2Hepatitis E virus: Epidemiology, diagnosis, clinical manifestations, and treatment显示文摘The hepatitis E virus(HEV)is the fifth known form of viral hepatitis and was first recognized as the cause of an epidemic of unexplained acute hepatitis in the early 1980s.Globally,it is one of the most frequent causes of acute viral hepatitis.The majority of HEV infections are asymptomatic and lead to the spontaneous clearance of the virus.Among the eight different genotypes identified to date,HEV genotype 1(HEV1),HEV2,HEV3,and HEV4 are the most frequent genotypes causing infections in humans.HEV1 and HEV2 are prevalent in developing regions and able to result in large-scale outbreaks originating from contaminated water supplies.They are also responsible for severe hepatitis in pregnant patients and infants.In contrast,HEV3 and HEV4 are zoonotic,and the transmission of these genotypes to humans occurs mainly through the fecal contamination of water and consumption of contaminated meat from infected animals.Their main reservoir is the pig,and they are mostly encountered in developed countries.The major risk groups for HEV infection and its ensuing adverse consequences are pregnant women,infants,older people,immunocompromised individuals,patients with underlying chronic liver diseases,and workers that come into close contact with HEV-infected animals.In the clinical perspective,HEV infections have diverse clinical manifestations including acute and self-limiting hepatitis,acute-on-chronic liver disease,chronic hepatitis,cirrhosis,and liver failure.Although HEV mainly results in acute selflimiting infection,chronic HEV infection may occur among immunocompromised patients(e.g.,solid-organ transplant recipients).Additionally,HEV-associated extrahepatic manifestations involving various organs have been reported in the last decade,although the causal link for many of them still needs to be proven.Ribavirin and interferon-alpha are the most widely used agents for the treatment of HEV infections with a certain level of success.However,ribavirin is contraindicated in pregnant patients,and interferon-alpha cannot be used in most transplant recipients.Therefore,there is an urgent need for novel antiviral compounds that are safe and effective particularly for patients having contraindications for ribavirin or interferon-alpha and infected by the ribavirinresistant HEV.In this review article,a literature search using PubMed and MEDLINE databases was performed,up to March 2020.Only the articles published in English were reviewed.Abdullah Tarık Aslan Hatice Yasemin Balaban 2020World Journal of Gastroenterology2020,26,37:15
3A deep learning-enabled portable imaging flow cytometer for cost-effective, highthroughput, and label-free analysis of natural water samples显示文摘We report a deep learning-enabled field-portable and cost-effective imaging flow cytometer that automatically captures phase-contrast color images of the contents of a continuously flowing water sample at a throughput of 100 mL/h.The device is based on partially coherent lens-free holographic microscopy and acquires the diffraction patterns of flowing micro-objects inside a microfluidic channel.These holographic diffraction patterns are reconstructed in real time using a deep learning-based phase-recovery and image-reconstruction method to produce a color image of each micro-object without the use of external labeling.Motion blur is eliminated by simultaneously illuminating the sample with red,green,and blue light-emitting diodes that are pulsed.Operated by a laptop computer,this portable device measures 15.5 cm×15 cm×12.5 cm,weighs 1 kg,and compared to standard imaging flow cytometers,it provides extreme reductions of cost,size and weight while also providing a high volumetric throughput over a large object size range.We demonstrated the capabilities of this device by measuring ocean samples at the Los Angeles coastline and obtaining images of its micro-and nanoplankton composition.Furthermore,we measured the concentration of a potentially toxic alga(Pseudo-nitzschia)in six public beaches in Los Angeles and achieved good agreement with measurements conducted by the California Department of Public Health.The cost-effectiveness,compactness,and simplicity of this computational platform might lead to the creation of a network of imaging flow cytometers for largescale and continuous monitoring of the ocean microbiome,including its plankton composition.Zoltán Gӧrӧcs Miu Tamamitsu Vittorio Bianco Patrick Wolf Shounak Roy Koyoshi Shindo Kyrollos Yanny Yichen Wu Hatice Ceylan Koydemir Yair Rivenson Aydogan Ozcan 2018Light(Science & Applications)2018,7,1:14
