1Addition of simvastatin to carvedilol non responders: A new pharmacological therapy for treatment of portal hypertension显示文摘AIM To determine whether addition of simvastatin could be an important pharmacological rescue therapy for carvedilol non-responders. METHODS One hundred and two consecutive patients of cirrhosis of liver with significant portal hypertension were included. Hepatic venous pressure gradient(HVPG) was measured at the base line and after proper optimization of dose; chronic response was assessed at 3 mo. Carvedilol non-responders were given simvastatin 20 mg per day(increased to 40 mg per day at day 15). Carvedilol plus simvastatin was continued for 1 mo and hemodynamic response was again measured at 1 mo.RESULTS A total of 102 patients with mean age of 58.3 ± 6.6 years were included. Mean baseline HVPG was 16.75 ± 2.12 mmH g and after optimization of dose and reassessment of HVPG at 3 mo, mean reduction of HVPG from baseline was 5.5 ± 1.7 mm Hg and 2.8 ± 1.6 mm Hg among responders and non-responders respectively(P < 0.001). Addition of simvastatin to carvedilol non-responders resulted in significant response in 16 patients(42.1%) and thus overall response with carvedilol and carvedilol plus simvastatin was seen in 78 patients(80%). 2Port site infection in laparoscopic surgery: A review of its management显示文摘Laparoscopic surgery(LS), also termed minimal access surgery, has brought a paradigm shift in the approach to modern surgical care. Early postoperative recovery, less pain, improved aesthesis and early return to work have led to its popularity both amongst surgeons and patients. Its application has progressed from cholecystectomies and appendectomies to various other fields including gastrointestinal surgery, urology, gynecology and oncosurgery. However, LS has its own package of complications. Port site infection(PSI), although infrequent, is one of the bothersome complications which undermine the benefits of minimal invasive surgery. Not only does it add to the morbidity of the patient but also spoils the reputation of the surgeon. Despite the advances in the field of antimicrobial agents, sterilization techniques, surgical techniques, operating room ventilation, PSIs still prevail. The emergence of rapid growing atypical mycobacteria with multidrug resistance, which are the causative organism in most of the cases, has further compounded the problem. PSIs are preventable if appropriate measures are taken preoperatively, intraoperatively and postoperatively. PSIs can often be treated non-surgically, with early identification and appropriate management. Macrolides, quinolones and aminoglycosides antibiotics do show promising activity against the atypical mycobacteria. This review article highlights the clinical burden, presentations and management of PSIs in LS as shared by various authors in the literature. We have given emphasis to atypical mycobacteria, which are emerging as a common etiological agent for PSIs in LS. 51,3-Bis(2-chloroethyl)-1-nitrosourea enhances the inhibitory effect of Resveratrol on 5-fluorouracil sensitive/resistant colon cancer cells显示文摘AIM:To study the mechanism of 5-fluorouracil(5-FU)resistance in colon cancer cells and to develop strategies for overcoming such resistance by combination treatment.METHODS:We established and characterized a 5-FU resistance(5-FU-R)cell line derived from continuous exposure(25μmol/L)to 5-FU for 20 wk in 5-FU sensitive HCT-116 cells.The proliferation and expression of different representative apoptosis and anti-apoptosis markers in 5-FU sensitive and 5-FU resistance cells were measured by the MTT assay and by Western blotting,respectively,after treatment with Resveratrol(Res)and/or 1,3-Bis(2-chloroethyl)-1-nitrosourea(BCNU).Apoptosis and cell cycle arrest was measured by 4',6'-diamidino-2-phenylindole hydrochloride staining and fluorescence-activated cell sorting analysis,respectively.The extent of DNA damage was measured by the Comet assay.We measured the visible changes in the DNA damage/repair cascade by Western blotting.RESULTS:The widely used chemotherapeutic agents BCNU and Res decreased the growth of 5-FU sensitive HCT-116 cells in a dose dependent manner.Combined application of BCNU and Res caused more apoptosis in5-FU sensitive cells in comparison to individual treatment.In addition,the combined application of BCNU and Res caused a significant decrease of major DNA base excision repair components in 5-FU sensitive cells.We established a 5-FU resistance cell line(5-FU-R)from 5-FU-sensitive HCT-116(mismatch repair deficient)cells that was not resistant to other chemotherapeutic agents(e.g.,BCNU,Res)except 5-FU.The 5-FU resistance of 5-FU-R cells was assessed by exposure to increasing concentrations of 5-FU followed by the MTT assay.There was no significant cell death noted in5-FU-R cells in comparison to 5-FU sensitive cells after5-FU treatment.This resistant cell line overexpressed anti-apoptotic[e.g.,AKT,nuclear factorκB,FLICE-like inhibitory protein),DNA repair(e.g.,DNA polymerase beta(POL-β),DNA polymerase eta(POLH),protein Flap endonuclease 1(FEN1),DNA damage-binding protein 2(DDB2)]and 5-FU-resistance proteins(thymidylate synthase)but under expressed pro-apoptotic proteins(e.g.,DAB2,CK1)in comparison to the parental cells.Increased genotoxicity and apoptosis were observed in resistant cells after combined application of BCNU and Res in comparison to untreated or parental cells.BCNU increased the sensitivity to Res of 5-FU resistant cells compared with parental cells.Fifty percent cell death were noted in parental cells when 18μmol/L of Res was associated with fixed concentration(20μmol/L)of BCNU,but a much lower concentration of Res(8μmol/L)was needed to achieve the same effect in 5-FU resistant cells.Interestingly,increased levels of adenomatous polyposis coli and decreased levels POL-β,POLH,FEN1 and DDB2 were noted after the same combined treatment in resistant cells.CONCLUSION:BCNU combined with Res exerts a synergistic effect that may prove useful for the treatment of colon cancer and to overcome drug resistance.Dipon Das Ranjan Preet Purusottam Mohapatra Shakti Ranjan Satapathy Chanakya Nath Kundu 2013World Journal of Gastroenterology2013,19,42:4

