| 4 | Delphi methodology in healthcare research:How to decide its appropriateness显示文摘The Delphi technique is a systematic process of forecasting using the collective opinion of panel members.The structured method of developing consensus among panel members using Delphi methodology has gained acceptance in diverse fields of medicine.The Delphi methods assumed a pivotal role in the last few decades to develop best practice guidance using collective intelligence where research is limited,ethically/logistically difficult or evidence is conflicting.However,the attempts to assess the quality standard of Delphi studies have reported significant variance,and details of the process followed are usually unclear.We recommend systematic quality tools for evaluation of Delphi methodology;identification of problem area of research,selection of panel,anonymity of panelists,controlled feedback,iterative Delphi rounds,consensus criteria,analysis of consensus,closing criteria,and stability of the results.Based on these nine qualitative evaluation points,we assessed the quality of Delphi studies in the medical field related to coronavirus disease 2019.There was inconsistency in reporting vital elements of Delphi methods such as identification of panel members,defining consensus,closing criteria for rounds,and presenting the results.We propose our evaluation points for researchers,medical journal editorial boards,and reviewers to evaluate the quality of the Delphi methods in healthcare research. | Prashant Nasa Ravi Jain Deven Juneja | 2021 | World Journal of Methodology2021,11,4: | 21 |

| 6 | Euglycemic diabetic ketoacidosis:A missed diagnosis显示文摘Euglycemic diabetic ketoacidosis(DKA)is an acute life-threatening metabolic emergency characterized by ketoacidosis and relatively lower blood glucose(less than 11 mmol/L).The absence of hyperglycemia is a conundrum for physicians in the emergency department and intensive care units;it may delay diagnosis and treatment causing worse outcomes.Euglycemic DKA is an uncommon diagnosis but can occur in patients with type 1 or type 2 diabetes mellitus.With the addition of sodium/glucose cotransporter-2 inhibitors in diabetes mellitus management,euglycemic DKA incidence has increased.The other causes of euglycemic DKA include pregnancy,fasting,bariatric surgery,gastroparesis,insulin pump failure,cocaine intoxication,chronic liver disease and glycogen storage disease.The pathophysiology of euglycemic DKA involves a relative or absolute carbohydrate deficit,milder degree of insulin deficiency or resistance and increased glucagon/insulin ratio.Euglycemic DKA is a diagnosis of exclusion and should be considered in the differential diagnosis of a sick patient with a history of diabetes mellitus despite lower blood glucose or absent urine ketones.The diagnostic workup includes arterial blood gas for metabolic acidosis,serum ketones and exclusion of other causes of high anion gap metabolic acidosis.Euglycemic DKA treatment is on the same principles as for DKA with correction of dehydration,electrolytes deficit and insulin replacement.The dextrosecontaining fluids should accompany intravenous insulin to correct metabolic acidosis,ketonemia and to avoid hypoglycemia. | Prashant Nasa Sandeep Chaudhary Pavan Kumar Shrivastava Aanchal Singh | 2021 | World Journal of Diabetes2021,12,5: | 11 |

| 9 | Severe sepsis and septic shock in the elderly:An overview显示文摘The incidence of severe sepsis and septic shock is increasing in the older population leading to increased admissions to the intensive care units(ICUs). The elderly are predisposed to sepsis due to co-existing comorbidities, repeated and prolonged hospitalizations, reduced immunity, functional limitations and above all due to the effects of aging itself. A lower threshold and a higher index of suspicion is required to diagnose sepsis in this patient population because the initial clinical picture may be ambiguous, and aging increases the risk of a sudden deterioration in sepsis to severe sepsis and septic shock. Management is largely based on standard international guidelines with a few modifications. Age itself is an independent risk factor for death in patients with severe sepsis, however, many patients respond well to timely and appropriate interventions. The treatment should not be limited or deferred in elderly patients with severe sepsis only on the grounds of physician prejudice, but patient and family preferences should also be taken into account as the outcomes are not dismal. | 10 | Patients with irritable bowel syndrome-diarrhea have lower disease-specific