3Pharmacogenetic considerations for optimizing tacrolimus dosing in liver and kidney transplant patients显示文摘The introduction of tacrolimus in clinical practice has improved patient survival after organ transplant.However,despite the long use of tacrolimus in clinical practice,the best way to use this agent is still a matter of intense debate.The start of the genomic era has generated new research areas,such as pharmacogenetics,which studies the variability of drug response in relation to the genetic factors involved in the processes responsible for the pharmacokinetics and/or the action mechanism of a drug in the body.This variability seems to be correlated with the presence of genetic polymorphisms.Genotyping is an attractive option especially for the initiation of the dosing of tacrolimus;also,unlike phenotypic tests,the genotype is a stable characteristic that needs to be determined only once for any given gene.However,prospective clinical studies must show that genotype determination before transplantation allows for better use of a given drug and improves the safety and clinical efficacy of that medication.At present,research has been able to reliably show that the CYP3A5 genotype,but not the CYP3A4 or ABCB1 ones,can modify the pharmacokinetics of tacrolimus.However,it has not been possible to incontrovertibly show that the corresponding changes in the pharmacokinetic profile are linked with different patient outcomes regarding tacrolimus efficacy and toxicity.For these reasons,pharmacogenetics and individualized medicine remain a fascinating area for further study and may ultimately become the face of future medical practice and drug dosing.Alessio Provenzani Andrew Santeusanio Erin Mathis Monica Notarbartolo Manuela Labbozzetta Paola Poma Ambra Provenzani Carlo Polidori Giovanni Vizzini Piera Polidori Natale D'Alessandro 2013World Journal of Gastroenterology2013,19,48:15
4Role of the endothelium in inflammatory bowel diseases显示文摘Inflammatory bowel diseases(IBD) are a complex group of diseases involving alterations in mucosal immunity and gastrointestinal physiology during both initiation and progressive phases of the disease.At the core of these alterations are endothelial cells,whose continual adjustments in structure and function coordinate vascular supply,immune cell emigration,and regulation of the tissue environment.Expansion of the endothelium in IBD(angiogenesis),mediated by inflammatory growth factors,cytokines and chemokines,is a hallmark of active gut disease and is closely related to disease severity.The endothelium in newly formed or inflamed vessels differs from that in normal vessels in the production of and response to inflammatory cytokines,growth factors,and adhesion molecules,altering coagulant capacity,barrier function and blood cell recruitment in injury.This review examines the roles of the endothelium in the initiation and propagation of IBD pathology and distinctive features of the intestinal endothelium contributing to these conditions.Walter E Cromer J Michael Mathis Daniel N Granger Ganta V Chaitanya J Steven Alexander 2011World Journal of Gastroenterology2011,17,5:10

9Calcineurin inhibitor sparing strategies in renal transplantation, part one: Late sparing strategies显示文摘Kidney transplantation improves quality of life and reduces the risk of mortality. A majority of the success of kidney transplantation is attributable to the calcineurin inhibitors(CNIs), cyclosporine and tacrolimus, and their ability to reduce acute rejection rates. However, longterm graft survival rates have not improved over time, and although controversial, evidence does suggest a role of chronic CNI toxicity in this failure to improve outcomes. Consequently, there is interest in reducing or removing CNIs from immunosuppressive regimens in an attempt to improve outcomes. Several strategies exist to spare calcineurin inhibitors, including use of agents such as mycophenolate mofetil(MMF), mycophenolate sodium(MPS), sirolimus, everolimus or belatacept to facilitate late calcineurin inhibitor withdrawal, beyond 6 mo post-transplant; or using these agents to plan early withdrawal within 6 mo; or to avoid the CNIs all together using CNI-free regimens. Although numerous reviews have been written on this topic, practice varies significantly between centers. This review organizes thedata based on patient characteristics(i.e., the baseline immunosuppressive regimen) as a means to aid the practicing clinician in caring for their patients, by matching up their situation with the relevant literature. The current review, the first in a series of two, examines the potential of immunosuppressive agents to facilitate late CNI withdrawal beyond 6 mo post-transplant, and has demonstrated that the strongest evidence resides with MMF/MPS. MMF or MPS can be successfully introduced/maintained to facilitate late CNI withdrawal