4Primary and secondary hyperoxaluria: Understanding the enigma显示文摘Hyperoxaluria is characterized by an increased urinary excretion of oxalate. Primary and secondary hyperoxaluria are two distinct clinical expressions of hyperoxaluria. Primary hyperoxaluria is an inherited error of metabolism due to defective enzyme activity. In contrast, secondary hyperoxaluria is caused by increased dietary ingestion of oxalate, precursors of oxalate or alteration in intestinal microflora. The disease spectrum extends from recurrent kidney stones, nephrocalcinosis and urinary tract infections to chronic kidney disease and end stage renal disease. When calcium oxalate burden exceeds the renal excretory ability, calcium oxalate starts to deposit in various organ systems in a process called systemic oxalosis. Increased urinary oxalate levels help to make the diagnosis while plasma oxalate levels are likely to be more accurate when patients develop chronic kidney disease. Definitive diagnosis of primary hyperoxaluria is achieved by genetic studies and if genetic studies prove inconclusive, liver biopsy is undertaken to establish diagnosis. Diagnostic clues pointing towards secondary hyperoxaluria are a supportive dietary history and tests to detect increased intestinal absorption of oxalate. Conservative treatment for both types of hyperoxaluria includes vigorous hydration and crystallization inhibitors to decrease calcium oxalate precipitation. Pyridoxine is also found to be helpful in approximately 30% patients with primary hyperoxaluria type 1. Liver-kidney and isolated kidney transplantation are the treatment of choice in primary hyperoxaluria type 1 and type 2 respectively. Data is scarce on role of transplantation in primary hyperoxaluria type 3 where there are no reports of end stage renal disease so far. There are ongoing investigations into newer modalities of diagnosis and treatment of hyperoxaluria. Clinical differentiation between primary and secondary hyperoxaluria and further between the types of primary hyperoxaluria is very important because of implications in treatment and diagnosis. Repetitive transcranial magnetic stimulation exhibits satisfactory outcomes in improving multiple sclerosis, stroke, spinal cord injury and cerebral palsy-induced spasticity. By contrast, transcranial direct current stimulation has only been studied in post-stroke spasticity. 6Prolonged QT dispersion in inflammatory bowel disease显示文摘AIM:To investigate the frequency and factors of prolonged QT dispersion that may lead to severe ventricular arrhythmias in patients with inflammatory bowel disease(IBD).METHODS:This study included 63 ulcerative colitis(UC) and 41 Crohn's disease(CD) patients.Forty-seven healthy patients were included as the control group.Heart rate was calculated using electrocardiography,corrected QT dispersion(QTcd) and the Bazett's formula.Homeostasis model assessment(HOMA) was used to determine insulin resistance(IR).HOMA values < 1 were considered normal and values > 2.5 indicated a high probability of IR.RESULTS:Prolonged QTcd was found in 12.2% of UC patients,and in 14.5% of CD patients compared with the control group(P < 0.05).A significant difference was found between the insulin values(CD:10.95 ± 6.10 vs 6.44 ± 3.28,P < 0.05;UC:10.88 ± 7.19 vs 7.20 ± 4.54,P < 0.05) and HOMA(CD:2.56 ± 1.43 vs 1.42 ± 0.75,P < 0.05;UC:2.94 ± 1.88 vs 1.90 ± 1.09,P < 0.05) in UC and CD patients with and without prolonged QTcd.Disease behavior types were determined in CD patients with prolonged QTcd.Increased systolic arterial pressure(125 ± 13.81 vs 114.09 ± 8.73,P < 0.01) and age(48.67 ± 13.93 vs 39.57 ± 11.58,P < 0.05) in UC patients were significantly associated with prolonged QTcd.CONCLUSION:Our data show that IBD patients have prolonged QTcd in relation to controls.The routine followup of IBD patients should include determination of HOMA,insulin values and electrocardiogram examination.Elif Yorulmaz Aslihan Sezgin Gupse Adali Hatice Yorulmaz Hilmi Ciftci 2013World Journal of Gastroenterology2013,19,1:8
