| 1 | International consensus statement on robotic hepatectomy surgery in 2018显示文摘The robotic surgical system has been applied in liver surgery. However,controversies concerns exist regarding a variety of factors including the safety,feasibility, efficacy, and cost-effectiveness of robotic surgery. To promote the development of robotic hepatectomy, this study aimed to evaluate the current status of robotic hepatectomy and provide sixty experts' consensus and recommendations to promote its development. Based on the World Health Organization Handbook for Guideline Development, a Consensus Steering Group and a Consensus Development Group were established to determine the topics, prepare evidence-based documents, and generate recommendations. The GRADE Grid method and Delphi vote were used to formulate the recommendations. A total of 22 topics were prepared analyzed and widely discussed during the 4 meetings. Based on the published articles and expert panel opinion, 7 recommendations were generated by the GRADE method using an evidence-based method, which focused on the safety, feasibility, indication,techniques and cost-effectiveness of hepatectomy. | 3 | Second-line treatment of metastatic gastric cancer:Current options and future directions显示文摘Gastric cancer remains one among the leading causes of cancer-related deaths, regardless of its decreasing incidence and newly available treatment options. Most patients present at an advanced stage and are treated with upfront systemic chemotherapy. Those patients receiving first-line therapy may initially respond to treatment, but many of them relapse over time. In such condition, second-line treatment for disease progression remains the only available option. Although there exists no standard approach in the second-line setting, several phase Ⅲ trials have shown modest survival benefit in patients receiving irinotecan, taxane and ramucirumab over the best supportive care or active agents. This review analyzes the currently available treatment regimens and future directions of research in the second-line setting for metastatic gastric cancer with the best available evidence. | 6 | Ground moving target signal model and power calculation in forward scattering micro radar显示文摘Forward scattering micro radar is used for situation awareness;its operational range is relatively short because of the battery power and local horizon,the free space propagation model is not appropriate. The ground moving targets,such as humans,cars and tanks,have only comparable size with the transmitted signal wavelength;the point target model and the linear change of observation angle are not applicable. In this paper,the signal model of ground moving target is developed based on the case of forward scattering micro radar,considering the two-ray propagation model and area target model,and nonlinear change of observation angle as well as high order phase error. Furthermore,the analytical form of the received power from moving target has been obtained. Using the simulated forward scattering radar cross section,the received power of theoretical calculation is near to that of measured data. In addition,the simulated signal model of ground moving target is perfectly matched with the experimented data. | 8 | Reversibility of minimal hepatic encephalopathy following liver transplantation in Egyptian cirrhotic patients显示文摘AIM To evaluate the reversibility of minimal hepatic encephalopathy(MHE) following liver transplantation(LT) in Egyptian cirrhotic patients. METHODS This prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty ageand sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A(TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score(PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTS Before LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls(P < 0.001). There was a statistically significant improvement in test values in the patient group after LT; however, their values were still significantly worse than those of the controls(P < 0.001). The PHES detected MHE in 16 patients(80%) before LT with a median value of -7 ± 3.5. The median PHES value was significantly improved following LT, reaching-4.5 ± 5(P < 0.001), and the number of patients with MHE decreased to 11(55%). The pre-transplant model for end-stage liver disease(MELD) score ≥ 15 was significantly related to the presence of post-transplant MHE(P = 0.005). In recent years, this innovative technology has reached a maturity stage which allows it to be considered for a wide range of applications. The paper covers the fundamental aspects of this technology as a radar system, such as the resolution, power budget and Point Spread Function(PSF) analysis, as well as its signal processing aspects and the state of the art in terms of advanced SAR techniques that it enables. | 19 | Wolf phase tomography(WPT) of transparent structures using partially coherent illumination显示文摘In 1969,Emil Wolf proposed diffraction tomography using coherent holographic imaging to extract 3D information from transparent,inhomogeneous objects.In the same era,the Wolf equations were first used to describe the propagation correlations associated with partially coherent fields.Combining these two concepts,we present Wolf phase tomography(WPT),which is a method for performing diffraction tomography using partially coherent fields.WPT reconstruction works directly in the space-time domain,without the need for Fourier transformation,and decouples the refractive index(RI)distribution from the thickness of the sample.We demonstrate the WPT principle using the data acquired by a quantitative-phase-imaging method that upgrades an existing phase-contrast microscope by introducing controlled phase shifts between the incident and scattered fields.The illumination field in WPT is partially spatially coherent(emerging from a ring-shaped pupil function)and of low temporal coherence(white light),and as such,it is well suited for the Wolf equations.From three intensity measurements corresponding to different phase-contrast frames,the 3D RI distribution is obtained immediately by computing the Laplacian and second time derivative of the measured complex correlation function.We validate WPT with measurements of standard samples(microbeads),spermatozoa,and live neural cultures.The high throughput and simplicity of this method enables the study of 3D,dynamic events in living cells across the entire multiwell plate,with an RI sensitivity on the order of 10^(−5). | Xi Chen Mikhail E.Kandel Chenfei Hu Young Jae Lee Gabriel Popescu | 2020 | Light(Science & Applications)2020,9,1: | 6 |
| 20 | Aging:A mitochondrial DNA perspective,critical analysis and an update显示文摘The mitochondrial theory of aging, a mainstream theory of aging which once included accumulation of mitochondrial DNA(mt DNA) damage by reactive oxygen species(ROS) as its cornerstone, has been increasingly losing ground and is undergoing extensive revision due to its inability to explain a growing body of emerging data. Concurrently, the notion of the central role for mtDNA in the aging process is being met with increased skepticism. Our progress in understanding the processes of mtDNA maintenance, repair, damage, and degradation in response to damage has largely refuted the view of mt DNA as being particularly susceptible to ROS-mediated mutagenesis due to its lack of 'protective' histones and reduced complement of available DNA repair pathways. Recent research on mitochondrial ROS production has led to the appreciation that mitochondria, even in vitro, produce much less ROS than previously thought, automatically leading to a decreased expectation of physiologically achievable levels of mtDNA damage. New evidence suggests that both experimentally induced oxidative stress and radiation therapy result in very low levels of mtDNA mutagenesis. Recent advances provide evidence against the existence of the 'vicious' cycle of mtDNA damage and ROS production. Meta-studies reveal no longevity benefit of increased antioxidant defenses. Simultaneously, exciting new observations from both comparative biology and experimental systems indicate that increased ROS production and oxidative damage to cellular macromolecules, including mtDNA, can be associated with extended longevity. A novel paradigm suggests that increased ROS production in aging may be the result of adaptive signaling rather than a detrimental byproduct of normal respiration that drives aging.
