1Establishment of a transgenic mouse model with liver-specific expression of secretory immunoglobulin D显示文摘Mutation of mevalonate kinase(MVK) is thought to account for most cases of hyperimmunoglobulinemia D syndrome(HIDS) with recurrent fever.However,its mechanism and the relationship between elevated serum immunoglobulin D(IgD) and the clinical features of HIDS are unclear.In this study,we generated by fusion PCR a vector to express high levels of chimeric secretory IgD(csIgD) specifically in the liver.We then generated seven founder lines of transgenic mice by co-microinjection,and verified them using genomic PCR and Southern blotting.We detected the expression of csIgD by reverse transcription PCR,quantitative PCR,western blotting,and enzyme-linked immunosorbent assays.We demonstrated that csIgD could be specifically and stably expressed in the liver.We used flow cytometry to show that overexpression of csIgD in the bone marrow and spleen cells had no effect on B cell development.Morphologic and anatomical observation of the transgenic mice revealed skin damage,hepatosplenomegaly,and nephromegaly in some transgenic mice;in these mice,pathological sections showed high levels of cell necrosis and protein-like sediments in the liver,spleen,and kidney.We demonstrated that the genomic insertion sites of the transgenes did not disrupt the MVK gene on mouse chromosome 5.This transgenic mouse will be useful to explore the pathogenesis of HIDS.WANG Ping WEI ZhiGuo YAN BoWen HUANG Tan GOU KeMian DAI YunPing ZHENG Min WANG MeiLi CHENG XueQian WANG XiFeng XU Chen SUN Yi 2012Science China(Life Sciences)2012,55,3:6
2Genome-wide detection of additional fetal chromosomal abnormalities by cell-free DNA testing of 15,626 consecutive pregnant women显示文摘Cell-free DNA(cfDNA) testing for common fetal trisomies(T21, T18, T13) is highly effective. However, the usefulness of cfDNA testing in detecting other chromosomal abnormalities is unclear. We evaluated the performance of cfDNA testing for genome-wide abnormalities, and analyzed the incremental yield by reporting extra abnormalities. We performed genome-wide cfDNA testing in 15,626 consecutive pregnancies prospectively enrolled in this study. cfDNA testing results were reported and counseling was given depending on the presence of extra chromosomal abnormalities. cfDNA testing identified 190 cases(1.2%)of chromosomal abnormalities including 100 common trisomies and 90 additional abnormalities. By expanding the cfDNA reporting range to genome-wide abnormalities, the false positive rate increased to 0.39%(P<0.001) and positive predictive value(PPV) was reduced to 65.58%(P=0.42). However, the detection yield increased from 0.44% to 0.65%(P=0.014), and cfDNA testing detected 38.61%(39/101) additional abnormalities with no ultrasound and biochemical screening findings. cfDNA testing outperformed biochemical screening by showing 60 times higher true positive rate and fewer false negative results.Genome-wide cfDNA testing significantly increased the diagnostic yield by detecting extra abnormalities, especially those without diagnostic indications. 4Prognostic factors in patients with stage IV non-small cell lung cancer显示文摘Objective: To investigate the prognostic factors for stage IV non-small cell lung cancer (NSCLC) with distant metas- tasis and establish a reliable model of clinical prognostic index. Methods: From January 1990 to April 2005, 313 primary NSCLC patients with metastasis, who had been treated in Shanghai Chest Hospital, were reviewed. Survival time was estimated accord- ing to the Kaplan-Meier method. Cox proportional hazard regression model was used for multivariate analysis. Results: Among the 313 cases of non-small cell lung cancer (NSCLC) at stage IV, there were 218 and 95 patients with metastasis to single and different organs, respectively. The overall median survival time for all 313 cases of NSCLC patients was 10.8 (9.00, 12.30) months and the overall 1-, 2-, 3-, 4- and 5-year survival rate was 45%, 18%, 12%, 4% and 0%. There were 63, 174, 127, 36, 18, 11 and 5 patients with metastasis to brain (20.13%), bone (55.59%), lung (40.58%), liver (11.50%), adrenal gland (5.75%), subcutaneous (3.51%) and others, respectively. The survival time was shortest in subcutaneous metastasis (4.6 months), and liver 7.0 months, brain 8.0 months, adrenal gland 8.6 months, bone 10.6 months, lung 11.8 months. Kaplan-Meier estimation showed that patients anatomic typing, KPS, numbers of organ with metastasis, appetite, liver, adrenal gland and subcutane- ous metastasis, body weight loss, smoking, index of smoking, chemotherapy, cycles of chemotherapy were the predictors of survival. Multivariate analysis showed survival statistically significant correlation with anatomic typing, KPS, appetite, liver and subcutaneous metastasis, body weight loss, cycles of chemotherapy. The relative risk (RR) was 1.51, 1.97, 1.55, 1.67, 2.56, and 2.56 respectively. Conclusion: Survival time decreases distinctly in patients who had distant metastasis to more than two different organs (P<0.01). Bone is the commonest organ for distant metastasis in lung cancer. The prognosis is poor when lung cancer appears subcutaneous metastasis and liver metastasis. [Methods] The removal rates of DPPH free radicals and ABTS free radicals and the reduction capacity of Fe^(3+) by ethanol extract( EE),acetone extract( AE) and ethyl acetate extract( EAE) of Blumea Megacephala were detected.