and improve renal function in the setting of graft deterioration, albeit with an increased risk of acute rejection and infection. Additional benefits may include improved blood pressure, lipid profile and serum glucose.Sirolimus has less data directly comparing CNI withdrawal to an active CNI-containing regimen, but modest improvement in short-term renal function is possible, with an increased risk of proteinuria, especially in the setting of baseline renal dysfunction and/or proteinuria. Renal outcomes may be improved when sirolimus is used in combination with MMF. 13Maintaining biodiversity will define our long-term success显示文摘Human beings are not only a part of our planet's ecosystems,but also,they are massively overusing them.This makes ecosystem protection,including biodiversity preservation,vital for humanity's future.The speed and scale of the threat are unprecedented in human history.The long arch of evolution has been confronted with such a high level of human impact,that we are now facing the sixth mass extinction event,66 million years after the last one.This threat heightens the imperative for bold human intervention.Our paper identifies three strategies for such an intervention.First,and possibly most challenging,human demand needs to be curbed so it fits within the bounds of what Earth's ecosystems can renew.Without meeting this quantitative goal,biodiversity preservation efforts will not be able to get scaled.Second,in the transition time,we must focus on those locations and areas where most biodiversity is concentrated.Such a focus on‘hotspots’will help safeguard the largest portion of biodiversity with least effort.Third,to direct biodiversity preservation strategies,we need to much better document the existence and distribution of biodiversity around the globe.New information technologies could help with this critical effort.In conclusion,biodiversity preservation is no longer just a concern for specialized biologist but is becoming a societal necessity if humanity wants to have a stable future.Peter Raven Mathis Wackernagel 2020Plant Diversity2020,42,4:3
14Current view and perspectives in amyotrophic lateral sclerosis显示文摘Amyotrophic lateral sclerosis(ALS), identified as a distinct clinical entity by Charcot since the end of the nineteenth century, is a devastating and fatal neurodegenerative disorder that affects motor neurons in the brain, brainstem and spinal cord. Survival of patients with ALS is associated with several factors such as clinical phenotype, age at onset, gender, early presence of respiratory failure, weight loss and treatment with Riluzole(the only disease-modifying drug approved for this disease). Nowadays, there is still no curative treatment for ALS: palliative care and symptomatic treatment are therefore essential components in the management of these patients. Nevertheless, the scientific knowledge in the field of ALS motor neuron degeneration is growing, with the prospect of new treatments. Based on this physiopathological knowledge, several new therapeutic targets are being studied, involving various mechanisms such as excitotoxicity, neuroinflammation, mitochondrial dysfunction, oxidative stress, RNA metabolism and other attractive concepts. Moreover, it is also important to identify reliable biomarkers that will be essential components for future therapeutic development and study design in ALS. 20Thermal characterization of bio-based phase changing materials in decorative wood-based panels for thermal energy storage显示文摘Decorative wood panels containing pouches of bio-based phase changing materials(PCMs) were prepared. Three different PCM mixtures were used: a blend of capric and lauric acids as well as two commercial products, Puretemp?20 and Puretemp?23(Puretemp). The panels consist of engraved Medium Density Fiberboard(MDF) filled with a plastic pouch filled with PCM. High density fiberboard(HDF) was used on top of the panels to enclose the PCM pouches. PCM mixtures were first tested by differential scanning calorimetry(DSC). Phase change temperature and total heat storage of the panels were measured for both fusion and solidification with a Dynamic Heat-Flow Meter Apparatus(DHFMA). DSC and DHFMA results were compared, allowing a better understanding of results gathered from these two techniques. DSC calibration has been revealed important when assessing PCMs. The panels present a phase change temperature and a latent heat storage suitable for buildings applications. The panel made with Puretemp?23 presented the highest energy, with 57.1 J g^(-1). Thermal cycling was conducted on the panels to investigate thermal reliability, which revealed small modifications of thermal properties for two products. For all cases, latent heat was found stable. Hygro-mechanical behavior of the panels was also evaluated as these where designed to be esthetic decorative panels.