8Synthesis of Co3O4 Nanoparticles Wrapped Within Full Carbon Matrix as an Anode Material for Lithium Ion Batteries显示文摘A facile polyol-assisted pyro-synthesis method was used to synthesize Co_3O_4 nanoparticles embedded into carbon matrix without using any conventional carbon source. The surface analysis by scanning electron microscopy showed that the Co_3O_4 nanoparticles(~20 ± 5 nm) are tightly enwrapped within the carbon matrix. CHN analysis determined the carbon content was only 0.11% in the final annealed sample. The Co_3O_4@carbon exhibited high capacities and excellent cycling performance as an anode at various current rates(such as 914.4 and 515.5 mAh g^(-1) at 0.25 and1.0 C, respectively, after 50 cycles; 318.2 mAh g^(-1) at a high current rate of 5.0 C after 25 cycles). 9Actinobacteria-enhanced plant growth, nutrient acquisition, and crop protection:Advances in soil, plant, and microbial multifactorial interactions显示文摘Agricultural areas of land are deteriorating every day owing to population increase, rapid urbanization, and industrialization. To feed today’s huge populations, increased crop production is required from smaller areas, which warrants the continuous application of high doses of inorganic fertilizers to agricultural land. These cause damage to soil health and, therefore, nutrient imbalance conditions in arable soils. Under these conditions, the benefits of microbial inoculants (such as Actinobacteria) as replacements for harmful chemicals and promoting ecofriendly sustainable farming practices have been made clear through recent technological advances. There are multifunctional traits involved in the production of different types of bioactive compounds responsible for plant growth promotion, and the biocontrol of phytopathogens has reduced the use of chemical fertilizers and pesticides. 11Diagnostic assays developed for the control of foot-andmouth disease in India显示文摘Foot-and-mouth disease(FMD) is a highly contagious and economically devastating disease of livestock, primarily affecting cattle, buffalo and pigs. FMD virus serotypes O, A and Asia1 are prevalent in India and systematic efforts are on to control and eventually eradicate the disease from the country. FMD epidemiology is complex due to factors like co-circulation, extinction, emergence and re-emergence of genotypes/lineages within the three serotypes, animal movement, diverse farm practices and large number of susceptible livestock in the country. Systematic vaccination, prompt diagnosis, strict biosecurity measures, and regular monitoring of vaccinal immunity and surveillance of virus circulation are indispensible features for the effective implementation of the control measures. Availability of suitable companion diagnostic tests is very important in this endeavour. 13Ore petrography of low-grade siliceous manganese ores from the Bonai-Keonjhar belt, Orissa, India: The influence of mineral-fabric on their beneficiation显示文摘Low grade siliceous manganese ores from the iron ore group of the Bonai-Keonjhar belt, Orissa, India are found mostly in shear zones. The ore characteristics of siliceous manganese ore samples from three differ- ent mines, viz. the Shankar (Barbil OMC lease hold area), the Sone-Patuli (Patmunda, OMM lease hold area), and the Musaghar (Roida, OMDC lease hold area), were studied. These siliceous manganese ores are of three types, respectively: (i) spongy-granular; (ii) massive-mosaic; and (iii) hard-mylonitized. The spongy-gran- ular type contains granular, saccharoidal quartz and the major manganese mineral present is pyrolusite. The second type contains well crystallized quartz and cryptomelane, while the third has cherty, fine grained quartz (mylonite) along with romanechite. All three ores were subjected to physical beneficiation under similar conditions. Both gravity and magnetic separation techniques were employed. The mineral-fabric of the ores has been correlated to the extent of their beneficiation using these physical techniques. Of these three ores only the spongy-granular type responded well to upgrading. The feed with 22% Mn content could be upgraded to 44% with a 28% yield and a 49% recovery. The good response to beneficiation of the spongy- granular sample could be due to the large euhedral crystals of pyrolusite and the friable nature of the sac- charoidal quartz. 14The Interaction of Oblique Flexural Gravity Waves With a Small Bottom Deformation on a Porous Ocean-Bed: Green's Function Approach显示文摘The interaction of oblique incident water waves with a small bottom deformation on a porous ocean-bed is examined analytically here within the framework of linear water wave theory. The upper surface of the ocean is assumed to be covered by an infinitely extended thin uniform elastic plate, while the lower surface is bounded by a porous bottom surface having a small deformation. By employing a simplified perturbation analysis, involving a small parameter δ(<<1), which measures the smallness of the deformation, the governing Boundary Value Problem(BVP) is reduced to a simpler BVP for the first-order correction of the potential function. This BVP is solved using a method based on Green's integral theorem with the introduction of suitable Green's function to obtain the first-order potential, and this potential function is then utilized to calculate the first-order reflection and transmission coefficients in terms of integrals involving the shape function c(x) representing the bottom deformation. Consideration of a patch of sinusoidal ripples shows that when the quotient of twice the component of the incident field wave number propagating just below the elastic plate and the ripple wave number approaches one, the theory predicts a resonant interaction between the bed and the surface below the elastic plate. Again, for small angles of incidence, the reflected wave energy is more as compared to the other angles of incidence. It is also observed that the reflected wave energy is somewhat sensitive to the changes in the flexural rigidity of the elastic plate, the porosity of the bed and the ripple wave numbers.
