| 2 | Determinants of percutaneous coronary intervention success in repeat chronic total occlusion procedures following an initial failed attempt显示文摘AIM To investigate the rates and determinants of success of repeat percutaneous coronary intervention(PCI) following an initial failed attempt at recanalising the chronic total occlusions(CTO) percutaneously.METHODS In 445 consecutive first attempt CTO-PCI procedures in our institution,procedural failure occurred in 149(33.5%). Sixty-four re-PCI procedures were performed in 58 patients(39%) all had a single CTO. Procedural and outcome data in the re-PCI population was entered into the institutional database. A retrospective analysis of clinical,angiographic and procedural data was performed. RESULTS Procedural success was achieved in 41(64%) procedures. Univariate analysis of clinical and angiographic characteristics showed that re-PCI success was associated with intravascular ultrasound(IVUS) guidance(19.5% vs 0%,P = 0.042),while failure was associated with severecalcification(30.4% vs 9.7%,P = 0.047) and a JCTO score > 3(56.5% vs 17.1% P = 0.003). Following multiple regression analysis the degree of lesion complexity(J-CTO score > 3),IVUS use,involvement of an experienced CTO operator and LAD CTO location were significant predictors of successful re-PCI. Overall the complication rate was low,with the only MACCE two periprocedural MI's neither of which required intervention. CONCLUSION Re-PCI substantially increases the overall success rate of CTO revascularization. | 3 | Presacral venous bleeding during mobilization in rectal cancer显示文摘AIM To analyze the anatomy of sacral venous plexus flow, the causes of injuries and the methods for controlling presacral hemorrhage during surgery for rectal cancer.METHODS A review of the databases MEDLINE? and Embase? was conducted, and relevant scientific articles published between January 1960 and June 2016 were examined. The anatomy of the sacrum and its venous plexus, as well as the factors that influence bleeding, the causes of this complication, and its surgical management were defined.RESULTS This is a review of 58 published articles on presacral venous plexus injury during the mobilization of the rectum and on techniques used to treat presacral venous bleeding. Due to the lack of cases published in the literature, there is no consensus on which is the best technique to use if there is presacral bleeding during mobilization in surgery for rectal cancer. | 7 | Coronary angiography findings in cardiac arrest patients with non-diagnostic post-resuscitation electrocardiogram:A comparison of shockable and non-shockable initial rhythms显示文摘AIM To investigate the impact of coronary artery disease in a cohort of patients resuscitated from cardiac arrest with non-diagnostic electrocardiogram.METHODS From March 2004 to February 2016, 203 consecutive patients resuscitated from in or out-of-hospital sudden cardiac arrest and non-diagnostic post-resuscitation electrocardiogram(defined as ST segment elevation or pre-sumably new left bundle branch block) whounderwent invasive coronary angiogram during hospitalization were included. For purpose of analysis and comparison, patients were classified in two groups: Initial shockable rhythm(ventricular tachycardia or ventricular fibrillation; n = 148, 72.9%) and initial non-shockable rhythm(n = 55, 27.1%). Baseline characteristics, coronary angiogram findings including Syntax Score and long-term survival rates were compared. RESULTS Sudden cardiac arrest was witnessed in 95.2% of cases, 66.7% were out-of-hospital patients and 72.4% were male. There were no significant differences in baseline characteristics between groups except for higher mean age(68.1 years vs 61 years, P = 0.001) in the nonshockable rhythm group. Overall 5-year mortality of the resuscitated patients was 37.4%. Patients with non-shockable rhythms had higher mortality(60% vs 29.1%, P < 0.001) and a worst neurological status at hospital discharge based on cerebral performance category score(CPC 1-2: 32.7% vs 53.4%, P = 0.02). Although there were no significant differences in global burden of coronary artery disease defined by Syntax Score(mean Syntax Score: 10.2 vs 10.3, P = 0.96) there was a trend towards a higher incidence of acute coronary lesions in patients with shockable rhythm(29.7% vs 16.4%, P = 0.054). There was also a higher need for ad-hoc percutaneous coronary intervention in this group(21.9% vs 9.1%, P = 0.03). | 19 | Breeding effects on the genotype×environment interaction for yield of durum wheat grown after the Green Revolution:The case of Spain显示文摘This study evaluates the changes caused by breeding in the genotype by environment(G×E)interaction of the durum wheat varieties most widely cultivated in Spain after the Green Revolution.A set of 12 cultivars was tested in 27 environments,which are understood as the combination of different sites,years,and treatments(water regime and planting dates),representative of the durum wheat growing conditions in Spain with average grain yields(GY)ranging between 2.8 and 9.1 Mg ha^-1.The most important environmental factors affecting the G×E interaction for yield were themaximum and the mean temperature during the entire crop cycle.An improvement in genetic yield was observed in warm environments and under optimal water conditions that resemble those where the germplasm originated(essentially as advanced lines)before its release in Spain.Therefore,the adaptation of semi-dwarf durum in Spain has shown a tendency to specific adaptation rather than large-scale adaptation.Two different patterns of selection have been reported due the G×E interaction and changes in the ranking of genotypes:in the high yielding environments(GY>5 Mg ha^-1),plants favor increased water uptake,with higher levels of transpiration and more open stomata(more negative values of carbon isotope composition,δ^13 C,and higher canopy temperature depression,CTD),whereas,in low yielding environments(GY<5 Mg ha^-1)plants close their stomata and favor greater water use efficiency(less negativeδ^13 C values and lower CTD values). | Fadia Chairi Nieves Aparicio Maria Dolores Serret José Luis Araus | 2020 | The Crop Journal2020,8,4: | 1 |
