3Restoration of energy level in the early phase of acute pediatric pancreatitis显示文摘Acute pancreatitis (AP) is a serious inflammatory disease with rising incidence both in the adult and pediatric populations. It has been shown that mitochondrial injury and energy depletion are the earliest intracellular events in the early phase of AP. Moreover, it has been revealed that restoration of intracellular ATP level restores cellular functions and defends the cells from death. We have recently shown in a systematic review and meta-analysis that early enteral feeding is beneficial in adults; however, no reviews are available concerning the effect of early enteral feeding in pediatric AP. In this minireview, our aim was to systematically analyse the literature on the treatmentof acute pediatric pancreatitis. The preferred reporting items for systematic review(PRISMA-P) were followed, and the question was drafted based on participants, intervention, comparison and outcomes: P: patients under the age of twenty-one suffering from acute pancreatitis; I: early enteral nutrition (per os and nasogastric- or nasojejunal tube started within 48 h); C: nil per os therapy; O: length of hospitalization, need for treatment at an intensive care unit, development of severe AP, lung injury (including lung oedema and pleural effusion), white blood cell count and pain score on admission. Altogether, 632 articles (Pub Med: 131; EMBASE: 501) were found. After detailed screening of eligible papers, five of them met inclusion criteria. Only retrospective clinical trials were available. Due to insufficient information from the authors, it was only possible to address length of hospitalization as an outcome of the study. Our mini-meta-analysis showed that early enteral nutrition significantly(SD = 0.806, P = 0.034) decreases length of hospitalization compared with nil per os diet in acute pediatric pancreatitis. In this minireview, we clearly show that early enteral nutrition, started within 24-48 h, is beneficial in acute pediatric pancreatitis. 4Metabolic diversity in sweet potato (Ipomoea batatas, Lam.) leaves and storage roots显示文摘Sweet potato(Ipomoea batatas,Lam.)is an important root vegetable in developing countries.After its domestication in Neotropical America,human migration led to the distribution of the sweet potato plant throughout the world.Both leaf and storage root are high in compounds of nutritional value.Yet,the storage roots are of particular value due to their significant content of provitamin A(β-carotene).The breeding effort for elite sweet potato lines led to the reduction of genetic diversity and the potential to improve other traits.The focus of the present study was to assess the metabolic diversity of 27 sweet potato cultivars including landraces and improved varieties.A metabolite profiling approach was optimised for sweet potato leaf and storage root tissue and 130 metabolites identified with three different analysis platforms.The data highlighted a lack of correlation between storage root phenotype and leaf metabolism.Furthermore,the metabolic diversity of storage roots was based on the secondary metabolism,including phenylpropanoids and carotenoids.Three cultivars of three different flesh colouration(yellow,orange and purple)showed a significant difference of the primary metabolism.This data demonstrates the value of metabolite profiling to breeding programs as a means of identifying differences in phenotypes/chemotypes and characterising parental material for future pre-breeding resources.Margit Drapal Genoveva Rossel Bettina Heider Paul D.Fraser 2019Horticulture Research2019,6,1:6