7Assessment of the correlation between serum prolidase and alpha-fetoprotein levels in patients with hepatocellular carcinoma显示文摘AIM: To determine the predictive value of increased prolidase activity that reflects increased collagen turnover in patients with hepatocellular carcinoma(HCC).METHODS: Sixty-eight patients with HCC(mean age of 69.1 ± 10.1), 31 cirrhosis patients(mean age of59.3 ± 6.3) and 33 healthy volunteers(mean age of51.4 ± 12.6) were enrolled in this study. Univariate and multivariate analysis were used to evaluate the association of serum α-fetoprotein(AFP) values with HCC clinicopathological features, such as tumor size,number and presence of vascular and macrovascular invasion. The patients with HCC were divided into groups according to tumor size, number and presence of vascular invasion(diameters; ≤ 3 cm, 3-5 cmand ≥ 5 cm, number; 1, 2 and ≥ 3, macrovascular invasion; yes/no). Barcelona-clinic liver cancer(BCLC)criteria were used to stage HCC patients. Serum samples for measurement of prolidase and alphafetoprotein levels were kept at-80 ℃ until use.Prolidase levels were measured spectrophotometrically and AFP concentrations were determined by a chemiluminescence immunometric commercial diagnostic assay.RESULTS: In patients with HCC, prolidase and AFP values were evaluated according to tumor size, number,presence of macrovascular invasion and BCLC staging classification. Prolidase values were significantly higher in patients with HCC compared with controls(P <0.001). Prolidase levels were significantly associated with tumor size and number(P < 0.001, P = 0.002,respectively). Prolidase levels also differed in patients in terms of BCLC staging classification(P < 0.001).Furthermore the prolidase levels in HCC patients showed a significant difference compared with patients with cirrhosis(P < 0.001). In HCC patients grouped according to tumor size, number and BCLC staging classification, AFP values differed separately(P = 0.032,P = 0.038, P = 0.015, respectively). 8Diet high in fructose leads to an overexpression of lipocalin-2 in rat fatty liver显示文摘AIM:To explore lipocalin-2(LCN-2)expression and its possible role and mechanism(s)of production in rat models of diet-inducible fatty liver.METHODS:Fatty liver was triggered in male SpragueDawley rats fed either with liquid Lieber-DeCarli(LDC)or LDC+70%cal fructose(L-HFr)diet for 4 or 8 wk.Chow-nourished animals served as controls.Hepatic expression of LCN-2 and other metabolic and inflammatory mediators was assessed by quantitative reverse transcription polymerase chain reaction and Western blotting.Serum LCN-2,fasting leptin,and lipid profile were evaluated via Enzyme-Linked Immunosorbent Assay,Radioimmunoassay,and colorimetric assays,respectively.The localization of LCN-2 in the liver was detected by using immunofluorescence staining.Furthermore,HE stain was used to evaluate hepatic fatdegeneration and inflammation.RESULTS:Both LDC-fed and L-HFr-fed rat histologically featured fatty liver.In the liver,mRNA transcriptions of Mcp-1,a2-m,Il-8 and Glut5 were increased in the L-HFr group at both time points(P<0.001),while the transcription of Tlr4,Inos,and Tnf-a was significantly up-regulated at week 4.Interestingly,hepatic Lcn-2 expression was 90-fold at week 4 and 507-fold at week 8 higher in L-HFr-subjected rats vs control(P<0.001).In contrast to HDL-cholesterol,systemic levels of LCN-2,fasting leptin and triglycerides were elevated in the L-HFr regimen(P<0.001).Moreover,protein expression of hepatic LCN-2,CD14,phosphoMAPK,caspase-9,cytochrome c and 4-hydroxynonenal was increased in the L-HFr group.Conversely,the hepatic expression of PGC-1a(a mitochondrial-biogenic protein)was reduced in the L-HFr category at week 8.The localization of LCN-2 in the liver was predominantly restricted to MPO+granulocytes.CONCLUSION:Fructose diet up-regulates hepatic LCN-2 expression,which correlates with the increased indicators of oxidative stress and mitochondrial dysfunction.The LCN-2 may be involved in liver protection.Salamah Mohammad Alwahsh Min Xu Hatice Ali Seyhan Shakil Ahmad Sabine Mihm Giuliano Ramadori Frank Christian Schultze 2014World Journal of Gastroenterology2014,20,7:7
