1Changes of proteins induced by anticoagulants can be more sensitively detected in urine than in plasma显示文摘The most fundamental property of biomarkers is change.But changes are hard to maintain in plasma since it is strictly controlled by homeostatic mechanisms of the body.There is no homeostatic mechanism for urine.Besides,urine is partly a filtration of blood,and systematic information can be reflected in urine.We hypothesize that change of blood can be reflected in urine more sensitively.Here we introduce the interference into the blood by two anticoagulants heparin or argatroban.Plasma and urine proteins were profiled by LC-MS/MS and then validated by Western blot in totally six SD female rats before and after the drug treatments.In argatroban treated group,with exactly the same experimental procedure and the same cutoff value for both plasma and urine proteins,62 proteins changed in urine,only one of which changed in plasma.In heparin treated group,27 proteins changed in urine but only three other proteins changed in plasma.Both LC-MS/MS and Western blot analyses demonstrated drug-induced increases in transferrin and hemopexin levels in urine but not in plasma.Our data indicates that urine may serve as a source for more sensitive detection of protein biomarkers than plasma.LI MengLin ZHAO MinDi GAO YouHe 2014Science China(Life Sciences)2014,57,7:18
2Comparing the intraoperative complication rate of femtosecond laser-assisted cataract surgery to traditional phacoemulsification显示文摘AIM: To compare the complication rate of femtosecond laser-assisted cataract surgery(FLACS) and traditional phacoemulsification for the first 18 mo of FLACS use at a private surgical center in Hawaii.METHODS: A retrospective chart review was conducted from January 2012 to June 2013. The first 273 consecutive eyes receiving FLACS and 553 eyes receiving traditional phacoemulsification were examined.All surgeries were performed at a single surgical center in Hawaii. The presence of intraoperative complications was used as the main outcome measure. Approval was obtained from the institutional review board of the University of Hawaii.RESULTS: The overall complication rate for FLACS was 1.8%, while that of the traditional procedure was5.8%(P <0.05). 4Comparison of renal safety of tenofovir and entecavir in patients with chronic hepatitis B: Systematic review with meta-analysis显示文摘BACKGROUND Recently, the American Association for the Study of Liver Disease suggested no preference between tenofovir(TDF) and entecavir(ETV) regarding potential long-term risks of renal complications. Over the years, renal safety has become a critical concern in nucleos(t)ide analog-treated patients due to the long-term use of these drugs. However, existing studies do not show significant differences in renal dysfunction between these two drugs. Further, there is a paucity of studies comparing the long-term renal effects of TDF and ETV.AIM To investigate the effects of TDF and ETV on renal function, we performed systematic review and meta-analysis.METHODS Two investigators independently searched the Cochrane Library, MEDLINE, and Embase databases for randomized controlled trials and nonrandomized studies(NRSs) using the keywords 'CHB', 'Tenofovir', and 'Entecavir', and additional references were obtained from the bibliographies of relevant articles published through December 2017. The quality of each study was assessed using the Newcastle-Ottawa scale and the Grading of Recommendations Assessment,Development and Evaluation criteria. The primary outcome was the change in serum creatinine level in the TDF and ETV groups at baseline, 6 mo, 12 mo and24 mo.RESULTSNine NRSs comprising 2263 participants met the inclusion criteria. Changes in creatinine levels were higher in the TDF group than in the ETV group at 6 mo[mean difference(MD) = 0.03 mg/dL;95%CI: 0.02-0.04;I2 = 0%], 12 mo(MD =0.05 mg/dL;95%CI: 0.02-0.08;I2 = 78%), and 24 mo(MD = 0.07 mg/dL;95%CI:0.01-0.13;I2 = 93%). 5The influence of surface roughness on cloud cavitation flow around hydrofoils显示文摘The aim of this study is to investigate experimentally the effect of surface roughness on cloud cavitation around Clark-Y hydrofoils. High-speed video and particle image velocimetry(PIV) were used to obtain cavitation patterns images(Prog. Aerosp. Sci. 37: 551–581, 2001), as well as velocity and vorticity fields. Results are presented for cloud cavitating conditions around a Clark-Y hydrofoil fixed at angle of attack of α = 8?for moderate Reynolds number of Re = 5.6 × 10~5. The results show that roughness had a great influence on the pattern, velocity and vorticity distribution of cloud cavitation. For cavitating flow around a smooth hydrofoil(A) and a rough hydrofoil(B), cloud cavitation occurred in the form of finger-like cavities and attached subulate cavities, respectively. The period of cloud cavitation around hydrofoil A was shorter than for hydrofoil B.Surface roughness had a great influence on the process of cloud cavitation. The development of cloud cavitation around hydrofoil