5Compared efficacy of preservation solutions on the outcome of liver transplantation:Meta-analysis显示文摘AIM To compare the effects of the four most commonly used preservation solutions on the outcome of liver transplantations.METHODS A systematic literature search was performed using MEDLINE, Scopus, EMBASE and the Cochrane Library databases up to January 31^(st), 2017. The inclusion criteria were comparative, randomized controlled trials(RCTs) for deceased donor liver(DDL) allografts with adult and pediatric donors using the gold standard University of Wisconsin(UW) solution or histidinetryptophan-ketoglutarate(HTK), Celsior(CS) and Institut Georges Lopez(IGL-1) solutions. Fifteen RCTs(1830 livers) were included; the primary outcomes were primary non-function(PNF) and one-year posttransplant graft survival(OGS-1). RESULTS All trials were homogenous with respect to donor and recipient characteristics. There was no statistical difference in the incidence of PNF with the use of UW, HTK, CS and IGL-1(RR = 0.02, 95%CI: 0.01-0.03, P = 0.356). Comparing OGS-1 also failed to reveal any difference between UW, HTK, CS and IGL-1(RR = 0.80, 95%CI: 0.80-0.80, P = 0.369). Two trials demonstrated higher PNF levels for UW in comparison with the HTK group, and individual studies described higher rates of biliary complications where HTK and CS were used compared to the UW and IGL-1 solutions. 6Expression and Chromosomal Mapping of Mouse Gpx2 Gene Encoding the Gastrointestinal Form of Glutathione Peroxidase, GPX-GI显示文摘GPX-GI is a cytosolic tetrameric Se-dependent glutathione peroxidase, similar in properties to GPX-1. Unlike the almost ubiquitous GPX-1, GPX-GI is mainly expressed in the epithelium of gastrointestinal tract. GPX-GI contributes to at least fifty percent of GPX activity in rodent small intestmal epithelium. The total GPX activity consists of at least 70% of selenium-dependent GPX activity in this compartment.By analyzing a panel of mouse mterspecies DNA from the Jackson Laboratory’s backcross resource,we mapped Gpx2 gene to mouse chromosome 12 between D12Mit4 and D12Mit5, near the Ccs1 locus which contains a colon cancer susceptibility gene. A pseudogene, Gpx2-ps is mapped to mouse chromosome 7.Comparison of Gpx2 gene expression in three pairs of C57BL/6Ha and ICR/Ha mice which are respectively resistant and sensitive to dimethylhydrazine-induced colon cancer, we found a higher Gpx2 mRNA level in C57BL/6Ha colon than ICR/Ha colon. Interestingly, a lower level of GPX activity is found in the resistant strain of mice. Because GPX-1 has three times higher specific activity than GPX GI, our data suggest that the decreased GPX activity may result from a higher level of Gpx2 gene expression in those cells co-express GPx1FONG-FONG CHU R. 7Successful use of adalimumab for treating fistulizing Crohn's disease with pyoderma gangrenosum:Two birds with one stone显示文摘Crohn's disease(CD) is a chronic relapsing and remitting autoinflammatory disorder of the gastrointestinal tract that has many intestinal and extraintestinal complications.The purpose of treatment is long-term remission,reduction of complications,and improvement of patients' quality of life.In many cases,this can be quite challenging and it is necessary to have a well thought out management strategy.We present the case of a 38-year-old woman with fistulizing CD that manifested as diffuse abdominal pain and bloody diarrhea accompanied by arthralgia.In addition,there were ulcerative lesions surrounded by cutaneous inflammation and erythema on her extremities,indicative of pyoderma gangrenosum.The patient was treated with high doses of parenteral methylprednisolone without any improvement and was started on adalimumab.A positive response to adalimumab therapy was observed:after 2 mo of therapy,the ulcerative skin lesion healed completely and the enterogastric fistula was closed after 5 mo adalimumab treatment.Adalimumab might be a suitable initial as well as maintenance therapy in patients with complicated CD.Eva Zold Arpad Nagy Katalin Devenyi Margit Zeher Zsolt Barta 2009World Journal of Gastroenterology2009,15,18:4