9Non-invasive brain stimulation to promote motor and functional recovery following spinal cord injury显示文摘We conducted a systematic review of studies using non-invasive brain stimulation(NIBS: repetitive transcranial magnetic stimulation(r TMS) and transcranial direct current stimulation(t DCS)) as a research and clinical tool aimed at improving motor and functional recovery or spasticity in patients following spinal cord injury(SCI) under the assumption that if the residual corticospinal circuits could be stimulated appropriately, the changes might be accompanied by functional recovery or an improvement in spasticity. 10Motility-based label-free detection of parasites in bodily fluids using holographic speckle analysis and deep learning显示文摘Parasitic infections constitute a major global public health issue.Existing screening methods that are based on manual microscopic examination often struggle to provide sufficient volumetric throughput and sensitivity to facilitate early diagnosis.Here,we demonstrate a motility-based label-free computational imaging platform to rapidly detect motile parasites in optically dense bodily fluids by utilizing the locomotion of the parasites as a specific biomarker and endogenous contrast mechanism.Based on this principle,a cost-effective and mobile instrument,which rapidly screens~3.2 mL of fluid sample in three dimensions,was built to automatically detect and count motile microorganisms using their holographic time-lapse speckle patterns.We demonstrate the capabilities of our platform by detecting trypanosomes,which are motile protozoan parasites,with various species that cause deadly diseases affecting millions of people worldwide.Using a holographic speckle analysis algorithm combined with deep learningbased classification,we demonstrate sensitive and label-free detection of trypanosomes within spiked whole blood and artificial cerebrospinal fluid(CSF)samples,achieving a limit of detection of ten trypanosomes per mL of whole blood(~five-fold better than the current state-of-the-art parasitological method)and three trypanosomes per mL of CSF.We further demonstrate that this platform can be applied to detect other motile parasites by imaging Trichomonas vaginalis,the causative agent of trichomoniasis,which affects 275 million people worldwide.With its costeffective,portable design and rapid screening time,this unique platform has the potential to be applied for sensitive and timely diagnosis of neglected tropical diseases caused by motile parasites and other parasitic infections in resource-limited regions.Yibo Zhang Hatice Ceylan Koydemir Michelle M.Shimogawa Sener Yalcin Alexander Guziak Tairan Liu Ilker Oguz Yujia Huang Bijie Bai Yilin Luo Yi Luo Zhensong Wei Hongda Wang Vittorio Bianco Bohan Zhang Rohan Nadkarni Kent Hill Aydogan Ozcan 2018Light(Science & Applications)2018,7,1:6
11Handheld high-throughput plasmonic biosensor using computational on-chip imaging显示文摘We demonstrate a handheld on-chip biosensing technology that employs plasmonic microarrays coupled with a lens-free computational imaging system towards multiplexed and high-throughput screening of biomolecular interactions for point-of-care applications and resource-limited settings.This lightweight and field-portable biosensing device,weighing 60 g and 7.5 cm tall,utilizes a compact optoelectronic sensor array to record the diffraction patterns of plasmonic nanostructures under uniform illumination by a single-light emitting diode tuned to the plasmonic mode of the nanoapertures.Employing a sensitive plasmonic array design that is combined with lens-free computational imaging,we demonstrate label-free and quantitative detection of biomolecules with a protein layer thickness down to 3 nm.Integrating large-scale plasmonic microarrays,our on-chip imaging platform enables simultaneous detection of protein mono-and bilayers on the same platform over a wide range of biomolecule concentrations.In this handheld device,we also employ an iterative phase retrieval-based image reconstruction method,which offers the ability to digitally image a highly multiplexed array of sensors on the same plasmonic chip,making this approach especially suitable for high-throughput diagnostic applications in field settings.Arif E Cetin Ahmet F Coskun Betty C Galarreta Min Huang David Herman Aydogan Ozcan Hatice Altug 2014Light(Science & Applications)2014,3,1:6

