| 2 | Hepatocellular carcinoma: From diagnosis to treatment显示文摘Hepatocellular carcinoma(HCC) is the sixth most prevalent malignancy worldwide and is a rising cause of cancer related mortality. Risk factors for HCC are well documented and effective surveillance and early diagnosis allow for curative therapies. The majority of HCC appears to be caused by cirrhosis from chronic hepatitis B and hepatitis C virus. Preventive strategies include vaccination programs and anti-viral treatments.Surveillance with ultrasonography detects early stage disease and improves survival rates. Many treatment options exist for individuals with HCC and are determined by stage of presentation. Liver transplantation is offered to patients who are within the Milan criteria and are not candidates for hepatic resection. | 4 | Inverted internal limiting membrane flap technique for very large macular hole显示文摘AIM: To assess the anatomical and visual outcome of idiopathic macular holes greater than 1000 μm using the inverted internal limiting membrane flap technique.METHODS: This retrospective case series included 5 eyes of 5 patients with idiopathic macular hole with base diameter greater than 1000 μm who underwent inverted internal limiting membrane flap technique along with standard 23 G pars plans vitrectomy with posterior hyaloid detachment and fluid gas exchange with 12%-14% perfluoropropane(C3F8). Preoperative and postoperative visual acuity and spectral domain optical coherence tomography images were evaluated. The main outcome measures were visual outcome and macular hole closure.RESULTS: Mean age was 63.2±8.4y with all 5 subjects being females. Mean duration of symptoms was 11±14 mo with a mean postoperative follow up of 13.2 ±13 mo. The mean base diameter of the macular holes was 1420±84.8 μm(1280-1480 μm). Type 1 closure was achieved in four out of five patients, while one patient had type 2 closure using the inverted internal limiting membrane(ILM) flap technique. Median baseline BCVA was 0.79 log MAR(Snellen's equivalent 20/120) and median final BCVA0.6 log MAR(Snellen's equivalent 20/80) with mean visual improvement of approximately three lines improvement. | 6 | What can imaging tell us about cognitive impairment and dementia?显示文摘Dementia is a contemporary global health issue with far reaching consequences, not only for affected individuals and their families, but for national and global socioeconomic conditions. The hallmark feature of dementia is that of irreversible cognitive decline, usually affecting memory, and impaired activities of daily living. Advances in healthcare worldwide have facilitated longer life spans, increasing the risks of developing cognitive decline and dementia in late life. Dementia remains a clinical diagnosis. The role of structural and molecular neuroimaging in patients with dementia is primarily supportive role rather than diagnostic, American and European guidelines recommending imaging to exclude treatable causes of dementia, such as tumor, hydrocephalus or intracranial haemorrhage, but also to distinguish between different dementia subtypes, the commonest of which is Alzheimer's disease. However, this depends on the availability of these imaging techniques at individual centres. Advanced magnetic resonance imaging(MRI) techniques, such as functional connectivity MRI, diffusion tensor imaging and magnetic resonance spectroscopy, and molecular imaging techniques, such as 18 F fluoro-deoxy glucose positron emission tomography(PET), amyloid PET, tau PET, are currently within the realm of dementia research but are available for clinical use. Increasingly the research focus is on earlier identification of at risk preclinical individuals, for example due to family history. | 8 | Machine learning enabled autonomous microstructural characterization in 3D samples显示文摘We introduce an unsupervised machine learning(ML)based technique for the identification and characterization of microstructures in three-dimensional(3D)samples obtained from molecular dynamics simulations,particle tracking data,or experiments.Our technique combines topology classification,image processing,and clustering algorithms,and can handle a wide range of microstructure types including grains in polycrystalline materials,voids in porous systems,and structures from self/directed assembly in soft-matter complex solutions.Our technique does not require a priori microstructure description of the target system and is insensitive to disorder such as extended defects in polycrystals arising from line and plane defects.We demonstrate quantitively that our technique provides unbiased microstructural information such as precise quantification of grains and their size distributions in 3D polycrystalline samples,characterizes features such as voids and porosity in 3D polymeric samples and micellar size distribution in 3D complex fluids.To demonstrate the efficacy of our ML approach,we benchmark it against a diverse set of synthetic data samples representing nanocrystalline metals,polymers and complex fluids as well as experimentally published characterization data.Our technique is computationally efficient and provides a way to quickly identify,track,and quantify complex microstructural features that impact the observed material behavior. | Henry Chan Mathew Cherukara Troy D.Loeffler Badri Narayanan Subramanian K.R.S.Sankaranarayanan | 2020 | npj Computational Materials2020,,1: | 5 |
| 9 | Metallurgical analysis of a failed maraging steel shear screw used in the band separation system of a satellite launch vehicle显示文摘Maraging steels have excellent combination of strength and toughness and are extensively used for a variety of aerospace applications. In one such critical application, this steel was used to fabricate shear screws of a stage separation system in a satellite launch vehicle. During assembly preparations, one of the shear screws which connected the separation band and band end block has failed at the first thread. Microstructural analysis revealed that the crack originated from the root of the thread and propagated in an intergranular mode. | 10 | Ivabradine in the treatment of systolic heart failure- A systematic review and meta-analysis显示文摘AIM To perform a systematic-review and meta-analysis to compare outcomes of ivabradine combined with betablocker to beta-blocker alone in heart failure with reduced ejection fraction(HFr EF).METHODS We searched PubM ed, Cochrane, EMBASE, CINAHL and Web of Science for trials comparing ivabradine + betablocker to beta-blocker alone in HFr EF. We performed a systematic-review and meta-analysis of published literature. Primary end-point was combined end point of cardiac death and hospitalization for heart failure.RESULTS Six studies with 17671 patients were included. Mean follow-up was 8.7 ± 7.9 mo. Combined end-point of heart failure readmission and cardiovascular death was better in ivabradine + beta-blocker group compared to beta-blocker alone(RR: 0.93, 95%CI: 0.79-1.09, P = 0.354). Mean difference(MD) in heart rate was higher in the ivabradine + beta-blocker group(MD: 6.14, 95%CI: 3.80-8.48, P < 0.001). There was no difference in all cause mortality(RR: 0.98, 95%CI: 0.89-1.07, P = 0.609), cardiovascular mortality(RR: 0.99, 95%CI: 0.86-1.15, P = 0.908) or heart failure hospitalization(RR: 0.87, 95%CI: 0.68-1.11, P = 0.271). CONCLUSION From the available clinical trials, ivabradine + betablocker resulted in a significantly greater reduction in HRcoupled with improvement in combined end-point of heart failure readmission and cardiovascular death but with no improvement in all cause or cardiovascular mortality. | 11 | Nonylphenol in bivalves and sediments in the northeast coast of China显示文摘To understand nonylphenol (NP) contamination in the northeast coastal environment in China, NP was determined in 19 sediment and 20 bivalve samples collected during November 2005. NP was identified in all sediment samples in the range of 8.8 to 1.0 × 10 3 ng/g dw (dry weight), with an average of 0.12 × 10 3 ng/g dw. Most of the bivalve samples (oysters and mussels) contained detectable amounts of NP that ranged from ND (not detected) to 7.6 × 10 3 ng/g dw, with an average value of 0.79 × 10 3 ng/g dw. These results indicated that NP is ubiquitous in sediments and bivalves from the northeast coast of China, and are even present in bivalves sold as seafood. Serious contamination with NP was recorded off the coast of Bohai Strait and in semi-enclosed coastal environment such as Jiaozhou Bay and Liaodong Bay.