A consisted of two stages:(1) Attached cavities developed along the surface to the trailing edge;(2) A reentrant jet developed, resulting in shedding and collapse of cluster bubbles or vortex structure. Meanwhile, its development for hydrofoil B included three stages:(1) Attached cavities developed along the surface to the trailing edge, with accumulation and rotation of bubbles at the trailing edge of the hydrofoil affecting the flow field;(2) Development of a reentrant jet resulted in the first shedding of cavities. Interaction and movement of flows from the pressure side and suction side brought liquid water from the pressure side to the suction side of the hydrofoil, finally forming a reentrant jet. The jet kept moving along the surface to the leading edge of the hydrofoil, resulting in large-scale shedding of cloud bubbles. 6Hepatocellular carcinoma in patients with non-alcoholic fatty liver disease显示文摘Nonalcoholic fatty liver disease(NAFLD) is the most common chronic liver disease in the United States and represents an increasingly important etiology of hepatocellular carcinoma(HCC) with annual cumulative incidence rates ranging from 2% to 12% in cohorts of NAFLD cirrhosis. While the risk of progression of NAFLD to HCC remains higher among patients with fibrosis or cirrhosis, an increasing amount of literature describes NAFLD-HCC as a disease that can occur in the absence of cirrhosis. Efforts to characterize the pathogenesis of NAFLD-HCC have suggested mechanisms that strongly associate with states of hyperinsulinemia and chronic inflammation, cellular mechanisms including adaptive immune responses and hepatic progenitor cell populations, and genetic polymorphisms including mutations of PNPLA3. Current literature describes NAFLD-HCC mostly as a disease of late presentation with lower rates of receipt of curative therapy and worse prognosis. However, a growing body of evidence has reported comparable and potentially more favorable disease-free and overall survival rates among patients with NAFLD-HCC after receipt of curative treatment. 7Current update on molecular cytogenetics, diagnosis and management of gastrointestinal stromal tumors显示文摘Gastrointestinal stromal tumors(GISTs)are the most common mesenchymal tumors of the gastrointestinal(GI)tract and are thought to arise from precursors of the interstitial cells of Cajal.GISTs can arise anywhere in the GI tract,but most commonly originate from the stomach and small intestine.The majority of GISTs occur as a result of activating mutations in two receptor protein tyrosine kinases:KIT and/or platelet-derived growth factor receptor-α.Mutational analyses allow for predicting patient prognosis and treatment response.Clinical presentations can vary from no symptoms,typical in the case of small incidentally found tumors,to GI bleeding,abdominal discomfort,and ulcer-related symptoms when the tumor is enlarged.Imaging plays a critical role in the diagnosis and management of these tumors with multiphasic computed tomography serving as the imaging modality of choice.Magnetic resonance imaging and positron emission tomography-computed tomography can serve as imaging adjuncts in lesion characterization,especially with liver metastases,and subsequent staging and assessment for treatment response or recurrence.Surgical resection is the preferred management for small GISTs,while tyrosine kinase inhibitors−imatinib mesylate and sunitinib malate−serve as crucial molecular-targeted therapies for locally advanced and metastatic GISTs.This review article highlights the clinical presentation,pathology and molecular cytogenetics,imaging features,and current management of GISTs.Mindy X Wang Catherine Devine Nicole Segaran Dhakshinamoorthy Ganeshan 2021World Journal of Gastroenterology2021,27,41:5

11Metabolic,autonomic and immune markers for cardiovascular disease in posttraumatic stress disorder显示文摘Posttraumatic stress disorder(PTSD) has been associated with significantly greater incidence of heart disease.Numerous studies have indicated that health problems for individuals with PTSD occur earlier in life than in the general population. Multiple mechanistic pathways have been suggested to explain cardiovascular disese(CVD)risk in PTSD,including neurochemical,behavioral,and immunological changes. The present paper is a review of recent research that examines cardiovascular and immune risk profiles of individuals with PTSD. First,we address the relatively new evidence that the constellation of risk factors commonly experienced in PTSD fits the profile of metabolic syndrome. Next we examine the findings concerning hypertension/blood pressure in particular. The literature on sympathetic and parasympathetic responsivity in PTSD is reviewed. Last,we discuss recent findings concerning immune functioning in PTSD that may have a bearing on the high rates of CVD and other illnesses. 