| 1 | Insulin sensitizers in treatment of nonalcoholic fatty liver disease:Systematic review显示文摘AIM: To summarize the evidence available for the clinical effectiveness of insulin sensitizers in the treatment of nonalcoholic fatty liver disease (NAFLD) systematically. METHODS: Relevant articles were located using computer-assisted searches of Medline (1966-March 2006), EMBASE (1988-March 2006), CINAHL (1982-March 2003), Educational Resource Information Center (1966-March 2006), Library, Information Science & Technology Abstracts (1967-March 2006), Cochrane Database of Systematic Reviews, Database of Abstracts of Reviews of Effects (1994-2006), dissertations in ProQuest and FirstSearch databases. Manual searches were made in the abstracts from meetings of the American Gastroenterological Association (1999-2006), and the American Association for the Study of Liver Diseases (2003-2005). Studies were retrieved using the following selection criteria: (1) clinical trials using insulin sensitizers in subjects with NAFLD, (2) adult patients, (3) published as full manuscripts or abstracts, and (4) English, Spanish, German, and French languages only. Data were abstracted independently by two reviewers following standardized procedures. A face-to- face comparison of data was conducted to ensure the completeness and reliability of the abstraction process. RESULTS: Nine studies were included, six using metformin and three using thiazolidinediones. Only two studies were placebo-controlled trials. The mediansample size for all studies was 18 subjects. In the placebo-controlled trials, metformin improved insulin resistance markers and liver function tests, but not histological scores. In the single-arm trials, metformin and thiazolidinediones improved insulin resistance markers and liver function tests, and beneficial histological changes were reported. There is limited high-quality information available from which to draw categorical conclusions about the clinical use of insulin sensitizers in NAFLD.CONCLUSION: Current information indicates that the use of insulin sensitizers in NAFLD improves insulin resistance and liver function. | 2 | Red pigment production by Penicillium purpurogenum GH2 is influenced by pH and temperature显示文摘The combined effects of pH and temperature on red pigment production and fungal morphology were evaluated in a submerged culture of Penicillium purpurogenum GH2, using Czapek-Dox media with D-xylose as a carbon source. An experimental design with a factorial fix was used: three pH values (5, 7, and 9) and two temperature levels (24 and 34 °C) were evaluated. The highest production of red pigment (2.46 g/L) was reached with a pH value of 5 and a temperature of 24 °C. Biomass and red pigment production were not directly associated. This study demon- strates that P. purpurogenum GH2 produces a pigment of potential interest to the food industry. It also shows the feasibility of producing and obtaining natural water-soluble pigments for potential use in food industries. | 3 | Effect of a bacterial inoculum and additive on dry matter in situ degradability of sugarcane silage显示文摘The objective of this study was to evaluate the effect of adding a bacterial inoculum and a handmade additive to sugarcane silage(SCS) on the in situ digestibility of dry matter(DM). The treatments were: T1) sugarcane silage(SCS) and T2) sugarcane silage with 1% inoculum and 1% additive(SCS+). The bacterial inoculum consisted of 10.0% molasses, 1.0% yogurt, 5.0% chicken manure, 0.5% urea, and 83.0% water, and the additive was formulated with 1.0% urea, 0.1% ammonium sulfate, and 0.25% phosphorus. In situ dry matter digestibility(DMD) was determined using the nylon bag technique with four cows equipped with ruminal fistulas. Cows were fed with ensiled sugarcane supplemented with 1 kg of commercial concentrate. 5 g of ground sample for each sugarcane treatment were weighted in nylon bags and incubated for 4, 8, 12, 24, 48, and 72 h in a completely randomized design with six replicates. The DMD(%) was higher(P<0.05) for SCS+ for all incubation times when compared with SCS. There were no differences in ruminal p H between the treatments for all the incubation times. | 4 | Maximum tolerated volume in drinking tests with water and a nutritional beverage for the diagnosis of functional dyspepsia显示文摘AIM: Recently, drinking load tests with water or nutritional beverages have been proposed as diagnostic tools for functional dyspepsia (FD), therefore we sought to reproduce if these tests can discriminate between FD patients and controls in a Mexican population. METHODS: Twenty FD-Rome Ⅱ patients were matched by age and gender with 20 healthy controls. All underwent both drinking tests at a 15 mL/min rate, randomly, 7 d apart. Every 5 min within each test, four symptoms were evaluated (satiety, bloating, nausea and pain) by Likert scales. Maximum tolerated volume (MTV) was defined as the ingested volume when a score of 5 was reached for any symptom or when the test had to be stopped because the patients could not tolerate more volume. Sensitivity and specificity were analyzed. RESULTS: FD patients had higher symptom scores for both tests compared to controls (water: t= 4.1, P= 0.001 <0.01; Nutren(R): t= 5.2, P= 0.001<0.01). The MTV forwater and Nutren(R) were significantly lower in FD (water: 1014±288 vs 1749±275 mL; t = 7.9, P = 0.001<0.01;Nutren(R): 652±168 vs 1278±286 mL; t= 6.7, P = 0.001<0.01). With the volume tolerated by the controls, the percentile 10 was determined as the lower limit fortolerance. Sensitivity and specificity were 0.90, 0.95 for water and 0.95, 0.95 for Nutren(R) tests.CONCLUSION: A drinking test with water or a nutritional beverage can discriminate between FD patients and healthy subjects in Mexico, with high sensitivity and specificity.
