| 1 | Surgical outcome after docetaxel-based neoadjuvant chemotherapy in locally-advanced gastric cancer显示文摘AIM:To investigate feasibility,morbidity and surgical mortality of a docetaxel-based chemotherapy regimen randomly administered before or after gastrectomy in patients suffering from locally-advanced resectable gastric cancer.METHODS:Patients suffering from locally-advanced(T3-4 any N M0 or any T N1-3 M0)gastric carcinoma,staged with endoscopic ultrasound,bone scan,computed tomography,and laparoscopy,were assigned to receive four 21 d/cycles of TCF(docetaxel 75 mg/m 2 day 1,cisplatin 75 mg/m 2 day 1,and fluorouracil 300 mg/m 2 per day for days 1-14),either before(Arm A)or after(Arm B)gastrectomy.Operative morbidity,overall mortality,and severe adverse events were compared by intention-to-treat analysis.RESULTS:From November 1999 to November 2005,70 patients were treated.After preoperative TCF(Arm A),thirty-two(94%)resections were performed,85% of which were R0.Pathological response was complete in 4 patients(11.7%),and partial in 18(55%).No surgical mortality and 28.5%morbidity rate were observed,similar to those of immediate surgery arm(P= 0.86).Serious chemotherapy adverse events tended to be more frequent in arm B(23%vs 11%,P=0.07),with a single death per arm.CONCLUSION:Surgery following docetaxel-based chemotherapy was safe and with similar morbidity to immediate surgery in patients with locally-advanced resectable gastric carcinoma. | Roberto Biffi Nicola Fazio Fabrizio Luca Antonio Chiappa Bruno Andreoni Maria Giulia Zampino Arnaud Roth Jan Christian Schuller Giancarla Fiori Franco Orsi Guido Bonomo Cristiano Crosta Olivier Huber | 2010 | World Journal of Gastroenterology2010，16，7： | 43 |

| 3 | 喀喇昆仑断裂带走滑过程中伴随的快速隆升作用:热年代学证据显示文摘喀喇昆仑断裂带是青藏高原西部的一条大型右旋走滑断裂带,它是喜马拉雅山脉西段北侧重要的地质边界。本文在岩石学、变形构造的研究基础上,对喀喇昆仑断裂带东南段阿伊拉日居山—噶尔盆地地区的喀喇昆仑韧性剪切带中变质岩石的同构造矿物进行了^(40)Ar/^(39)Ar热年代学研究。显微构造研究表明,剪切带中的矿物记录了从高温(>600℃)到低温(<250℃)条件下的连续变形,表现为近水平的右旋剪切运动转变成斜向的右旋正滑,使绿片岩相的变形作用叠加在中-高温变形之上,暗示出走滑过程中存在隆升作用,热年代学结果显示其连续剪切变形作用从早中新世以来至少持续到4Ma,并且出现三个快速冷却阶段:第一个快速冷却阶段为从25~22Ma 到21~18Ma 期间,可能代表的是浅部高温剪切过程中变形局部停止或减慢的过程;第二个快速冷却时期为从15Ma 到12~10Ma,是喀喇昆仑断裂带走滑过程中,阿伊拉日居山的快速隆升、噶尔盆地开始形成以及主要河流深切过程阶段;9Ma 以来是第三个快速冷却过程,使阿伊拉日居山脉进一步快速隆升、噶尔盆地定形过程。根据不同年代地表地貌特征的右旋错位距离以及不同层次变形特征,估算出喀喇昆仑断裂带长期滑移速率为8~10mm/a,伴随的隆升速率为1mm/a。从显微构造和热年代学证据表明,晚第四纪以来该断裂经历了强烈的右旋走滑运动的同时伴随强烈的隆升作用。 | 李海兵 VALLI Franck ARNAUD Nicolas 陈松永 许志琴 TAPPONNIER Paul LACASSIN Robin 司家亮 邱祝礼 | 2008 | 岩石学报2008，24，7： | 14 |
| 4 | High tacrolimus intra-patient variability is associated with graft rejection,and de novo donor-specific antibodies occurrence after liver transplantation显示文摘AIM To investigate the role of tacrolimus intra-patient variability(IPV) in adult liver-transplant recipients.METHODS We retrospectively assessed tacrolimus variability in a cohort of liver-transplant recipients and analyzed its effect on the occurrence of graft rejection and de novo donor-specific antibodies(dn DSAs), as well as graft survival during the first 2 years posttransplantation. Between 02/08 and 06/2015, 116 patients that received tacrolimus plus mycophenolate mofetil(with or without steroids) were included. RESULTS Twenty-two patients(18.5%) experienced at least one acute-rejection episode(BPAR). Predictive factors for a BPAR were a tacrolimus IPV of > 35% [OR = 3.07 95%CI(1.14-8.24), P = 0.03] or > 40% [OR = 4.16(1.38-12.50), P = 0.01), and a tacrolimus trough level of < 5 ng/mL [OR=3.68(1.3-10.4), P =0.014]. Thirteen patients(11.2%) developed at least one dn DSA during the follow-up. Tacrolimus IPV [coded as a continuous variable: OR = 1.1, 95%CI(1.0-1.12), P = 0.006] of > 35% [OR = 4.83, 95%CI(1.39-16.72), P = 0.01] and > 40% [OR = 9.73, 95%CI(2.65-35.76), P = 0.001] were identified as predictors to detect dn DSAs. IPV did not impact on patient-or graft-survival rates during the follow-up. | 6 | Prognostic value of ^(18)F-FDG PET/CT in liver transplantation for hepatocarcinoma显示文摘AIM:To evaluate the prognostic value of pretreatment F D G p o s i t r o n e m i s s i o n t o m o g ra p h y c o m p u t e d tomography(PET-CT) in patients with hepatocarcinoma treated by liver transplantation(LT).METHODS:The authors retrospectively analyzed the data of 27 patients(mean age 58 ± 9 years) who underwent FDG PET-CT before LT for hepatocarcinoma.Mean follow-up was 26 ± 18 mo.The FDG PET/CT was performed according to a standard clinical protocol:4 MBq FDG/kg body weight,uptake 60 min,low-dose non-enhanced CT.The authors measured the SUVmax and SUVmean of the tumor and the normal liver.The tumor/liver activity ratios(RSUVmax and RSUVmean) were tested as prognostic factors and compared to the following conventional prognostic factors:MILAN,CLIP,OKUDA,TNM stage,alphafoetoprotein level,portal thrombosis,size of the largest nodule,tumor differentiation,microvascular invasion,underlying cirrhosis and liver function.RESULTS:Overall and recurrence free survivals were80.7%and 67.4%at 3 years,and 70.6%and 67.4%at 5 years,respectively.According to a multivariate Cox model,only FDG PET/CT RSUVmax predicted recurrence free survival.Even though the MILAN criteria alone were not predictive,it is worth noting that none of the patients outside the MILAN criteria and with RSUVmax<1.15 relapsed.CONCLUSION:FDG PET/CT with an RSUVmax cutoff value of 1.15 is a strong prognostic factor for recurrence and death in patients with HCC treated by LT in this retrospective series.Further prospectivestudies should test whether this metabolic index should be systematically included in the preoperative assessment. | Olivier Detry Laurence Govaerts Arnaud Deroover Morgan Vandermeulen Nicolas Meurisse Serge Malenga Noella Bletard Charles Mbendi Anne Lamproye Pierre Honoré Paul Meunier Jean Delwaide Roland Hustinx | 2015 | World Journal of Gastroenterology2015，21，10： | 5 |
