| 1 | Value of screening endoscopy in evaluation of esophageal,gastric and colon cancers显示文摘Esophageal,gastric,and colorectal cancers are deadly diseases that continue to plague our world today.The value of screening endoscopy in evaluating these types of cancers is a critical area of discussion due to a potential reduction in morbidity and mortality.This article describes how to identify a good screening test and explains what are important criteria in the field of screening endoscopy.Furthermore,the current status and progress of screening endoscopy for esophageal,gastric,and colorectal cancer will be evaluated and discussed.Mass screening programs have not been implemented for esophageal and gastric carcinomas in those with average or low risk populations.However,studies of high-risk populations have found value and a cost-benefit in conducting screening endoscopy.Colorectal cancer,on the other hand,has had mass screening programs in place for many years due to the clear evidence of improved outcomes.As the role of endoscopy as a screening tool has continued to develop,newer technology and techniques have emerged to improve its utility.Many new image enhancement techniques and computer processing programs have shown promise and may have a significant role in the future of endoscopic screening.These developments are paving the way for improving the diagnostic and therapeutic capability of endoscopy in the field of gastroenterology. | Tae H Ro Michelle A Mathew Subhasis Misra | 2015 | World Journal of Gastroenterology2015，21，33： | 9 |
| 2 | Genomic change in hepatitis B virus associated with development of hepatocellular carcinoma显示文摘AIM: To determine the genomic changes in hepatitis B virus(HBV) and evaluate their role in the development of hepatocellular carcinoma(HCC) in patients chronically infected with genotype C HBV.METHODS: Two hundred and forty chronic hepatitis B(CHB) patients were subjected and followed for a median of 105 mo. HCC was diagnosed in accordance with AASLD guidelines. The whole X, S, basal core promoter(BCP), and precore regions of HBV were sequenced using the direct sequencing method.RESULTS: All of the subjects were infected with genotype C HBV. Out of 240 CHB patients, 25(10%) had C1653 T and 33(14%) had T1753 V mutation in X region; 157(65%) had A1762T/G1764 A mutations in BCP region, 50(21%) had G1896 A mutation in precore region and 67(28%) had pre-S deletions. HCC occurred in 6 patients(3%). The prevalence of T1753 V mutation was significantly higher in patients who developed HCC than in those without HCC. | 5 | Endoscopic management of colorectal polyps: From benign to malignant polyps显示文摘Colorectal cancer(CRC)is the third most common cancer worldwide and the second leading cause of cancer related death in the world.The early detection and removal of CRC precursor lesions has been shown to reduce the incidence of CRC and cancer-related mortality.Endoscopic resection has become the first-line treatment for the removal of most precursor benign colorectal lesions and selected malignant polyps.Detailed lesion assessment is the first critical step in the evaluation and management of colorectal polyps.Polyp size,location and both macro-and micro-features provide important information regarding histological grade and endoscopic resectability.Benign polyps and even malignant polyps with superficial submucosal invasion and favorable histological features can be adequately removed endoscopically.When compared to surgery,endoscopic resection is associated with lower morbidity,mortality,and higher patient quality of life.Conversely,malignant polyps with deep submucosal invasion and/or high risk for lymph node metastasis will require surgery.From a practical standpoint,the most appropriate strategy for each patient will need to be individualized,based not only on polyp-and patient-related characteristics,but also on local resources and expertise availability.In this review,we provide a broad overview and present a potential decision tree algorithm for the evaluation and management of colorectal polyps that can be widely adopted into clinical practice. | April A Mathews Peter V Draganov Dennis Yang | 2021 | World Journal of Gastrointestinal Endoscopy2021，13，9： | 2 |