quality of life than irritable bowel syndrome-constipation显示文摘AIM: To determine effect of irritable bowel syndrome(IBS) subtype on IBS-specific quality of life(QOL) questionnaire and its subscales.METHODS: We studied IBS patients visiting our functional gastroenterology disorder clinic at a tertiary care center of Unites States.IBS and IBS subtype were diagnosed using Rome-Ⅲ questionnaire.QOL was assessed using IBS-QOL questionnaire.IBSQOL assesses quality of life along eight subscales: dysphoria,interference with activities,body image,health worry,food avoidance,social reactions,sexual health,and effect on relationships.IBS-QOL and its subscales were both scored on a range of 0-100 with higher scores suggestive of better QOL.Results of overall IBS-QOL scores and subscale scores are expressed as means with 95%CI.We compared mean IBS-QOL score and its subscales among various IBSsubtypes.Analysis of variance(ANOVA) was used to compare the mean difference between more than two groups after controlling for age and gender.A posthoc analysis using Bonferroni correction was used only when P value for ANOVA was less than 0.05.RESULTS: Of 542 patients screened,243 had IBS as per Rome-Ⅲ criteria.IBS-mixed(IBS-M) was the most common IBS subtype(121 patients,49.8%) followed by IBS- diarrhea(IBS-D)(56 patients,23.1%),IBSconstipation(IBS-C)(54 patients,22.2%) and IBSunspecified(IBS-U)(12 patients,4.9%).Overall IBSQOL scores were significantly different among various IBS-subtypes(P = 0.01).IBS-QOL of patients with IBS-D(61.6,95%CI: 54.0-69.1) and IBS-M(63.0,95%CI: 58.1-68.0) was significantly lower than patients with IBS-C(74.5,95%CI: 66.9-82.1)(P = 0.03 and 0.02 respectively).IBS-D patients scored significantly lower than IBS-C on food avoidance(45.0,95%CI: 34.8-55.2 vs 61.1,95%CI: 50.8-71.3,P = 0.04) and interference with activity(59.6,95%CI: 51.4-67.7 vs 82.3,95%CI: 74.1-90.6,P < 0.001).IBS-M patients had more interference in their activities(61.6,95%CI: 56.3-66.9 vs 82.3,95%CI: 74.1-90.6,P = 0.001) and greater impact on their relationships(73.3,95%CI: 68.4-78.2 vs 84.7,95%CI: 77.2-92.2,P = 0.02) than IBS-C patients.Patients with IBS-M also scored significantly lower than IBS-C on food avoidance(47.2,95%CI: 40.7-53.7 vs 61.1,95%CI: 50.8-71.3,P = 0.04) and social reaction(66.1,95%CI: 61.1-71.1 vs 80.0,95%CI: 72.1-87.7,P = 0.005).CONCLUSION: IBS-D and IBS-M patients have lower IBS-QOL than IBS-C patients.Clinicians should recognize food avoidance,effects on daily activities and relationship problems in these patients. | Prashant Singh Kyle Staller Kenneth Barshop Elaine Dai Jennifer Newman Sonia Yoon Shahar Castel Braden Kuo | 2015 | World Journal of Gastroenterology2015,21,26: | 9 |

This host-pathogen barrier will be possibly countered in future research by comparative genomics data from available genome sequence data of rice and M. oryzae for durable resistance. | 14 | Spectroscopic analysis on the binding interaction of biologically active pyrimidine derivative with bovine serum albumin显示文摘A biologically active antibacterial reagent, 2–amino-6-hydroxy–4–(4-N, N-dimethylaminophenyl)-pyrimidine-5-carbonitrile(AHDMAPPC), was synthesized. It was employed to investigate the binding interaction with the bovine serum albumin(BSA) in detail using different spectroscopic methods. It exhibited antibacterial activity against Escherichia coli and Staphylococcus aureus which are common food poisoning bacteria. The experimental results showed that the fluorescence quenching of model carrier protein BSA by AHDMAPPC was due to static quenching. The site binding constants and number of binding sites(n E 1) were determined at three different temperatures based on fluorescence quenching results. The thermodynamic parameters, enthalpy change(ΔH), free energy(ΔG) and entropy change(ΔS) for the reaction were calculated to be 15.15 k J/mol, –36.11 k J/mol and 51.26 J/mol K according to van't Hoff equation, respectively. The results indicated that the reaction was an endothermic and spontaneous process, and hydrophobic interactions played a major role in the binding between drug and BSA. The distance between donor and acceptor is 2.79 nm according to F?rster's theory. The alterations of the BSA secondary structure in the presence of AHDMAPPC were confirmed by UV–visible, synchronous fluorescence, circular dichroism(CD) and three-dimensional fluorescence spectra. All these results indicated that AHDMAPPC can bind to BSA and be effectively transported and eliminated in the body. It can be a useful guideline for further drug design. | Vishwas D. | 17 | 含氨燃料预混火焰的层流火焰速度及NO排放特性显示文摘将甲烷或氢气与氨气共燃可以克服NH_(3)火焰的点火能量高、燃烧速度慢的缺点。为了解NH_(3)作为燃料的燃烧特性,对含NH_(3)燃料进行一维层流预混火焰数值模拟,研究其层流火焰速度及NO排放特性。采用文献中5个简化反应机理进行数值计算,发现Okafor机理模拟NH_(3)/CH_(4)/air火焰精度更高;Xiao机理模拟NH_(3)/H_(2)/air、NH_(3)/air精度适中,计算时间较短。此外,开展了当量比、燃料混合物组分比例、压力等参数对含NH_(3)燃料燃烧时烟气中NO浓度影响的研究。研究发现:含NH_(3)燃料燃烧时NO主要通过OH、H、O自由基和O_(2)分子的消耗而生成,主要通过与NH_(i)(i=0,1,2)自由基反应消耗;含NH_(3)燃料在富燃状态下燃烧可有效减少NO排放,但富燃燃烧效率低,可采用富燃-贫燃分级燃烧技术来提高燃烧效率,同时保持NO的低排放;掺有较多NH_(3)的含NH_(3)燃料在中高压下燃烧时可有效减少NO排放。 | 毛晨林 王平 Prashant Shrotriya 何宏凯 Antonio Ferrante | 2021 | 化工学报2021,72,10: | 6 |