12Features of primary pancreatic lymphoma:A bi-institutional review with an emphasis on typical and atypical imaging features显示文摘BACKGROUND Primary pancreatic lymphoma(PPL)is a rare neoplasm.Being able to distinguish it from other pancreatic malignancies such as pancreatic ductal adenocarcinoma(PDAC)is important for appropriate management.Unlike PDAC,PPL is highly sensitive to chemotherapy and usually does not require surgery.Therefore,being able to identify PPL preoperatively will not only direct physicians towards the correct avenue of treatment,it will also avoid unnecessary surgical intervention.AIM To evaluate the typical and atypical multi-phasic computed tomography(CT)imaging features of PPL.METHODS A retrospective review was conducted of the clinical,radiological,and pathological records of all subjects with pathologically proven PPL who presented to our institutions between January 2000 and December 2020.Institutional review board approval was obtained for this investigation.The collected data were analyzed for subject demographics,clinical presentation,laboratory values,CT imaging features,and the treatment received.Presence of all CT imaging findings including size,site,morphology and imaging characteristics of PPL such as the presence or absence of nodal,vascular and ductal involvement in these subjects were recorded.Only those subjects who had a pre-treatment multiphasic CT of the abdomen were included in the study.RESULTS Twenty-nine cases of PPL were diagnosed between January 2000 and December 2020(mean age 66 years;13 males/16 females).All twenty-nine subjects were symptomatic but only 4 of the 29 subjects(14%)had B symptoms.Obstructive jaundice occurred in 24%of subjects.Elevated lactate dehydrogenase was seen in 81%of cases,whereas elevated cancer antigen 19-9 levels were present in only 10%of cases for which levels were recorded.The vast majority(90%)of tumors involved the pancreatic head and uncinate process.Mean tumor size was 7.8 cm(range,4.0-13.8 cm).PPL presented homogenous hypoenhancement on CT in 72%of cases.Small volume peripancreatic lymphadenopathy was seen in 28%of subjects.Tumors demonstrated encasement of superior mesenteric vessels in 69%of cases but vascular stenosis or occlusion only manifested in 5 out of the twentynine individuals(17%).Mild pancreatic duct dilatation was also infrequent and seen in only 17%of cases,whereas common bile duct(CBD)dilation was seen in 41%of subjects.Necrosis occurred in 10%of cases.Size did not impact the prevalence of pancreatic and CBD dilation,necrosis,or mesenteric root infiltration(P=0.525,P=0.294,P=0.543,and P=0.097,respectively).Pancreatic atrophy was not present in any of the subjects.CONCLUSION PPL is an uncommon diagnosis best made preoperatively to avoid unnecessary surgery and ensure adequate treatment.In addition to the typical CT findings of PPL,such as homogeneous hypoenhancement,absence of vascular stenosis and occlusion despite encasement,and peripancreatic lymphadenopathy,this study highlighted many less typical findings,including small volume necrosis and pancreatic and bile duct dilation.Nicole Segaran Kumaresan Sandrasegaran Catherine Devine Mindy X Wang Chintan Shah Dhakshinamoorthy Ganeshan 2021World Journal of Clinical Oncology2021,12,9:2

15Structural Characterization and Photocatalytic Activity of Synthesized Carbon Modified TiO2 for Phenol Degradation显示文摘To promote the photocatalytic performance TiO2 and enlarge its application in visible region, carbon doped TiO2 (C/TiO2) composites were synthesized by wet impregnation method using sucrose as a precursor and used for phenol photocatalytic reaction. The synthesized products were characterized by Nitrogen adsorption-desorption isotherms (BET), X-ray diffraction (XRD), transmission electron microscopy (TEM) and UV-visible diffuse reflectance spectroscopy (UV-vis) techniques. The results showed that the obtained TiO2 was anatase phase in the C/TiO2 products, and its crystallite size was 11.7 nm, respectively. Carbon amount and calcined temperature of C/TiO2 can promote phenol removal. In this experiment, 5% carbon and 500 ℃ are the best choice for photocatalyst preparation. Under the UV light irradiation, 5%C/TiO2 (500 ℃, 2 h) exhibited the efficiency of 70.0% for phenol degradation within 150 min whereas TiO2 (500 ℃, 2 h) had 53.0% in the same duration of time. Also 5%C/TiO2 (500 ℃, 2 h) has higher photocatalytic performance under sunlight than pure TiO2. 17Hepatitis B surface antigen escape mutations: Indications for initiation of antiviral therapy revisited显示文摘Approximately 240 million people are chronically infected with hepatitis B. The implementation of rigorous vaccination programs has led to an overall decrease in the prevalence of this disease worldwide but this may also have led to emergence of viral mutations that can escape the protection of hepatitis B surface antibody. As this phenomenon is increasingly recognized, concern for transmission to vaccinated individuals has also been raised. Herein, we describe two cases where the suspected presence of a hepatitis B surface antigen escape mutation impacted the decision to initiate early antiviral therapy, as well as provide a brief review of these mutations.
