| 1 | Interstitial cells of Cajal, the Maestro in health and disease显示文摘Interstitial cells of Cajal (ICC) are important players in the symphony of gut motility. They have a very signif icant physiological role orchestrating the normal peristaltic activity of the digestive system. They are the pacemaker cells in gastrointestinal (GI) muscles. Absence, reduction in number or altered integrity of the ICC network may have a dramatic effect on GI system motility. More understanding of ICC physiology will foster advances in physiology of gut motility which will help in a future breakthrough in the pharmacological interventions to restore normal motor function of GI tract. | 2 | Validation of a serum microRNA panel as biomarkers for early diagnosis of hepatocellular carcinoma post-hepatitis C infection in Egyptian patients显示文摘AIM To investigate the prospective importance of serum micro(mi)RNAs(mi R-125 b, mi R-138 b, mi R-1269, mi R-214-5p, mi R-494, mi R375 and mi R-145) as early biomarkers for the diagnosis of hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC).METHODS Two-hundred and fifty HCV4 a patients, 224 HCV4 aHCC patients, and 84 healthy controls were enrolled in the study. Expression levels of mi R214-5p, mi R-125 b, mi R-1269 and mi R-375 were quantified using quantitative real-time PCR.RESULTS Expression of the selected mi RNAs in serum wassignificantly lower in HCC patients than in the healthy controls, except for mi R-1269 and mi R-494. There was a significant difference between HCC and HCV patients, in particular for HCC and late stage fibrosis, rather than HCV patients and early fibrosis. It is obvious that mi R-1269 was significantly upregulated in HCC cases compared to hepatic fibrosis cases. Each mi RNA can show HCC progression. Multivariate logistic regression analysis indicated that the tested panel of mi RNAs(mi R214-5p, mi R-125 b, mi R-1269 and mi R-375) represent accurate and specific indictors of HCC development.CONCLUSION This study presents a panel of mi RNAs with strong power as putative diagnostic and prognostic biomarkers for HCV-induced HCC. | 3 | Aetiology and risk factors of ischaemic cholangiopathy after liver transplantation显示文摘Liver transplantation(LT)is the best treatment for endstage hepatic failure,with an excellent survival rates over the last decade.Biliary complications after LT pose a major challenge especially with the increasing number of procured organs after circulatory death.Ischaemic cholangiopathy(IC)is a set of disorders characterized by multiple diffuse strictures affecting the graft biliary system in the absence of hepatic artery thrombosis or stenosis.It commonly presents with cholestasis and cholangitis resulting in higher readmission rates,longer length of stay,repeated therapeutic interventions,and eventually re-transplantation with consequent effects on the patient’s quality of life and increased health care costs.The pathogenesis of IC is unclear and exhibits a higher prevalence with prolonged ischaemia time,donation after circulatory death(DCD),rejection,and cytomegalovirus infection.The majority of IC occurs within 12 mo after LT.Prolonged warm ischaemic times predispose to a profound injury with a subsequently higher prevalence of IC.Biliary complications and IC rates are between 16%and 29%in DCD grafts com-pared to between 3%and 17%in donation after brain death(DBD)grafts.The majority of ischaemic biliary lesions occur within 30 d in DCD compared to 90 d in DBD grafts following transplantation.However,there are many other risk factors for IC that should be considered.The benefits of DCD in expanding the donor pool are hindered by the higher incidence of IC with increased rates of re-transplantation.Careful donor selection and procurement might help to optimize the utilization of DCD grafts. | Moustafa Mabrouk Mourad Abdullah Algarni Christos Liossis Simon R Bramhall | 2014 | World Journal of Gastroenterology2014,20,20: | 16 |

| 5 | Multitarget therapeutic strategies for Alzheimer's disease显示文摘Neurodegenerative diseases such as Alzheimer's, Huntington's and Parkinson's diseases have multifaceted nature because of the different factors contributing to their progression. The complex nature of neurodegenerative diseases has developed a pressing need to design multitarget-directed ligands to address the complementary pathways involved in these diseases. The major enzyme targets for development of therapeutics for Alzheimer's disease are cholinesterase and β-secretase enzymes. In this review, we discuss recent advances in profiling single target inhibitors based on these enzymes to multitarget-directed ligands as potential therapeutics for this devastating disease. Patients at greatest risk were those identified to have infected pancreatic necrosis and/or organ failure. This review seeks to highlight the potential vascular complications associated with pancreatitis that despite being relatively uncommon are associated with mortality in the region of 34%-52%. We examine the current evidence base to determine the most appropriate method by which to image and treat pseudo-aneurysms that arise as the result of acute and chronic inflammation of pancreas. | 8 | Toward an operating speed profile model for rural two-lane roads in Egypt显示文摘The geometric design, especially the horizontal and vertical alignments, of rural two-lane highway facilities is considered one of the most important factors affecting the quality of traffic service and safety. A consistent highway geometric design is defined to be one that conforms to the driver's expectations. In order to calculate the main measures of design consistency, an accurate operating speed profile model for road alignment is needed.Studies have shown that operating speed models are country dependent due to varying demographics, driver attitudes, habits, etc. This paper develops a speed profile model for two-lane rural roads in Egypt. This includes the development of operating speed models for horizontal curves and tangents, as well as the study of the characteristics and relationships between acceleration and deceleration rates before and after horizontal curves. The study uses a desert rural two-lane, two-way road sections that connect the city of Sohag with the city of Hurghada, in Upper Egypt. All geometric characteristics were obtained from highway authority. Speed data regarding individual