13Frequency,types,and treatment of anemia in Turkish patients with inflammatory bowel disease显示文摘AIM to specify the type and prevalence of anemia along with a treatment approach for inflammatory bowel disease(IBD).METHODS We conducted a retrospective study on 465 patients who were diagnosed with IBD and followed up at our hospital from June 2015 to June 2016 [male: 254, female: 211; average age: 47 ± 14.4; Crohn's disease(CD): 257, Ulcerative Colitis(UC): 208]. Epidemiological and clinical data, such as sex, age, age of diagnosis, type of IBD, disease extension, disease behavior and duration, treatments for IBD and anemia, and surgical history were obtained for each patient. Per World Health Organization guidelines, anemia was diagnosed for males if hemoglobin values were less than 13 g/dL and for females if hemoglobin values were less than 12 g/d L.RESULTS We determined that 51.6% of the patients had anemia, which was more frequent in women then men(64% vs 41.3%, P < 0.001). Anemia frequency was higher in CD cases(57.6%) than in UC cases(44.2%)(P = 0.004). CD involvements were as follows: 48.2% in ileal involvement, 19% in colonic involvement, and 32.8% in ileocolonic involvement. Furthermore, 27.5% of UC patients had proctitis(E1) involvement, 41% ofthem had involvement in left colitis(E2), and 31.5% had pancolitis involvement. There was no significant relationship between anemia frequency and duration of disease(P = 0.55). Iron deficiency anemia(IDA) was the most common type of anemia in this cohort. Moreover, because anemia parameters have not been evaluated during follow-up of 15.3% of patients, the etiology of anemia has not been clarified. Fifty percent of patients with anemia received treatment. Twentythree percent of IDA patients had oral iron intake and forty-one percent of IDA patients had parenteral iron treatment. Fifty-three percent of patients who were suffering from megaloblastic anemia received B12/folic acid treatment.CONCLUSION We found out that almost half of all IBD patients(51.6%) had anemia, the most frequent of which was IDA. Almost half of these patients received treatment. 14Adipokines and ghrelin in gastric cancer cachexia显示文摘AIM:To investigate the roles of the adipocytokines,ghrelin and leptin in gastric cancer cachexia. METHODS:Resistin,ghrelin,leptin,adiponectin,insulin and insulin-like growth factor(IGF-Ⅰ),were measured in 30 healthy subjects,and 60 gastric cancer patients of which 30 suffered from cancer-induced cachexia and 30 served as a control group. The relationships between hormones,body mass index(BMI) loss ratio,age,gender,and Glasgow Prognostic Score(GPS) were investigated. RESULTS:Cachexia patients had higher tumor stage and GPS when compared with non-cachexia patients(P < 0.05). Ghrelin,resistin,leptin,adiponectin and IGF-Ⅰ,showed a significant correlation with BMI loss ratio and GPS(P < 0.05). A strong correlation was seen between GPS and BMI loss(R = -0.570,P < 0.0001). Multivariate analysis indicated that BMI loss was significantly independent as a predictor of ghrelin,resistin,leptin and IGF-Ⅰ(P < 0.05). Existence of an important significant relationship between resistin and insulin resistance was also noted. CONCLUSION:These results showed that serum ghrelin,leptin,adiponectin,and IGF-Ⅰ play important roles in cachexia-related gastric cancers. No relationshipwas found between resistin and cancer cachexia. 16Characterization of recuperating talent of white-rot fungi cells to dye-contaminated soil/water显示文摘This study was purposed to explore the decolorization of dyes by fungi on either a soil or in a liquid medium and to determine the application through batch shaking system. Two commercial dyes were decolorized and studied with four fungal strains in three media. Fungal growth is the best in malt extract/glucose medium for all organisms. Decolorization of reactive blue 220 and methyl red was investigated in soil medium by Trametes versicolor. These dyes were removed 91% and 80% for methyl red and reactive blue 220 respectively(dye concentration; 100 mg·L^(-1)) by both organisms. Enzymatic activities were monitored. Laccase(Lac) and manganese peroxidase(Mn P) were detected. MnP enzyme had important role for the dye decolorization. This study demonstrates that it is possible to decolorize some synthetic dyes, which would be highly advanced for dye containing wastewater and soil. 17Epidemiological and clinical features of paediatric open globe injuries in southwestern Turkey显示文摘AIM:To evaluate the epidemiologic,anatomic,and clinical features of open globe injuries in children.METHODS:The medical files of patients under the age of 16 who had been operated for an open globe injury at Akdeniz University Hospital's Department of Ophthalmology were retrospectively evaluated.RESULTS:A total of 90 patients were evaluated in this study.Among these patients,26(28.9%)were female and64(71.1%)were male.The