| 3 | Lateral patellofemoral ligament reconstruction to restore functional capacity in patients previously undergoing lateral retinacular release显示文摘AIM: To study patient outcomes after surgical correction for iatrogenic patellar instability.METHODS: This retrospective study looked at 17 patients(19 knees) suffering from disabling medial patellar instability following lateral release surgery. All patients underwent lateral patellofemoral ligament(LPFL) reconstruction by a single surgeon. Assessments in all 19 cases included functional outcome scores, range of motion, and assessment for the presence of apprehension sign of the patella to determine if LPFL reconstruction surgery was successful at restoring patellofemoral stability.RESULTS: No patients reported any residual postoperative symptoms of patellar instability. Also no patients demonstrated medial patellar apprehension or examiner induced subluxation with the medial instability test described earlier following LPFL reconstruction. Furthermore, all patients recovered normal range of motion compared to the contralateral limb. For patients with pre and postoperative outcome scores, the mean overall knee injury and osteoarthritis outcome score increased significantly, from 34.39 preoperatively(range: 7.7-70.12) to 69.54 postoperatively(range:26.82-91.46) at final follow-up(P < 0.0001). | 12 | Evolution of shoals and vegetation of Jiuduansha in the Changjiang River Estuary of China in the last 30 years显示文摘The evolution of the shoals and vegetation plays an important role in maintaining the stability of the river regime and the estuarine ecosystem. However, the interaction between the evolution of shoals and vegetation dynamic has rarely been reported. In this study, we determined the interaction between the shoal and vegetation evolution of Jiuduansha in the Changjiang River Estuary in the last 30 years. We did this through the collection and summarization of the existing data of the regional hydrological processes, wading engineering, and vegetation,and combined it with the analysis of nautical charts and remote sensing images. During the past 30 years, the expansion of the shoals within the 0 m isobath in Jiuduansha was obvious, with an increase of 176.5%, while the expansion of the shoals within the 5 m isobath was relatively slow. The regional hydrological characteristics in the Jiuduansha area changed dramatically, especially the sediment discharges. The area of vegetation in Jiuduansha increased from 9.1 km^2 in 1990 to 65.68 km^2 in 2015, while the variations in the different vegetation types were different. The best combination of environmental factors with a significant correlation on the shoals within the 0 m isobath is the area of Spartina alterniflora and Phragmites australis. The evolution of Jiuduansha shoals was significantly affected by the variations in hydrological characteristics. Meanwhile, on a long-term scale, the expansion of the shoals could promote the regional vegetation expansions due to the suitable elevation and environmental conditions it provides. The interaction between the shoal and vegetation evolution varied in the different vegetation types and different elevations. However, data on how a high-fat highcarbohydrate(HFHC) diet influences the effect of different intensities of PA on cardiometabolic health and cardiovascular function in a controlled setting are yet to be fully established. This study investigated the effect of sedentary behavior, light-intensity training(LIT), and high-intensity interval training(HIIT) on cardiometabolic markers and vascular and cardiac function in HFHC-fed adult rats.Methods: Twelve-week-old Wistar rats were randomly allocated to 4 groups(12 rats/group): control(CTL), sedentary(SED), LIT, and HIIT.Biometric indices, glucose and lipid control, inflammatory and oxidative stress markers, vascular reactivity, and cardiac electrophysiology of the experimental groups were examined after 12 weeks of HFHC-diet feeding and PA interventions.Results: The SED group had slower cardiac conduction(p = 0.0426) and greater thoracic aortic contractile responses(p < 0.05) compared with the CTL group. The LIT group showed improved cardiac conduction compared with the SED group(p = 0.0003), and the HIIT group showed decreased mesenteric artery contractile responses compared with all other groups and improved endothelium-dependent mesenteric artery relaxation compared with the LIT group(both p < 0.05). The LIT and HIIT groups had lower visceral(p = 0.0057 for LIT, p = 0.0120 for HIIT)and epididymal fat(p < 0.0001 for LIT, p = 0.0002 for HIIT) compared with the CTL group.Conclusion: LIT induced positive adaptations on fat accumulation and cardiac conduction, and HIIT induced a positive effect on fat accumulation,mesenteric artery contraction, and endothelium-dependent relaxation. No other differences were observed between groups. | 15 | An Awareness-Adoption Matrix for Strategic Decision Making in Agricultural Development Projects: A Case Study in Yunnan Province, China显示文摘Significant achievements have been made in generating sustainable agricultural technologies in developing countries. Usually, these new technologies have been more effective in increasing production and productivity than existing technologies. However, many new technologies considered ‘effective’ have not been successful in alleviating the associated sustainability problems, due to poor adoption by targeted users. Success of any novel technical intervention should be judged on the basis of how widespread adoption is in the target area by targeted users. In this context, a case study identified the factors affecting farmers’ adoption of improved technologies extended by an agricultural development project in a rural Village in Yunnan Province, China. Initial adoption/adaptation of project technologies was influenced by farmers’ awareness of the technology. Farmers were more aware of polythene mulch, contour cultivation, intercropping and tree planting technologies than others. This led to a comparatively high initial uptake of polythene mulch, contour cultivation, sweet chestnut, and intercropping technologies. Farmers had inadequate knowledge about some Project technologies, the adoption of which was particularly low. This reveals the need for increasing farmers’ awareness about the rationale for Project technologies to achieve greater adoption/adaptation of project technologies by farmers over wider areas. Farmers’ testing of the technology leads to better adoption/adaptation compared to mere awareness. However, considerable time and resources are required for testing/trying technologies. This justifies the usefulness of comparatively quick and less resource demanding options for awareness creation. Development of effective cropping technologies is important for sustainable agricultural development. The success of any agricultural development project, especially in terms of improving sustainability, depends on how widely those improved technologies are adopted/adapted by farmers in the targeted region. Therefore, farmers’ adoption of technologies should be a key criterion for judging the success of any project. The awareness-adoption matrix is a useful tool to guide research and development projects to achieve expected adoption/adaptation of technologies.