drivers traveling on selected two-lane rural road sections were sampled using an on-board GPS system. The study confirms the finding in previous research that curvature is the most important factor in determining the speed on horizontal curves. Moreover, tangent length is the most important factor in determining operating speeds at tangents. The acceleration and deceleration characteristics were derived to gain an understanding of the behavior of individual vehicles traveling through curves of varying radii and lengths as well as preceding tangent length. Several operating speed models were developed for tangents and curves as well as for acceleration and deceleration rates. | 11 | Analysis of Unsteady Flow over Offshore Wind Turbines in Combination with Different types of Foundations显示文摘Environmental effects have an important influence on Offshore Wind Turbine (OWT) power generation efficiency and the structural stability of such turbines. In this study, we use an in-house Boundary Element (BEM)-panMARE code-to simulate the unsteady flow behavior of a full OWT with various combinations of aerodynamic and hydrodynamic loads in the time domain. This code is implemented to simulate potential flows for different applications and is based on a three-dimensional first-order panel method. Three different OWT configurations consisting of a generic 5 MW NREL rotor with three different types of foundations (Monopile, Tripod, and Jacket) are investigated. These three configurations are analyzed using the RANSE solver which is carried out using ANSYS CFX for validating the corresponding results. The simulations are performed under the same environmental atmospheric wind shear and rotor angular velocity, and the wave properties are wave height of 4 m and wave period of 7.16 s. In the present work, wave environmental effects were investigated firstly for the two solvers, and good agreement is achieved. Moreover, pressure distribution in each OWT case is presented, including detailed information about local flow fields. | 12 | MicroRNA signature in patients with hepatocellular carcinoma associated with type 2 diabetes显示文摘BACKGROUND Nonalcoholic steatohepatitis-related cirrhosis is one of the liver complications in type 2 diabetes mellitus(T2DM)and reported to be a risk factor for developing hepatocellular carcinoma(HCC).A reliable screening biomarker of liver cirrhosis(LC)and HCC among T2DM patients is important to reduce the morbidity and mortality of this disease.MicroRNA(miRNA)is considered a key player in HCC and T2DM,and it might be a hidden culprit in diabetes-associated HCC,making it a promising reliable prognostic tool.AIM To investigate the signature of serum miRNAs as early biomarkers for the screening of HCC among diabetic patients.METHODS Expression profiles of miRNAs in serum samples of diabetic LC and diabetic HCC patients were assessed using Illumina sequencing;then,RT-qPCR was used to validate significantly altered miRNAs between the two groups.Candidate miRNAs were tested in serum samples of 200 T2DM patients,270 LC patients,200 HCC patients,and 225 healthy control subjects.Additionally,receiver operating characteristic(ROC)analysis,with area under the curve(AUC),was performed to assess the diagnostic performance of the screened miRNAs for discriminating HCC from LC and nonmalignant patients(LC+T2DM).RESULTS Expression of the sequenced miRNAs in serum was different in HCC vs LCpositive T2DM patients.Two miRNAs(miR-34a,miR-221)were significantly upregulated and five miRNAs(miR-16,miR-23-3p,miR-122-5p,miR-198,miR-199a-3p)were significantly down-regulated in HCC compared to LC patients.Analysis of ROC curve demonstrated that the combination of these seven miRNAs can be used as a reliable biomarker for detection of HCC in diabetic patients,as it could identify HCC with high diagnostic accuracy in diabetic LC patients(AUC=0.993)and in diabetic nonmalignant patients(AUC=0.961).CONCLUSION This study validates a panel of serum miRNAs that can be used as a reliable noninvasive screening biomarker of HCC among T2DM cirrhotic and noncirrhotic patients.The study recommends further research to shed light on a possible role of c-Met in T2DM-associated HCC via the miRNA regulatory pathway. | Moustafa Nouh Elemeery Marwa Anwar Mohamed Marwa Ahmed Madkour Mohammed Mohammed Shamseya Noha Mahmoud Issa Ahmed Noah Badr Doaa Ahmed Ghareeb Cheol-Ho Pan | 2019 | World Journal of Gastroenterology2019,25,42: | 3 |

Results indicate that, the rocks were formed in an alteration complex of medium temperature and pressure. The area was under intense polymetamorphism (ductile shearing, and hydrothermal metamorphism), which occurred along the deformation zone stretching from Jinzhou to Fuxin. The area bears some propylitic, phyllic (or sericitic) and argillic alteration styles. Gold minerals are contained in fine grained pyrite in silicified zones, and associated with metamorphosed Precambrian rocks. | 19 | Enhancing IoT anomaly detection performance for federated learning显示文摘Federated Learning (FL) with mobile computing and the Internet of Things (IoT) is an effective cooperative learning approach. However, several technical challenges still need to be addressed. For instance, dividing the training process among several devices may impact the performance of Machine Learning (ML) algorithms, often significantly degrading prediction accuracy compared to centralized learning. One of the primary reasons for such performance degradation is that each device can access only a small fraction of data (that it generates), which limits the efficacy of the local ML model constructed on that device. The performance degradation could be exacerbated when the participating devices produce different classes of events, which is known as the class balance problem. Moreover, if the participating devices are of different types, each device may never observe the same types of events, which leads to the device heterogeneity problem. In this study, we investigate how data augmentation can be applied to address these challenges and improving detection performance in an anomaly detection task using IoT datasets. Our extensive experimental results with three publicly accessible IoT datasets show the performance improvement of up to 22.9% with the approach of data augmentation, compared to the baseline (without relying on data augmentation).