| 18 | 基于强化学习的自主式水下潜器障碍规避技术(英文)显示文摘Obstacle avoidance becomes a very challenging task for an autonomous underwater vehicle(AUV)in an unknown underwater environment during exploration process.Successful control in such case may be achieved using the model-based classical control techniques like PID and MPC but it required an accurate mathematical model of AUV and may fail due to parametric uncertainties,disturbance,or plant model mismatch.On the other hand,model-free reinforcement learning(RL)algorithm can be designed using actual behavior of AUV plant in an unknown environment and the learned control may not get affected by model uncertainties like a classical control approach.Unlike model-based control model-free RL based controller does not require to manually tune controller with the changing environment.A standard RL based one-step Q-learning based control can be utilized for obstacle avoidance but it has tendency to explore all possible actions at given state which may increase number of collision.Hence a modified Q-learning based control approach is proposed to deal with these problems in unknown environment.Furthermore,function approximation is utilized using neural network(NN)to overcome the continuous states and large statespace problems which arise in RL-based controller design.The proposed modified Q-learning algorithm is validated using MATLAB simulations by comparing it with standard Q-learning algorithm for single obstacle avoidance.Also,the same algorithm is utilized to deal with multiple obstacle avoidance problems. | Prashant Bhopale Faruk Kazi Navdeep Singh | 2019 | Journal of Marine Science and Application2019,18,2: | 5 |
| 19 | 插层FeSe高温超导体的高压研究进展显示文摘通过对FeSe进行化学插层可以将其超导转变温度(T_c)从约8 K提高到40 K以上,实现高温超导电性.最近,我们对两种插层FeSe高温超导材料(Li_(0.84)Fe_(0.16))OHFe_(0.98)Se和Li_(0.36)(NH_3)_yFe_2Se_2开展了高压调控研究,发现压力会首先抑制高温超导相(称为SC-I相),然后在临界压力P_c以上诱导出第二个高温超导相(称为SC-Ⅱ相),呈现出双拱形T-P超导相图.这两个体系的P_c分别约为5和2 GPa,两个体系SC-Ⅱ相的最高T_c分别可以达到约52和55 K,比相应SC-I相的初始T_c提高了10 K.对(Li_(0.84)Fe_(0.16))OHFe_(0.98)Se的正常态电输运性质分析表明,SC-Ⅰ和SC-Ⅱ相的正常态分别具有费米液体和非费米液体行为,意味着这两个超导相可能存在显著差异.此外,还发现这两个体系的SC-Ⅱ相的T_c与霍尔系数倒数1/R_H (∝载流子浓度n_e)具有很好的线性依赖关系.对(Li_(0.84)Fe_(0.16))OHFe_(0.98)Se的高压X射线衍射测量排除了其在10 GPa以内发生结构相变的可能,因此P_c以上SC-Ⅱ相的出现和载流子浓度的增加很可能起源于压力导致的费米面重构. | 孙建平 Prashant Shahi 周花雪 倪顺利 王少华 雷和畅 王铂森 董晓莉 赵忠贤 程金光 | 2018 | 物理学报2018,67,20: | 4 |
| 20 | COVID-19 and the liver: What do we know so far?显示文摘The coronavirus disease 2019(COVID-19)pandemic has caused unprecedented pressure on public health and healthcare.The pandemic surge and resultant lockdown have affected the standard-of-care of many medical conditions and diseases.The initial uncertainty and fear of cross transmission of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)have changed the routine management of patients with pre-existing liver diseases,hepatocellular carcinoma,and patients either listed for or received a liver transplant.COVID-19 is best described as a multisystem disease caused by SARS-CoV-2,and it can cause acute liver injury or decompensation of the pre-existing liver disease.There has been considerable research on the pathophysiology,infection transmission,and treatment of COVID-19 in the last few months.The pathogenesis of liver involvement in COVID-19 includes viral cytotoxicity,the secondary effect of immune dysregulation,hypoxia resulting from respiratory failure,ischemic damage caused by vascular endotheliitis,congestion because of right heart failure,or drug-induced liver injury.Patients with chronic liver diseases,cirrhosis,and hepatocellular carcinoma are at high risk for severe COVID-19 and mortality.The phase Ⅲ trials of recently approved vaccines for SARS-CoV-2 did not include enough patients with pre-existing liver diseases and excluded immunocompromised patients or those on immunomodulators.This article reviews the currently published research on the effect of COVID-19 on the liver and the management of patients with pre-existing liver disease,including SARS-CoV-2 vaccines. | Prashant Nasa George Alexander | 2021 | World Journal of Hepatology2021,13,5: | 4 |
