisting of platinum,fluoropyrimidine and anthracycline was considered for operable patients(World Health Organization performance status≤2)with clinical stage T2-4 N0-1.Response to neoadjuvant chemotherapy(NAC)was assessed using TRG,as described by Mandard et al.In addition lymph node downstaging was also assessed.Lymph node downstaging was defined by cN1 at diagnosis:assessed radiologically(computed tomography,positron emission tomography,endoscopic ultrasonography),then pathologically recorded as N0 after surgery;ypN0 if NAC given prior to surgery,or pN0if surgery alone.Patients were followed up for 5 years post surgery.Recurrence was defined radiologically,with or without pathological confirmation.An association was examined between t TRG and lymph node downstaging with disease free survival(DFS)and a comprehensive range of clinicopathological characteristics.RESULTS:Two hundred and eighteen patients underwent esophageal resection during the study interval with a mean follow up of 3 years(median follow up:2.552,95%CI:2.022-3.081).There was a 1.8%(n=4)inpatient mortality rate.One hundred and thirty-six(62.4%)patients received NAC,with 74.3%(n=101)of patients demonstrating some signs of pathological tumour regression(TRG 1-4)and 5.9%(n=8)having a complete pathological response.Forty four point one percent(n=60)had downstaging of their nodal disease(cN1 to ypN0),compared to only 15.9%(n=13)that underwent surgery alone(pre-operatively overstaged:cN1 to pN0),(P<0.0001).Response to NAC was associated with significantly increased DFS(mean DFS;TRG 1-2:5.1years,95%CI:4.6-5.6 vs TRG 3-5:2.8 years,95%CI:2.2-3.3,P<0.0001).Nodal down-staging conferred a significant DFS advantage for those patients with a poor primary tumour response to NAC(median DFS;TRG 3-5 and nodal down-staging:5.533 years,95%CI:3.558-7.531 vs TRG 3-5 and no nodal down-staging:1.114 years,95%CI:0.961-1.267,P<0.0001).CONCLUSION:Response to NAC in the primary tumour and in the lymph nodes are both independently associated with improved DFS. | Fergus Noble Luke Nolan Adrian C Bateman James P Byrne Jamie J Kelly Ian S Bailey Donna M Sharland Charlotte N Rees Timothy J Iveson Tim J Underwood Andrew R Bateman | 2013 | World Journal of Gastroenterology2013,19,48: | 4 |

| 14 | DNA barcoding of Pedicularis L.(Orobanchaceae):Evaluating four universal barcode loci in a large and hemiparasitic genus显示文摘One application of DNA barcoding is species identification based on sequences of a short and standardized DNA region.In plants,various DNA regions,alone or in combination,have been proposed and investigated,but consensus on a universal plant barcode remains elusive.In this study,we tested the utility of four candidate barcoding regions(rbcL,matK,trnH-psbA,and internal transcribed spacer(ITS)) as DNA barcodes for discriminating species in a large and hemiparasitic genus Pedicularis(Orobanchaceae).Amplification and sequencing was successful using single primer pairs for rbcL,trnH-psbA,and ITS,whereas two primer pairs were required for matK.Patterns of sequence divergence commonly showed a 'barcoding gap',that is,a bimodal frequency distribution of pairwise distances representing genetic diversity within and between species,respectively.Considering primer universality,ease of amplification and sequencing,and performance in discriminating species,we found the most effective single-region barcode for Pedicularis to be ITS,and the most effective two-region barcode to be rbcL + ITS.Both discriminated at least 78% of the 88 species and correctly identified at least 89% of the sequences in our sample,and were effective in placing unidentified samples in known species groups.Our results suggest that DNA barcoding has the potential to aid taxonomic research in Pedicularis,a species-rich cosmopolitan clade much in need of revision,as well as ecological studies in its center of diversity,the Hengduan Mountains region of China. | Wen-Bin YU Pan-Hui HUANG Richard H.