8Novel biomarkers of acute kidney injury: Evaluation and evidence in urologic surgery显示文摘Patients undergoing urologic surgery are at risk of acute kidney injury(AKI) and consequently longterm deterioration in renal function. AKI is further associated with significantly higher odds of perioperative complications, prolonged hospital stay, higher mortality and costs. Therefore, better awareness and detection of AKI, as well as identification of AKI determinants in the urological surgery setting is warranted to pre-empt and mitigate further deterioration of renal function in patients at special risk. New consensus criteria provide precise definitions of diagnosis and description of the severity of AKI. However, they rely on serum creatinine(SCr), which is known to be an inaccurate marker of early changes in renal function. Therefore, several new urinary and serum biomarkers promise to address the gap associated with the use of SCr. Novel biomarkers may complement SCr measurement or most likely improve the diagnostic accuracy of AKI when used in combinations. However, novel biomarkers have to prove their clinical applicability, accuracy, and cost effectiveness prior to implementation into clinical practice. Most preferably, novel biomarkers should help to positively improve a patient's long-term renal functional outcomes. 10联合分析血小板特异性抗体和瞬逝生物传感器技术检测血小板抗体的方法比较显示文摘目的应用联合分析血小板特异性抗体(specificpplateletantibodies,SASPA)和瞬逝生物传感器技术(evanescentbiosensortechnology,EVA)分别检测血小板特异性同种抗体和自身抗体,对两者进行方法学比较。方法收集94例存在HPA-1a、HPA-Sb、HLA-I和自身抗体的患者血浆标本,制备血小板膜糖蛋白-血小板特异性抗体复合物,分别用SASPA和EVA方法对复合物进行检测,对SASPA检测所得平均荧光强度值(MFI)和EVA检测所得斜率值(Slope)作统计学分析和比较。结果SASPA方法对HPA-1a、HPA-Sb、HLA-I和自身抗体检测的敏感度分别为74.1%、86.9%、86.9%和40.9%,EVA方法的敏感度分别为88.9%、78.3%、60.9%和72.7%。人类血小板抗原(HPA)-1a抗体检测中,MFI与Slope表现为强相关性,相关性系数为r=0.95(P〈0.05)。结论SASPA和EVA技术对传统的单克隆抗体特异性血小板抗原固定实验(MAIPA)作了继承和改良,SASPA方法可以用极少量的血小板同时检测多种血小板特异性抗体,提高了检测效率;EVA方法将血小板裂解后的实验时间缩短为10min,极大地简化了操作过程,减少了检测时间。SASPA和EVA检测方法的建立对临床血小板免疫相关疾病的诊断和血小板输血领域的发展具有深远意义。宋文倩 Torsten J Schulze Margit Baade Harald Kluter Peter Bugert 2017国际免疫学杂志2017,40,1:3
11Microscopic colitis: A retrospective study of clinical presentation in 53 patients显示文摘AIM: To evaluate the relationship between symptoms and microscopic colitis (MC) subtypes: to test whether collagenous colitis (CC) and/or lymphocytic colitis (LC)might be related to both constipation and diarrhea.METHODS: A cohort of patients with independently confirmed typical histopathological changes was investigated. Fifty-three patients with histologically proved MC (46 with CC, 7 with LC) were included. The existence of diarrhea or constipation and the co-existence of autoimmune diseases were also investigated and all data were retrospectively analyzed.RESULTS: Twenty-three (43.39%) of MC patients had chronic constipation (20 in CC, 3 in LC patients). Twentyfour(45.28%) of MC patients had autoimmune disease and the diagnosis of autoimmune disease was always prior to MC. Sjogren's syndrome was associated only with the constipation subgroup.CONCLUSION: The Janus face of MC resembles the subgroups of irritable bowel syndrome. 13Pulse cyclophosphamide therapy for inflammatory bowel disease显示文摘AIM:To assess the efficacy of intravenous cyclophos-phamide pulse therapy for refractory inflammatory boweldisease(IBD).METHODS:We included in our cohort eight patientswith(moderate/severe)steroid refractory IBD(4 withulcerative colitis and 4 with Crohn's disease).They allreceived 6 cycles of intravenous cyclophosphamide(800mg)per month.RESULTS:Patients entered into remission after the sec-ond/third cyclophosphamide pulse.Disease activity de-creased.There were no side effects and toxicity.All thepatients went into long lasting remission.All Crohn's diseasepatients and 3 of 4 ulcerative colitis patients achievedcomplete remission.One patient with ulcerative colitisshowed an impressive clinical response but did not enterinto remission.For the maintenance,patients with Crohn'sdisease were treated with methotrexate(15 mg/wk)andpatients with ulcerative colitis were treated with azathio-prine(2.5 mg/kg body weight/d).CONCLUSION:Remission was maintained in all pa-tients for 6 mo on the average.The drug was well toler-ated.These findings suggest that aggressive immuno-suppressive therapy may be useful in some refractorypatients and further controlled study should be consid-ered in order to fully evaluate this type of treatment as apotential therapy for IBD.Zsolt Barta László Tóth Margit Zeher 2006World Journal of Gastroenterology2006,12,8:2