mean age of the patients was7.7±4.2 years.The male/female ratio was observed to increase with increasing age(P=0.006,P=7.48).Injuries were most likely to occur in spring and autumn(P=0.028).The time interval between the injury and the surgical repair was 9.36±27.4h.Forty(44.4%)of the injuries occurred in the home,27(30%)occurred in the yard,and 21(23.3%)happened while playing in the street.The most common causes of injury were sharp metal objects(P<0.001).Injury to the cornea occurred in47(52.2%)of the patients(P<0.001).The most common complication to occur was cataract formation.Additional operations were necessary for 37(41.1%)of the patients.The final visual acuity was correlated with both the initial visual acuity of the wounded eye prior to surgery and the length of the wound(P<0.001,r=0.502 and P<0.001,r=-0.442,respectively).CONCLUSION:Open globe injuries that are suffered in childhood generally occur either at home,in the yard,or on the street,with sharp metal objects being the most common cause of injury.The initial visual acuity and the length of the wound are the most important determinants of the final visual acuity.Hatice Deniz Ilhan Ahmet Burak Bilgin Ash Cetinkaya Mustafa Unal Iclal Yucel 2013International Journal of Ophthalmology(English edition)2013,6,6:3
18Double-layer graphene for enhanced tunable infrared plasmonics显示文摘Graphene is emerging as a promising material for photonic applications owing to its unique optoelectronic properties.Graphene supports tunable,long-lived and extremely confined plasmons that have great potential for applications such as biosensing and optical communications.However,in order to excite plasmonic resonances in graphene,this material requires a high doping level,which is challenging to achieve without degrading carrier mobility and stability.Here,we demonstrate that the infrared plasmonic response of a graphene multilayer stack is analogous to that of a highly doped single layer of graphene,preserving mobility and supporting plasmonic resonances with higher oscillator strength than previously explored single-layer devices.Particularly,we find that the optically equivalent carrier density in multilayer graphene is larger than the sum of those in the individual layers.Furthermore,electrostatic biasing in multilayer graphene is enhanced with respect to single layer due to the redistribution of carriers over different layers,thus extending the spectral tuning range of the plasmonic structure.The superior effective doping and improved tunability of multilayer graphene stacks should enable a plethora of future infrared plasmonic devices with high optical performance and wide tunability.Daniel Rodrigo Andreas Tittl Odeta Limaj F Javier García de Abajo Valerio Pruneri Hatice Altug 2017Light(Science & Applications)2017,6,1:3
19Contact lens impact on quality of life in keratoconus patients: rigid gas permeable versus soft siliconehydrogel keratoconus lenses显示文摘AIM: To determine the impact of rigid gas permeable(RGP) and silicone-hydrogel keratoconus lenses on the quality of life(Qo L) in keratoconus(KCN) patients using the self-reported results from the Contact Lens Impact on Quality of Life(CLIQ) Questionnaire.·METHODS: From January 2013 to April 2013, 27 consecutive KCN patients who wore RGP contact lenses(conflexair100 UV KE Zeiss-Whlk) or soft silicone-hydrogel contact lenses(SHCLs) for KCN(Kera Soft ICBausch&Lomb or Hydrocone Toris K-Swiss lens)completed the CLIQ questionnaire.·RESULTS: The mean age of 27 patients was 29.6±8.0y.Fifteen patients were RGP user. The groups were comparable with respect to the mean patient age, sex,and mean K values(P =0.1, P =0.8 and P =0.1, respectively).The mean CLIQperson measure was 42.8 ±5.5 in RGP group and 39.6 ±5.5 in SHCLs for KCN group(P =0.06).CLIQperson measure was positively correlated with steep K value(r =0.301, P =0.04). 20Agenesis of the dorsal pancreas显示文摘Developmental anomalies of the pancreas have been reported but dorsal pancreatic agenesis is an extremely rare entity. We report an asymptomatic 62-year-old woman with complete agenesis of the dorsal pancreas. Abdominal computed tomography (CT) revealed a normal pancreatic head, but pancreatic body and tail were not visualized. Magnetic resonance imaging (MRI) findings were similar to CT. At magnetic resonance cholangiopancreatography (MRCP), the major pancreatic duct was short and the dorsal pancreatic duct was not visualized.
