| 1 | Role of Joshi's external stabilization system with percutaneous screw fixation in high-energy tibial condylar fractures associated with severe soft tissue injuries显示文摘Purpose: The treatment of high-energy tibial condylar fractures which are associated with severe soft tissue injuries remains contentious and challenging. In this study, we assessed the results of Joshi's external stabilization system (JESS) by using the principle of ligamentotaxis and percutaneous screw fixation for managing high-energy tibial condylar fractures associated with severe soft tissue injuries. Methods: Between June 2008 and June 2010, 25 consecutive patients who were 17-71 years (mean, 39.7), underwent the JESS fixation for high-energy tibial condylar fractures associated with severe soft tissue injuries. Out of 25 patients, 2 were lost during follow-up and in 1 case early removal of frame was done, leaving 22 cases for final follow-up. Among them, 11 had poor skin condition with abrasions and blisters and 2 were open injuries (Gustilo-Anderson grade I & 11). The injury mechanisms were motor vehicle accidents (n - 19), fall from a height (n = 2) and assault (n - 1). The fractures were classified according to Schatzker classification system. Results: There were 7 type-V, 14 type-Vl and 1 type-IV Schatzker's tibial plateau fractures. The average interval between the injury and surgery was 6.8 days (range 2-13). The average hospital stay was 13 days (range, 7-22). The average interval between the surgery and full weight bearing was 13.6 weeks (range 11-20). The average range of knee flexion was 121° (range 105°-135°). The normal extension of the knee was observed in 20 patients, and an extensor lag of 5°-8° was noted in 2 patients. The complications included superficial pin tract infections (n 4) with no knee stiffness. Conclusion: JESS with lag screw fixation combines the benefit of traction, external fixation, and limited internal fixation, at the same time as allowing the ease of access to the soft tissue for wound checks, pin care, dressing changes, measurement of compartment pressure, and the monitoring of the neurovascular status. | 3 | 在在印度的心血管的风险因素的地区性的变化: 印度心手表显示文摘 Cardiovascular disease(CVD) is an important cause of mortality and morbidity in India.Mortality statistics and morbidity surveys indicate substantial regional variations in CVD prevalence and mortality rates.Data from the Registrar General of India reported greater ageadjusted cardiovascular mortality in southern and eastern states of the country.Coronary heart disease(CHD) mortality is greater in south India while stroke is more common in the eastern Indian states.CHD prevalence is higher in urban Indian populations while stroke mortality is similar in urban and rural regions.Case-control studies in India have identified that the common major risk factors account for more than 90% of incident myocardial infarctions and stroke.The case-control INTERHEART and INTERSTROKE studies reported that hypertension,lipid abnormalities,smoking,obesity,diabetes,sedentary lifestyle,low fruit and vegetable intake,and psychosocial stress are as important in India as in other populations of the world.Individual studies have reported that there are substantial regional variations in risk factors in India.At a macro-level these regional variations in risk factors explain some of the regional differences in CVD mortality.However,there is need to study the prevalence of multiple cardiovascular risk factors in different regions of India and to correlate them with variations in CVD mortality using a uniform protocol.There is also a need to determine the 'causes of the causes' or fundamental determinants of these risk factors.The India Heart Watch study has been designed to study socioeconomic,anthropometric and biochemical risk factors in urban populations in different regions of the country in order to identify regional differences. | Rajeev Gupta Soneil Guptha Krishna Kumar Sharma Arvind Gupta Prakash Deedwania | 2012 | World Journal of Cardiology2012,4,4: | 3 |
| 4 | Biloma: An unusual complication in a patient with pancreatic cancer显示文摘The term biloma describes an encapsulated collection of bile within the abdomen, usually secondary to bile duct disruption. The commonest causes reported in the literature are iatrogenic (secondary to hepatobiliary surgery), trauma or complications due to choledocholithiasis. A few cases have been reported as complications of cholangiocarcinoma or acute cholecystitis. We report the case of a 64-year-old man initially diagnosed with a non-obstructive malignancy of the pancreas, who developed a spontaneous intrahepatic biloma 8 mo later. This was identifi ed following a 1-wk history of fever, rigors and icterus. The biloma was identifi ed on computed tomography and subsequently drained under ultrasound guidance. Forty-eight hours later, a stent was inserted endoscopically into his common bile duct and he made an uneventful in-hospital recovery. | 5 | Morpho-anatomical and physicochemical studies of Fumaria indica(Hausskn.)Pugsley显示文摘Objective:To study morpho-anatomical characters and physicochemical analysis of Fumaria indica(F.indica)(Hausskn.)Pugsley,(Fumariaceae),an important medicinal plant used extensively for treating a variety of ailments in various system of indigenous medicine.Methods:Evaluation of the different parts of the plant was carried out to determine the morphoanatomical,physicochemical,phytochemical and HPTLC fingerprinting profile of F.indica and other WHO recommended methods were performed for standardization.Results:Morphoanatomical studies showed compound and pinnatifid leaf,4 to 6 cm in length,linear and oblong in shape and anomocytic arrangement of stomata,thin walled parenchymatous cells,scattered,sclerenchymatous,capped vascular bundles and radiating medullary rays.Physicochemical studies showed foreign matter 0.2%,loss on drying 6.8%,total ash 16.77%,alcohol and water soluble extractives 8.92%and 20.26%,respectively,sugar 17.75%,starch 22.97%and tannins 2.37%.Phytochemical evaluation revealed the presence of carbohydrate,alkaloids,flavonoids,saponins,tannins and sterol.Thin layer chromatography was carried out with different solvents and the best solvent system was chloroform and methanol in 80:20 ratio and revealed 12 spots with different R_f value under UV light 366λ.Conclusions:The results of the study can serve as a valuable source of information and provide suitable standards for identification of this plant material for future investigations and applications. | Prakash Chandra Gupta Ch V Rao | 2012 | Asian Pacific Journal of Tropical Biomedicine2012,2,10: | 2 |

| 7 | A review on ethnobotany,phytochemistry and pharmacology of Fumaria indica(Fumitory)显示文摘Fumaria indica(Hausskn.) Pugsley(Fumariaceae),known as 'Fumitory',is an annual herb found as a common weed all over the plains of India and Pakistan.The whole plant is widely used in traditional and folkloric systems of medicine.In traditional systems of medicine,the plant is reputed for its anthelmintic,diuretic,diaphoretic,laxative,cholagoguc,stomachic and sedative activities and is used to purify blood and in liver obstruction in ethnopharmacology.
