| 2 | The predictive value of triglyceride-glucose index for in-hospital and one-year mortality in elderly non-diabetic patients with ST-segment elevation myocardial infarction显示文摘BACKGROUND Triglyceride-glucose(TyG)index,a reliable marker of insulin resistance,was associated with cardiovascular morbidity and mortality.This study aimed to evaluate the predictive value of TyG index for mortality in elderly non-diabetic pa-tients with ST-segment elevation myocardial infarction(STEMI).METHODS In total,430 non-diabetic patients aged over 65 years with STEMI were consecutively included.The TyG index was calculated by using the following formula:TyG index=ln(fasting triglyceride×fasting glucose/2).The 5-item modified frailty index score was utilized to determine comorbidities.Patients were divided into three groups based on TyG index tertiles.RESULTS Patients included into high TyG index tertile were male and had higher body mass index,glucose,triglyceride,low-density lipoprotein cholesterol,5-item modified frailty index and GRACE risk score;and had low systolic blood pressure,estim-ated glomerular filtration rate and left ventricular ejection fraction.A one-unit increase in TyG index was associated with 3.03 ex-tra cases per 1000 person-day for in-hospital mortality and 0.29 extra cases per 1000 person-day for long-term mortality.There was a non-linear relationship between TyG index and the risk of mortality with an increased risk above 8.5 for TyG index.Kaplan-Meier survival curves revealed that patients in high TyG index tertile had higher in-hospital and long-term mortality rates than those in low TyG index tertile.CONCLUSIONS This was the first study to demonstrate that the TyG index could predict in-hospital and long-term mortality in elderly non-diabetic STEMI patients. | FaysalŞaylık TufanÇınar Murat Selçuk İbrahim Halil Tanboğa | 2022 | Journal of Geriatric Cardiology2022，19，8： | 3 |

| 4 | Alvarado, Eskelinen, Ohhmann and Raja Isteri Pengiran Anak Saleha Appendicitis scores for diagnosis of acute appendicitis显示文摘AIM:To assess the reliability and practical applicability of the widely used Alvarado,Eskelinen,Ohhmann and Raja Isteri Pengiran Anak Saleha Appendicitis(RIPASA)scoring systems in patients with suspected acute appendicitis.METHODS:Patients admitted to our tertiary center due to suspected acute appendicitis constituted the study group.Patients were divided into two groups.appendicitis group(Group A)consisted of patients who underwent appendectomy and were histopathologically diagnosed with acute appendicitis,and non-appendicitis group(Group N-A)consisted of patients who underwent negative appendectomy and were diagnosed with pathologies other than appendicitis and patients that were followed non-operatively.The operative findings for the patients,the additional analyses from follow up of the patients and the results of those analyses were recorded using the follow-up forms.RESULTS:One hundred and thirteen patients with suspected acute appendicitis were included in the study.Of the 113 patients(62 males,51 females),the mean age was 30.2±10.1(range 18-67)years.Of the 113patients,94 patients underwent surgery,while the rest were followed non-operatively.Of the 94 patients,77patients were histopathologically diagnosed with acute appendicitis.Our study showed a sensitivity level of81%for the Alvarado system when a cut-off value of 6.5was used,a sensitivity level of 83.1%for the Ohmann system when a cut-off value of 13.75 was used,a sensitivity level of 80.5%for the Eskelinen system when a cut-off value of 63.72 was used,and a sensitivity level of 83.1%for the RIPASA system when a cut-off value of 10.25 was used.CONCLUSION:The Ohmann and RIPASA scoring systems had the highest specificity for the diagnosis of acute appendicitis. | Hasan Erdem Süleyman etinkünar Koray Das Enver Reyhan Cumhur Deger Mehmet Aziret Hilmi Bozkurt Seluk Uzun Selim Szen Oktay rkrücü | 2013 | World Journal of Gastroenterology2013，19，47： | 2 |
| 5 | COVID-19:Gastrointestinal manifestations,liver injury and recommendations显示文摘Coronavirus disease 2019(COVID-19)has caused a pandemic that affected all countries with nearly 270 million patients and 5 million deaths,as of as of December,2021.The severe acute respiratory syndrome coronavirus 2 virus targets the receptor,angiotensin-converting enzyme 2,which is frequently found in human intestinal epithelial cells,bile duct epithelial cells,and liver cells,and all gastrointestinal system organs are affected by COVID-19 infection.The aim of this study is to review the gastrointestinal manifestations and liver damage of COVID-19 infection and investigate the severe COVID-19 infection risk in patients that have chronic gastrointestinal disease,along with current treatment guidelines.A literature search was conducted on electronic databases of PubMed,Scopus,and Cochran Library,consisting of COVID-19,liver injury,gastrointestinal system findings,and treatment.Liver and intestinal involvements are the most common manifestations.Diarrhea,anorexia,nausea/vomiting,abdominal pain are the most frequent symptoms seen in intestinal involvement.Mild hepatitis occurs with elevated levels of transaminases.Gastrointestinal involvement is associated with long hospital stay,severity of the disease,and intensive care unit necessity.Treatments and follow-up of patients with inflammatory bowel diseases,cirrhosis,hepatocellular carcinoma,or liver transplant have been negatively affected during the pandemic.Patients with cirrhosis,hepatocellular carcinoma,auto-immune diseases,or liver transplantation may have a greater risk for severe COVID-19.Diagnostic or therapeutic procedures should be restricted with specific conditions.Telemedicine should be used in non-urgent periodic patient follow up.COVID-19 treatment should not be delayed in patients at the risk group.COVID-19 vaccination should be prioritized in this group. | Zulal Ozkurt EsraÇınar Tanrıverdi | 2022 | World Journal of Clinical Cases2022，10，4： | 2 |
