| 1 | Epigenetic profiles of pre-diabetes transitioning to type 2 diabetes and nephropathy显示文摘AIM: To examine DNA methylation profiles in a longitudinal comparison of pre-diabetes mellitus(Pre-DM) subjects who transitioned to type 2 diabetes mellitus(T2DM).METHODS: We performed DNA methylation study in bisulphite converted DNA from Pre-DM(n = 11) at baseline and at their transition to T2 DM using Illumina Infinium Human Methylation27 Bead Chip, that enables the query of 27578 individual cytosines at Cp G loci throughout the genome, which are focused on the promoter regions of 14495 genes.RESULTS: There were 694 Cp G sites hypomethylated and 174 Cp G sites hypermethylated in progression from Pre-DM to T2 DM, representing putative genes involved in glucose and fructose metabolism, inflammation, oxidative and mitochondrial stress, and fatty acid metabolism. These results suggest that this high throughput platform is able to identify hundreds of prospective Cp G sites associated with diverse genes that may reflect differences in Pre-DM compared with T2 DM. In addition, there were Cp G hypomethylation changes associated with a number of genes that may be associated with development of complications of diabetes, such as nephropathy. These hypomethylation changes were observed in all of the subjects.CONCLUSION: These data suggest that some epigenomic changes that may be involved in the progression of diabetes and/or the development of complications may be apparent at the Pre-DM state or during the transition to diabetes. | 6 | A programmed wave of uridylation-primed mRNA degradation is essential for meiotic progression and mammalian spermatogenesis显示文摘Several developmental stages of spermatogenesis are transcriptionally quiescent which presents major challenges associated with the regulation of gene expression. Here we identify that the zygotene to pachytene transit!on is not only associated with the resumption of transcription but also a wave of programmed mRNA degradation that is essential for meiotic progress!on. We explored whether terminal uridydyl transferase 4-(TUT4-) or TUT7-mediated 3' mRNA uridylation contributes to this wave of mRNA degradation during pachynema. Indeed, both TUT4 and TUT7 are expressed throughout most of spermatogenesis, however, loss of either TUT4 or TUT7 does not have any major impact upon spermatogenesis. Combined TUT4 and TUT7 (TUT4/7) deficiency results in embryonic growth defects, while conditional gene targeting revealed an essential role for TUT4/7 in pachytene progression. Loss of TUT4/7 results in the reduction of miRNA, piRNA and mRNA 31 uridylation. Although this reduction does not greatly alter miRNA or piRNA expression, TUT4/7-mediated uridylation is required for the clearanee of many zygotene-expressed transcripts in pachytene cells. We find that TUT4/7-regulated transcripts in pachytene spermatocytes are characterized by having long 3Z UTRs with length-adjusted enrichment for AU-rich elements. We also observed these features in TUT4/7-regulated maternal transcripts whose dosage was recently shown to be essential for sculpting a functional maternal transcriptome and meiosis. Therefore, mRNA 3Z uridylation is a critical determinant of both male and female germline transcriptomes. | 7 | 肺动脉导管对重症患者的影响——随机临床试验汇总分析显示文摘背景:评估肺动脉导管(pulmonary artery catheter,PAC)的随机临床试验(randomized clinical trials,RCTs)一直受到研究样本量较小的限制。一些非随机研究表明,PAC的应用会增加患者的发病率和死亡率。
目的:评估PAC对重症患者的影响。数据来源:在MEDLINE(1985~2005年)数据库、Cochrane对照试验登记站(1988~2005年)、国立卫生研究院ClinicalTrials.gov网站数据库以及美国食品与药物管理局网站上对将患者随机分配至PAC组和非PAC组的RCTs进行检索。在重症心力衰竭患者中进行的ESCAPE试验结果也被纳入。检索词包括肺动脉导管、右心导管、导管和Swan-Ganz。
研究选择:符合条件的研究应包含如下患者:接受手术治疗的患者,进入重症监护病房(intensive care unit,ICU)的患者,因重度心力衰竭入院或者诊断为急性呼吸窘迫综合征和/或败血症的患者。作为观察指标,入选的研究报告了患者死亡、住院天数或ICU住院天数。
数据提取:各研究的入选标准、基线特征、干预措施、预后以及方法学质量的信息由2位作者提取。不同意见通过协商达成共识解决。
数据综合:13项RCTs共包含5051例随机患者。各试验的血流动力学目标和治疗策略各不相同。使用随机效应模型估计患者死亡、住院天数以及使用变力和静脉血管扩张药物的优势比(odds ratios,ORs)。死亡率的合并OR为1.04(95%可信区间[confidence interval,CI],0.90~1.20;P=0.59)。PAC组平均住院天数与非PAC组平均住院天数的差为0.11天(95%CI,-0.51~0.74;P=0.73)。PAC与变力药物(OR,1.58;95%CI,1.19~2.12;P=0.002)和静脉内血管扩张药物的使用率较高相关(OR,2.35;95%CI,1.75~3.15;P〈0.001)。
结论:对于重症患者而言,PAC的使用既不增加总体死亡率或住院天数,也没有给患者带来益处。尽管相关RCTs已进行了近20年,但是仍未得出可以提高患者存活率的明确的PAC使用策略。PAC对临床预后的中性作用可能为对其在各种重症患者的使用缺乏有效的循证治疗所致。 | Monica R. Shah Vic Hasselblad Lynne W.
